@adc0417, sorry no experience with injecting estrogen. And I didn’t mean the ERA would help prepare you as such, just that it doesn’t stop you from stimming the cycle after. Also if they do an endometrial scratch whilst they are in there it might help with implantation. So I think its only positive you’re doing it this time.
@hopefuljj Hope the priming does its job without over suppressing - fx for tomorrow!

@dreamscapes_ it’s so wrong that money plays a role in all this, especially even for testing. I hope you are able to find enough to do it this month, so at least you can have the appointment with the results and know which options are possible to make the next plans.
@jamieh2000 it also sucks that the natural/ normal decision needs to be money related. Though for natural they talked to me about really natural - ie monitoring for natural ovulation - so no meds at all, just monitoring costs. But if you have lining issues that’s maybe not an option anyhow.

AFM monitoring was this morning, and I’m not back in again until Monday. I had two follies on the left at 11mm (though my left ovary was seriously hiding and I had to get into a weird position for him to even find it), and there could be a few antral follicles on each side which haven’t woken up, but too small to see.
So good that I have two, and they are growing together (no runaway follie like the previous two times). But definitely not growing fast. And although I hoped for two, I left the appointment feeling very emotional and disheartened.
So my request to my ovaries for Monday’s appointment is to grow these two follies nice and steady and healthy to be in the right range to trigger on Monday or Tuesday night. If they would like to give me a third follie which has caught up with the first two I would be extremely thankful.

@hopefuljj- So hoping for good news at your baseline tomorrow! Keep us posted!!

@adc0417- If you are injecting estrogen to grow your lining, its usually enough to suppress your FSH and prevent follicles and ovulation, so I don't know it could happen naturally this month. Some ladies ovulate through estrogen, but you don't want that when you are doing a transfer. Do they have you on lupron too? My clinic always does lupron and estrogen for transfer cycles.

Everything about this process is heart breaking. None of us should have to go thru any of this to have what most have so easily. I am sending you lots of prayers that both of those follies grow strong together! And of course a third follicle would be totally welcome!!

AFM- My RE said I could do another transfer, but recommended we do the ERA since she says I should be pregnant from the last transfer since the lining and embryos were beautiful. She said she has only found 10% of ladies have their transfer timing off, but I am still glad we are doing it to check. She seemed very confident we would get me pregnant this year. I feel like they are always confident, yet I keep defying their odds (DE transfers have a 80-90% success rate at my clinic). I asked her if she has ever had someone do DE and not get pregnant and she couldn't remember anyone... I am sure there was someone, but hopefully it won't be me. Now if I can just mentally make it through as many transfers as it takes. I didn't ask her about adding a steroid to my protocol next time, but will once we start the transfer cycle. We are also doing a medicated FET (instead of a stimulated FET) because it would just be so much meds for the stimulated again. I am nervous my lining won't be as good, but hopefully with the polyps gone now, that protocol will work for me

@emmasemm how many days stemming are you now. Slow and steady wins the race. I know it's so hard! I would love another to pop up but I was just like u! Praying the ones that are growing continue to grow together and be ready to go!! Just keep talking to them and I'm still sending miracle grow your way!!

This whole process is a fortune, by 12wk blood tests with the high risk dr is $900 which I'm waiting to see what the insurance is covering and hope I don't have to pay for the rest!! It's insane especially because I'm suppose to be covered for blood work etc. This is such a business it disgusts me!!

