How are things going?: they're moving along this cycle. Obviously, I'm still unhappy about DH's repeat SA results, but it is what it is. He feels really bad because he knows that I always beat myself up for being the problem and now he could be a part of the problem and he feels guilty almost. Since we had already planned on moving onto IVF, I wonder if they would even require further testing and such... (see next line)
Any questions?: If DH had a high count but low morph, do you suspect any further testing before IVF? I would like to start IVF in October so I just want to make sure we get the testing taken care of in time for that.
GTKY: What super power would you love to have and why? I agree with the majority and I would do teleportation. It would allow SO much more available time to relax. You don't have to leave 30+ minutes early to go somewhere. Or you don't have to drive 9 hours to see family. You just show up!
@jennifer_louise I LOVE new girl!! I could re-watch that show over and over and never get sick of it! I have a very hard time figuring out which of the guys is my favorite though!

@mokay19 The Acrosome defect was picked up in the SA that they did through the clinic. The one they do there is quite a lot more detailed than the regular SA. Due to the low morph we were recommended to go ahead with IVF w/ICSI. Which I suspect will be the recommendation for you as well. Unfortunately for us, the RE feels that the sperm is just too badly damaged and even though we got 10 eggs fertilized, the sperm just didn't do their part. There was no further testing to be done though, or at least not in our case there wasn't. If you have questions, let me know.

@mokay19 I'm sorry YH is feeling guilty but I can totally see why he would have a hard time. I think as long as you're doing IVF with ICSI, you should be able to start when you want and October seems like plenty of time if YH needs another test or two. Your the one that will need to start meds. Frankly, he's not even needed until the retrieval and if they do ICSI, they'll pick the good sperm and won't leave anything to chance.

My principal knows about my first loss, so I think he guesses something is going on, but I will only share what I need to. I trust him, but there are some things I want to keep private.

@NYTino24 lots of hugs to you. I am 37.5 and I worry constantly about my egg quality. Since both my losses were too early, we've had nothing to analyze. My RE says it's either the embryos we created we abnormal or my body couldn't support them (or both). It blows that "peak" fertility is in our 20s but not too many of us are in a position to have children at that point. I know I wasn't.
@jennifer_louise I am definitely sweet...my messing with people would be some light trolling. So excited for your RE consult this week! Be prepared they might do some bloodwork. Hydrate, hydrate, hydrate!

@nitnat007 did you do ICSI? And did your RE think the sperm didn’t do it’s part because of the acrosome or because of the low morph? Fortunately, our clinic has ICSI included in the price so we won’t have to worry about specifying. I had originally told the doctor that I don’t want to risk too many eggs since I have PCOS but now that we also have MFI stuff going on, I want to be like PUMP ME FULL OF DRUGS!
@kpearse99 It’s taking a while to get in because the consult couldn’t be put in until I had all my bloodwork done. They are currently booking about 2-3 months out, but I’m still waiting for an appointment date. I’m glad you were able to get in for the testing to confirm if you did O. Hopefully you did and your timing was just right!
@splashmountain Glad you have the beginnings of a plan in place. Must be a relief for you. What kind of MFI are you dealing with (if it’s ok to ask)?

I’ve been reading a lot of mixed things about how significant a low morph is. Was IVF with ICSI recommended based on morph alone or also because of the acrosome defect? I guess I’m just wondering what I should expect going in to my first RE appointment. My H’s morph was at 6%, which I know isn’t considered that low, but he also has low count and motility.

What are you doing this cycle? (Testing? Treatment?): All of our RPL testing so far has come back negative/normal. Waiting on karyotyping (we just went and had that done this weekend, so it’ll be a few weeks) and will schedule SHG when cycle starts.
How are things going?: Pretty good, glad everything so far has come back normal...just anxious to get the rest of the testing done. So, I got good news (I think) from the RE’s office! I called to get my Provera prescription to jump start my cycle and thought I’d double check about having to sit out the SHG cycle (*TW due to loss - End TW*)...this nurse said, no we can try during the SHG cycle and went ahead and called in Letrozole for me to start on CD3 of my next cycle. I’m super excited, but also confused by the conflicting info from the 2 different nurses. I may triple check when I call to schedule my SHG...but I guess worst case is that he tells me not to try after my SHG if something shows up or if he had intended me to benched that cycle anyways. At least I’d have a normal cycle with being medicated and wouldn’t need Provera again.

We can only be selfless for a certain period of time and then need to do what’s best for us. How’d your conversation with YH go about moving to IUI next cycle? I tried to have the same conversation with MH last night and he basically shut me down.

