Status (WTO/TWW/TTA): Waiting for FET!

What are you doing this cycle? (Testing? Treatment?): FET is on Friday! I started PIO last night.

How are things going?: I'm feeling better about my lining not being super thick. Thanks to everyone who weighed in on that issue. I'm trying to stay positive leading up to FET, but I keep having pangs of worry that my embryo won't survive thaw etc. Overall I'm doing okay. Just random worries here and there.

Any questions?: Any lurkers or those in the know have tips for what to do post-transfer? Did you all do all the old wives' tales of eating pineapple core, eating warm foods and wearing socks all the time?

GTKY: What does your user name mean? (If it's not too personal to share.) There is a twitch streamer that MH watches who is British. He curses a lot on his channel now, but in the beginning, he never did, and would instead say "Oh sugarpuff!" when something bad happened. Except with his accent it sounded like "shoogapoff". So I made that my twitch account name, and it stuck for many other accounts down the road.

How are things going?: Just met with my OB. We're going to meet again a week from tomorrow. If I'm KU, we start blood work and all that fun stuff. If I'm not, DH has a script to get a SA done and we will start medicated cycles. I asked which medication he was planning and he said Letrozole. So I'm excited to have a good plan going forward but hopefully we won't need it. AF is due Monday but I'm hoping it stays far away.

Any questions?: Probably a million but I literally just got home from the doctor's office so I'm still processing.

How are things going?: Pretty good! I took my last does of Clomid on Saturday night, so I’m starting to feel a little better. Hopefully I can get a good nights sleep tonight and continue to feel good. DH was super adorable and made me “rage cookies” to make me smile yesterday. They were delish. He is my favorite human. That’s pretty much it lol. I have a monitoring appointment on Thursday so hoping to get some good news about the follies!

How are things going?: officially CD 1. I called the RE and the nurse was seriously stressing me out. I schedule the first u/s for Thursday morning. There are a bunch of consent forms and some online videos we have to watch but she was making it sound like she was "making it work" for us to get everything going for this cycle since we hadn't done the videos yet. Like we did the blood work, I got a pap, we were doing things to prepare but if this gets delayed because we didn't watch a stupid 20 minute video I'm going to be pissed. I'm just feeling more anxious than anything.

Any questions?: not yet but I'm sure I will.

What are you doing this cycle? (Testing? Treatment?): Stopped the estradiol priming. Did several days of Letrozole already. Starting stims today. Will go for my first checkup on Saturday. I barely updated last week, but basically we had a second whirlwind trip to Arizona/Mexico for the second LIT treatment, and then the baseline ultrasound at Vida in Mexico/border of San Diego to start this new round of IVF. Coordinating the travel has gotten challenging and expensive. But *still* cheaper than doing it OOP in the States. Plus, I picked up some free medicine at the specialty pharmacy in AZ so that made it feel worthwhile in addition to the invaluable info from Dr.Q. This is getting long, so details about testing in spoiler.

We are still waiting on our DQ Alpha match test results a month later! The lab is re-running them, idk why. Supposedly this is normal. We got the other test results that indicate whether we need LIT, and I’m feeling really good about seeing Dr.Quiroga in Nogales because the test results appeared “normal” to my other doctors. Both REs said that the very low numbers show that my immune system is working great & fighting off any infections. Dr.Quiroga said my numbers must be much much higher to show that I’ve developed an immunity to MH’s DNA. He thinks this second treatment will do the trick. 🙏🏽

How are things going?: Stressful. Lots happening in my work life, and then this IF/ IVF stuff is like a second job so it’s been a lot recently. Just trying to stay calm, centered, & positive. ✨

Any questions?: Has anyone done their IVF meds at night only? My timing has been thrown off from travel and I was wondering about starting stims at night. I’m only doing once a day this time instead of twice a day shots.

Do you guys use OPK's for medicated cycles?

A little update, I spent an hour and a half on the phone this morning talking to the RE, insurance and the pharmacy but everything seems to be ready to go. I think it's funny that I will get my prescriptions covered but the treatments from my RE are not. Well if I hit my out of network deductible then it will be covered 60% which is after $16,000

But my baseline u/s is tomorrow and I start letrozole.

@krash_ I was for my first cycle, but then I ran out and asked the nurses if I should even bother getting more. They pretty much told me since everything is so closely monitored there’s no need. Sorry that your treatments aren’t covered!! That’s so frustrating.