Hi @Hopeful_mom, Glad to hear that everything is going well with your little one. I love all your updates. I am possibly pursuing IVF in the upcoming months and was wondering if your protocols are the mini-ivfs? Have you tried the full on IVF with high dose stimming? Did you feel like there was a difference with the quality of the eggs retrieved? I'm in the Dallas area and there are two REs who specialize in mini-ivfs but neither are my current RE. So i'm really debating changing REs, but at the same time, i know if i do, the new RE is going to make me test all the same tests over again, so i'm not too sure yet. Wishing you and H&H 9 months

Hi @babyhopeful442 are you able to see my signature? Which I have to update! I know it seems like mini IVF from some of my response but it was actually a full ivf cycle. I actually had a conversation with my clinic on how I should be charged as a mini IVF patient since I was responding low etc. I was still paying for all the monitoring I was having regardless. And paying for all the meds so I was frustrated paying so much when I was responding so low. Since doing 8 cycles I now knew my body didn't respond great meaning not producing 20 eggs BUT still thrilled when I would walk away with 5. THe one cycle I walked away with 9eggs I had all abnormal embryos so quantity with me was out the window.
Some people respond well to clinics used during IUI and mini IVF. The most I got on 150mg was 3 so i always opted to do the meds once I saw how I responded.....however was it the right decision. If I was younger I would've tried a mini IVF cycle I was just full steam ahead and didn't have enough time. Originally I thought I would do another 2/3 cycles after my MC bank enough normal embryos and move on. Unfortunately that was not the case and I first had to do 5 cycles.
My very first cycle I started meds at 450 and bumped it to 600 as we progressed. Since it took a few days for my follies to wake up we always started at 600, 300 memupar and 300 gonal f. I truly can't answer the quality question as I got normals and abnormal embryos on the same protocol. Along with estrogen priming sometimes it was ok and others it wasn't. I wish I knew the winning potion but for me it was luck of the draw and slow and steady wins the race I guess....
how old are u and what's your numbers? On my mobile so forgive me if you have it in ur signature.
do you plan on doing pgs? Are your meds covered by insurance? Are you looking to do a fresh transfer or freeze all? To give advice on which way to go is tough. Do u like your RE? I think 2nd opinions are a good idea and you should certainly look into it so you have all your information. Tests are annoying only if it slows u down normally it's good to have them done. It still sounds like you're in the beginning so I wouldn't worry too much about the tests. Just know you will need an HSG done if you haven't gotten that one done. I wish I knew that in advance as it delayed me another month to start just for that one.
Dont want to confuse u too much, think I answered your questions. Happy to share any knowledge I have.

Just stopping by to leave a quick update. Baseline went ok. It looks exactly like my last baseline and so I’m trying not to get excited because last time I was really pleased and then follies disappeared and didn’t grow and I got canceled. I’m stopping estrogen priming and waiting to Start stims! on Saturday in hopes that some of the smaller ones will catch up to the larger ones and a few might grow together. Fingers crossed. Nervous with the size differences though and how similar it is to last cycle. But happy that I have follicles as I have had plenty of scans where there are none. I had my appointment at 8am so I traveled in the blizzard all the way into the city and then my boss emailed saying I should head home after my doctor appointment because everyone was working from home so I went back to Grand Central and got back on a train at 9am but the train got stopped when we were barely out of the city due to fallen trees AND a frozen switch! Luckily I had a WiFi jet pack and my computer so I worked for 2 hours trapped on the train and then hiked up the huge hill by my place through the storm back home and continued to work from home for the rest of the day. It was a super crazy day:)

