Re: November 2018 IUI
Introduction: I hope its ok that I’m jumping in over here (and starting the Nov IUI thread already). I’m a semi-regular over on TTGP. MH and I have been TTC for 1.5 years now. All the tests have come back normal so that puts us in the unexplained category. We did 2 rounds of clomid and TI with my OB. After those failed I was finally mentally ready for the next step in seeing an RE. We met with her last week and we are all set to do our first IUI for next cycle! I’m excited and want to chat with other IUIers.
IUI number: 1
IUI protocol: I’ll be getting a baseline U/S then taking Clomid (not sure of dose or days yet). I will then get a mid-cycle U/S to check follicular growth and then trigger. I will also be taking progesterone during my LP so that my lining doesn’t get too thin from taking clomid.
Date of planned IUI: If everything stays on track then it should be the first full week of Nov.
Testing: Not sure yet if I’ll be going in for betas or if I’ll just be testing at home.
R/R: My first patient appointment wasn’t the best with my RE but I’m just really excited to finally be doing something more. I wish there were more test that they could run so that we could point our finger at something. I don’t like the approach of try IUI first and then test more later if it doesn’t work. I’ve convinced myself that I have hostile CM. I asked my RE about getting a post coital test done but she said they aren’t a good indication of success so there’s no point. Um ok… Anyways, since I’m so convinced that it’s my CM I’m starting to freak myself out that the IUI is going to work too well and we’re going to have multiples. Twins would be fine but more than that would be scary!

I think my RE said she wants to see 2-4 follicles. I don't know what the max cut off will be but I'm hoping she starts me out slow. I'm sorry about the spotting. Have you tested at all? Your LP has always been all over the place, right? Have you ever had this much spotting before? What change in protocol are you hoping for?

I did test today and I didn't see anything. It may be TMI but there's tissue in the spotting so I think it's just taking awhile for my lining breakdown? It's very frustrating because I just want to get started on another cycle already. My LP has been all over, which is also frustrating. Last cycle I had an 11 day LP with spotting beginning on day 10, and today is day 13 and spotting began on day 10. I don't know anything to do but wait at this point. I'm not sure what if anything they can change to help us have better chances. Honestly at this point I just want to go to IVF. It's just too hard mentally to continue doing this.
@crookshanks10 - Ugh sorry to hear you're still spotting girl. I can't remember, are they doing a beta for you? How many days has it been since IUI? Also not sure if you did progesterone suppositories next cycle but I started doing them after IUI's and have had better results according to my RE. I'm going to also look into adding a baby aspirin after ovulation (so around the same time I start progesterone) this next cycle. It's supposed to help with inflammation and lining... who knows- I'll try anything at this point lol!
@LJMoon6 excited for you and your husband has great counts!! 2-4 follies is a very nice target for Clomid! Did they do an ultrasound when you did Clomid with TI to see how you responded? Keep us posted. I personally love monitoring and seeing those follies grow along with blood work but realize that's not always feasible for everyone.

As for Lap- why are they suggesting this? It seems really early unless you had something in your tests to make them think endometriosis or something could be a possibility. Are you diagnosed unexplained? Did they give you an explanation as to concerns they would want to figure out from a lap? I would highly recommend getting as much info as possible before moving forward with it- it's real surgery (you are under general and are intubated with a catheter). TBH my doctor didn't push it and I waited 2 full years before I did it. There were two reasons I ended up going forward 1) I had hit my insurance out of pocket max so it was free - thats why I did it at EOY 2) I get crazy bad cramps which are WAY worse when I run. I often will sit on the side of the road in pain until like the contracting pain stops so I thought it was a possibility. My lap ended up negative and in fact she only went through my belly button and took a bunch of pictures and said I have textbook ovaries so she didn't need to do a second incision so It's basically like it never happened- I can't even see a mark because of how she went in on the fold. So basically even with the negative I'm glad I did it to rule it out, but if my insurance didn't pay for it and I hadn't done lots of procedures that kept me in the unexplained camp, I'm not sure I would have gone forward.

