Can we talk about inductions?

My doctor brought up the possibility of inducing labor at 37 weeks for medical reasons to keep everybody safe, (also, oh ehm gee, that's in two weeks!!!) I know we've touched on it during the randoms thread but can we talk about it more?

FTM questions? STM Experiences? All that good stuff.

I had planned to try to avoid inducing, since I've heard it can raise the C section rate and cause more painful contractions, so I feel like I don't have any knowledge about the process really. At my birthing class they went over vaginal births, and the process once you go into labor, and C sections, but not really how to force labor if it didn't start up on it's own.

  • STM here with previous induction.  I will preface to say that my birth plan then and now states "I am willing to have labor induced if my health care providers deem it medically necessary, though I would like to delay induction as long as advisable"

    Out of curiosity, what are the medical reasons for the doctor wishing to induce at 37 weeks?  I tend to trust my doctors' judgment, but I'm curious why he/she would know now that induction that early is necessary (as opposed to C-section).  

    I dug up an excerpt from my post last month on the "planned induction" thread...

    ... I had a really wonderful planned induction with DD, which I feel is rare! Ours was planned at 41w1d simply because DD was past due. My doc was concerned about the complications associated with delivering later, and I was ready to get the show on the road!  I suspect my experience went well because I was already dilated about 2-3cm, so didn't need any cervadil. I started pitocin around 8:30am, started pushing around 6:15pm, and DD was born at 8:10pm at 8lbs 2oz. Yes I pushed a long time, but the epidural made it feel like it wasn't much time at all. I had a first degree tear, which I never felt (happening or during healing). Funny thing, my doctors all predicted a tiny baby, so I feel like measuring early/late is a total guessing game, and I even had an ultrasound at 40w. 

    Sounds like it may not apply to your situation, but I would try to wait to schedule an induction until you are dilated a little bit so you won't have to do cervadil. 

    And if my previous situation presented itself with this baby, I'd totally do the induction again. I was able to shower at home in the morning, have hair and make-up done, and feel rested before labor and delivery. And doing so this time would allow me to make solid plans for DD without being rushed.

  • Ask about your Bishops Score.  Typically that will indicate which type of induction you would start with. 

    With my first, I was not very favorable (only a 2-3 on the bishops score) so I started with a cervical ripening (cervidil) in the evening.  By morning my water broke naturally which was good and I only needed a small supplement of pitocin.  

    With my second baby I was more favorable and already soft, so my OB said we could go straight to pitocin.  It didn't really work (I wasn't in pain, I just wasn't progressing) and so they broke my water to start labor and things went very quick and smoothly from there. 

    Both were vaginal births with an epidural for very large babies. 

  • I'm one of the induction stories that ends in c section. My bishop score was not favorable at all, I think a 2 maybe, but they induced me at 40w4d due to high blood pressure. 

    They had me come in at 5pm and start with the Foley catheter(bulb) thing which is supposed to put pressure on the cervix to get you to a 4 or 5 maybe. It's supposed to fall out but mine never did so the next morning around 5am they just took it out and started on the pitocin. I think I was dilated to a 3 at this point. 

    My my blood pressure shot up really high to 180/110 so they gave me some BP meds and then I got an epidural which caused my BP to bottom out at like 90/50. All that finally leveled out but then after about 30 hours of being in labor I never dilated past a 5 so we just decided to go ahead with a c section. 

    DD ended up being sunny side up which caused terrible back labor. While I wish I could've experienced a vaginal birth I know that we got her out the safest way for us. And now we can fully plan her next birth with a RCS so that's a plus.

    @peachgal23 Possible cholestasis. I was tested for it yesterday, we'll find out next week. I've had gallbladder issues for months with this pregnancy, and ultrasound showed my liver was enlarged when they were checking my gallbladder. My hands and feet have been itching HORRIBLY the last couple days, which is apparently a warning sign, so we'll see!