@Hopeful_mom Thank you for your response. I do plan on doing pgs testing, but both REs that i interviewed says it can only be done on day 5. I hope i have embryos from day 5. I'm 32 and my amh is 0.898, FSH 6 back in august of 2017. However, i recently had my FSH retested and it was a 8.7. So now i'm freaking out that my amh might have dropped again despite all my supplements and acupuncture. my meds are not covered by insurance and I think i will be doing a freeze all for the best success. So far, i've spoken with two REs. RE1 is my current RE, who feels i should go directly to IVF and not waste anymore time with iui and he wants to do the full IVF with 2 weeks of BC to start. This alarms me as i know people can be over suppressed when they have low AMH. He doesn't believe in mini ivfs and his costs are very high for the Dallas area, ranging from $22,000-27,200(this includes the meds). His stats from the CDC website is 8/19 live birth per cycle for fresh transfer and 0/2 live birth per cycle for frozen. They generally don't do too many DOR patients it seems even though they are quite a big clinic with 622 cycles per year. RE2 does mini-ivfs and doesn't use BC and his cost is $11,000 (meds included) per cycle and it includes ICSI as well as Assisted hatching. RE2 does 525 cycles per year. RE2 says he would split up the package price if i was to do multiple retrievals and just pay for retrieval ($5400) if i have cycles with no embryos, where as RE1 doesn't split up the cost at all. I am hoping to bank embryos like you if i can. RE2 stats are 50% live birth per cycle and 5/12 for FET. What do you think? I'm leaning towards RE2 so far and he's willing to do another IUI on me just to see since i have full coverage for that. I just don't know if mini is more successful than conventional IVF. have you read much about it? Sorry for the long post...any advice would be appreciated. There's a third RE that i'm interviewing in two weeks because his mini-ivfs are even cheaper. 3600 per retrieval and this includes anesthesia! and 2200 for transfer.

@babyhopeful442 just chiming in with my opinion here. Mini IVF is very successful for some ladies with DOR and not with others. That seems to be the case with most protocols. When I was getting ready to move on from IUI’s I was obsessively reading about mini IVF vs regular and it seems to depend on how many eggs you would end up growing successfully and retrieving on a regular stim cycle to determine if mini might be as or more successful. There are great arguments for it and against it. I know this isn’t helpful but I just wanted to mention it because I drove myself crazy reading both. I really respect Dr Sher and Dr Gleichers opinions in the DOR world but even they disagree on certain areas so this entire journey has most of us just torn in different directions and eventually trying different things or just deciding to pick an RE and follow their lead with some poking and prodding along the way to add in this or that or try this or that.

@babyhopeful442 one more thing, FSH fluxtuates from cycle to cycle even when testing on day 3 and yours isn’t alarming at all so I wouldn’t be worried about the AMH having gone down.

@babyhopeful442- I agree with the other ladies... mini vs full IVF is always a debate I think. Personally, I think I would try one full IVF with max stims to see if you can get more than a few eggs, since this is a numbers game. If you don't respond well, it may then be worth trying mini IVF since some ladies will get the same amount of eggs on mini, and then its cheaper to do multiple rounds. I think having an RE who has experience in DOR is key, and someone you feel is really behind you and invested in you.

@hopeful_mom- Thanks! I hope the ERA is the missing piece. and I really hope my body behaves so I can get it in before the trip. I wouldn't have wanted to transfer before the trip anyways, so this is good timing if we can make it work!

@jamieh2000 thanks a lot. I’m feeling much better the last two days, but I think the stress will build again Sunday ready for Monday’s monitoring. Doing repeat cycles with hardly any follies is like reliving a nightmare over and over...
Then again I guess you might feel some FET PTSD too... I’m pleased your RE is doing the ERA even if it’s just to save you heartache - let alone money and embies. You didn’t have them PGS tested I think? So even in a young donor 20% might not be chromosomally normal but the chances the next one is will be super high (like 8 out of 9).

@hopeful_mom thanks for sending the miracle grow!!! Tonight will be night 8, so by the time I go in for monitoring we will have done 9 nights. My first cycle we only did 8 nights and I had the good egg. But last time I think it was 14 and no good eggs. Steady but not too steady is thus the aim! I think if we’re at 20mm then he will trigger me Monday night for Wednesday ER. And if we’re not there keep going for 1-2 more nights. They don’t do weekend ERs (bloody German laws...) which I hate as I either have to trigger on Wednesday or wait until Saturday... and as we know over cooking eggs for three days does not help...