Little update: my follies are better than they were on CD12 last cycle, but still not quite there yet. I have one at 13 and one at 12. I go back on Saturday for another u/s to take another look! Just over here trying to think positive thoughts haha

@krash_ I did for the 1st and 2nd cycles. I would say do it for your first (and hopefully there's no need for a 2nd), because when I went for my Day 11 u/s, my LH was already starting to surge (my lead follicle was 26mm at that time), and the Dr that did my u/s kind of 'panicked' that I might have ovulated early. She had me do an LH blood draw to check levels, which came back elevated and told me to HIO that evening just in case.

I don't really subscribe to the old wives' tales, but I admittedly did get McD's fries on the way home (it is literally on the same road as my fertility clinic and also yum fries so why not) and I did wear socks the week after. I drank a ton of cold drinks though and did a terrible job of relaxing/not stressing (but would say to def relax if you can). Oh, also, take the valium if it's offered, I really think that helped me. Will be stalking you to see how it goes!

Another Q, do you HIO during FW still? My only concern is that we are going to watch how many follies are matured and if there are too many then we wont HIO to avoid the risk of a higher multiple pregnancy but if we abstain for too long before go time then his swimmers might be a little slow.

@krash_ for a medicated cycle with TI, unless YH has known MFI and your Dr tells you otherwise, then yes we would HIO EOD during FW then once we got a little closer we'd wait until our RE gave us the green light for TI. Typically we wouldn't go more than 3 days without HIO just to make sure there was always a fresh supply, but that it wouldn't completely drain him.

Putting this in a spoiler for a TMI warning, lol.

On days we didn't want to HIO or were told not to I'd either make him masturbate or I'd do it for him until juuuust before climax then stop before he actually came. We found that by doing this we could fairly significantly increase his count. I used to call it "priming the pump"

He hated me for it but it worked to get his #s up so he went along with it.

@krash_ oh I'd completely crush him if I had to admit just how many times I never really got to finish before he did

his fragile ego could NOT handle that LOL. So yes, it makes him very grumpy but we turned it into a game so now when I'm tired I tell him we can "practice" priming the pump aaaaand then I let myself off the hook. Ha! Given how much it has impacted his sperm count for the better, he's been all for it just for that temporary boost in confidence that he feels like he has more to contribute, literally and figuratively.

Okay, I'm back for all my tags! It's been a crazy week work-wise, trying to finish up work so I can have a long weekend. Here we go:

@And846 I completely agree with you that the dailies can get overwhelming; I've multiple times considered not posting there, but ultimately, I just end up taking a break until I feel like I can jump in again. Usually I take a break once I gotten a BFN around 12DPO and waiting for AF, and in the first week of my cycle. Hopefully participating here and in the other weekly threads will give you what you need from this community.

@runningoncookies I'm so happy you have such a supportive H! And I'm sorry the Clomid affects you so much. But hopefully those 2 follies grow sufficiently by Saturday and that your IUI goes ahead soon!

@krash_ I'm sorry you got the 3rd degree from the nurse. From what I gathered, finding good fertility care is very difficult and having shitty fertility makes you lose trust in providers. I hope that you only get good nursing support from here on out! Also, re: HIO, since MH has a known MFI, we HIO then day before my monitoring u/s, that way if I'm ready to trigger on day of the monitoring u/s, with an IUI 36 hours later, there is a 2.5-3 days of abstinence. Although, the priming the pump method that @kiki047 recommends is extremely curious. I... will bring it up with MH but I fully expect to hear "HARD PASS!" 🤣

@keikilove so happy to get an update from you this week! You've been a busy lady! I can definitely see how your treatment and travel you have to do for it feels like a 2nd job. It IS a 2nd job! I'm so happy to hear that Dr.Q thinks that this second LIT treatment will do the trick. Good luck with the stims! Are you hanging out in Mexico right now? I have so many questions for you and your treatment plan. And hooray for free medication! I 💯 agree that your treatments, even with travel, will come out being cheaper than anywhere in the US.

Diagnosis (if you've been): Hot off the press: multi-factorial infertility! Hahaha. Read on for more juicy details.

Status (WTO/TWW/TTA): TWW

What are you doing this cycle? (Testing? Treatment?): Was gonna do IUI, but that was cancelled in favor of Letrazole + Trigger + TI.

How are things going?: Boy oh boy, the last 10 days have been quite the roller coaster. So, gather round ladies, while I tell you all about it. Putting everything in spoiler because 1) it's really long; 2) the spoiler feature is being a total biatch right now.

