@JamieH2000 Oh no! I'm so so sorry to hear you've had so many setbacks and yet another delay. It's truly awful that you've had so many "odd" things happen to you that have delayed your progress. I hope your talk with the RE on Wednesday goes well and you can find a plan that will work for you.
AFM- I met with my RE today and was hoping to get input from you very knowledgeable ladies on the things she said and recommended, since I'm very new to all of this. **TW-will mention miscarriage** First off, she said my AFC during my baseline u/s on cd4 of this cycle was only 3, not 6-8 like I had thought she said during the u/s. She told me to start taking CoQ10, which I've already been taking for a month. She also gave me a lab slip to have my vitamin D checked, as well as the full genetic testing that will include the Fragile X test. She said since my AMH was so low (.12) and I've had 3 miscarriages since August, it's plausible that my egg quality isn't very good. On the other hand, she said they could be really good quality, but there's no way to know. She laid out all my options: keep trying on our own, IUI with clomid, or IVF with PGS. If we decide to keep trying on our own, she suggested that we put a short-ish timeline on it since my AFC was so low and it's not clear how long I have left to conceive. I'm leaning towards trying on our own for a while longer. I know it takes CoQ10 at least 3 months to have an affect, so I think I should give it a chance. And I doubt I'll stop ovulating in the next couple of months! I know I'm capable of getting pregnant, but I'm still a bit concerned about my egg quality because of my miscarriages. But it could have also just been really rotten luck too. I guess my questions are: 1) Is there anything wrong with me waiting and trying on my own for a while, or do you think because of my low numbers it's time to be more aggressive? I know that's a personal question and no one can tell me what to do, but I thought some advice about the meanings of these numbers could be really helpful. 2) Is an IUI even that helpful for someone with low AMH/low AFC? Since it doesn't improve egg quality, I'm not sure we should even consider that. Now that I've written so much, I'll leave it at that. Thank you for your help as I'm truly at a loss with all of this.

@ilky sounds like your cycle is moving forwards! Getting to ER and finding out what’s going on with your eggs can be a really important step in itself to learn more about which steps issues might be at. Fx your monitoring goes well tomorrow - how big is your largest follie?
@offtoneverland oh big hugs again. First of all I suggest you read the link I sent you in the PM over the next days. Second there is no right or wrong way to go, just different ways. I will be short as I need to go to sleep (but was stalking here and TTGP in case you posted an update) - personally I would go straight to IVF (it’s actually what I did and do not regret it for one moment - but like you say it’s absolutely a personal decision) - if you have enough embies to try growing to blast PGS Test the first batch, if not do a fresh transfer but grow to blast, test and freeze the rest if you are lucky enough to have some. Of course it depends a lot on insurance coverage and finances - but if you can afford to, my advice is doing lower level interventions will not really help speed things up (or get to the root cause). And be prepared for multiple rounds - some get lucky on the first try, but some of us need a lot more rounds to find our golden egg. Check your RE is not a “one size fits all” IVFer - we need specially tailored protocols. With your history I would ask them again to run RPL and autoimmune testing just to be sure you don’t have a second issue (as we can’t afford to waste any normies we get). Back for more tomorrow. @jamieH2000 uuuuuugh lady I'm so sorry. I mean I'm happy if this is the solution you have been waiting for to get your BFP but i'm just so sorry that there is more waiting and another obstacle. It really is something every single time! I still think about when the donor you had picked got pregnant and just scream. Of ALL the things. you really are such an incredible trooper. @emmasemm Your consult sounded really great and through! I'm sorry about the positive Lupus test:-/ I called Dr. Jeffery Scher and price out his immune stuff but then because he was out of town for many weeks and only takes a small number of new patients on and cost, I decided I might not end up doing it. But I did talk to my RE about all my concerns and had my normal 3 pages of notes. hahaha.
I will give the short version of the story but basically asked for Lovenox, Prednisone, Intralipid therapy, and an ERA. She said she will treat me like i have had multiple losses and do all the testing she does for that due to the small amount of embryos to transfer after 9 months of IVF but she won't give me Lovenox unless i come back with a clotting issue. Even though as far as SOME research is concerned, being Homozygous for MTHFP that is a clotting issue. She said no Intralipids unless i get test results that she thinks warrants but if they do she is happy to do it...and Prednisone is also a no for now. I mean, come on, i have Hash!! and everything i read says that means i will very likely have implantation issues So we did an Immune panel which was just 6 vials of blood from me and one from my husband (to see if I am at risk of attacking his DNA) and then I did a blood clot panel which was a whopping 23 vials of blood after fasting. I'm awaiting those results. Depending on what we find we will see what i do prior to or during transfer. As for the ERA, you can't get it in NY?!??!? Anymore??? SO she said i can go to New Jersey and have a doctor do it but she recommends the Yale EFT or the Receptiva DX or RX or something like that. I am concerned with the fact I don't think i shed my lining enough every cycle so when i go in for the Yale EFT (which will act as a scratch and also test for Endometritis and other infection as well as for how receptive my lining is) she is going to do a few passes (so like a scratch and then some) to clean out my lining, so she is recommending I take the twilight anesthesia for this. So that is what I'm doing. I still feel like I'm not doing nearly enough but I"m glad I put the brakes on her from moving forward immediately with transfer.

