Re: Low AMH Level

  • @hopefuljj Thank you!  We got five eggs, so now to wait for the fertility report tomorrow!

  • How is everyone doing today?

We got the call yesterday; of the five retrieved, four were mature, and three fertilized.  Since we're kind of taking a gamble and pushing through to day 5, it's going to be a stressful week of hoping that all three continue to develop.

Unfortunately it's my own RE who will be doing the transfer, and she is very opposed to putting three back.  We're really going to have to push hard for it if we still have three on Thursday.

  • @bakerstreetboys I am praying that all 3 make it day 5. How are you managing the stress this week? hope you are keeping busy and being kind to yourself.  Looking forward to hear about the progress.

    How is everyone doing?
@emmasemm Your appointment must be coming up soon!

  • @bakerstreetboys yay!! Such great news!!     Crossing everything for you, I know this wait is excruciating!   Hi @Hopeful_mom :) :) my donor was supposed to finally fly into NY today  for her mandatory psychiatric evaluation and inperson medical (scan etc) after psssing the recent bloodwork she got done on day 3 and after passing all the onpaper medical stuff... but things just never go smoothly, lol, so last night shortly after boarding the plane and it taxying out, her plane was towed back to the gate after a mechanical failure , the agency couldn’t get her on a flight to make it on time for the appointment here in New York this AM, and now everything has been rescheduled for mid April because the donor appointments take a while to book at the clinic and now it’s all full:(.  I mean....honestly I expected nothing less. I was specifically not sharing many details so I wouldn’t jinx it.  Yet here we are. Lol.   Is what it is.:)

    Me: Hashimoto Autoimune Disease
    Homozygous MTHFR Cr677

    IVF 1 -3 made it do day 5!    2 5AA 1 6BB  PGS results =  all abnormal :(
IVF 2 - stimmed for 19 days. insanity. Had 5 follicles that looked mature but only 2 contained mature eggs the others were empty.  Nothing made it to day 5
IVF 3 - added in HGH - 4 mature follicles (had some empties again) 3 made it to day 5/6 for PGS (held for testing with next retrieval
IVF 4 - Upped my dosage for the first time - Canceled 11 days into sims - looked like 1 follicle and we were afraid it was empty
IVF 5 - Canceled -  looked like 1 follicle and we were afraid it was empty
IVF 6 - 3 mature, 2 fertalized 1 made it to day 5 and sent for PGS

Sent in IVF round 3 and 6 (4 embryos) for PGS testing and got 2 normal embryos!!!  1Boy and 1Girl. Took a little over a year to get those two embryos so demanded all sorts of test prior to attempting transfer. Did EFT, ERA and NK Cells testing.

FET#1 - Intralipid drips, Prednisone, and did a depot lupron shot before starting the FET protocol
    Chemical pregnancy
    FET#2 - PGS normal boy - BFN
Natural pregnancy - cycle following the FET2????   Miscarriage at 8 weeks.  viability scan had shown an enlarged yolk sac so I had been warned that it was likely not chromosonally normal :(

@hopefuljj I read your post and had to just walk away... I mean how many movie plot lines can we have on this board??????? I do hope she her next flight soon and you can get going again...

@bakerstreetboys keeping everything crossed for you. My RE is also willing to transfer 3 untested embryos at a time... but we will probably go for less unless but mostly as we have 3 sets of 2 and a single so we don’t need to defrost 3 at a time.

    @imajin99 how are you doing?

@hopeful_mom hope all is good with you

@jamieh2000 hope all is ok with you as I didn’t see you post on Facebook recently either

AFM well we have the appointment with the specialist on Thursday for fibroids and polyps. Hoping to get a surgery date for the polyps very soon (let’s see...) and hoping they say that they don’t advise surgery for the fibroid but also that it shouldn’t affect implantation...

  • @bakerstreetboys that’s so exciting! Happy for you! I got the more in depth Fragile X testing back from lab. My risk has been reduced to about 2% of having a baby with full permutation but since you can’t test for that if you’re pregnant until 10 weeks I’m not sure how I’m going to proceed. It would be a horrible decision to make if I got pregnant and received positive results of a full permutation. I have a WTF appointment tomorrow with the RE. mostly just scared at this point. Hoping for no more surgery for @emmasemm and news from @dreamscapes_ Omg @hopefuljj what frustrating delays. Following your story, I might be right behind ya with donor eggs and I really hope yours gets back on track. I had no idea how strong women are who have to deal with this. You guys really are amazing for even putting one foot in front of the other.