@shoogapoff I’m sorry for the BFN and that you’re struggling. TOTALLY there with you, IF is the pits. I’m excited about your plan for the next cycle! If it helps, I know several people who have been successful with IUIs. FX it works for you! I wouldn’t say you HAVE to take off work the day of the IUI, but if you can swing at least a half day (if not a full day) to do something nice/relaxing for yourself (like coffee!), I totally would!

Phew, I think I got everyone! My monitoring appt went well yesterday. I had a 19.4(I think) mm follie on my left side again as well as a 14mm one. I was able to get some questions answered as well, including supplements I can safely add with letrozole. I told her I just want to feel like I’m doing everything I can to help since I’m not doing anything else (OPKs, BBT, was too scared to do any supplements). I really like my RE- I’m pretty sure they were super busy, but she still sat there and let me pepper her with questions haha. I’ll be doing the trigger shot solo tonight (CD 11) and I feel better about the timing of this one as opposed to last month (day 12). Soooo we’ll see!

@mokay19 Yes, we did do ICSI as well, which we were told is automatic for low morph. Our RE felt that the poor sperm quality was more due to the acrosome defect, and not necessarily due to the morph. Because of that we will likely have to go with a donor. I do not suspect any improvement on that front. And yes, I will be asking to get pumped full of all the drugs this time too! Given that I responded well on round one, I don't see there being an issue with that. Hopefully they will allow you to do that given your PCOS. Close monitoring, no fresh transfer and just taking it easy. Worth it!
@misshelen86 ICSI is our clinic' standard recommendation with low morph. The acrosome defect caused issues outside of that unfortunately. 6% is definitely not that low. National Health Standards is set at 4%, or at least that is the range that is used here. Not sure how that would change with low count etc. You will have to let us know how it goes.
@bluetickgal That is great that you are able to start during your SHG cycle. Hopefully you get some magic there.

I just got the news today that our next appointment is Oct 2nd (a little later than I wanted since I was originally told he was booking in Sept a week ago, but the receptionist dropped the ball on that one). So trying to get as much stuff out of the way as possible now. Pap, HSG, bloodwork etc. So we can dive right back in. Really good update from your monitoring appointment. You are my hero doing your own shots though! Pull that trigger!!

@misshelen86 4% was the bottom of the range. I believe it was between 15 and 4% was considered 'normal'. But yes, everyone uses a different scale. MH is navy and when we tried to talk to the base docs about it they wafted and used something completely different too. Our last test was 2%, so really low. google is not always helpful.
Soooo bloodwork came back and progesterone is still at 0.3... so I didn’t O. Even after 2 sets of LH surges on OPKs. WTF??!! I’m upset and angry and beyond frustrated. I don’t get it. All of my tests the last few months came back normal and great and nothing indicated I wasn’t Oing until now. I guess we will just continue with EOD or every other 2 days of HIO.
Would you keep testing with the OPKs? Or just go with the flow the rest of “cycle”?
What if I’m not Oing... what do they do for that? I mean, we are upping my letrozole dose next cycle but is that enough? Along with the trigger?

@kpearse99 I’m so sorry your progesterone came back low again!! I’d continue with the EOD just in case! I would thinking upping your Letrozole dose should do the trick, especially with the trigger if the Letrozole can get your follies to the right size. @kpearse99 ugh I totally thought you ovulated. That must be so frustrating. I’m sorry if you’ve said this before but do you have PCOS? I do and I’ve had issues ovulating on my own that clomid/Letrozole plus trigger seemed to help with. So maybe upping your dose will help. Sending you big hugs.

@kpearse99 oh my goodness... I'm so sorry about the continued confusion! So, on my cycle that I didn't respond to the Letrozole, they did bloodwork to check if I was at baseline--even though I was timing-wise in the middle of the cycle--and an ultrasound to check lining and follicles. Rather than needing to force a period, they determined that my body was basically still at the start and they put me on a higher dose of Letrozole right away. That might be an option for you if they just checked your bloodwork rather than needed to wait and wait and still possibly force a period. Since you had follicles start to develop and then go away, I would think your body is basically back to baseline. I'd continue with the OPKs because I would be interested if the false surges consistently happen or if it was just these times.
@kpearse99 Oh no, I'm so sorry. I would definitely just go with the flow at this point. You've got to be so burned out. Did your doctor say whether they would up your Letrozole?
@b_1029 I have betas twice next week and they'll only call me after the second test. I happen to have a big presentation after my second beta, so I think I'm going ask that they not call me until later in the afternoon. So I may just wait it out.
@ruby696 yes, I only did 2.5 this cycle and he's going to up it to 7.5 next time around! So hopefully that will help. I am feeling burned out now, I started off so positive too! I think we'll keep up with EOD(ish) but from a more relaxed approach then.
@mokay19 you sharing your experience is so helpful! I had 4 rounds of bloodwork and 4 ultrasounds at the monitoring appointments, so they saw all the numbers and stuff like that - including lining thickness and follie size and count. They never said anything about my cycle essentially starting over again though. At this point he wants me to just wait for AF or a BFP (good luck with that happening if I'm not Oing!) and then we start with the increased dose of letrozole. He said he's hopeful and optimistic that increasing to 7.5 will do the trick, that in addition to the trigger which will force my body to O so we can do the IUI as well as some TI.
@hanshotfirst77 I haven't been diagnosed with PCOS, even years ago when my periods were so very random and never consistent. And now seeing the RE, he's never mentioned it because all of my tests came back normal, so I don't think I have it - I just don't know why my cycles are being wonky all of a sudden.

