Re: February 2018 IUI
Today I am on day 5 (started Clomid two days ago) and am having weird stabbing pains I haven’t had before. Every five or ten minutes, a get a sharp stab in my right ovary area, almost like a strong pinch or knife jabbing me. I also have either an endometrioma or hemoraghic cyst there - not sure if that is the cause. Unfortunately since it is Sunday I can’t ask my doctor if this is normal...

I've only done this once so I may be getting the process confused. I called last week on the day I started my period. She told me when to start the clomid and scheduled me to come in for the ultrasound this coming Sunday (2/5 I believe). My husband leaves at 6AM on Monday and will not return until Wednesday evening.
My first round of Clomid was on Dec 27th (the length of my cycle varies...one month it will be 35 days, then the next its 21 days. This month I think it was 27 days).
When I went in last month for the ultrasound, he said follicles but my blood test indicated I hadn't ovulated. So he wrote be a script for estrodial inserts and the progesterone shot to take if the inserts didn't trigger ovulation.
The estrodial inserts thankfully triggered ovulation so I didn't have to give myself the shot. However, I got the positive O on that Monday night and he had to leave on Wednesday and was gone for the rest of the week.
So, if this month is similar to last month....I will get an ovulation positive on Monday night....when he is gone.
So, I honestly have no idea what the heck is going on, what I am talking about or what to do.
But, if he leaves on Wednesday - and you get a + on Monday, I'd have sex Monday and Tuesday if possible (maybe Wed before he leaves?) because your + OPK shows that you're gearing up to ovulate, so that's the time to start having sex. Then you'll still have great timing, and if he's going to not be able to provide a sample for the IUI, you'd still have great timing with TI (timed intercourse).
Sorry I'm all over the place - I'd start with calling your RE, explain YH will be out of town on potential IUI day, and ask what your options are.

@tinjp78 - Your RE will do cycles while you have a cyst? Both my previous and current REs have cancelled cycles for a cyst and said I couldn't take clomid until they went away.

@kistra Yes, we are proceeding with the cycles even with my 28mm chocolate cyst. On my last IUI last month, Dr had me take Duphaston to help treat that cyst. I really hope it goes away by itself because I don't want another lap- it could damage my ovaries in the process. I'm trying to search an do everything to treat this nasty choco cyst.
@kistra you sound like my twin. My last two cycles were cancelled due to cysts (including one where I had three big cysts). On my last ultrasound, they found what is either an endometrioma or a hemoraghic cyst. They can’t tell which and will try to see with time. I kind of wish they could give more definitive answers. If it is an endometrioma, then one probably has endometriosis, or is that a bad assumption?
@RVAmom315 I am scheduled for an ultrasound on Friday so I will just wait since all the pain disappeared. But I will definitely ask about it. Curious what my RE has to say.
@kistra and @scully127 I've been on the look out for somebody I can talk with about chocolate cyst/endometrioma etc. I know that it is common to have aches on the ovaries when we have them but sometimes I'm really not certain if it is just me or others experience them too. During my AF My ovaries area became sensitive to the touch and a bit uncomfortable, it went about 3days and it's gradually getting better now. Do you have the same experience?
So, I spoke with the nurse and I’m going in on Sun for my US and labs. They’ll schedule the IUI then. And we’ll be able to use his sample.
i asked the nurse today about success rates...are they diff with each clinic? She said it was only like a 15% chance but that’s not too different from intercourse chances.
What do I need to be doing to make my eggs stronger? Acupuncture? Certain vitamins?
High protein, high veggies, low carb, low sugar - basically all Whole Foods nothing processed (not the grocery store it just auto-corrects)

A couple of other questions:
Does anyone's office do an u/s the day of the procedures, or just a couple of days before to measure eggs?
Also, I see @fertilizemeplease mentioned having an IUI on Sunday. My RE office is only open M-F with a half day on Friday. Are all of your RE offices open on weekends? Starting to doubt my RE...
My RE is open 363 days a year - however they ONLY do procedures on the weekends (and IVF monitoring for ER) I had to go in Friday (last week) for a CD 2 U/S because they didn't do it on Saturdays (CD 3 which is the usual day for that). I mean, what does your RE do if someone's ready to trigger on a Friday morning? You couldn't wait until Monday to do the IUI, you'd have to do it Saturday afternoon, or Sunday morning at the latest. That seems odd to me considering so much of this process HAS to be done on a certain time/day.

@FutureMrsMerati - I would check in to other clinics around you and see if they are open on weekends. My current RE clinic is open on weekends for monitoring ultrasounds, lab work, and procedures if indicated. IUI/IVF is time sensitive and I'd want a clinic that is open 7 days week.

Updated to say that although my ovary was not super visible, they believe I have at least one mature follicle measuring about 20. Doing the Ovadril trigger tonight at 9PM and IUI Friday morning.

@scully127 - this is my first time with a chocolatecyst/endometrioma/hemorrhagic cyst I was definitely achy during AF. It seems a little better now but I don't know how much of that is wishful thinking as I really want the cyst to be gone so we can move forward with treatments. I didn't see my regular doctor because it was a saturday when i went in for my CD3 u/s in Jan but he seemed hopeful it would still go away in one month. When I had a cycle cancelled before it was just a regular cyst and didn't cause any pain and was gone the next month.
My old RE only did IVF on the weekend and not IUI, would either trigger in time for Friday or wait for Monday. My new RE is open on Saturday. My first RE would only do one U/S and then told me when to come back for the IUI a few days from then. My new RE is apparently going to do daily u/s until trigger day then IUI the next day.

Edited to let you know that our doctor recommends BD two days before and to avoid it the night before IUI to ensure a good supply.

Both my IUIs I had 3 measurable follicles. IUI #1 were smaller, two 14s and a 16 (50mg Clomid, 300 units GonalF) IUI #2 were better 17, 20 and 21(100 mg Clomid, 150 units GonalF) I’m on the same protocol as IUI #2 so we’ll see how my follies look on Tuesday.
I didn’t notice any side effects from the injectables (Gonal F) either time

@FutureMrsMerati Did you BD 2 days ago or yesterday as your Dr recommended? If so then you shouldn't be so worried since the sperms could stay in you for 3 days waiting for your egg to fertilize. If not, I guess it would still be ok to BD tonight if it'll make you feel at ease but the supply of the SA that YH would give tomorrow might not be enough as your dr had warned. This is just my 2 cents, i agree with @kistra on getting some advice from your clinic.
@FutureMrsMerati Let us know how it goes today! Also, in all my TI cycles I've had just one (twice) or two (once) predominant follicles. From what I recall, I think they ideally want one so that you don't risk multiples (though I would welcome them!!), and two is ok, but going beyond two can be risky in terms of multiples.

AFM, today at my day 11 u/s I had a few follicles in the 12/13 range and one at 14. since they don't do u/s over the weekend, they want me to use OPK to monitor ovulation. If I'm surging Sunday, they'll schedule IUI Monday. If not, I'll go in for another u/s on Monday, trigger, and probably IUI Wednesday. I'm feeling anxious because I've always been ready to trigger on day 12 in the past so I'm feeling like waiting the whole weekend is risky and Wednesday seems so late for IUI. Then again, our bodies are not machines so I will just trust the RE's advice. He told us to BD tomorrow (in case IUI is Monday), so we'll go with that.