@lucyb1954 thanks!!! Hope your RE appointment goes/went well and you have a clear path.

@bakerstreetboys I so hope you have your THB in there!

So first of all the good news - someone cancelled whilst I was in my appointment and so I got in for my polyp removal already next week! It was so lucky as if not first dates were end of July! We are ok in this respect to transfer as soon as next cycle as long as it goes to plan.

Also good news, monster fibroid can stay and just needs watching in 3rd trimester. Shouldn’t affect implantation or 1st trimester at all.

    But.
My appointment lasted nearly 4 hours... and I saw 5 doctors each doing another ultrasound. They used the highest resolution US I’ve ever seen - it’s the one the fetal cardiologist uses apparently.
Because they wanted to correlate with the MRI and other scans what they thought - and I ended up with a new diagnosis of adenomyosis.
So now is the question do we do depot Lupron for 3 months which is basically the only suggestion for adenomyosis, or do we try a transfer first without it... It might not be needed, it might not work - or it might be the magic key... and if we do it we’d need to redo the ERA, so more time. Basically we’d be lucky to transfer within this year.
Booked a consult with the RE for two weeks time (after the polyps are out, just in case of further complications).

Just spent an hour explaining to DH about it all. I thought he might cry. That hit me hard so think we both need a few weeks to digest.

  • @emmasemm That's great that you're getting in next week for the polyp removal, and that you don't have to have the fibroid removed.  But omg, I can't believe that you've hit another obstacle!  I hope that the RE has some solid advice for you on whether or not to do the Lupron and wait, or to transfer now.  How many do you have frozen?  7?
@lucyb1954 That's great that your risk has been reduced, but I understand how scary it is.  I hope you get more information from your RE.

  • @bakerstreetboys omg!!!!!!!! This is so exciting!!! I’ll remain cautiously optimistic over here:)!! @emmasemm oh for godsness sakes! Seriously?? Do you have really heavy bleeding and pain?? Ugh, here we are back at another one staring down the barrel of 3 months of Depot Lupron!!!! Ugh. You know how I feel about that. I’m so frustrated for you!! Take some time and think it all through. I hate how much we just don’t know when it comes to this stuff. Specifically with things like Endo, and adenomyosis or glandular developmental arrest or anything they throw 3 months of Lupron at. Keep us posted on what you decide. I’m sorry:-/ you’ve got this, you are a rockstar. @lucyb1954 I’m so sorry that you have to worry about that. I was just listening to a podcast about a couple in a similar scenario who decided to just see what happens and have a baby. How did your WTF appointment go?? AFM just waiting over here as usual.   • Awww thanks @hopefuljj I’m sending prayers your way that things get started already! My WTF appointment was hard. Given my AMH level, doctor thinks IVF would be 10-15% chance successful. The alternative of trying with IUI is open to us but having to wait 12 weeks to genetic test potential baby would be so hard. She recommended mini IVF given my FSH/AMH levels. SO and I haven’t decided what to do but I’d like to try it before we move to DE. I just feel like I need to. I know IVF is a hard process but hopefully a mini cycle would be less difficult than a full. @emmasemm omg I’m so sorry about all this crap that keeps getting thrown at you. It’s great you can get in for the surgery soon but how disappointing. Hugs to you. Thinking good thoughts for everyone else too! @bakerstreetboys ;

@emmasemm I am screaming with you, that is so frustrating.  I am so sorry I do not know anything about that condition.  Will read up on it, so interesting that there is so much information I have received from all you ladies that I was not even aware about. Whatever direction you choose, I wish you lots of luck.

@hopefuljj  Hope everything goes well when your donor arrives. I will really need to speak with some of you as here in Canada they do not have egg banks and our option is to have a frozen eggs from egg donor sent from the US.  My husband's family is in NY/NJ area so we do have an option of going there to do a fresh cycle but will result in a lot of time off work.

@bakerstreetboys Congratulations my dear, double up beta! Sending you a big virtual hug.

