Re: November IVF

  • @bethann1022 I knew you’d be right! So glad you didn’t go with their flow!!  Sounds like a big batch of eggs!!
I am not going to do contrast dye... i just feel like it’s too risky just to look at my uterus... my mom was hospitalized after using it and I’ve heard too many stories... it’s not life or death so to hell with it. I don’t know if they got any usable non contrast images as I was sobbing and possibly not perfectly still (though I tried) I might consider an open unit, but first that woman needs to make sure she absolutely needs me to do it... felt like I was going to outer space in that damn thing!!
@mac627 @baseballhoney thanks ladies... had a lovely glass of sparkling rose, and it helped!!

  • Phew! Good thing you insisted on going in today @bethann1022 ! Yay for triggering! Hope it goes well for you!

AFM doing some research tonight- yes that’s my Saturday night while DH is away lol. Any thoughts on transferring one vs. two embryos? It seems like it depends but was just wondering if you ladies had any thoughts.

  • @mac627 how old are you? Under 35 and with generally good numbers they will prefer 1. Older and depending on your dx and 2 might make more sense. I xferred 2 during my (tw) positive but ending in mc FET. Only one had implanted. But I’m in my 40’s so that’s pretty standard
  • @dragonette505 I’m 31 but my diagnosis is DOR so idk where I fall. It might also make more sense to do 2 on a 3 day transfer but 1 if we have a 5 day transfer?

I still think in your case they may look at the embie quality... if you  seem to have a bunch of strong ones it will make more sense to transfer 1 on either day and freeze what’s left if any, but if it’s looking iffy I say toss them back in... I know some girls your age (and younger) that put 3 back (because the third was looking so unlikely). And DOR is still a siding scale... I’m more AMA than DOR but I’m really both.... I just get treated more for age issues than numbers issues. In your case DOR may or may not indicate difficulties with quality.... if you look like you’re having good embie quality (even if the numbers are lower) they may say 1 is best....
eta I meant at the end one is best not isn’t best... durn autocorrect
  • @dragonette505 Thanks for all the info! I guess I’ll see what they recommend and go from there. I’m anxious to do 2 because of all the obvious reasons (finances, upping the chances, etc.) but I also understand that multiples would be higher risk.

So I’m officially transferring day 5 on Tuesday! I’m glad because then I can go with my DH instead of my friend so that’ll be nice. The embryologist said I have “really nice embryos” - yay! The phone call woke me up so I didn’t get too much more information but she did give me the status of each. I have 5 8-cell grade 1 and two others that aren’t quite as good. Hope they keep growing! Feeling optimistic.

@bethann1022 my clinic also doesn't do PIO with fresh transfers, so FX for you if a fresh is what you want! (I am doing fresh as well unless I develop/am at risk for OHSS)
@dragonette505 I am sorry your MRI was so horrible
@mac627 My clinic is super against transferring 2 unless you are over 37. They also only do day 5 transfers though, so if you were doing a day 3 maybe 2, but since you are doing a day 5, I would probably only do 1, but thats just me

AFM- I'm just chugging along on my estrogen. Its making me a bit nauseated, but other than that, no side effects really. I am suuuuper sick though from some nasty bug I caught on a plane (of course. Plane people are disgusting). Someone send help for my sinuses!
  • @kiwi2628 yes! Plane ppl are gross!!!
  • @dragonette505, I'm so sorry for your horrible MRI experience. Hopefully they can figure out a way to get the info they need without making you go through that again.

@kiwi2628, sorry to hear your sick, that sucks. Planes are a breeding ground for germs. Ugh.

AFM: unless something drastically and surprisingly changes on my bw today, we are triggering tonight for retrieval Tues morning.
  • @kiwi2628 hope you feel better soon! Being sick is the last thing we need during this process! Ugh!

@zwink1 yay for triggering! Hope you get the call and can do the ER with time to spare before the holiday!

  • @2MomsHoping how are you feeling on your day without injections?

@mac627- awesome news about going for the 5 day transfer and it sounds like you're embryos are progressing nicely so hopefully you'll have some to freeze as well!

@zwink1- Good luck with the trigger tonight and your retrieval on Tuesday, how were your ultrasounds and bloodwork?  Do you have an estimate of mature follicles going into retrieval (of course we all know this number can be very inconsistent with actual mature eggs).