@llky do you go back for a scan and blood work today? The estrogen is hard for me because my best cycle I had low estrogen and we retrieved 7 and 6 were mature eggs and my estrogen was below 1000 right before retrieval. Then this last round my estrogen looked great!! Highest it has ever been! And we only got 3 mature. The estrogen also really goes up once the eggs are in the mature range so there is definitely time for it to go up for you. Crossing my fingers for you! I always have one a bit bigger too and I have been able to retrieve it on occasion some I’m rooting for your smaller follicles to hurry up! ANs thr larger one to slooooow down:)

@capps42613 and @kp214- you ladies are SO kind to keep checking in and cheering us all on! Really so nice of you, since I am sure you are both pretty busy now with little ones! Hopefully SOON I will have good news to share after over 1.5 years on this board!
@ilky- it sounds promising! Of course we would always like more, but I have definitely learned that its quality over quantity on here. While I was able to get a few more eggs than alot of ladies on this board, my quality was much worse. So don't count yourself out yet! For estrogen, the rule of thumb (which can be wrong) is ~200 per mature egg. But that is at trigger time I believe. So you still have some time and estrogen normally exponentially climbs as you get closer to trigger. This part is really stressful, but hang in there!!
@offtoneverland- I wish I had some wisdom for you, but this is all such a hard decision. My point of view is to not wait and do IVF immediately, if you can afford it. Unfortunately much of this comes down to financial decisions. It really sucks and some day when I get through this I would love to advocate to get the laws change. I am assuming you have checked if your insurance will cover anything? I would also assess how many rounds you could afford, as it will likely take a few rounds. I would reco banking embryos, as @emmasemm suggests. I wouldn't waste any $$ or time on IUI. The upside of IVF/banking embryos is if you are able to get more than 1 or 2, you can also save them for a second child, if that is something you want. You do seem to get pregnant naturally easily, but I am sure the miscarriages are absolutely devastating, so only you can know how much of that you can handle. Feel free to ask us any questions. I wish someone had a crystal ball to tell us all what to do and how it will turn out, but its really just all a gamble unfortunately
@hopefuljj- you are so sweet and you are such a trooper too! Our time just HAS to be coming right?! Your RE appt sounds like it went well and good for you for advocating for yourself! That is so interesting that NY doesn't do the ERA. I don't really get the difference between the ERA and EFT, but I think its great you are able to do the EFT at least! I think if she is going to be roughing it up in there with the biopsy, twilight anethsia is a good idea. Can she also look around in there for polyps or anything else, kind of like a hysterscopy? I am interested to hear the results of your bloodwork. I am wondering if I should request an immune/RPL panel if i am delayed another month. After all of my delays, we might as well check EVERYTHING we can at this time. Are you able to get started on your mock cycle for the EFT soon?!

@hopefuljj and @JamieH2000 thank you! I just came from my appointment. There seems to be now 8 follies measuring, 18, 16, 16, 16, 15, 14 and 11. I'm going back on Thursday and most likely everyday after that. Dr. said the ER will be probably on Sunday or Monday and the slow growth was normal. He added that usually it takes 2 weeks, so I hope thats true. I'm kind of worried about the slow growth in spite of 425IU follistim and 2 vials of menopur. I guess I just have to trust him, but it is just so hard for someone like me, who is a control freak lol. Havent gotten the E2 results but I hope its also in line with expectations. I just need to be a bit more patient...Ugh!
@ilky- yay for more and a good group together! I am a little surprised he thinks you won't trigger until Friday or Saturday night. my guess is you would be ready Thursday night after two more nights of stimming! But they will check on Thursday and adjust timing if needed! It would be nice to have a weekend ER too FX that the estrogen has a good rise today too!

@offtoneverland I agree with everyone above. Pls don’t waste your time and money with IUI and go straight to IVF, of course depending on the finances etc. My RE who first diagnosed me with DOR wasted my time with 3 IUIs 1.5 Years back. Wish she had gone to trying an IVF right away; my hormones would’ve cooperated more then.

@emmasemm@adc0417@JamieH2000@hopefuljj Thank you all so so much for the advice. It is so incredibly helpful to get advice and tips from you all since I have no idea what I'm doing right now. I'm glad I was not off base in thinking that an IUI would likely be a waste of time. Our insurance does not cover any fertility treatments at all, so it would all be OOP, unfortunately. If I had the option, I would try to move to IVF sooner rather than later to reduce the chance of any more losses and to make sure my AMH doesn't keep dropping. We decided to try on our own a couple of months and reassess after that. I'm hoping I can catch that "golden egg" but if not, we will consider the pros and cons of IVF. Again, thank you so so much for your words of wisdom! @hopefuljj That's great that you're getting some extra blood work and testing done to check all the boxes. I'm sorry your RE was not on board with all the medications you were hoping might help. Hopefully everything will be all set for whenever you're ready for the transfer. @Ilky Good luck with the transfer. I hope everything goes smoothly up until then. @adc0417 I'm sorry to hear you wasted all that time on IUIs. Thank you for sharing that with me. It sucks to look back and wish you had done things differently, so thank you for helping me with my decisions.
@offtoneverland- I don’t think things will change drastically in a few month and maybe you will get lucky! You can also use that time to save for ivf if it’s needed.

@llky that sounds really really GREAT!!!!! What day of stims are you on? My follicles grow really slow some cycles and my RE has to wait for the estrogen to rise before triggering me sometimes so it feels like I trigger really late.
@offtoneverland good luck lady! I hope you catch that good egg! It certainly does happen for DOR ladies sometimes!! I am so sorry you would be paying out of pocket. It makes me so incredibly mad that many women (and couples) have to struggle with this process entirely out of pocket. I’m with Jamie on this one, if I ever make it to the other side I would love to find out how to become and involved in the fight to get mandated infertility coverage in more states. It is so unfair.

But I’ve just decided to search the house before I go to work for any similar things Also, make sure you get the full results from all that blood work - if your RE sees something in the results I hope they will change their mind about adding anything else to the protocol

@offtoneverland I think your plan is great - but most of all it is if you and Your DH are happy with it. Just be aware that with some clinics they have a wait list to cycle, also some want test results to be within 3 or 6 months or you have to test again before you can start. Or suddenly come up with an extra bunch of tests. Several of the IF ladies on TTGP decided to do IVF last summer but only did their first cycle in January. So make sure you check your clinic’s policy so you don’t have extra delays if you decide to go for IVF.
But of course best would be you just get KU naturally during the next months and your RE has some good tricks up his sleeve to avoid your history repeating. Fx!

