IVF after 40

Hi all,

I won't bore you with my 5+ year backstory because it's too long and awful, but I have a quick question and I'm hoping people will weigh in. I'm trying to decide how much I should push my doctors to get me in for another IVF retrieval in December, as opposed to having to wait until February or March. I would need to push HARD - like refusing some genetic screens and being a real PITA to the  nurses, but I feel like those 2-3 months might make a big difference since I turned 40 at the end of the summer. I do not have diminished ovarian  reserve, but egg quality might be a concern. I haven't had a fresh retrieval in years, so I have no idea what how it will go.

Tl;dr: How much does each month REALLY matter after age 40?

    I don’t think that few months would make much difference.

    I’m 42 and I have only 1 embryo left. So I asked my dr if I should squeeze in IVF quickly before using my last embryo. Her answer was: “few months here of there won’t make any difference”

    Surgery 10/2015: Planned to start trying again but had a surgery. (Not related to fertility)
    Surgery 5/2016: Planned to start trying again but had another surgery. (Not related to fertility)
    IUI  #3-5 (with Clomid): summer 2016 BFN
    IVF #1: 11/2016. 30R; 21M; 20F; 8B (6 day5 & 2 day6); 4 normal after PGS
    Medicated FET #1: 1/31/2017 transferred 1 embryo 3AA. BFP. Embryo stopped growing at 6w 1d. MUA at 9w 3d.
    Medicated FET #2: June 2017 - cancelled...
    Hysteroscopy #2: June 2017
    Medicated FET #2: 8/7/2017 transferred 1 embryo 5BB. BFP. Ended in CP.
    Medicated FET #3: 10/11/2017 transferred 1 embryo 3AA. BFN
    ERA: December 2017 - need an extra 12 hours of PIO
    Medicated FET #4: 1/24/2018 transferred 1 embryo 4AA. BFN
    Out of embryos.  :'(

    I think prudence9-2’s response is helpful.  I may not technically count as my last retrieval was not quite a couple of weeks before my 40th birthday last month.  But I’d done three retrievals this year (Jan-Jun - which felt back to back because of luteal lupron).  I get the now or never feeling, but we took the summer off (upped the ubiquinol to 600mg/day) and my September cycle (with a new RE, new protocol, new lab) was our best by a lot (not sure if you can see details in my signature). I understand that everyone is different, but I don’t think everything instantly went south when you turned 40.  I think the CDC age categories for ART cycle outcomes are 38-40 then 41-42, in which case, you haven’t even moved to the next age bracket yet!