I am ready to get these eggs out tomorrow that is for sure.  The RN yesterday told me not to expect a call today unless there was an issue so you can imagine that when I received a call from a Restricted number and answered and it was my RE himself, I got really nervous for a minute.  He started to review the two triggers I took last night and the bloodwork results from this morning, and all I could think of was that the lupron trigger failed to mature my eggs or he was expecting a poor result tomorrow.  Instead, it was sort of the opposite, but good and bad.

My progesterone and LH look "excellent," but my E2 jumped from 2300 to almost 4000 today and I am at very high risk for severe OHSS if we do a fresh transfer.  I have had mild OHSS in the past, but of course it never gets bad because we do freeze alls so once my ovaries rest for a few days, they start to feel better.  I was disappointed to say the least as I really wanted to try a fresh transfer since I have had 4 failed FETs already this year.  I asked if we could wait 2-3 days after retrieval to see if I have any signs of OHSS, but he said even if I don't or just mild, a hopeful pregnancy would still put me at risk for severe.  I had a strong feeling that I would be changed to an FET tomorrow at retrieval, as my ovaries are already larger and more swollen than my previous 2 cycles and I remember at the first retrieval the RE wasn't expecting OHSS and came out after, when I was still recovering from anesthesia and told my husband that I was at very high risk of OHSS and I needed extra rest and was prescribed cabergoline. The cabergoline itself made me not feel very well, dizzy and nauseous and because I didn't start it until the day of ER, my RE thought it was too late to have much of an affect anyway.  So an FET it is I guess, on a high note, he said he is actually pleased that my E2 rose so much in terms of egg quality and quantity as he expects the dual trigger matured them nicely, so for that I am happy.  I guess we will wait and see how many eggs we get yesterday and at least during the 5-6 days while embryos are growing, I will have a busy 4 days to stay occupied.

  • @bethann1022 so sorry you won’t get to do a fresh transfer. That’s so disappointing. But it sounds like you’ll get some great eggs! So when your body is ready for a transfer you should be all set! Hope the ER goes well tomorrow!

  • @bethann1022 us and bw were good, and the plan is the same, so I'll trigger tonight. I haven't really asked my RE how many mature follicles we have because I sort of don't want to have any expectations. She's measuring 10-11 follicles per ovary right now, and said it's good that they're all growing fairly consistently,  so we'll see.

I'm sorry you won't get to try a fresh transfer, that's disappointing, but hopefully all goes well with retrieval and those 4 days fly by!
  • @zwink1 yay for triggering! Good luck tonight!
@bethann1022 ugh.... I got mild ohss this time (got High 3000’s myself) but I was already not planning a transfer. I know how much it sux... hopefully you won’t need the cabergoline... it does make you feel gross.  Sorry you have to freeze all, but I hear it’s wretched to deal with ohss and pregnancy :(
  • @dragonette505 that sounds just awful.  I can’t handle tight spaces like that either or the noise!

@bethann1022 yay cycle buddy.  Good luck tomorrow!  I’m sorry that you got switch to a FET I hope you are ok and the OHSS doesn’t get to bad and though it stinks to be delayed it is best to be cautious.  Fingers crossed that you get lots of mature eggs :)  I put all my sharps containers away since we aren't doing POI for the transfer. It's so odd to put it away I keep feeling like I should be doing something.

@mac627 I’m totally biased on this but I’m a one embryo kind of girl.  Depending on your age and history a twin pregnancy from two embryos may be pretty small BUT Twins are risky business and I have had three close families to me have heartbreaking outcomes from twin pregnancies.  I think the risk is not given as much credit and I would much rather wait and save that embryo for another transfer one day.

@zwink1 yay for your trigger!  Have a great retrial on Tuesday

I’m excited to get these eggs retrieved tomorrow.  The retrieval is early enough that DW can take me to the clinic and hang out for a little while while we wait.  She will probably have to leave about 15 minutes before they take me back but this is definitely better than nothing.  We lined up a car service but I had a change of heat this afternoon and called up one of my besties who was extremely understanding and she will be meeting me there and taking me home.  She also reminded me of a story from about 20 years ago when I picked her up from an embarrassing medical procedure.  It kinda warmed my little heart and melted all the stress away.  I’m feeling good!

@dragonette505 Ugh, that sounds awful. I've never had to have an MRI, but I suspect I'd feel the same way as you. I do not like enclosed spaces.