@hopefuljj- haha thank you! I needed that! I do remember that brady bunch episode. We were just in Hawaii, but made sure to not bring back any rocks or sands because of the bad luck myth! It sure does seem like I have some sort of bad luck curse lately.... That is interesting about the EFT. So it sounds like it doesn't measure receptivity really? Since its 10 days post ovulation? I think all those are things are good to get checked for still though!!

I'm 37 years old, married almost 9 years, and mom to 2 beautiful boys. Our first son passed away when he was 8 months old due to liver failure (born without enough bile ducts, we still don't know why). Our second son will be 3 in a couple of months. No trouble conceiving. We've been trying for #3 for about 2 years. Had one missed miscarriage after trying Clomid with my OB/GYN last year, D&C December 2016.

Last year we switched to a RE, AMH=0.6, did a few medicated cycles and IUI and, surprising no one, no success. Thought we were done because I wasn't sure I could handle the emotional toll of spending so much and maybe ending up with nothing. There's only so much in a lifetime I can take. Ended up with surprise IVF coverage in 2018 of one round, so we're on to that. AMH a year later lower at 0.5. Our first attempt last month was 16 days of priming with estrogen and progesterone, 3 days of Cetrotide, then antagonist protocol, 350 follistim and 300 menopur. Cancelled, only 3 good looking follicles and 2 little ones, with estrogen at 750.
We have it in us (and our wallets/insurance) for one more try. So this one is it, cancelled or not. Just had bloodwork today and should hear back later today about priming and new protocol. I know our RE plans to add Lupron but haven't heard past that.
I am scheduling a call with the RE to talk through approach, and would love thoughts from you ladies on questions to ask if you're willing to offer assistance! Basically, I want to make sure we're going in as aggressively as possible for this last try. I know most women with DOR have to go through multiple cycles to get to one beautiful egg, but I just can't. I want to at least make sure we're giving our one shot all we've got, but I don't know what more to ask for. Stronger meds? Different meds? If you have any advice, or protocols that have worked for you that I should ask about, it would be very much appreciated!
@JamieH2000 the EFT tests the receptivity of your lining. They grade it in 3 levels. If they come back and say severe which is the worst then I’m not sure what you would do other than maybe a month of Lupron to improve the lining? And I’m super worried that my lining needs some work beforehand I transfer...but not sure what I would do to fix it if it does.
@Mel&John2013 I’m so sorry about your son:( that is heartbreaking. I’m also sorry you find yourself here but it is a good group of ladies with lots of knowledge:). I swear I’ll get a signature set up one day soon...this weekend! That is my goal. I’m at work but just wanted to pop in real fast! I’ll write more after work. The one thing I will say is that every cycle is different so it is hard to know what to change sometimes. Some ladies have very different outcomes with the same protocol different months.

@hopefuljj thank you for the welcome! Reading through the conversations, I have to agree that it's an amazing group of ladies here. I was reading through your updates and am not sure I follow everything (still learning!!), but am so impressed with how you're approaching this as an educated patient and asking for what you want. I'm a big believer in being your own advocate, and it sounds like you've got that down. Love it!
@hopefulljj I had 10 days of stims and today will be 11. I'm just concerned about my E2, since it was only 630 yesterday.

@mel&john2013 welcome to our group and I'm sorry for what you had to go through. My AMH is same as yours and my second IVF resulted in 3 follicles as well. We cancelled that cycle because our insurance doesnt cover any fertility treatment. The following IVF attempt didnt work because of high FSH and low count. I took two months break and focused on my well being, did mayan abdominal massages, some witch cures, some meditation, took additional supplements (highly recommend Ubiquional - form of CoQ10). This IVF cycle I have more follicles, but I'm still in the process and don't know the results. I just wanted wish you the best of luck! P.S: my doctor is doing the same protocol as before and said every cycle is different.
@emmasemm Thank you so much for the tips about how long it takes to get started on IVF. I definitely did *not* know that, so that is really helpful. I will for sure look into those grants you mentioned as well; I don't know those existed! @Mel&John2013 I'm so sorry to hear about your son. I'm also so sorry you find yourself here! I don't have any advice for you, unfortunately, as I still have a lot to learn about all of this, but the ladies here have been super helpful and I'm sure they will help put you on the right track. @Mel&John2013 I’m very sorry you are having difficulties and for what you have gone through so far. If you only have one shot with ivf and then you are done, I would thinking about what would give you the most peace about the cycle of it were to be disappointing in terms of numbers, quality, etc. Would you regret not having supplemented a couple of months prior? Would you regret not giving up stuff to see if that helped your eggs? Would you regret not trying acupuncture or doing a certain protocol? Results can vary from cycle to cycle, but also finding the right protocol/supplements can make a difference. I would definitely see if you could use HGH for several days during stimulation. EPP and lower dose stims worked really well for me; however, that’s not the case for some women with DOR. I also took a few months off to supplement before trying again. Good luck.

I really like what kp214 is saying above. I think it is important to think about what you would most regret not doing. I also second the HGH. It is an added cost but it seems to really add value.