  • @hopefuljj Just over a week to go!  Keeping everything crossed for a smooth flight and a great appointment.

My betas are doubling so far, though I haven't even had the blood test with my fertility clinic yet!  I just don't understand how people can wait that long!  I guess patience isn't my best strength.

  • @emmasemm- omg I am so frustrated for you! How many roadblocks can there be? You guys are such troopers. That is interesting they could diagnosis adenmyosis via ultrasound. I didn’t think they could but I’m sure there are constant new developments on that front. So hard to know what to do. You have waited sooo long but those embryos are precious of course!! Thinking of you always!

@hopefuljj- yay!! Let’s get this party started. Delays are sooo annoying but hopefully the donor will check out fine and smooth sailing from here!! Soo excited for you!

@bakerstreetboys- so thrilled for you!! Can’t wait for your first scan. This has to be it for you!!!

@bakerstreetboys keeping everything crossed for continued good appointments for you

@hopefuljj a week to go until the donor flies in! I hope everything runs smoothly this time!

@lucyb1954 10-15% is a very good prognosis for DOR... I’ve been given a less than 1% (actually said less than 0.01%) chance with own eggs and less than 5% with DE...
The only thing that a mini IVF would be is lower cost of medications - basically everything else is the same.

@imajin99 how are you doing yourself? Recently on best Infertility and infertile mafia podcasts they had people talking about egg banks (as part of surrogacy stories... but the egg bank info was useful). Maybe have a listen and look them up. One of them did mention having a lot of Canadian patients.

@hopeful_mom still not sure yet. I’m really on the fence. However I think they gave me antibiotics rectally after my last Hysteroscopy due to all the bleeding and time they were in there. In which case I would need to repeat ERA anyhow potentially depending on which used as antibiotic use is known to change receptive window...

@jamieh2000 they diagnosed with combination of MRI, ultrasound and symptoms. Though I never thought I had bad enough symptoms, apparently they complete the picture and my idea of “not so bad” is apparently what others call “terrible” ;-) The US they used was super powerful - the special one used to diagnose and monitor fetal heart conditions. They said that in two places the adenomyosis is bad/big enough to see clearly on US if you know what you’re looking for and have the MRI to cross reference with. I did quiz them quite intensively!

AFM polyps have been removed! They were in the same spot as 6 weeks ago and hadn’t grown. They send them off to histology and check for endometritis too whilst they were there.
I also got some new pictures of inside my uterus to add to my collection. They reconfirmed it’s heart shaped but this cycle showed no “rough looking” piece in the middle of the Fundus, so either it was disturbance last time or just a one cycle thing. They said that can happen from time to time anyhow and especially with adenomyosis.
Still trying to fix a mutually convenient time where DH and I can talk to our RE together, hopefully in the next week or so, then we can make a plan for what to do next and when!

  • @emmasemm yay no more polyps!!! I swear, women in general just have such a much higher tolerance for pain that sometimes we don’t know that other people might classify it as horrible while we can withstand it.

I dont know if my prognosis includes the fact the fact that some of my eggs might be crappy because of Fragile X. I felt like she was trying to steer me away from IVF but I’ve been so effed over by doctors I don’t care what she thinks. Trying to be hopeful but realistic. A fine line.

Im going to check out that podcast too as I might be in the DE boat as well! Anyway lots of good juju to all on here @bakerstreetboys @hopefuljj @Hopeful_mom @dreamscapes_

  • @emmasemm so glad ur polyp free still
shocked with all the extra testing and US that they were able to see! I’m glad u were able to get more answers! My RE originally thought I had endo as I complained about my pd however she said no.  I always think after the ER wouldn’t u be able to see them if there was an issue but I was told that’s not how it works. FYI - I pm you.

@hopefuljj of course a snow storm is coming but I’m positive all will go smoothly for u!

@bakerstreetboys ok this means ur only a few days to go woohoo!!!

@lucy1954 the IF road is a tough when and I always say it shows the true warriors!!! My stats were even less due to my age but I still kept trying. One dr straight up told me to use DE after my 2nd cycle was cancelled and I cried.  I just wanted to get through a cycle and try with my own eggs first so I went back to the previous protocol used and got to an ER.  Granted I went through over 8 cycles to get my miracle but it does happen, u got this!