Okay, so last Monday, I was feeling really positive ahead of my Day 10 monitoring u/s on Tuesday, because we froze sperm this cycle in case MH didn't have enough good quality sperm on day of IUI. Then Tuesday rolls around, RE recommends we cancel the cycle due to risk of a post-mature egg, which might mean waste of $. Since I complained ad nauseam here about cost of infertility treatments in San Francisco Bay Area, I really appreciated it. Needless to say I was devastated. But, by Wednesday, I had picked myself up and we had a follow-up with RE that evening and decided we should just go ahead with IVF. I then spent Thursday and Friday talking to various ppl at the clinic to see if it was possible to do an egg retrieval next cycle if we weren't successful this cycle. Well, by Friday evening, it was looking more and more like it were going to be possible! So, I was feeling pretty good heading into the weekend.

(I'll pause here to say... remember how I said I wouldn't do IVF before the New Year and just take the rest of the year to chill out and get mentally prepared? Well, that was out the window at this point. Reason explained below. Also, my anxiety meds are really helping me stay cool, calm, collected and positive.)

Then, on Saturday morning, I receive the second opinion report from Dr. Gleicher at the Centre For Reproductive Studies in New Year. I had applied for the 2nd opinion ~ 1 week earlier because this doctor is specialized in women with low androgens. This 1.25 page report was pretty succinct and contained a lot of ALL CAPS SENTENCES but the gist of it was: you have PCOS, but a relatively rare phenotype - phenotype D or Lean PCOS - (did you know there are 4 different phenotypes?) which, in women over 35, generally becomes hypo-androgenic. There was more written, but basically the take-away message was: DO NOT RUSH INTO IVF before we have an accurate diagnosis (you need more testing) as you're at a higher risk for miscarriage because 45% of women with this PCOS phenotype have Hashimoto's and 85% of those women have hyperactive immune system / another autoimmunity issue. Fun fact: this doctor made no mention on my 2009 ultrasound which actually showed PCOS-like ovaries. More on this later.

Okay, at this point, my heart once again plummets because I definitely don't want to rush into anything that could put me at a higher risk of miscarriage. So, as part of my 2nd opinion package, I paid for a video consultation and I called the clinic to book it. At the same time, I emailed my new RE a copy of the report. She agreed that we should wait and see what he had to say in person before proceeding, effectively putting our ER on hold. Once again, I was exhilarated by having a doctor that listened and didn't have an ego that prevented her from hearing what this doctor had to say.

Now, on Monday, we had scheduled a consult at Main Line Fertility Clinic (in Philly) because - I think I mentioned it last week or the week before - I have been looking into creative ways to fund IVF, and discovered that some clinics that do research, offer portions of the treatment for free. MLFC is one of those clinics and I had been pre-screened by them for an upcoming study which seeks to study difference in use of Crinone vs PIO to support luteal phase post transfer. However, this study is starting in January 2021, so in order to participate, I'd have to be ready to do a transfer in 2 months. Hence the rush to see if we could get an ER started next cycle. Anyway, we had our consult on Monday with a very lovely doctor and laid out the whole situation to her, including the 2nd opinion report which I promised to send to her. She recommended that we proceed with using eggs I had previously frozen, because we would learn a lot more about our infertility equation by doing so.

Side note here to explain the frozen eggs: in 2017, post a very traumatic breakup, and before MH and I were seriously dating, I started looking into/going through the motions of egg freezing because at that point I was convinced I was gonna be a single mother by choice in a few years. I completed that journey in Oct 2019, when MH and I were basically engaged (we got engaged 2 days after my ER). In June 2019, I was feeling really on the fence about doing it because MH and I were doing really well, and headed down the wedding/babies isle but since I'd already done all the work up, I didn't want to just abandon it. So, I had asked MH to do the at-home sperm test, and when it came back low everything, it basically convinced me that I needed to complete that journey. However, I did this in Canada! (Cause, home... and cheaper... or so I thought.)**

I had always considered these eggs as my "eggs of last resort"; I actually called them like that to my doctor(s), but here I had a doctor telling me that I should just use them because we'd learn a lot. I don't disagree with that... we don't know how good quality they are, and we don't know if MHs sperm can fertilize them. So, this will be a good learning opportunity. If we end up with 0 genetically normal embryos, we'll do an ER and at least I won't rely on these frozen eggs that are worthless. Waiting to do an ER for 3 months is not going to "hurt me." But I'm really hoping mine will be a situation similar to @misshelen86.

