Sorry I've been MIA all day. My grandmother is in the hospital, so I spent the day with her and warned my family that I wouldn't be able to get back for a visit for a few weeks (because ER and transfer, which they don't know about). Trying really hard not to be stressed for the next couple of weeks.

I mentioned I would post my list of questions...I feel like most of them pertain to our situation in particular, but here it is anyways:

There is a second page with 2 more questions, 1 about having a cervical polyp removed and another about how he would like to be contacted with questions following the appt. I also made a list of all the meds I currently take or have taken in previous cycles, but figured that wasn’t necessary to post here. Please excuse the sloppy handwriting, I had already re-written it once! I’m also going to take copies of all my charts since we started trying, just in case he wants to see them. I’m nervous & excited about this...but mostly excited! I’ll post an update this evening.

@bluetickgal sorry, I can't remember but do you already take progesterone? I just didn't see it on the questions but that might be something to ask about. Supplementing with it and getting yours checked during the cycle.

@mokay19 I do not. I typically have 14-16 day LP, so I figured progesterone wasn’t an issue. I’ve had it checked on CD21 every cycle, and it is always above 15 (even when it was less than 7DPO). *TW - during both pregnancies/losses, it was above 20 but then dropped quickly along with HCG - End TW* I’ll add it to the list, I’m definitely open to trying whatever could possibly help! Thanks!

@bluetickgal and a lot of doctors will say that adding progesterone hasn't fully been proven to help... but it also doesn't hurt. So my mindset is "why the hell not?!" haha

AFM - I just had my appointment to check my follies! He said I am responding really well to the letrozole! I feel like everything happened so quick this morning and I have such a strong feeling that I can only describe as being "overwhelmed"!! I have 4 follies (Is that a normal number?) - they are measuring at 13, 14, 11 and 9 mm - and he wants them to be at least 20mm. So he said they need to cook for about 2 more days. He also said my uterus lining needs to be at least 7mm but I don't remember what he said it was measuring it.

I am to go back Saturday morning and he's guessing I'll be able to do the trigger shot Saturday night and come in Monday morning (before my flight to Atlanta) for the IUI!! He knows that I'm away Monday afternoon - Wednesday afternoon but he kept saying he thinks the timing is going to work out perfectly.

Gosh, I am so nervous and I have so many thoughts and feelings... i actually cried on my way to work (and I'm not even really sure why!) They weren't sad tears, I think maybe it was just a release and being excited/nervous for Monday morning and hoping the timing really does work out. I'm also really scared of how I'll react if this first cycle doesn't work. I know I'll have 3 more cycles after that but the thought of it not working has finally crept into my mind and I can't seem to push it away yet.

Anyway, here's hoping everything looks great and ready on Saturday!

@kpearse99 everyone responds differently to letrozole. I only ever got 1 good follicle but I was only on 5mg I think. I totally understand you crying on your way to work! I definitely did that a few times after getting good news at my appointments. I don't blame you for feeling emotional at the idea that the IUI is going to be timed well with your travel!

Quick question about the trigger shot - I know when you go to inject it you pinch the skin below the belly button line - BUT - do you release the pinch after the needle is in and before you push the meds in? Does that make sense?

@kpearse99 So glad it sounds like the timing is going to work out and you have a few follies! So exciting! Someone else who has done the trigger shot may be a better source of info.. but generally with a subq injection you would pinch and keep it pinched until the injection is done.

Another thing I thought of was to ask if there was anything in particular he suggests we avoid (foods/exercise) or add in (supplements/foods) that could be beneficial? Oh, thanks for your list @jennifer_louise I need to ask about HSG too! It was mentioned on the initial paperwork, so I think it’ll happen...but I definitely want to ask if it isn’t brought up!

@inthewoods23 our shower doesn't have a door (I feel like the previous owners were going for a european-style open concept bathroom but it's still weird it only has 2 walls plus a glass half wall, so one entire side is open). There's lots of room to be creative lol, however we decided not to risk it. MH still somehow hurt his hip in bed lying at a funny angle. Getting old sucks.
@mokay19 my legs are still too short to comfortably be on our hands and knees. It takes some creative maneuvering but it can be done

thankfully we didn't have to resort to that, however unfortunately MH had it all in his head that we had to get in one more try and then of course it was a NF. Poor guy, performance anxiety is a terribly thing y'all. Also, yay for moving on to your next cycle!!

@kpearse99 that's great news about your appt and follies! Your doctor sounds like a character. 4 follies seems right. Mine were around 20mm, 21mm, 24mm and 25mm by the time I triggered. I don't know how many actually released but my RE was thrilled (the more the merrier, he says. Besides, we can't seem to be able to fertilize even ONE egg so the chances of more than one actually taking is pretty low). You totally hit the nail on the head when you said it was just a cathartic kind of 'release' cry. As soon as my RE left the room after my IUI I burst into tears. MH kept asking me what was wrong but I think I was just so overwhelmed with the whole situation all I could do was cry.