@Kp214 very good points! I’ve been supplementing so I feel confident there, but you’re right about giving stuff up. I’m a coffee drinker and cut back, but just from 3 cups to 2 because the doctor said not to stress my body too much. But I’m used to it now and it’s time to cut more. And acupuncture is something I keep being curious about. It’s time to stop being curious and pull out all the stops. Thanks for giving me great points to think about!
@mel&john2013 sorry you’re here and for your loss. I cannot imagine what that is like. The other lovely ladies already gave you some great advice. When it is really your last IVF shot I suggest being super critical of those day 3 labs. If FSH is really high and AFC below your average then wait and find a better month in the next few to do it. Also some of the supplements take a few months. Additionally just remember even after that, low AMH does not mean it’s impossible to get KU naturally, so the end of IVF is not necessarily the end. Some REs think too high FSH doses in stimming can fry your eggs, so be cautious if that’s his only trick. Micro dose Lupron flare protocol seems to either be a miracle to get more good eggs or a complete disaster with horrible quality. So that’s a gamble if you never used it before. I second that the HGH isn’t supposed to do any harm, so go for it (if you’re in or close to a state that will prescribe it). And don’t be afraid to cancel a week in if things aren’t progressing as expected - most insurance doesn’t “count” the cycle until you have ER.
AFM just trying not to break my DH in organizing everything. I can see he’s stressed with all the things we have going on and needing to manage to get the overseas topic running and with some other legal issues. Last night i was really low and thinking this is never going to work and all this time, money, effort, heart and soul will be wasted. This morning I’m feeling more positive again, but it’s tough.
@jamieh2000 I’m on CD22 so I hope AF doesn’t show up yet! I’ve finally got a temp rise, but no clear sign if I really ovulated or not in the middle there. I’m feeling super emotional which I assume means my progesterone rose, which would suggest I ovulated though lol. If AF comes on time next week then I will assume I did O normally, if not then more waiting. Whichever way, as I can only see the haematologist next week then no chance for results and follow up in time to start next IVF round with next cycle. I’m hoping we can do it over Easter then.

@jaimeh2000 I had a lot to catch up on and all I have retained is your dr has the flu and can't talk to you really? Put a cough drop in your mouth and lets get on with it!! I know its just 2 days but still I would be annoyed too...I just feel like you're so close!!
@mel&john2013 I am so very sorry for your loss I can't even imagine! I'm glad you got some coverage and you get to try! You've already been given similar advice that I wouldve said from these extremely wise ladies!! Not sure if you can see my signature even though I have to update it BUT I did 8 cycles of IVF with 6 ER. And I still can't tell you the magical protocol that works. It sux that every cycle is different. I can tell you that for me it was quality over quantity! The one cycle I retrieved the most eggs, was the least that made it to blast...go figure. The antagonist protocol worked best for me som estrogen priming but not all. I did not respond well to microdose flare lupron BUT many DOR ladies have success with lurpron so its tough. I'm not a fan of acupuncture but I did go just to know I was giving it my all through the cycles.
@offtoneverland I really should be saying the same to you as I said to Mel above. I wouldve given you the same advice. There are miracles on this board where people conceive naturally all the time, so why can't the magic happen for you!! I did 7 IUIs before moving on to IVF and a part of me thinks I should've moved on to IVF sooner mostly due to my age. I would take the advice of making sure you like your RE and are comfortable with what they want to do and how to proceed. Low AMH is very overwhelming and so is this process, but this board helped me learn so much and it will for you too!
My RE was great and I was able to email her questions all the time. And I did so I felt like I wasn't waiting and ready for the next step. For example HGH many mention it but yet my clinic does not use it as its still considered experimental by them...and then tell me I can go elsewhere if I like. Its just good to keep going over your options and what will work for you!

@mel&john2013- I am so sorry to hear of the loss of your son. That is absolutely heart breaking. I can totally understand you wanting to keep building your family. DOR is a hard diagnosis, but there is hope. I agree with the others advice. Since you may only have one shot, I would make sure everything else that can be tested before any transfer is done, including a saline sono or even hysterscopy to look for polyps/fibroids, maybe advanced bloodwork for clotting, and even maybe an ERA test to make sure you are transferring at the right time. I realize these aren't without costs, but it could prevent heartbreak if you only have a few embryos to work with. While most of the time, a negative is an embryo issue, that is not always the case. If you read through some of the recent pages, you will see I had a failed DE transfer of two perfect blasts, and now I am in the process of an ERA and we look to have been transferring too late. Another option I will throw out there if this one IVF doesn't work... embryo donation/adoption can be a very economical option to have another child if you can accept the genetic loss. Not that you will get to the point, its just not a very well known option so I wanted to mention it since a lot of people are ok with adoption, but its often to heartbreaking and expensive to pursue.
@hopefuljj- I wish I could do both the ERA and EFT since they both seem to do something the other one doesn't. I am particularly interested in what my EFT would say about endo, since I had mild endo removed 1.5 years ago. I wish both published more specific research/scientific results about HOW they figured things out and how many women tend to have certain issues they find. I think you are going to be in a great spot after all this testing to have success on your transfer!!
@emmasemm- I hope all the timing works out perfectly for you! It is def hard to do naturally timed cycles with traveling. Can I ask what country you decided on? I remember looking into IB in Spain as a potential option. Hope your bloodwork comes back ASAP!
@hopeful_mom- yes I had heard from my RE via email on Friday. She said she should have time Monday, but then of course monday I couldn't get an appt until Wed. Then Tues she got the flu. So now my appt is tomorrow afternoon and I am just hoping nothing else comes up. Mel&John2013 I didnt really post about my witch cures here because mainly i started two months ago after my 3rd failed IVF attempt. there is this onion tea cure: I know it sounds gross but i promise it tastes better and sweeter than it sounds. It's supposed to cure any inflammation, fibroids, ovarian cyst, endo, polyps etc. Every morning and evening 10 mins before you eat food, you drink a cup of onion tea. Boil 2 cups of water, add chopped organic yellow onion, continue boiling for exactly 5 mins and turn it off. Steep it for 10 mins, strain and drink. Also, drinking Lady's Mantle tea is very good, but I didnt continue that mainly I was lazy. You are supposed to steep one teaspoon of herb into one cup boiled water and steep for 15 mins. Then drink it two times a day between meals. In addition to these cures, I take vitamin d, prenatal, b12+folate, b complex, magnesium supplements. Again, I havent gotten any good results yet, so I'm not the best indicator of how much these things works, but I'd do anything if it remotely helps my situation. Oh, dont forget to eat healthy, grass fed, pasture raised meats and proteins and use non-toxic cosmetics, cleaning stuff, shampoo etc.
@JamieH2000 I agree! I’m constantly scouring for published work around these procedures and larger bodies of work in general around what is found and how it is treated with success. The EFT doesn’t test for endometriosis but endometritis. I follow a couple ladies who have tested positive from a biopsy after a failed transfer or early loss. It’s caused by infection and is inflammation in the uterus. After all the retrieval’s I want to make sure I test negative for that. I still feel totally panicked about not finding a DR in Jersey to do an ERA....trying to stay zen:).