@bethann1022 Good luck today! I'm sorry about not being able to do a fresh transfer. I have similar fears since I've had mild OHSS before with very low dose Gonal F and 10 follicles. We're aiming for a fresh transfer but I may have to ditch that plan and be forced to do a FET as well. FX everything goes smoothly today!

@2MomsHoping Good luck with the ER today! Glad your DW can make it for at least part of the appointment. So glad you could find a friend to bring you home instead of having to use a car service.

@mac627 Wohoo! Great news about your embryos! Just one more day until transfer!!! My clinic usually transfers one embryo unless there looks like there are quality issues. Because I'm doing a government-funded cycle, the rule is one embryo only if your eggs are under 35 (so even if you're 37 but the retrieval was done prior to 35, you can only put one embryo back). If we happen to have embryos to freeze and we have successive failures, we will likely opt out of the funded program once we get down to our last two embryos and pay OOP to have two transferred.

    @zwink1 Yay for triggering!

@kiwi2628 Ugh, sorry you're so sick. Hope you're feeling better soon. I'm also just chugging along on my Estrace. I do notice that I'm a bit nauseated here and there, but I actually never attributed it to the estrogen because I usually feel that way post-O anyway. I do notice that about 2-3 hours after taking the pills I hit a WALL. I get soooo tired. Seems my innards are also feeling a little sleepy as they're a bit...sluggish too. Colace has been my friend this week! ETA: How much Estrace are you on? Most women in the posts I've read are on 4mg/day but I've seen some that are more or less too. I'm on 4mg/day (two, 2 mg pills once a day at lunch time - I take too many other supplements in the morning!).

  • Good luck today @bethann1022 and @2momshoping!

@mac627 how did the PIO go?

@dragonette505  my clinic has us take antibiotics for any of the injected dye x rays. the clinicians say the risk is low that most doctors don't require you to be on antibiotics before the HSG etc.

@kiwi2628 Just so you have this for reference... I went in with 8 eggs and I am stiming super slow.  Once they saw 12 eggs yesterday they took me off the menopur and now I am still on the gonal -f.  They said they could back down on the meds easier than ramp up. Hopefully the dosage works out well for you.

    @zwink1 congrats on ER tomorrow

AFM- Thank you ALL for giving me a few sane moments when reading your responses to my IVF interfering with my grandfathers funeral.  It's hard to have other people get it. I am super slow at egg growing. My cycle has always been 45-60 days so it's no surprise they think I am going to take 14!!!  Holy pins and needles- batman!  I am no longer on the menopur so that is great.  I am sizes 4-10 Qty 12 (after 6 days of stims)  Crazy.

  • @sunnyinatx I never even let them do the dye :( ugh.... Also.... I know families can get a little out of control when someone passes.... Obviously you would be with the family as much as you could... IVF stops for nothing and no one... once you are in a cycle, youre in. Its a terrible thing to have to deal with along with the natural anxieties you have during a cycle... so sorry youre dealing with all that. BTW I have totally stimmed for 14 days... I tend to be a slow grower too :/
@tosh24 Estrace kicks my *ss too. By the time I'm done with priming I'm SO done.

  • Hi everyone I’m happily back home.  Everything went well and we got 12 eggs!  12!!!! I’m shocked and I made the nurse double check to make sure she had the right patient.  Definitely far exceeded my expectations.  Keeping my fingers crossed for the fertilization report tomorrow.  Now for some pj’s and a movie marathon.

@sunnyinatx I’m sorry your family doesn’t understand but if I were in your shoes I would be doing the exact same thing.  Try not to let it get to you and keep those little follicles growing.

  • @tosh24 I am on 2 mg twice daily (q12h). The nausea is definitely from the estrogen, but its honestly not too bad.

@bethann1022 I am sorry your E2 jumped. Hopefully that means lots of happy, mature eggs, but I am sorry you don't get to try for a fresh.

@tosh24 (sorry I had to do this twice, my computer spazzed out)- also now that you mention it, my GI system is also moving slower. Womp womp.

@sunnyinatx thats what my doctor said too. Since I have variable AFCs (I have had anywhere from 4-18 as a count) he would rather over-stim me to start and get more and back off, than under-stim.