@babyhopeful442 my first two IVF cycles were kind of mini as we “only” used 225 FSH. Most others are using 400-600 when you combine the FSH in the follistim/gonalf with that in the menopur. For me, the lower dose only gave me a few more follies - I do ovulate one well on my own each month. Going to see what 300 gives this time.
I think @hopeful_mom and @hopefuljj gave good advice and experience in their posts. I would say for us I think flexibility is key as you don’t know if you will get to ER or transfer each cycle. And although AMH is important, what your AFC is each cycle is what will determine your upper potential. If you have an AFC of 6-10 it’s totally different to having an AFC of 1-3 like me. I know many ladies with lower AMH than me, but much higher average AFCs. So it’s not a perfect correlation.

@hopefuljj pleased to hear you had a good baseline! When do you go for first monitoring?

AFM night 8 of stims and night 3 of Ganirelix tonight. Really really trying to encourage my ovaries to grow those to follies nice and healthy and strong ready for Monday’s monitoring. Come on ovaries, you can do it!

@emmasemm- Thanks girl. I totally hear you. I think I have U/S PTSD most out of anything. I literally shake sometime after having cycles cancelled, cysts found for month, fluid in my lining and transfer cancelled... I can't imagine the anxiety I will have when we finally get to an U/S to see the heartbeat. We did not do PGS, but my donor's past cycle did and they were 70% normal. So we should have really had one normal out of the two we transferred. I'm hoping the ERA will be the unlock for me. That is so frustrating they don't do weekend ER! I mean some things are time sensitive.. I assume they do emergency surgery on the weekends in Germany? this is not that much different! Praying for good news for you. You have been such a support to us all and I want this for you so bad!

@jamieh2000 oh I forgot you transferred two. Ok then out of ten. That should be seven normies out of eight. With ERA as well to check timing then you will be all set! I’m super convinced.
I want for you to be successful soooo much too. And soon. You have had far too many delays. So ERA-trip-transfer - that’s how it’s going to be! No more fluid or delays. Smooth sailing only from here on in!

And yes, they do emergency surgery - but I mean truly emergency. My DH has kidney stones and has several times ended up in the hospital with them - but twice it’s been on the weekend, when all they do is give muscle relaxers and pain killers (IV) and wait to operate on Monday... when he went in the other times in the week then they operated same day... apparently there’s different levels of urgent... and IVF is seen as elective... so unless my life was in danger eg from OHSS I cannot get an ER on a weekend... and I mean the chance of me getting OHSS is less than seeing a pink elephant fly past our window... so I need my follies to really be good to go for ER latest Friday, if not we have to choose between doing ER but risking them being immature or waiting and frying them... another reason we’re looking overseas for round four...

I have to keep hoping that some day soon it will be over and somehow I’ll be thankful for all of it. Wow, I can’t believe they wouldn’t even handle kidney stones on the weekend!! I mean I get weekends are important, but gosh! Probably doesn’t hurt to consider a new place for ivf number 4, which hopefully won’t be needed!! I did a little bit of research about EU when I was first looking at DE over seas and one of the clinics I found with great reviews was institute bernabeau or something like that in Spain. It’s on the coast, so I figured that would be a benefit to help relax during treatments... I’m sure you know way more than me about food clinics over there but just thought I would mention it!

@hopefuljj when's your first monitoring?

As for this morning - well I have two follies on the left at 20mm. So I really do have two follies growing well together! Way to go ovaries!!! I also have one at 14mm on the left which probably won't catch up enough, but of course when they go in for retrieval they will try. On the right I have something at 24mm which doesn't look nice and clear and dark like the ones on the left, so he thinks its an empty cyst from November's cycle and will "give it a poke" whilst they're in there for retrieval.

One more night of stims and Ganirelix tonight (I'm just going to empty the syringe of Pergoveris... will probably be 350 instead of 300 but he said it's ok as it won't keep until next time anyhow. Then trigger Tuesday night for Thursday morning ER! Let's get this show on the road!

Super nervous as last time we were lookings at 2-3 follies, but they were not as well grouped together and we waited too long on the big ones but not long enough for the small ones. So I hope this time we can hit the sweet spot and if we get two mature eggs I will be soooooooo happy.

