@shortstack930 I know you touched on this in a previous post, but they want you to HIO before YH gives a sample for the IUI? Wouldn’t that hurt the numbers? (I thought it was 2-3 day abstinence) Maybe that’s just if MFI is an issue? Anyway yay! Hopefully I won’t be too far behind you

I've heard conflicting advice about whether "every day" or "every other day" is better, anyway....
@RVAmom315 they've always had us HIO either the day before or two days before IUI so that the sperm is better quality. The RE had said even if there were fewer sperm they would be good quality and would be put right up there during the IUI so abstaining wouldn't make a difference. I would imagine it would be different if MFI were an issue though.

So, holy s**t, unexpectedly, we are rolling! Consult went really well. I got -most- of my questions answered. I left a few for later be sis my RE was being a chatty Cathy and we never would have gotten out of there, so I thought I'd leave the FET questions for later. So anyway, she does a 21 day birth control protocol to prep the ovaries. I was supposed to call/text her on CD1 to let her know it was CD1 and then start the BC that day... weeeeellllll I got back to my office, did some work, and took a break...only to find that CD1 had started (as far as I was aware, I hadn't even ovulated yet so talk about a surprise! Apparently this cycle was annovulatory, and short for me). Soooo, I started BC tonight. She's sending the calendar our way, but it's looking like a mid-Nov retrieval.

Eek! So I had my consult this morning- my doctor is gonna start the ball rolling once CD1 comes. We will still do a medicated cycle next month (that will be cycle 6 for insurance), but post-ovulation next cycle my RE is gonna start me on something (I think estrogen?) and then the following cycle we will start swimming and do IVF. He thinks mid-December for ER! He was very hopeful about my chances given my age and bloodwork. He did say that depending on egg numbers he would like to do half 'natural' fertilization and the other half with ICSI. His goal with me is 15-20 eggs, and he said he is gonna use a slightly higher dose of meds with me than he would with other 30 year olds since I have had weird AFCs through the cycles. Like, when they first looked I had 16 (great!), the next I only had 6. The next I had 12. The next I had 8, then I had 14...so he is gonna do more drugs with me hoping I get slightly more than less.

Well, sometimes mine seems to work perfectly fine. And sometimes it takes
FOREVER to beep and is still incrementally working it’s way up. Sometimes the
temperatures just seem flat-out wrong. So I think after over 2 years of using
it, the new one is the way to go to comfort myself that the temps will be
actually accurate. Not, well, hmm… that seems off… is it the thermometer or is
it me?

@kiwi2628 sounds like a great plan!!! I wanted to ask my doc about doing half natural and half ICSI because I really wanted to also see if they would fertilize. Did he say why he was interested in doing it that way?
I wrote a super long summary of my RE appointment yesterday
on WTO. If anyone wants to take a look, I’d love to hear your feedback! The
short version is that all my tests look normal and my diagnosis is unexplained
IF. The RE is recommending letrozole + iui. I felt like the RE was not
personalizing things very much and was instead just following a formula of “unexplained
= iui.” He showed no inclination to talk about why I might be experiencing IF,
seemed to think there was no further testing we could do, and wasn’t interested
in any of my bw except for thyroid and AMH. Getting a second opinion/new doctor
would likely delay things a full cycle, which wouldn’t be great. My insurance
covers IF treatment (but not IVF) up to $5k per year, so if I could get two
cycles in this year, I could potentially afford up to 4 more next year, if
needed. Right now I’m leaning towards doing one cycle with this RE while
simultaneously scheduling a consult with a new RE. Thank you all for the
support and suggestions for questions to ask!
@madspunk The clinic actually immediately provided us with a
print out that told us how to contact them with more questions, who to talk to,
how long it would take for them to respond, etc. I guess they get that question
a lot! I’m sorry ttc is feeling like such a downer right now for you. I know