Me: Hashimoto Autoimune Disease Homozygous MTHFR Cr677 DOR - AMH .27 High FSH (don't even know how high) 2 iui's to start - BFN IVF 1 -3 made it do day 5! 2 5AA 1 6BB PGS results = all abnormal IVF 2 - stimmed for 19 days. insanity. Had 5 follicles that looked mature but only 2 contained mature eggs the others were empty. Nothing made it to day 5 IVF 3 - added in HGH - 4 mature follicles (had some empties again) 3 made it to day 5/6 for PGS (held for testing with next retrieval IVF 4 - Upped my dosage for the first time - Canceled 11 days into sims - looked like 1 follicle and we were afraid it was empty IVF 5 - Canceled - looked like 1 follicle and we were afraid it was empty IVF 6 - 3 mature, 2 fertalized 1 made it to day 5 and sent for PGS
Sent in IVF round 3 and 6 (4 embryos) for PGS testing and got 2 normal embryos!!! 1Boy and 1Girl. Took a little over a year to get those two embryos so demanded all sorts of test prior to attempting transfer. Did EFT, ERA and NK Cells testing.
FET#1 - Intralipid drips, Prednisone, and did a depot lupron shot before starting the FET protocol Chemical pregnancy FET#2 - PGS normal boy - BFN Natural pregnancy - cycle following the FET2???? Miscarriage at 8 weeks. viability scan had shown an enlarged yolk sac so I had been warned that it was likely not chromosonally normal
DEIVF cycle - picked a donor who is willing to be open! Her cycle resulted in in 4 PGS Normal embryos 1st transfer - chemical pregnancy 2nd transfer - success, May 17 2020 due date!
@hopefuljj- hmm when you say that about endometritis, it makes me wonder if I have that too! I think once you have a few rare health issues (like DOR), I am a bit of a hypochondriac and worry everything is wrong with me! Which so far, has kind of been the case. I try to trust my RE and she is great, but she was certain it was my eggs, but now low and behold we have an implantation timing issue as well. Its hard to know if its your gut being right and just your worrying side that worries everything is wrong. My appt is this afternoon, so hopefully I will have a plan in less than 8 hours!

Well i FINALLY got to talk to my RE and my ERA reco was 24 hours LESS of progesterone and a biopsy to retest (meaning they aren't 100% confident in the timing). We agreed it was best to repeat the ERA since my RE has never see anyone need that little of progesterone (but I could be a first she said- gosh I am so special in so many ways i tell you ladies!). Annoyed at a 4-5 week delay, but if it will work, I am ok with it. She said she has been having so much success with the ERA lately and lots of twins, since most have multiple failures and are usually transferring two. Once they get the right timing, they end up with twins! Anyways... I hope I am the only left on here by April... and then joining you all on the other side

I have a question about e2 levels. My levels finally went up on wednesday to 1226 and there were 10 follies. Today it dropped to 1135 but i still have 10 follies that are around 20-22mm. What does this mean? I read online and it doesnt seem like a good sign that the levels dropped spontaneously and means bad egg quality etc. Anyone has experience with this? I'm super worried now, but the Dr. didnt mention anything about this just told me to continue stims tonight and tomorrow night we'll trigger.
@ilky- I am just surprised they are 20-22 and you haven’t triggered or aren’t triggering tonight. My understanding was that trigger was a last little growth spurt. I am pretty sure my RE triggers when the first ones get to 18-20, so even 14-15mm get to mature usually by ER. I am not sure about the e2, but it didn’t decrease too much so hopefully it doesn’t really mean much. I just hope they get those eggs out soon...

@JamieH2000 yeah, that worries me as well. he is a well known doctor with high success rates, so I should trust him, but cant really fully. I've had so many health issues before and doctors generally dont really care about our well being, or whats best for us... so, i hope it wont cause overcooking my egss

@ilky- Its great 10 follicles are showing and its also possible that some RE measure differently then others and yours are maybe a tad smaller. I agree with Jamie as I would've thought you would've been triggered tonight or tomorrow! E2 is really hard. The one cycle I had 10 vs 6 another my E2 wasn't extremely different. It various for everyone. My acupuncturist used to tell me that every 200 is a mature egg BUT that really didn't hold true for me each time...I wouldn't stress over it. You're so close so lets just pray your golden eggs are in there!!

@hopeful_mom i could choose to do the EFT this cycle with my RE and then find a Dr. in jersey and schedule an ERA and then schedule an FET. I’ve never had a loss and my RE’s point is I’m already spending all this money on all these other things...I was just coming to peace with not doing it because I’m doing all the other extensive testing...I guess I should look into it again. I can’t remember, did you do one?

@ilky another gal I follow on a different feed had the same fear with the estrogen drop based on her previous result she but it happened again this cycle and she has great results! Ended up with 5 day 5 embryos to test. My E2 levels have been all over and not a great indicator for my outcome personally:-/