    @Katm89 One of my best friends is a reflexologist! I will call her and see if she can come up the weekend before they are talking about inducing. Great suggestion, I hadn't even thought of that. I'm all for more "natural" induction, (ie, not Pitocin)





  • @Allisun85 I did not have a scheduled induction with my daughter but I did stall during labor and they gave me pitocin. I've never really investigated the issues with using pitocin, but I had zero issues when they gave it to me and it did just as it was supposed to. However I had been dilated when I got to the hospital.

    @bsckgb7 I did have an epidural-- I had an anterior placenta so it was all back labour.  However, the epidural didn't kick in until I was pushing, so I felt everything up to that point.  I'm hoping to avoid an epidural this time (but no judgment for those who use them!). 

    When I talked to my midwives about my induction, they said an induction, back labour, and not being able to move around because I had to be constantly monitored (some meconium when they broke my water) was a "good use" of an epidural.  I'm hoping my midwives will be able to help me move/reposition during contractions this time around.

  • With DD I was induced at 41 weeks. I was already at 3 cm so just had to go in in the morning and be hooked up to the pitocin. They broke my water right away and less than an hour later I was having regular contractions that weren't painful, but I could definitely feel them. This was at about 9. I labored until about noon when suddenly I could hardly stand the pain and the contractions were one on top of the other. I opted for an epidural at that point. DDs heart rate kept dropping about every 30 minutes or so and they would come in to make me change positions, but we later found out it was because the cord was wrapped around her neck. The nurses didn't seem too concerned, but it scared the heck out of me a couple of times. My epidural never fully numbed me, but it took the edge off for me to sleep. At 11 pm I started to have a lot of pain and pressure and by midnight I was fully dialated. They were going to have me labor down for a while, but I was in too much pain and felt like I had to push. 45 minutes later she was out. I hope I go into labor naturally this time, but I wouldn't be opposed to having to be induced again. It was nice knowing exactly when I would start labor. 
  • STM here - with my first I was induced at 38+6 for preeclampsia.  I didn't require any cervadil because I was already 2-3 cm dilated before they started the Pitocin.  I was able to get my epidural after it was clear my labor was progressing - I believe I was 4-5 cm when I got it, but I didn't feel like I needed it.  I only knew I was contracting by looking at the monitor.  The anesthesiologist wanted to wait until I "needed" the epidural but my doctor wanted to go ahead and get it in if I wanted it before my contractions became painful.  After my epidural my doctor broke my water.  They started my Pitocin around noon and at 8:30 p.m. that night my daughter was born.  

    I am hoping to go naturally this time, but will do anything to guarantee a safe delivery of my second daughter.  I am definitely an advocate of making sure you understand why the induction is necessary.  I had a very positive induction experience but yes, induction COULD make labor harder on you and on the baby and it can prolong labor.  If your labor drags out too long a doctor MAY want to do a c-section, but that's something you should bring up to them and ask about.  It does increase the intensity of your contractions which typically leads to an epidural.  That may not be a big deal if you're leaning towards an epidural anyway.  I am hoping to avoid an epidural this time around because I hated the way it made me feel - I felt very woozy and it made it extremely difficult for me to focus during delivery.

    I think it really helps to just make sure you stay relaxed and don't get too stressed out.  I know that can be hard depending on the circumstances of your induction, but my preeclampsia was pretty mild and they were going to talk about a planned induction at 39 weeks anyway because of my hypertension alone, so I wasn't worried about anything going wrong.  Just try to stay relaxed.

  • Hi, I've been lurking for quite a while without posting, but wanted to share my previous experience. I was scheduled to be induced with my son (now almost 4) at 39 weeks due to high bp. I was scheduled for a Monday evening, but decided to try acupuncture on Friday night after the decision was made. My acupuncturist hooked me up to the electro-acupuncture machine (not painful, just buzzy) and I went into labor on my own on Sunday evening. Still ended up with a little pitocin on Monday afternoon when labor stalled, but ended with a vaginal birth on Monday night. I think anything you can do to get the ball rolling a little in advance will probably help make the process easier, and acupuncture worked well for me. I was slightly effaced but not dilated when I went in for the acupuncture.
  • @Allisun85  Thank you so much for starting this tread!! I am scheduled for a C-Section at 37 weeks due to Pre-Eclampsia and DD continuing to be breech for so long. Having these stories to read is helping ease my worries.