*hugs* it sounds like we're in the same boat emotionally, excited but scared to be hopeful because a BFN is going to be devastating.

Update - turns out I have 3 follicles: 2 on the right side that are measuring at 17mm and 1 on the left side at 13mm. And my uterus lining is at 5.5mm! So that's better than what I thought I heard him tell me when I was there (I told you it all happened so fast!)

@kpearse99 I had no clue what I was doing for the SC injection so I pinched then let go so I could hold the syringe with both hands cuz I was nervous. Didn't seem to make any difference as far as I could tell, didn't hurt either way. I iced the area for a bit because it was sore, then again the next day. It's a little tender (like a bruise) but no redness or swelling.

@bluetickgal great questions! I hope you leave your appointment feeling good about your RE and whatever plan you decide on.

AFM, our appointment went exactly how we expected. DH isn't comfortable with IUI just yet, which I understand. *TW* He's encouraged by the "ttc math" and the fact that I did get KU twice within 1 year, even though both ended in loss. It seems like the issue isn't getting pregnant, but staying pregnant, so IUI wouldn't make a huge difference. *End TW* The RPL testing doesn't show any reasons for our losses, which we already knew from checking the patient portal. We decided we're going to do TI with progesterone post-O this cycle, then probably IUI in October. We have to do 2 cycles of IUI before insurance will cover IVF. Our RE already put in the insurance request and the nurse reviewed it with us, so if we decide to do IUI next cycle instead of waiting for October, we can. It's going to be really tricky trying to time everything, since DH and I are both teachers so taking time off to do IUI is complicated. We'll make it work, though.

@kpearse99 My trigger was intramuscular, so that's not helpful to you, but I do my Lupron injection subcutaneously. Pinch the skin and when you're done, count to three after you've finished depressing the syringe to make sure all the drips are out. Honestly, the needle slides right in and you don't even feel it because of the pinch. I pinch pretty hard (I'd rather feel that than a needle) and push the needle in pretty slowly, and I swear I don't really feel anything at all. You can do it!!

@emeraldduchess I agree with you guys about waiting on the IUI. I think you have a good plan in place, and I hope you feel better and more hopeful again about it all! That's awesome that your insurance will cover the IVF if you need to go down that path. My insurance covers the IUIs but won't cover any IVF (not that we were even considering that path). You guys will figure the timing out!

We just got back from the RE appt, and we were both super happy with how it went! The doctor is straightforward and seems super knowledgeable & confident...no warm fuzzies from him, but that’s totally ok. *TW - Since we have gotten pregnant twice fairly quickly (within 3-4 cycles each time), he is keeping meds (Metformin & Letrozole) the same next cycle. Because both those pregnancies ended in losses, we are really looking at RPL testing and figuring out what could be the cause. End TW* I had a lot of blood work done today for RPL...if all of it comes back normal, we will do karyotyping next for both of us (insurance covers it partially, but we want to rule out other stuff first). When I get my cycle, I’ll call to schedule SHG for a week later. We will continue with TI only but add in progesterone after I ovulate (he is a huge proponent of using progesterone). If we don’t get pregnant in 6 months, we may look at trigger and possibly IUI. He didn’t see a point in doing the SA at this point, but would suggest it after 6 months without getting pregnant again. DH is of course relieved! Overall we feel good about the course of action, and hopefully we will have some answers once our testing comes back.

I'm also a big fan of starting with a plan and building on it, so if you know letrozole + metformin works for you, hopefully adding in the progesterone will do the trick before delving into the more serious stuff. I hope the RPL testing gives you some more answers too.

Thanks for the good thoughts, ladies. 💚 I left the appointment feeling a little uncertain about not going right for IUI since it's been a year of ttc for us. But now that we think our issue is progesterone, that might be all we need. Plus, DH isn't quite ready to move to IUI yet and I need to respect that.

My RE said that the blood test numbers for progesterone aren’t totally accurate, so I guess it is possible that has been an issue for me all along. I’m definitely up for trying if it could help! I understand being a little uncertain about not moving on to the next step just yet, but that’s good you have a plan for when to move forward...hope it’s not needed though! ☺️

AFM my meds came this week. And had my baseline appt this morning. And it was mostly good. My lining is thin and I have about 15 small follies on each ovary. But I have a cyst, it was there during my last scan too. Doesn't look like it got any bigger. And RE thinks we should be fine to ignore it and continue with the plan. But we will see what my bloodwork today shows. I guess if the cyst is producing estrogen or something like that then we may have to cancel. So I am hoping so hard that my bloodwork is good and we do not have to cancel. They said no news is good news. So Fx they don't call! Nurse went over injections with me so hopefully I am prepared for tomorrow to start Stims!