AFM- I met with my RE today and was hoping to get input from you very knowledgeable ladies on the things she said and recommended, since I'm very new to all of this. **TW-will mention miscarriage** First off, she said my AFC during my baseline u/s on cd4 of this cycle was only 3, not 6-8 like I had thought she said during the u/s. She told me to start taking CoQ10, which I've already been taking for a month. She also gave me a lab slip to have my vitamin D checked, as well as the full genetic testing that will include the Fragile X test.
She said since my AMH was so low (.12) and I've had 3 miscarriages since August, it's plausible that my egg quality isn't very good. On the other hand, she said they could be really good quality, but there's no way to know. She laid out all my options: keep trying on our own, IUI with clomid, or IVF with PGS. If we decide to keep trying on our own, she suggested that we put a short-ish timeline on it since my AFC was so low and it's not clear how long I have left to conceive.
I'm leaning towards trying on our own for a while longer. I know it takes CoQ10 at least 3 months to have an affect, so I think I should give it a chance. And I doubt I'll stop ovulating in the next couple of months! I know I'm capable of getting pregnant, but I'm still a bit concerned about my egg quality because of my miscarriages. But it could have also just been really rotten luck too.
I guess my questions are: 1) Is there anything wrong with me waiting and trying on my own for a while, or do you think because of my low numbers it's time to be more aggressive? I know that's a personal question and no one can tell me what to do, but I thought some advice about the meanings of these numbers could be really helpful. 2) Is an IUI even that helpful for someone with low AMH/low AFC? Since it doesn't improve egg quality, I'm not sure we should even consider that.
Now that I've written so much, I'll leave it at that. Thank you for your help as I'm truly at a loss with all of this.

@ilky sounds like your cycle is moving forwards! Getting to ER and finding out what’s going on with your eggs can be a really important step in itself to learn more about which steps issues might be at. Fx your monitoring goes well tomorrow - how big is your largest follie?

First of all I suggest you read the link I sent you in the PM over the next days.
Second there is no right or wrong way to go, just different ways.
I will be short as I need to go to sleep (but was stalking here and TTGP in case you posted an update) - personally I would go straight to IVF (it’s actually what I did and do not regret it for one moment - but like you say it’s absolutely a personal decision) - if you have enough embies to try growing to blast PGS Test the first batch, if not do a fresh transfer but grow to blast, test and freeze the rest if you are lucky enough to have some.
Of course it depends a lot on insurance coverage and finances - but if you can afford to, my advice is doing lower level interventions will not really help speed things up (or get to the root cause). And be prepared for multiple rounds - some get lucky on the first try, but some of us need a lot more rounds to find our golden egg.
Check your RE is not a “one size fits all” IVFer - we need specially tailored protocols.
With your history I would ask them again to run RPL and autoimmune testing just to be sure you don’t have a second issue (as we can’t afford to waste any normies we get).

@emmasemm Your consult sounded really great and through! I'm sorry about the positive Lupus test:-/ I called Dr. Jeffery Scher and price out his immune stuff but then because he was out of town for many weeks and only takes a small number of new patients on and cost, I decided I might not end up doing it. But I did talk to my RE about all my concerns and had my normal 3 pages of notes. hahaha.
I will give the short version of the story but basically asked for Lovenox, Prednisone, Intralipid therapy, and an ERA. She said she will treat me like i have had multiple losses and do all the testing she does for that due to the small amount of embryos to transfer after 9 months of IVF but she won't give me Lovenox unless i come back with a clotting issue. Even though as far as SOME research is concerned, being Homozygous for MTHFP that is a clotting issue. She said no Intralipids unless i get test results that she thinks warrants but if they do she is happy to do it...and Prednisone is also a no for now. I mean, come on, i have Hash!! and everything i read says that means i will very likely have implantation issues
So we did an Immune panel which was just 6 vials of blood from me and one from my husband (to see if I am at risk of attacking his DNA) and then I did a blood clot panel which was a whopping 23 vials of blood after fasting. I'm awaiting those results. Depending on what we find we will see what i do prior to or during transfer.
As for the ERA, you can't get it in NY?!??!? Anymore??? SO she said i can go to New Jersey and have a doctor do it but she recommends the Yale EFT or the Receptiva DX or RX or something like that. I am concerned with the fact I don't think i shed my lining enough every cycle so when i go in for the Yale EFT (which will act as a scratch and also test for Endometritis and other infection as well as for how receptive my lining is) she is going to do a few passes (so like a scratch and then some) to clean out my lining, so she is recommending I take the twilight anesthesia for this. So that is what I'm doing. I still feel like I'm not doing nearly enough but I"m glad I put the brakes on her from moving forward immediately with transfer.

@offtoneverland I think trying for a couple months while the suppliments work their magic sounds like a good idea. I would consider moving directly to IVF after that. I'm sorry you are here and going through this. It really sucks. DOR really sucks

@ilky- it sounds promising! Of course we would always like more, but I have definitely learned that its quality over quantity on here. While I was able to get a few more eggs than alot of ladies on this board, my quality was much worse. So don't count yourself out yet! For estrogen, the rule of thumb (which can be wrong) is ~200 per mature egg. But that is at trigger time I believe. So you still have some time and estrogen normally exponentially climbs as you get closer to trigger. This part is really stressful, but hang in there!!
@offtoneverland- I wish I had some wisdom for you, but this is all such a hard decision. My point of view is to not wait and do IVF immediately, if you can afford it. Unfortunately much of this comes down to financial decisions. It really sucks and some day when I get through this I would love to advocate to get the laws change. I am assuming you have checked if your insurance will cover anything? I would also assess how many rounds you could afford, as it will likely take a few rounds. I would reco banking embryos, as @emmasemm suggests. I wouldn't waste any $$ or time on IUI. The upside of IVF/banking embryos is if you are able to get more than 1 or 2, you can also save them for a second child, if that is something you want. You do seem to get pregnant naturally easily, but I am sure the miscarriages are absolutely devastating, so only you can know how much of that you can handle. Feel free to ask us any questions. I wish someone had a crystal ball to tell us all what to do and how it will turn out, but its really just all a gamble unfortunately
@hopefuljj- you are so sweet and you are such a trooper too! Our time just HAS to be coming right?! Your RE appt sounds like it went well and good for you for advocating for yourself! That is so interesting that NY doesn't do the ERA. I don't really get the difference between the ERA and EFT, but I think its great you are able to do the EFT at least! I think if she is going to be roughing it up in there with the biopsy, twilight anethsia is a good idea. Can she also look around in there for polyps or anything else, kind of like a hysterscopy? I am interested to hear the results of your bloodwork. I am wondering if I should request an immune/RPL panel if i am delayed another month. After all of my delays, we might as well check EVERYTHING we can at this time. Are you able to get started on your mock cycle for the EFT soon?!