@emmasemm- YAY!!! two at 20 is great!!! I'm surprised your RE will stim for 1 more day... my RE will trigger once they get to 20, because the trigger and 1.5 days still adds some growth. I think there is a good chance the 14 could make it too! Praying you get at least 2, if not 3, eggs!! Can't wait for Thursday!

Yay @emmasemm !!!!! I’m so happy and hopeful for you! I bet if your RE is stimming for one more day he/she measures them like one of the other RE’s at my clinic which is by the longest part of the follicle, opposed to how my RE measures them which is an average of length and width. My RE always warns me if the other doctor is covering on a weekend when I need to go in for monitoring because his numbers will sound much larger because of that measuring technique. So he triggers patients when they sound much larger. Woohoo to one more night of stims!!!!!

afm I’m back in on Wednesday for monitoring. Hoping to not get canceled!

im 19weeks today and thrilled beyond! I won't lie I'm still scared everyday! Each test terrifies me! I keep hoping by the viable week maybe things will change! I am pregnant and trying to enjoy it. Haven't bought a thing as many have already...
All I did was complain of my nausea and I can finally say its no longer morning, noon and night now its more towards midday through the night but it's something. I have indigestion like I'm in my last trimester it's insane how awful it's been since day one! I know it's sounds like I'm complaining, I guess I am but more sharing with all of you! I can't wait for everyone to be on the other side and when you experience some of the same not so lovely symptoms I want u to know you're not alone!!!!

can u believe my vag is bac to giving me issues, come on! Being a chic!

AFM not canceled yet but yet again a bunch of follicles disappeared. Wtf. It must be a poor egg quality issue. So I have 4 follies now and i have no idea how many are real and how many are fake outs. Seems like at least 1/3 or 1/2 end up as fakeouts when it comes time for retrieval. Hope I make it to retrieval!!!!

Sooooo... we got three eggs!!! Well done ovaries! Way to go!!!
Now the wait for tomorrow’s maturity and fert report... and we’re just asking that after a great party with DHs sperm overnight and we have three strong couples by the morning. Two would be good, three would be out of this world...

@hopeful_mom if you can sprinkle some magic fertilizing dust that would awesome (as clearly your miracle grow worked!) So pleased to hear your little nugget is growing away. Though thanks for the hint on which end was the head hehehe. But come on vag... can’t she just give you a break now???

@hopefuljj one step at a time - yay for not getting cancelled so far!!! But boo at disappearing follies again... That really is annoying... but I guess it equally could be that there are or aren’t eggs in each follie... when’s your next monitoring? And what did your RE say? Do you already use HGH?

No news on my front. DH finally scheduled his SA, so that's on Wednesday afternoon. We're spending Monday and Tuesday with my parents, so we won't have much chance to HIO, so the timing actually works out really well for not being able to do anything. Once his test is done, we'll set up the follow up for all the results.

Against all the odds I have a day 4 embie on board! It was at 8 cells when they took the photo at 7.30am, but when they put it on the screen the embryologist said she could see 12 cells (I couldn’t see much through my tears!!!) and that one area looked like it was starting to compact. They did assisted hatching on it too.

I have never been so nervous in my life as I was this morning - every time the phone rang I thought it would be the clinic to cancel. I even threw up a few times.

After Transfer I went straight to acupuncture, which definitely helped me relax, now a few hours of meetings and home for an early steak dinner tonight.

Hopefully we can have a week or so to breathe before I start symptom spotting and having to convince myself not to test too early...

@emmasemm- YAYY!! Congrats on being PUPO! I am so happy for you! and I EXACTLY the feeling you were having this morning. I swear my heart rate doubles whenever i see the clinic call. The anxiety during this process is insane. Glad you were able to do accupuncture to relax! Sending you all my sticky vibes!

@emmasemm OMG OMG!! Just read your update and so so happy for you. You’ve given me so much hope, although off course each case is different(my AMH is lower than yours). I’m in tears and oh so excited for you.