@2MomsHoping Yay for 12! I hope so many of them fert and turns to blasts!
  • @2MomsHoping so happy you got 12! Hopefully you can get a bunch of great blastocysts from those.
  • @2MomsHoping yay!!! That's such a good number! Cant wait for tomorrows Fert report!
  • @2MomsHoping wow! What a great result!! Can’t wait to hear about the fert report!

@sunnyinatx giving myself the pio wasn’t bad! I worked myself up more than necessary ha. I didn’t find it any worse than any other shot other than the angle obviously made it hard. I also found massaging the area and using a heating pad immediately after really helped make it less sore!

I’m now in IVF#4, different protocol and working on staying positive that THIS is our cycle.  I’m thinkin’ of you, girlie (hug)
@dragonette505 wahoooo! Nice number of blasts, lady! Good looking and strong are the best words those embryologists can say, amirite??
@2MomsHoping congrats on the nice egg haul! Now rest up! FX for a great fert report :)
    @mac627 GL tomorrow!!!

AFM, just plugging away at shots... two in the morning, two at night. Ooof, they hurt this time around for some reason.  #pincushion I’ll go in for my first monitoring Wed. Baseline AFC showed 5 on one side and 4 on the other...here’s to growing ‘em slow and getting a bunch more to pop up!!! Estimated trigger looks to be next Monday(ish) for a Wed retrieval if they have me stim 10 days...
  • @mac627 good luck tomorrow!

  • @dragonette505 congrats on 3 good looking blasts, you now have how many banked? Excited for you to start transferring!

@mac627 good luck at transfer, happy to hear PIO wasn't as bad as you expected

@2MomsHoping awesome outcome, 12 eggs! What was the number of follicles they told you a couple days ago if you don't mind me asking, just curious to see the comparison. Can't wait to hear your report tomorrow

My retrieval went well earlier, honestly I was expecting around 15 as in previous cycles based on last ultrasound and e2 on trigger day, bu t we got 25! I was so uncomfortable this morning and the last 2 days from how large my ovaries got this cycle, that I actually felt relief after the retrieval was over. My last two rounds I woke up with severe cramps and pain, but this time even with 10 eggs, felt so much better! My RE unfortunately did not do the retrieval but I give credit to the one who did because he must be very skilled, I always felt like the doc at my old clinic who happened to do both my retrievals was rushed and am thinking maybe it caused more inflammation.

Strangely though, the RE who did the retrieval called and left me 2 voicemails before I even left, actually only about 30 mins after the retrievasl, stating that there was a problem with my husband's sample and the count was 1mil so they wanted to do ICSI. The weird thing is, we arrived and he gave them sample at 1030, I met with the RE at 1230 to discuss procedure and confirm regular fertilization, then was taken back for retrievaal and he called me at 145. They didnt know this at 1230 when he met with us? Als o, his count has always been over 120mil, 50mil washed, so a drop to 1mil from 2 months ago seems unlikely. I'm thinking perhaps maybe a lab problem causing issue with his sample or perhaps after seeing eggs maybe they felt they would fertilize better with icsi? They kept repeating the good news is that my insurance will cover because of their 1mil requirement and I know my RE mentioned they may prefer to do icsi and would try to get insurance coverage. Sorry to go on about it, but just find it off since we've always had a 75% feet rate with regular IVF, why change now.

They also could not tell me specifically what days I would get updates except tomorrow with mature and fertilization report. Hoping and praying for good numbers

I did call my RN after and asked if my RE might be willing to consider fresh as both the nurse manager and RE who did retrieval were surprised it was a freeze all. They said typically they use trigger day estrogen which was 2100 as guideline asnd also lupron trigger should prevent ohss. But my RE called himself to say i was still very high risk and they didn't know the severity of my PCOS and my history. I told him I was just hoping to try something different since I've had 4 failed FETs, he understood but my health is obviously our primary concern. So hopefully we will have an FET in Dec or January.

At this point I am just happy to have these eggs out of me and start recovering from this stim cycle!

  • @bethann1022 I have 8 frosties now! No transfer till I figure out what’s up with my uterus (septum, scar tissue, polyp)
Congrats on the big egg haul!! Crazy good! You should def keep an eye out for ohss...

@2MomsHoping congrats on good ER! I hope you're feeling okay, and that a bunch of those 12 fertilize!

@mac627 GL with your transfer tomorrow!!!

@bethann1022, I'm sorry for the confusion following your retrieval, but congrats on the fantastic number!