@llky Good luck!! Excited and keeping FX. Hoping your RE is making the right call. For the most part we have to trust them, but we still have to be our own advocates.
@hopefuljj That's great that you're getting some extra blood work and testing done to check all the boxes. I'm sorry your RE was not on board with all the medications you were hoping might help. Hopefully everything will be all set for whenever you're ready for the transfer.

@offtoneverland good luck lady! I hope you catch that good egg! It certainly does happen for DOR ladies sometimes!! I am so sorry you would be paying out of pocket. It makes me so incredibly mad that many women (and couples) have to struggle with this process entirely out of pocket. I’m with Jamie on this one, if I ever make it to the other side I would love to find out how to become and involved in the fight to get mandated infertility coverage in more states. It is so unfair.

Also, make sure you get the full results from all that blood work - if your RE sees something in the results I hope they will change their mind about adding anything else to the protocol

I think your plan is great - but most of all it is if you and Your DH are happy with it.
Just be aware that with some clinics they have a wait list to cycle, also some want test results to be within 3 or 6 months or you have to test again before you can start. Or suddenly come up with an extra bunch of tests. Several of the IF ladies on TTGP decided to do IVF last summer but only did their first cycle in January.
So make sure you check your clinic’s policy so you don’t have extra delays if you decide to go for IVF.
Also in case it’s of interest there are quite a lot of grants out there to apply for to help financially. You could use the time applying. https://www.fertilitywithinreach.org/financial-assistance-grants/
But of course best would be you just get KU naturally during the next months and your RE has some good tricks up his sleeve to avoid your history repeating. Fx!

I'm 37 years old, married almost 9 years, and mom to 2 beautiful boys. Our first son passed away when he was 8 months old due to liver failure (born without enough bile ducts, we still don't know why). Our second son will be 3 in a couple of months. No trouble conceiving. We've been trying for #3 for about 2 years. Had one missed miscarriage after trying Clomid with my OB/GYN last year, D&C December 2016.

Last year we switched to a RE, AMH=0.6, did a few medicated cycles and IUI and, surprising no one, no success. Thought we were done because I wasn't sure I could handle the emotional toll of spending so much and maybe ending up with nothing. There's only so much in a lifetime I can take.
Ended up with surprise IVF coverage in 2018 of one round, so we're on to that. AMH a year later lower at 0.5. Our first attempt last month was 16 days of priming with estrogen and progesterone, 3 days of Cetrotide, then antagonist protocol, 350 follistim and 300 menopur. Cancelled, only 3 good looking follicles and 2 little ones, with estrogen at 750.
We have it in us (and our wallets/insurance) for one more try. So this one is it, cancelled or not. Just had bloodwork today and should hear back later today about priming and new protocol. I know our RE plans to add Lupron but haven't heard past that.
I am scheduling a call with the RE to talk through approach, and would love thoughts from you ladies on questions to ask if you're willing to offer assistance! Basically, I want to make sure we're going in as aggressively as possible for this last try. I know most women with DOR have to go through multiple cycles to get to one beautiful egg, but I just can't. I want to at least make sure we're giving our one shot all we've got, but I don't know what more to ask for. Stronger meds? Different meds? If you have any advice, or protocols that have worked for you that I should ask about, it would be very much appreciated!

I mean....I dunno...I would rather an ERA but glad I’m doing something! And I’m super worried that my lining needs some work beforehand I transfer...but not sure what I would do to fix it if it does.
@Mel&John2013 I’m so sorry about your son:( that is heartbreaking. I’m also sorry you find yourself here but it is a good group of ladies with lots of knowledge:). @mel&john2013 welcome to our group and I'm sorry for what you had to go through. My AMH is same as yours and my second IVF resulted in 3 follicles as well. We cancelled that cycle because our insurance doesnt cover any fertility treatment. The following IVF attempt didnt work because of high FSH and low count. I took two months break and focused on my well being, did mayan abdominal massages, some witch cures, some meditation, took additional supplements (highly recommend Ubiquional - form of CoQ10). This IVF cycle I have more follicles, but I'm still in the process and don't know the results. I just wanted wish you the best of luck! P.S: my doctor is doing the same protocol as before and said every cycle is different.

@Kp214 very good points! I’ve been supplementing so I feel confident there, but you’re right about giving stuff up. I’m a coffee drinker and cut back, but just from 3 cups to 2 because the doctor said not to stress my body too much. But I’m used to it now and it’s time to cut more. And acupuncture is something I keep being curious about. It’s time to stop being curious and pull out all the stops. Thanks for giving me great points to think about!
The other lovely ladies already gave you some great advice.
When it is really your last IVF shot I suggest being super critical of those day 3 labs. If FSH is really high and AFC below your average then wait and find a better month in the next few to do it. Also some of the supplements take a few months.
Additionally just remember even after that, low AMH does not mean it’s impossible to get KU naturally, so the end of IVF is not necessarily the end.
Some REs think too high FSH doses in stimming can fry your eggs, so be cautious if that’s his only trick. Micro dose Lupron flare protocol seems to either be a miracle to get more good eggs or a complete disaster with horrible quality. So that’s a gamble if you never used it before.
I second that the HGH isn’t supposed to do any harm, so go for it (if you’re in or close to a state that will prescribe it).
And don’t be afraid to cancel a week in if things aren’t progressing as expected - most insurance doesn’t “count” the cycle until you have ER.
AFM just trying not to break my DH in organizing everything. I can see he’s stressed with all the things we have going on and needing to manage to get the overseas topic running and with some other legal issues. Last night i was really low and thinking this is never going to work and all this time, money, effort, heart and soul will be wasted. This morning I’m feeling more positive again, but it’s tough.
@jamieh2000 I’m on CD22 so I hope AF doesn’t show up yet! I’ve finally got a temp rise, but no clear sign if I really ovulated or not in the middle there. I’m feeling super emotional which I assume means my progesterone rose, which would suggest I ovulated though lol. If AF comes on time next week then I will assume I did O normally, if not then more waiting. Whichever way, as I can only see the haematologist next week then no chance for results and follow up in time to start next IVF round with next cycle. I’m hoping we can do it over Easter then.