BBT for several years and it still seems reliable. I’m sure the various brands

mom is pretty against iui and ivf though, because she’s basically suspicious of

@beachbunnyxo123 he sometimes likes to do that with his unexplained because sometimes it can give us the cause of the infertility, while also not wasting eggs. Like, if the problem is the actual sperm and egg combining, either due to poor sperm quality or a problem with the acrosome, or a problem with the shell of the egg dissolving and allowing sperm to actually get in, or even egg quality, the only way to tell thats the problem is to get eggs and sperm together and see a failure of fertilization. So we could do all natural, see a majority fail to fertilize and know thats the problem and next IVF cycle do IVF...or we could just do ICSI and bypass those things, but if you just do ICSI you never know thats the problem. If you do half natural and half ICSI, you give yourself both options- you get to see if that is the problem, but you also give yourself the chance at not wasting an IVF cycle if that is the problem.
Does that make sense? I got like no sleep last night, so I wrote that 3 different times and I am still not sure I am explaining it well.
@kiwi2628 that makes perfect sense! Thanks so much for the information I’m gonna present that to my RE. When I spoke with another RE from my practice they say we should do all ICSI because of better chances but I wanted to know the same thing since we are unexplained is that part of the reason for infertility. I am going to memorize this explanation in my head and talk to my doctor about it in preparation for our RE!
@beachbunnyxo123 I should say that my RE did say it was all mature egg number dependent- like if I only have 8, he would recommend ICSI for all just in case. If I have 20, then he may do half and half. He also says for most people with unexplained they start with natural, but I made it clear I wanted to be....aggressive, so he said we could do ICSI to up our chances.
@darkstar42 - what was YH's SA like? Did he test normal too? You may have already mentioned his results on another post - sorry if I missed it!
I guess the protocol makes sense since this is your first rodeo. You probably shouldn't jump into injects until you see how you respond to gentler stimulation. Sorry your RE didn't give you more personalized care, though I think these doctors get into a pattern and think "This worked for the majority of my patients in your situation so it will work for you, too." Given your insurance covers IUI starting with that to see how it goes seems like a prudent choice.
Are there other REs nearby you can see, or anyone else in the same practice who has a better bedside manner? That can make a difference in how satisfied you are with your treatments, for sure.
@darkstar42 per your concerns about your RE: its super important to like and trust your RE, so if you don't feel that, absolutely look into finding another. With that said, a lot of IF is kind of cookie cutter. With few exceptions: try medicated IUI hoping it works, then move onto IVF if it doesn't. Thats especially true for unexplained, since there is nothing concrete to treat/chase after, they try medicated IUI first and then after 3-4 cycles move on to IVF, so I wouldn't worry about treatment being too personalized on the first cycle. They can also tailor it more based on your response to the first cycle if they feel the need.
@wishiwaspreggo MH's SA was good, except that morphology was a little bit low (3%, when the cutoff for "normal" is 4%). The RE didn't seem at all concerned about that and didn't want to do any further tests on MH. My AMH is relatively high (5.72), so the RE thinks I would respond well to a low dose of letrozole (5mg). We were actually hoping for a less aggressive approach to begin with, rather than more aggressive. Mostly because I still haven't come to terms with an IF diagnosis. @kiwi2628 That is very helpful to hear. I think this is all so new to me that I keep thinking, yeah, but there must be SOME explanation! Basically I'm having a case of "I'm a special snowflake!" I'm also nervous about the medications because I tend to be someone who gets the weird drug side effects. When they say "only 7% of people will experience xyz side effect," I'm usually in that 7%. Taking hormones scares me because, although I logically know this is a totally different drug, birth control pills were a nightmare for me. Major mood effects on three different very low dose pills. I think switching to a doctor who will acknowledge my concerns would make me feel a lot better about everything.

@Redpuma119 this is my second cycle on letrozole. I have had like no side effects from it. None. I didn't produce as many eggs on letrozole as I did on clomid though (2 as opposed to 3 or 4)
@kiwi2628 yes that completely makes sense depending on how many mature eggs are retrieved I totally agree with that!
@Redpuma119 I’ve been on letrozole for about 7 different cycles. Barely any side effects at all I was just teary and emotional at times but any hormone med will do that. My REs never put me on clomid. Only thing is I produced about 1-2 mature follies from it each cycle which could’ve been the same for me with clomid though so who knows
@RedPuma119 This is my third cycle with Letrozole. I have had very minimal side effects with it, just some bloating and moodiness, and I usually produce 1 mature follicle. However, my first very cycle was a clomid cycle where I ended up overstimulating, and for that reason I now only take a 2.5mg dose of letrozole.
Thank you ladies for all the well wishes! The IUI went well and I had a lovely nurse that kept singing "just keep swimming!" when they put the sperm in .
@Redpuma119 this is my first cycle on Femara and I haven't had any side effects. I was on Clomid for three cycles and had horrible side effects and produced 3-4 eggs each time. This cycle was just one egg.