No, we just used this same protocol and it worked much faster last time. It seems that my body is reacting a little differently and his SA is obviously a new factor, too.
@bluetickgal so glad your testing has come back normal so far. While it's tough not to have an answer, it's a relief to know that you don't need any additional procedures.
@splashmountain it boggles my mind that only 16 states have laws requiring at least some form if IF coverage. Completely unacceptable. I live in NA where we do have the law, but the amount of coverage varies from plan to plan.

Any questions?: If DH had a high count but low morph, do you suspect any further testing before IVF? I would like to start IVF in October so I just want to make sure we get the testing taken care of in time for that.

@mokay19 I'm sorry YH is feeling guilty but I can totally see why he would have a hard time. I think as long as you're doing IVF with ICSI, you should be able to start when you want and October seems like plenty of time if YH needs another test or two. Your the one that will need to start meds. Frankly, he's not even needed until the retrieval and if they do ICSI, they'll pick the good sperm and won't leave anything to chance.

@splashmountain @shoogapoff I can't really say too much about IF and IF treatment at work. It's a tiny school community and information has a way of spreading to everyone whether we want it to or not. My principal knows about my first loss, so I think he guesses something is going on, but I will only share what I need to. I trust him, but there are some things I want to keep private.
@mokay19 I'm sorry about the not-so-good results from your DH's SA. Hopefully if there's more testing to be done you can take care of it before October!

@NYTino24 lots of hugs to you. I am 37.5 and I worry constantly about my egg quality. Since both my losses were too early, we've had nothing to analyze. My RE says it's either the embryos we created we abnormal or my body couldn't support them (or both). It blows that "peak" fertility is in our 20s but not too many of us are in a position to have children at that point. I know I wasn't.

I’ve been reading a lot of mixed things about how significant a low morph is. Was IVF with ICSI recommended based on morph alone or also because of the acrosome defect? I guess I’m just wondering what I should expect going in to my first RE appointment. My H’s morph was at 6%, which I know isn’t considered that low, but he also has low count and motility.

What are you doing this cycle? (Testing? Treatment?): All of our RPL testing so far has come back negative/normal. Waiting on karyotyping (we just went and had that done this weekend, so it’ll be a few weeks) and will schedule SHG when cycle starts.
How are things going?: Pretty good, glad everything so far has come back normal...just anxious to get the rest of the testing done. So, I got good news (I think) from the RE’s office! I called to get my Provera prescription to jump start my cycle and thought I’d double check about having to sit out the SHG cycle (*TW due to loss - End TW*)...this nurse said, no we can try during the SHG cycle and went ahead and called in Letrozole for me to start on CD3 of my next cycle. I’m super excited, but also confused by the conflicting info from the 2 different nurses. I may triple check when I call to schedule my SHG...but I guess worst case is that he tells me not to try after my SHG if something shows up or if he had intended me to benched that cycle anyways. At least I’d have a normal cycle with being medicated and wouldn’t need Provera again.

GTKY: What super power would you love to have and why? I’m gonna jump on the bandwagon and say teleportation too! It would be pretty awesome to never have to drive/fly again...I feel like so much time would be saved!

@emeraldduchess it definitely could be the end of summer blues! I totally agree- you need to put you and your family first. We can only be selfless for a certain period of time and then need to do what’s best for us. How’d your conversation with YH go about moving to IUI next cycle? I tried to have the same conversation with MH last night and he basically shut me down.
@misshelen86 that’s a great strategy! I’m sorry for AF and officially hitting the year mark. I’ve found that waiting is the worst part of IF (ya know, besides IF) and I’m sorry you don’t have an appointment date yet. I’m so glad you’ve found a counselor who’s helping you cope with everything!
@kpearse99 I’m sorry this cycle is confusing but that’s awesome you’re learning more about your body! Did the doctor confirm O today? That would totally make sense about the longer cycles if you are having a 2nd surge.

