  • @Ivorytower2 The MFM practice I’m going to is the only MFM practice in my mid-sized city and serves all the local hospitals. MFMs are the specialists you go to for these types of issues. There are other MFMs at the practice beside the dr I saw though and at minimum we would get an opinion from another dr within the practice before making a major decision like selective termination. Possibly I would travel further if it made sense to do so for another opinion (we are about 2.5 hours from two other larger cities). At this moment there isn’t much to get a second opinion on because we have only one data point - we literally had a scan days before where the CH wasn’t seen, possibly because the u/s machine wasn’t high level enough. 

    The particular MFM I saw is probably the least experienced MFM in the practice. She graduated 7 years ago and I think she moved here recently from out of state. On the one hand she probably has less hands on experience than some of her colleagues, but she also would have been trained and studied more recently and medicine and standard of care for monochorianic twins has transformed radically in the past 15 years with the advent of laser surgery for one of the most deadly conditions impacting monochorianic twins, twin-twin transfusion syndrome (which our twins are now at higher risk for due to disparate NT measurements). 

    Monochorianic twins are a rare form of twins (most are dichorianic) and then cystic hygromas are extremely rare so I wouldn’t be surprised if she has little first hand experience with this specific combination of factors despite her highly specialized training. I know some of the drs at her practice have decades of experience so when we know more I will likely ask to have an appt with one of the more seasoned doctors. Sometimes seasoned drs are old fashioned though so I think it’s good to have a range of perspectives.

  • @zamoraspin2 completely agree with getting both the “newer age” and “old school” perspectives. Wonder if anyone at another hospital would take you via a video consult if you end up going that route for a third opinion or beyond.

    It seems like you guys have done a lot of research and are really thinking clearly about this which is really amazing and hard to do under stress.

  • I've been taking a break from all platforms for a bit to destress, but got some bad news this week and thought I should give a final update. 
    Couldn't feel him wiggling anymore yesterday and I had a bad feeling. Midwives got me in to check and confirmed no heart beat at 13wks. Had been fine the week before and they couldn't see anything else abnormal on the scan. NIPT had confirmed the PGS test that he was chromosomally normal, so we'll never know why. Heartbroken. Waiting to have my D&C scheduled.