At this point, we're agreed with the doctor (who is really really really nice, and btw, a lurker in this community recommended her to me!) and she gave me a bunch of phone numbers to call to start the arrangements.

Okay, so then today rolls around and we have our consult with Dr. Gleicher. It was actually very good. He spent a full hour with us, going through our medical history in fine detail. So yeah, his conclusion is that I have this rare PCOS phenotype. In younger women, it's characterized by polycystic ovaries but normal weight levels, and normal androgens. This is unlike most common PCOS phenotype that has the insulin resistance, likely high BMI, anovulation, etc. What he said is as women with this phenotype age, their androgens levels drop. Which is my issue. He says that my FSH level is high for my age (9.23) and not consistent with my AMH (2.3). I also have a LOT of follicles on my ovaries; at my last AFC, the nurse didn't outright count them all, but hinted there were about 10 on either side. This is a lot of follicles for a 37 yr old. All that to say, nothing is lost. He says that before I do IVF (specifically egg retrieval), I need to boost my androgens so I need to take DHEA. Side bar here again: I started taken DHEA in August, after doing the IUI orientation at our original fertility clinic. Then in mid-Sep, I ran my list of supplements by our former RE and she said that I don't need to take DHEA because I don't have DOR. So I stopped. But then I retested my androgens again at the end of October, and with @keikilove recommendation read more about it in Rebecca Fett's book, re-started taking it on Oct 30. Now he wants me to take a different brand of DHEA (which is of course very expensive) and get my levels retested 6 weeks after starting the new supplement, and then monthly thereafter. He said what I have is andrenal DHEAS insufficiency, and not andrenal & ovarian kind, which is better. If it was the latter, "all hope would be lost" or something like that. Basically it means that once I correct my androgens, my ovulation should get much better and that would be required for a successful IVF round.

He also said that I need to get tested for other possible auto-immune conditions as well as inflammatory markers. In the meanwhile, he said "IF YOU SUSPECT THAT YOU ARE PREGNANT CALL US IMMEDIATELY" because "YOU WILL MOST LIKELY NEED IMMUNOLOGICAL AND HORMONAL SUPPORT IN YOUR PREGNANCY." I am mimicking his tone in the 2nd opinion report in which he hilariously uses ALL CAPS to make get his point across. After the call, MH said "if you look up 'professor' in the dictionary, this guy's photo will be there." I mean, he's been practicing in this field for basically ever.

He also said that MH's inconsistent SAs lead him to believe that we probably do have some kind of MFI; hence the multi-factorial infertility.

I hope you enjoyed my story thus far. Where are we at? Well, I'm currently making plans to ship my eggs from Montreal to Philadelphia. Also hoping that we got pregnant this month on our own. But mostly, trying to thrive instead of just survive!

Oooh! And I've also started tuning to The Egg Whisper live (on Instagram) where I can ask questions in chat! It's way better than submitting a question ahead of time, since it take about a month for them to get answered on her shows.

**So, I thought that doing the egg freezing in Canada would be cheaper, and it some aspects it was. For e.g. I paid $2000CAD for 10 years of storage. But, my protocol only had me come in for 2 ultrasounds (Day 6 and 9) and 2 blood tests, which I now know is very different from how things are done here in the US. Are those extra appointments necessary? I don't know... but I do feel that we could have gotten more eggs fro me since my AFC at the time was 22. 🤷‍♀️I know that statistically speaking, we're looking at getting about 1.5 genetically normal embryos from those eggs, but a girl could hope that it's more! 🤞🏻

GTKY: What does your user name mean? (If it's not too personal to share.) Well, it was supposed to be a placeholder user name while I thought of something clever because that's how I am. And then I realized that it in itself was a clever user name, so I kept it! (I mean, originally I was going to use one of my frequent usernames, but I figured those were quite identifiable.)

Thank you everyone for your well-wishes and tips! FET is in the books! My embryo was graded a 5BB at thaw, so that's good that it wasn't downgraded. Im relaxing at home. My BFF is bringing me Whataburger fries (can you tell I'm a Texan?)

Also, I thought this was really cool: I bought new nail polish to wear to the transfer. I picked out an olive green color I liked. Then I read the name: "Precious Cargo" !! I thought that felt like a good omen.