@shoogapoff I’m sorry for the BFN and that you’re struggling. TOTALLY there with you, IF is the pits. I’m excited about your plan for the next cycle! If it helps, I know several people who have been successful with IUIs. FX it works for you! I wouldn’t say you HAVE to take off work the day of the IUI, but if you can swing at least a half day (if not a full day) to do something nice/relaxing for yourself (like coffee!), I totally would!

@mokay19 ugh sorry YH is taking the SA so hard. At the end of the day, you’re both doing all you can do to help. I wish I could help answer your Q but I’m not sure what the process is.
Phew, I think I got everyone! My monitoring appt went well yesterday. I had a 19.4(I think) mm follie on my left side again as well as a 14mm one. I was able to get some questions answered as well, including supplements I can safely add with letrozole. I told her I just want to feel like I’m doing everything I can to help since I’m not doing anything else (OPKs, BBT, was too scared to do any supplements). I really like my RE- I’m pretty sure they were super busy, but she still sat there and let me pepper her with questions haha. I’ll be doing the trigger shot solo tonight (CD 11) and I feel better about the timing of this one as opposed to last month (day 12). Soooo we’ll see!
@mokay19 Yes, we did do ICSI as well, which we were told is automatic for low morph. Our RE felt that the poor sperm quality was more due to the acrosome defect, and not necessarily due to the morph. Because of that we will likely have to go with a donor. I do not suspect any improvement on that front. And yes, I will be asking to get pumped full of all the drugs this time too! Given that I responded well on round one, I don't see there being an issue with that. Hopefully they will allow you to do that given your PCOS. Close monitoring, no fresh transfer and just taking it easy. Worth it!

@misshelen86 ICSI is our clinic' standard recommendation with low morph. The acrosome defect caused issues outside of that unfortunately. 6% is definitely not that low. National Health Standards is set at 4%, or at least that is the range that is used here. Not sure how that would change with low count etc. You will have to let us know how it goes.
@bluetickgal That is great that you are able to start during your SHG cycle. Hopefully you get some magic there.
@b_1029 Thanks friend! I just got the news today that our next appointment is Oct 2nd (a little later than I wanted since I was originally told he was booking in Sept a week ago, but the receptionist dropped the ball on that one). So trying to get as much stuff out of the way as possible now. Pap, HSG, bloodwork etc. So we can dive right back in.

@kpearse99 I’m so sorry your progesterone came back low again!! I’d continue with the EOD just in case! I would thinking upping your Letrozole dose should do the trick, especially with the trigger if the Letrozole can get your follies to the right size. I love the New Girl gif...it’s one of our fav shows!

@kpearse99 ugh I totally thought you ovulated. That must be so frustrating. I’m sorry if you’ve said this before but do you have PCOS? I do and I’ve had issues ovulating on my own that clomid/Letrozole plus trigger seemed to help with. So maybe upping your dose will help. Sending you big hugs.
@NYTino24 I’m so sorry you’re going through this again. I can imagine it must be so frustrating to have nothing to test from them. Egg quality is a concern of mine too after *TW* 2 miscarriages. Fx for the rest of your testing. Hugs ❤️

Since you had follicles start to develop and then go away, I would think your body is basically back to baseline. I'd continue with the OPKs because I would be interested if the false surges consistently happen or if it was just these times.
@b_1029 I have betas twice next week and they'll only call me after the second test. I happen to have a big presentation after my second beta, so I think I'm going ask that they not call me until later in the afternoon. So I may just wait it out.
@mokay19 you sharing your experience is so helpful! I had 4 rounds of bloodwork and 4 ultrasounds at the monitoring appointments, so they saw all the numbers and stuff like that - including lining thickness and follie size and count. They never said anything about my cycle essentially starting over again though. At this point he wants me to just wait for AF or a BFP (good luck with that happening if I'm not Oing!) and then we start with the increased dose of letrozole. He said he's hopeful and optimistic that increasing to 7.5 will do the trick, that in addition to the trigger which will force my body to O so we can do the IUI as well as some TI.
@hanshotfirst77 I haven't been diagnosed with PCOS, even years ago when my periods were so very random and never consistent. And now seeing the RE, he's never mentioned it because all of my tests came back normal, so I don't think I have it - I just don't know why my cycles are being wonky all of a sudden.

@emeraldduchess yeah, my insurance makes me ragey too. I actually have 2 insurances, mine covers the testing and the one from H's work covers nothing fertility wise. It's ridiculous! I have a disease of my reproductive system, it should be covered!
