Sorry this is long and has some tough topics.
Today’s appointment wasn’t necessarily good. They confirmed all the findings that were discovered on Tuesday at my MFM. I was hoping we would know if baby A is growing on their own growth curve but apparently we need close to two weeks time to determine growth rather than a few days. Baby A is 37-40% discordance to baby B, who is on track. Baby As cord is velamentous which is also not the greatest. This biggest concern is that there is some reverse flow in the baby’s umbilical cord. The doctor didnan educated guessing that baby A only has 10-20% placenta share.
We now have an appointment on Monday and after they do the same exam (by different staff for a non biased view) we will have two options depending on findings. 1: expectant management - go in twice a week (I might push for 3 times) and as long as things don’t become worse, continue on to see if baby A is growing on their own growth curve and see if things improve. Risks associated with this option. Baby A could die in between monitoring cashing a 50% risk in baby B of either dying or getting brain damage. I have seen many success stories with this approach but I’m not sure how many were in my same combination of situations or this early on in pregnancy (currently 16+5). 2: surgery to separate the placenta from the two babies. Risks associated: its giving baby A a very low chance of survival (maybe 10%) but almost garuntees the safety of baby B although there is a small risk baby B can die too (5%).
What makes my case more more interesting is how early I am, making the decisions harder to make, whereas expectant management would probably be the quick choice if nearing viability which seems so far away at the moment (approx. 26 weeks).
What I am hoping for and would love prayer/good vibes from all who are willing is that at the very least baby A doesn’t show any reverse flow in the umbilical cord come our Monday appointment. BabyA is proportionate for their size currently, seems fairly active in utero (we saw him sucking his fingers) and had a good heartbeat (135)- all these things make it very hard for me to choose the second option. Obviously if the doctor strongly urged the surgery on Monday, that may be different, but at this point he was willing to do either option.
This journey will be long and extremely emotional. I feel guilty telling you guys this and getting all your support when I feel I haven’t been very active in the community like I should be.
For those that that want to follow the journey, I’ll continue to update this thread so it doesn’t take over other threads. Again, thank you everyone for such kind words and prayers! You all are amazing.
One thing we want to do is have a name for baby A by Monday so that if things go badly at least he has a name and it would then become the middle name of baby B in honor of him. If anyone has suggestions my husband would like the name to have meaning relative to warrior, fighter, strong, etc. Feel free to share suggestions.

First of all, I am so sorry for the bad news at your appointment. My son was severe IUGR as well. It was discovered at 20 weeks, and he was born at 34 weeks after we found reverse umbilical cord flow. We’ve spent 7 of the last 9 months in the hospital due to myriad of health issues. But guess what - he’s still here fighting each and every day. I know this road is scary and unpredictable. You live and die with each ultrasound.