@crookshanks10 That sucks. Have you told your RE about the spotting? I wonder, does letrozole impact your lining? Could it just be super thin to begin with so you’re only going to get spotting? I’m surprised you haven’t gotten your progesterone results back yet. Have they been fine in the past?
@cximo08 I followed your story from Oct IUI. I’m sorry for the crazy weird cycle. Is this your first cycle with injectables? What have you tried in the past? They did not do an U/S when we did clomid and TI. So I have no idea how I responded. They did test my progesterone mid LP, which was normal. I have been wondering though, if you ovulate more than one egg will your progesterone be higher? Since progesterone is produced by the corpus luteum, if there’s multiple corpus luteum, I would think the level would be higher. Maybe not tho? I’m very excited to actually be monitored to see what’s all going on. Thanks for the advice on how to cut cost (I know that was directed at crookshanks)! I never even thought about free samples.

@LJMoon6 Thanks! Ya unfortunately I'm an IUI vet lol, This Nov will be my 9th IUI - 7th on injectables and I have a couple IVFs under the belt. The weird thing about the last cycle was my stims were actually very low for me but my estrogen rocketed. Honestly I think the two CP's even though they ended up being MC's changed me somehow- my body feels REALLy different- I don't know how to explain it but everything seems more intense and this last cycle showed that for sure. AFU- same applies to the Estrate- I would have them keep an eye on your lining with US for this cycle to make sure that your lining is getting nice and thick and isn't effected by the Clomid. It's so weird you bring up that progesterone question because I was googling that this week with the luteal phase issues and I thought it was a logical assumption - I didn't find anything though to say that was the case. I do remember I had one IVF cycle where my 7dpo P4 level was high ~35 (normally ~20 no matter how many follies I had) and the nurse was super excited with the level so I googled up a storm, but there was nothing I could find that said that P4 7dpo (peak) was correlated with number of follies or HCG.

@cxsimo08 "They say" that the month or two after a loss you have a higher chance of conceiving. So hopefully the changes you've been feeling are a good sign. I only want to try one or 2 more rounds of clomid, then I'm going to request a change to letrozole. I was trying to google that question about progesterone too but couldn't find much. I was also wondering if progesterone supplements affected your serum test. I asked my OB about this because she had ordered BW while I was taking supplements and her nurse responded back that it wouldn't... But then my progesterone levels came back extremely high (like in the 50s when it was normally in the 15s) so I don't think I believe the nurse. What made you decide to go back to IUIs after IVF?

@LJMoon6 we did two IVF's unsuccessfully. Had a good fertilization rate but none of the embryos either time grew to blasts so there was nothing to freeze or test or anything. My embryos grew to ~day 2-3 but not to where they should be by day 5 either time. My doctor doesn't think IVF is the right path for me after those two terrible outcomes. She basically said some women do better with everything staying in the body (which I've now had two BFP's from IUI so that seems to be true- but results were CP/Ectopic).

I'm cramping like crazy. Feels like AF is coming early, isn't supposed to be here till Friday. If she does come early then it throws off my plan of not having to take too much time off (baseline and follow up U/S would have fallen on my days off). Now I have to decide if I want to tell my boss or keep it a mystery.
This is my first natural cycle after taking clomid the last 2 cycles. Is it normal to have a wacky cycle after a treatment cycle?

What I've learned from all my cycles is it is SO hard to plan around no matter how regular you are. Totally personal decision of if you tell people at work or not. I'm a consultant and didn't tell anyone for a while but it got impossibly hard to manage (especially given how hormonal I was). I ended up not only telling my colleagues at my consulting firm, but also opening up to clients. I'm glad to say that I've received an incredible amount of support and understanding over the years. Honestly, for me telling people has been the best thing I've done. That being said, there are unfortunate things to worry about when disclosing the desire to have kids in the workplace so I completely understand and respect the decision to go either way. My doctor has always offered to write notes and so if your cycle conflicts with your schedule you should definitely ask if that's an option. I'm sure there are lots of women on this board that have some awesome ways they have approached their boss without disclosing what the treatments are for!
AFM- I am kind of a mess right now. My back has been a mess since Friday- no idea why - and I'm having ovulation type cramps/pain on both sides but I'm on BCP so it doesn't make sense to me. The only think I can think of is it's my cysts acting up but they should be shrinking not growing. I have an appointment Thursday to see if I'm ready to start up again- hopefully all will be good.

Introduction/DX: Hello everyone! DH and I have been TTC for 4 years now and we started seeking help at the beggining of this year. Testing showed I had low AMH levels and DH tests came back normal. Did one iui cycle that resulted in a cp. Had a sono to rule out possibility of polyps. Since the Sono showed that there were no polyps, I get to start the next cycle!
IUI number: 2
IUI protocol: 100mg Clomid cd 3-8, U/S to check follicles, then trigger
Date of planned IUI: Waiting for af, then will have a solid date! Probably 2nd or 3rd week of nov!
Testing: Wait 15 days after iui, then taking a test if af doesn't show

@sarahtothea Hey! It looks like we will be doing the same protocol. I did 50mg of clomid for a couple cycles before and was fine on it, but now we're jumping to 100mg for the IUI. I'm a little worried I might have some bad side effects on the higher dose. What dose did you take for your first IUI?