@mel&john2013 I am so very sorry for your loss I can't even imagine! I'm glad you got some coverage and you get to try! You've already been given similar advice that I wouldve said from these extremely wise ladies!! Not sure if you can see my signature even though I have to update it BUT I did 8 cycles of IVF with 6 ER. And I still can't tell you the magical protocol that works. It sux that every cycle is different. I can tell you that for me it was quality over quantity! The one cycle I retrieved the most eggs, was the least that made it to blast...go figure. The antagonist protocol worked best for me som estrogen priming but not all. I did not respond well to microdose flare lupron BUT many DOR ladies have success with lurpron so its tough. I'm not a fan of acupuncture but I did go just to know I was giving it my all through the cycles.
@offtoneverland I really should be saying the same to you as I said to Mel above. I wouldve given you the same advice. There are miracles on this board where people conceive naturally all the time, so why can't the magic happen for you!! I did 7 IUIs before moving on to IVF and a part of me thinks I should've moved on to IVF sooner mostly due to my age. I would take the advice of making sure you like your RE and are comfortable with what they want to do and how to proceed. Low AMH is very overwhelming and so is this process, but this board helped me learn so much and it will for you too!
My RE was great and I was able to email her questions all the time. And I did so I felt like I wasn't waiting and ready for the next step. For example HGH many mention it but yet my clinic does not use it as its still considered experimental by them...and then tell me I can go elsewhere if I like. Its just good to keep going over your options and what will work for you!

@hopefuljj- I wish I could do both the ERA and EFT since they both seem to do something the other one doesn't. I am particularly interested in what my EFT would say about endo, since I had mild endo removed 1.5 years ago. I wish both published more specific research/scientific results about HOW they figured things out and how many women tend to have certain issues they find. I think you are going to be in a great spot after all this testing to have success on your transfer!!
@emmasemm- I hope all the timing works out perfectly for you! It is def hard to do naturally timed cycles with traveling. Can I ask what country you decided on? I remember looking into IB in Spain as a potential option. Hope your bloodwork comes back ASAP!
@hopeful_mom- yes I had heard from my RE via email on Friday. She said she should have time Monday, but then of course monday I couldn't get an appt until Wed. Then Tues she got the flu. IVF 1 -3 made it do day 5! 2 5AA 1 6BB PGS results = all abnormal
IVF 2 - stimmed for 19 days. insanity. Had 5 follicles that looked mature but only 2 contained mature eggs the others were empty. Nothing made it to day 5
IVF 3 - added in HGH - 4 mature follicles (had some empties again) 3 made it to day 5/6 for PGS (held for testing with next retrieval
IVF 4 - Upped my dosage for the first time - Canceled 11 days into sims - looked like 1 follicle and we were afraid it was empty
IVF 5 - Canceled - looked like 1 follicle and we were afraid it was empty
IVF 6 - 3 mature, 2 fertalized 1 made it to day 5 and sent for PGS
Sent in IVF round 3 and 6 (4 embryos) for PGS testing and got 2 normal embryos!!! 1Boy and 1Girl. Took a little over a year to get those two embryos so demanded all sorts of test prior to attempting transfer. Did EFT, ERA and NK Cells testing.
FET#1 - Intralipid drips, Prednisone, and did a depot lupron shot before starting the FET protocol

FET#2 - PGS normal boy - BFN
Natural pregnancy - cycle following the FET2???? Miscarriage at 8 weeks. viability scan had shown an enlarged yolk sac so I had been warned that it was likely not chromosonally normal
DEIVF cycle - picked a donor who is willing to be open! Her cycle resulted in in 4 PGS Normal embryos
1st transfer - chemical pregnancy
2nd transfer - success, May 17 2020 due date!

I have a question about e2 levels. My levels finally went up on wednesday to 1226 and there were 10 follies. Today it dropped to 1135 but i still have 10 follies that are around 20-22mm. What does this mean? I read online and it doesnt seem like a good sign that the levels dropped spontaneously and means bad egg quality etc. Anyone has experience with this? I'm super worried now, but the Dr. didnt mention anything about this just told me to continue stims tonight and tomorrow night we'll trigger.

@jamieh2000 lady you are very special indeed!! I hate you another era! Can this one be on them I"m excited to hear twins thats a party!! I get how frustrating it is to wait! Truly hoping this will be eat for you!! Glad you have a plan but annoying nontheless.
@ilky- Its great 10 follicles are showing and its also possible that some RE measure differently then others and yours are maybe a tad smaller. I agree with Jamie as I would've thought you would've been triggered tonight or tomorrow! E2 is really hard. The one cycle I had 10 vs 6 another my E2 wasn't extremely different. It various for everyone. My acupuncturist used to tell me that every 200 is a mature egg BUT that really didn't hold true for me each time...I wouldn't stress over it. You're so close so lets just pray your golden eggs are in there!!

@JamieH2000 I know how frustrating this is for you, I feel like this really might be the missing puzzle piece for you though so I’m so hoping the added wait is going to lead you finally to your baby!!!!! My heart totally goes out to you. I feel so crazy myself after all this time and never attempting a single transfer, it’s like at every turn I couldn’t take the next step and you have been through so much more! Ugh, I’m sorry. Thinking about you and hoping for the absolute best!
@ilky another gal I follow on a different feed had the same fear with the estrogen drop based on her previous result she but it happened again this cycle and she has great results! Ended up with 5 day 5 embryos to test. My E2 levels have been all over and not a great indicator for my outcome personally:-/