  • I was induced at 38 weeks with DS due to high blood pressure.  Went to the hospital at 10am and he was born at 9pm. I had pitocin and an epidural. It was my first, so I have nothing to compare to, but all went so smoothly and I hope this delivery goes as well!  

    One word of advice if you're planning on an epidural...as soon as I checked in, my nurse gave me the advice to request the epidural AS SOON as I start to be uncomfortable with contractions.  It takes a while for the anesthesiologist to get in there, then takes time for the meds to kick in.  If you request it "early," by the time you're REALLY starting to hurt hopefully it'll be doing its job.  I followed her advice, and really only had painful contractions for 30 minutes or so.  I had no weird side effects of it either....could still feel the contractions, more just a pressure than a pain, and I liked still having some sensation of what was happening.  Planning on doing it again with no hesitation!
  • I had 2 inductions that both went great. I did have epidurals with both so I can't tell you if the labor was worse than when my labor started on it's own.
  • My success story is similar to @PeachGal23. Induced at 41w1d because I was overdue and it was mostly a good experience. 

    Went in for scheduled induction in the morning. I had to get Cervadil because I was completely closed and not effaced at all. Cervadil started at 9am and most of that day was just spent with what felt like period cramps. My water was manually broken late afternoon, pitocin was started, and that's when the action started. Contractions became more painful throughout that evening and by the time I was around 3-4cm I was asking for the epidural. (I knew I would want one going in, just decided to wait until I really wanted it.) So yes, @bsckgb7 I did get an epidural around 11pm... and then I SLEPT through centimeters 5 through 10! (I know they're different for everyone, but for me, EPIDURAL = BEST.THING.EVER.) Woke up around 3:30-4am feeling pain even through the epidural so I buzzed the nurse to check me. I was at 10cm! Pushed for 40 minutes and he was born at 4:43am. 

    So here's the not-so-great part, (and @Allisun85 this probably won't apply to you since you'd be induced early). DS was 9 pounds so I had 3rd degree tears and 21 stitches. It took longer to stitch me up than it did to push him out (in fact, I still say that getting stitched up was the worst part of the whole experience)! I have since wondered if we had waited til my body was ready, would I have torn so badly? But then again, he would have been bigger so... we will never know.

  • @bsckgb7 yes, I had an epidural.  I always knew that I wanted one as soon as I hit 4-5cm.  I received the epidural shortly after my waters were broken.  I hadn't felt much until doctor broke my water, even having been on pitocin for several hours, but the 2-3 post-water breakage, pre-epidural contractions were BAD.  Agree with @Sunnyn75 to ask for it early (if it's something you know you want).


  • Thanks for starting this thread. Reading people's experiences has been so helpful. I'm scheduled for an induction on July 18 (37w2d) due to high BP. Yesterday, I was 2cm dilated and 30% effaced, so these stories give me hope for a positive experience. Fingers crossed for me and everyone else who needs an induction!

    TI - Injectables (follistim + Gonal-F, Ganirelix, & 1/2 dose Pregnyl) + prometrium (AM & PM) 9/15 --> 3-7 mature follicles (3 definites and 4+ that could have matured due to trigger) @ O -->BFN + 5 large cysts

    TI - Injectables (Gonal-F, Ganirelix & full dose Pregnyl) + prometrium (AM & PM) 10/15 --> 1 mature follicle --> BFN

  • My doctor said I can be induced at 39 weeks, and while I'm not sure if I want to go with an induction, my labors were so short with my first two that I'm afraid to go into labor on my own. These stories are definitely encouraging! 
  • I was induced with DS and DD at 39 weeks. Both times I was not dilated at all. Because of my health issues they induced anyways and I had 2 uncomplicated vaginal deliveries. I would try and wait till 38-39 weeks if possible, but if you or the baby's health is in danger, then I would induce ASAP.