  • Hi everyone. I had a baseline ultrasound today before starting a cycle to check out where I am. RE only saw one follie on my right side and 3 on my left. I know this is bad news... can anyone tell me how bad? My RE thinks it will probably be more effective to do some mini stim cycles and bank embryos (doing PGD because of Fragile X premutation). Feeling discouraged 🙁
  • @lucyb1954 I always have a low AFC.  It was five on my cycle, but we still got four mature eggs, three of which fertilized and made it to day 5 blasts.  I never expect to get high numbers with DOR, but it's quality over quantity.  I hope your RE comes up with a good protocol for you.

  • Thanks @bakerstreetboys It’s just bad news on top of bad news during this process so far. Rough stuff 🙁 how do people walk the line of trying to be hopeful but realistic? Just feeling down today, I’ll bounce back tomorrow but some days are worse than others. My RE  is thinking it might be more effective to do a couple of mini stim cycles and bank embryos rather than go to conventional first. What did you do?
  • @lucyb1954 Since I was already 40 when we started the IVF process, and we only wanted one baby out of it, we did full fresh cycles every time.  We had one frozen embryo after our second transfer, but instead of using that, we did another full round.  I just didn't feel that transferring one gave us enough of a chance, given our history.  I think it depends on how many babies you hope to have, whether you're doing PGS, and how you respond.  I know that the mini stim cycles work for some women.  We were also paying out of pocket for everything, so we had a limit on how many cycles we were willing to do.

    *tw
Our first cycle, we put back two day 3s and had a CP
Cycle two, we put back 2 day 5 blasts, and one took.  Unfortunately he had T-18 and we had to say goodbye to him at 16 weeks.
Cycle 3, we put back 3 day 5 blasts and I'm currently 6 weeks with one baby.

  • @bakerstreetboys keeping everything crossed that things keep going smoothly for you

@hopefuljj I really hope her flying in this time works out. I mean we have enough to deal with... the amount of bumps with DE cycles is just not something you think of if you haven’t been in this group  :D

    @dreamscapes_ hugs to you

@jamieh2000 hope you’re ready for a fun first Easter with your little man

@hopeful_mom hey you could do an FET in a few months and pair up with me  ;)

@lucyb1954 personally for DOR patients, I would say forget anyone who tries to push you to mini or max stim protocols. Your Doctor should tailor your stims to maximize the number of healthy mature eggs you can retrieve. If you need 300 of gonal f or menopur to get those follicles growing, then do it. If using clomid or letrozole the first days before injectibles works, then do that. But no fixed protocols in advance. I strongly suggest you go for estrogen priming though.
I have done banking and from 4 cycles have 7 frozen embryos (though the first 3 weren’t top quality - this improved after we switched to MDL and triggered when follicles were bigger). My body definitely needs LH to mature those eggs, not only FSH to make them grow.
Do your baseline on day 1 or 2 and if you have a high FSH or estrogen over 50 skip that cycle and prime again for the next one. Otherwise the drugs won’t have so much effect.
Check with your clinic and the testing lab for your PGD - my clinic lets you biopsy cycle by cycle, then sends all biopsies together when you’ve finished banking for testing as then you only have to pay for testing once (most testing is “for up to x embryos”). This might help keep costs down a little.
If they suggest ER without anesthesia to keep cost down, don’t do it if it’s not super expensive - for me was only €150 extra each time so worth that as it’s a drop in the ocean of the total cost.

    And AFM
Well we had the follow up with our RE on Wednesday and we are starting to prep for FET!
The decision on the Lupron might seem very familiar to some hehehe but we will do approx 3 weeks of it starting day 21 of my next cycle and hope that calms things down enough without over stressing my body with the Lupron side effects. Otherwise protocol will be same as for my ERA! It means, if all baselines and scans and labs etc go ok at each step we should be transferring around mid June sometime.
We were so excited Wednesday and Thursday- but reality and nerves hit yesterday and we were both very tense all day. We’ve talked it all through and we know our emotions will be up and down over the next months. That mix of excitement and pure terror is something else...
After all we’ve done to fight for those embryos it’s understandable I guess...
But in the meantime I’m going to try to relax and make the most of the time in between!