@LJMoon6 I totally get you girl. I don't think I've ever felt implantation cramping but I definitely can feel the difference when it comes to AF. That b*tch is painful!! ;-) You know your body! How are you feeling today?
@crookshanks10 FINALLY!!! lol - was so worried for you with all the spotting. Sometimes things are just off after medicine.
I'm going in tomorrow to check on my cysts. Not feeling terribly optimistic because I've been having sever back pain all weekend and ovulation type cramping for days. I can usually feel when there is more then one follicle in there and I'm definitely feeing this. Hoping this last day of BCP will do the trick... ugh FX!

@cxsimo08 From what I’ve read, implantation cramping is not a thing lol. But that doesn’t stop my crazy stupid mind from thinking it. Cramps have subsided the last couple days, on and off. Still no AF and BFN (I can’t help it). So I’m hoping it was just a fluke and she shows up when she’s supposed to. I’m excited to hear your update for today. Hopefully the cysts are all gone and you are green for go this cycle!
@sarahtothea I’m definitely hoping the 100mg will be fine. I’m going to stick to taking them at night. It seemed to be the best advice I’d found.

@crookshanks10 for IUI it depends on the RE how many follies they are OK with for trigger. I have seen some ladies on here who said their RE's only wanted 1 follie, while others had RE's that let them trigger and do IUI with 6-7 follies. For the most part as @cxsimo08 said 2-4 follies is pretty typical for IUI. @LJMoon6 if your really not comfortable with the chance of multiples or HOM have a talk with your RE and a long heart to heart with your SO about how many follies your willing to trigger with.
@LJMoon6 so vaginal progesterone works locally and should not increase your progesterone levels. injectable progesterone will increase your systemic levels. The research for progesterone supplementation if you do not have a progesterone level issue is mixed. it may not help but its not going to hurt.

@cxsimo08 Oh man! That sucks! Crazy how well we start to understand our body through all of this. Do you have the option to have it drained? I’ve heard of some women doing that and then still continuing with their cycle. I would say don’t give up! But only you know what your limits are. Take care of yourself.
@wabash15 Hey there! So I talked to my RE and they said if I had more than 5 follies they would cancel the IUI. Of course they would rather see 2-4. I also had a sit down with MH. We would make it work if we did get KU with multiples. Of course there’s some fear of the unknown like would my body be able to handle carrying them. But then we also looked at the statistics and realized that, ya twins are a higher chance, but more than that is not really statistically higher. This has really calmed me quite a bit. Also just talking to MH and knowing how all in no matter what he is has really helped.
Interesting about the progesterone levels. It makes sense that the suppositories wouldn’t increase your serum levels. What about taking them orally? I would think that’d be the same as injections then?
Update for me. AF held out till today! Yay! So now I just have to make the call and get the baseline scheduled for Monday. I’m so incredibly excited to be doing this! My anxiety is going to get the best of me this cycle.

@LJMoon6 The selective reduction convo is terrifying. I've done IUI with 6,7 and 8 follies only because my RE knows my history with IVF and IUI but she has a very intense SR convo with me every time and puts it in the disclosure I sign as a risk. It's so personal and scary and I'm glad you looked at the data and facts to think calmly and understand how unlikely it is.

2 IVFs, 8 IUIs (2 Clomid, 6 Injectables)
Update: Just had my first appointment for this cycle. Baseline on CD4 showed my lining at a 4.8B (I have no idea what the B means) and 7-8 follicles on each side. Starting clomid tonight. It was really interesting to see how this clinic does things. They had 2 monitors in the U/S room, one for the tech to look at and another on the wall opposite me so that I could watch. She also had me writing on her clipboard for her (the lining thickness and the follicular count). I thought that was a little weird but it did give me a sense of control. Overall a good appointment. I was nervous for this first one.

12/2018: Letrozole + IUI + Progesterone \\ Cancelled due to cyst
1/2019: Letrozole + IUI + Progesterone = BFP! \\ EDD Sept 30th, 2019
10/7/2019: Healthy baby boy!