    @bsckgb7 with DS I did = best thing ever. No pain, just pressure. With DD I had awful back labor and the epi only worked when I was flat on my back which dropped her heart rate, so needless to say her labor was very intense and painful. I went from a 3cm-10cm in 3 hrs! Eeek. I am praying for the epi to work with DS #2 as well as my epi did with DS #1 because it truly makes the whole experience more bearable.  

    IMO I would try and hold off on them breaking your water. Once the water breaks there is only so much time you have till they will do a c section. If you have your water intact they will usually let you go a few days of laboring before a c section. I'd say wait till you are at a 5cm to let them break your water.

  • I too had a positive induction experience.  With DS1, I went into labor and my water broke on its own. I still ended up on Pitocin because labor stalled. 
    With DS2,  I was induced at 39w6d. I went in at 7 am and was already dilated 2cm Dr. came in at 7:15 and broke my water then had me walk for an hour. My contractions were sporadic,  so they started me on Pitocin.  After a few hours of the Pitocin, I asked for the epidural.  I started pushing around 5pm, and DS2 was born at 6:18. 
    My induction was a much better birth experience than my first L&D!

    It does cause more painful and frequent contractions, but when done correctly, it does not increase c-section rates.

When I asked my OB specifically about this, she didn't answer spot-on buy seemed to indicate it would depend on the state of my cervix at the time of induction, so if I was completely closed, she would try to push the induction date unless my BP wouldn't allow her to safely do so. Then, we'd discuss scheduled C-section or attempted induction. She never answered the question directly, but that was our conversation. The nurse who taught my childbirth class stated that the more intervention  (i.e. induction), the risk of c-section is increased.

  • I've been lurking for months, and this is my first time posting. I am due with my third in a few weeks, and both of my previous deliveries were induced. My first was induced at 37 weeks due to preeclampsia. I was 75% effaced and 1cm so they started the pitocin around 2am. I had contractions visible on the monitor all day but they didn't hurt, and I never progressed past 1 cm. My doctor had the pitocin drip turned off that evening so I could sleep, then turned it back on at 5am the next morning and broke my water at 7am.  That was when the pain started! I had strong contractions but still was only 1 cm so my nurse asked if I wanted to try an epidural. I decided to get one, and 30 minutes after it was in I was at 10 cm. I know they tell you that an epidural can slow down dilation, but I ended up being the exact opposite. My daughter was born a few hours later and weighed 10 pounds 10 ounces (and no, did not have GD), and I was so glad I had the epidural. My second was induced at 38 weeks 3 days for preeclampsia-like symptoms. I got the epidural early this time and within 45 minutes was fully dilated. I slept for several hours and my son was born a healthy 9 pounds 1 ounce. Hope this helps ease any anxiety some of you may have, and good luck to everyone!
  • Induced for pre-e at 38w4d, I was already 3cm and 70% effaced.  Started with a balloon overnight on a Friday.  Balloon out Saturday morning, it did nothing.  Started on Pitocin, got up to the highest dose, did nothing, by Saturday evening still not dilated or effaced any more than I was two day before we started the induction.  Around 8pm Saturday night they broke my water and that finally gave me contractions that we're painful and started to do something.  Got the epidural around 11pm so I could rest.  That slowed my contractions down again.  Put back on pitocin Sunday morning but still did nothing to bring the contractions back to where they were.  By 3pm I was at about 5cm, and that was generous, and maybe 80% effaced, but DD was still at -3 station and they thought her head might have been tilted trying to go thru my pelvis.  By 3:30pm DH and I were sooooo over it.  They said it was up to us if we wanted to wait a few more hours. We were done, we both said C-section, and she was born at 5:03pm on Sunday at 38w6d.  I would never allow for an induction again, I'd rather go straight to C-section.

  • @Wolfie646 it seems most of these moms that had inductions did so because of dangerous conditions. Pre e and high bp can be very dangerous. I almost had a c section with my first after going into labor naturally because of my bp. Another 10 minutes and I would have been in surgery. I would have preferred to be induced earlier and avoid almost seizing or dying trying to give birth. 
    I know there are others that were induced just for being past thier due dates, but there are risks with being overdue as well. 
  • I know someone IRL who has been told she'll be induced at 37w, which is 3 weeks away for her.  I know it's because of other complications that she has, but as someone who was induced, what I don't understand is how they even know at this point whether or not she'll even be a candidate to be induced.  If I wasn't 3cm and 70%, my midwives would not have induced me even with the high BP because it likely wouldn't have worked (and it didn't, anyway).

  • Inductions do lead to a higher level of c-sections but if it's a safety concern, I would do it.  After going into labor naturally, I had pitocin after stalling out at 8 cm and it was horrible.  It also didn't work and I ended up with a c-section anyway.  Unless there is a legitimate medical reason I will never go on pitocin again.  
  • @Allisun85 -  I have known of several moms to make it through pitocin, at some point in labor, without an epidural (me included).   The contractions do get a little crazy but it definitely can be done!
  • @Wolfie646 I was induced for high BP that turned into preeclampsia.  That was my first pregnancy and honestly I didn't give much thought to how I wanted to deliver.  However, if the situation presents itself again during this pregnancy, I would likely agree to the induction for the safety of all involved. Fortunately, I was 38+6 and my daughter came out perfectly healthy.  I have watched "The Business of Being Born" this time around and have decided that I won't go with any interventions that aren't medically necessary.  I have an OB who is very understanding and cooperative and has agreed to discuss what will be negotiable and non-negotiable during my labor and deliver at my 36 week appointment.  They have been very up front with me in saying that they will do everything they can to help me do it all naturally, but because of my history I should keep an open mind, which I will.  

    Also, my aunt had to be induced with her last baby and her midwife agreed to turn the Pitocin off after the labor was well established, and everything turned out perfectly.  I had not thought about that possibility until she brought it up to me, but it's something I want to ask my doctor about in case I do have to be induced for any reason.

    If I recall correctly, inductions can lead to a higher level of c-section because the Pitocin makes your contractions stronger than they would be naturally.  The stronger contractions can cause extra stress on the baby and could affect oxygen and heart rate, which would be reason for a c-section for a lot of doctors.

    Yes - but my midwife recommended it for uterine health, not anything to do with inducing labor. I made a big batch iced in the fridge and was told to drink about a quart a day.
  • I've been drinking a cup a day of the raspberry leaf tea for the past couple weeks or so-- since 34 weeks.  No noticeable change in Braxton hicks, but it has helped me be a little more ahem... regular... and after already taking fiber supplements and eating prunes, I'm welcoming of the extra help there.

    I drank it daily for a few weeks before my due date with DS but it never made a difference. I still ended up being induced 8 days past my due date.

  • I was induced at 40 weeks 3 days because of low fluid levels.  I had an epidural, it was amazing.  DD was delivered 12 hours after the pit started (maybe a little less?).  I pushed for an hour and a half, and DD came out with the cord wrapped around her neck fairly loosely one time.  She was purple-ish.  Other than that she was perfectly healthy, was a great sleeper at first, and breast fed like a champ.

    @riveridgional, ACOG says that inductions don't increase c-section rates and I trust them as they are the experts in the United States on obstetrics.  In fact, two ACOG doctors stated in May, "the major risk associated with a failed induction at 39 weeks is cesarean delivery, but both Dr Lockwood and Dr Norwitz said that there does not appear to be any evidence of an increase, and that perhaps it might even decrease rates."