@darkstar42 - Letrozole has minimal side effects so I wouldn't worry about it too much. Clomid seems to be the culprit for a lot of people when it comes to impacting mood, hot flashes, etc. You'll be on such a low dose I doubt you will notice anything.
I just got a very disappointing/concerning call from the RE’s office. Originally they said we wouldn’t get our day 5 report until Friday and I almost wish they would have just waited until then.
They said only 2 made it to day 5 and they are watching 2 more. They also said they will not be able to do PGS testing on any of them, given the number of cells that they are seeing. To add insult to injury, they still said we won’t be able to do our FET until December.
I feel like I’ve somehow done something horribly wrong. How did we go from 16-18 fertilized to only 2 making it? Even if the other 2 do make it, that’s a horrible percentage. And I am really torn up about not being able to do the PGS testing. I just want to leave work and cry.

*percentage is averaging comfort levels reported by 10 women done in a study 15 years ago. May not accurately reflect comfort levels in 2017 or your own experience.
purplg8r Well, sometimes mine seems to work perfectly fine. And sometimes it takes FOREVER to beep and is still incrementally working it’s way up. Sometimes the temperatures just seem flat-out wrong. So I think after over 2 years of using it, the new one is the way to go to comfort myself that the temps will be actually accurate. Not, well, hmm… that seems off… is it the thermometer or is it me?

@kiwi2628 sounds like a great plan!!! I wanted to ask my doc about doing half natural and half ICSI because I really wanted to also see if they would fertilize. Did he say why he was interested in doing it that way?

I wrote a super long summary of my RE appointment yesterday on WTO. If anyone wants to take a look, I’d love to hear your feedback! The short version is that all my tests look normal and my diagnosis is unexplained IF. The RE is recommending letrozole + iui. I felt like the RE was not personalizing things very much and was instead just following a formula of “unexplained = iui.” He showed no inclination to talk about why I might be experiencing IF, seemed to think there was no further testing we could do, and wasn’t interested in any of my bw except for thyroid and AMH. Getting a second opinion/new doctor would likely delay things a full cycle, which wouldn’t be great. My insurance covers IF treatment (but not IVF) up to $5k per year, so if I could get two cycles in this year, I could potentially afford up to 4 more next year, if needed. Right now I’m leaning towards doing one cycle with this RE while simultaneously scheduling a consult with a new RE. Thank you all for the support and suggestions for questions to ask!
@madspunk The clinic actually immediately provided us with a print out that told us how to contact them with more questions, who to talk to, how long it would take for them to respond, etc. I guess they get that question a lot! I’m sorry ttc is feeling like such a downer right now for you. I know that feeling. *hugs*

@sapphire86 Thank you! I am thinking about going to get a second opinion (or just switch to a new doctor) because I didn’t think this guy was great. I don’t want to delay treatment though, so we might do one cycle with him and then switch. I have to figure out how my insurance would handle that though. So exciting that your right ovary is responding so well! Fx for you!
@dpjennifer For what it’s worth, I’ve been using the same BBT for several years and it still seems reliable. I’m sure the various brands differ though.

@KristoKekerooni I felt like the suggestions in ISWTE helped me feel like I had a little bit more control over ttc, which was good. I would definitely recommend it.

@beachbunnyxo123 Thanks! It does help to have support. My mom is pretty against iui and ivf though, because she’s basically suspicious of most medical intervention for anything. So we’ll see how she handles things if we do iui.

I guess the protocol makes sense since this is your first rodeo. You probably shouldn't jump into injects until you see how you respond to gentler stimulation. Sorry your RE didn't give you more personalized care, though
Are there other REs nearby you can see, or anyone else in the same practice who has a better bedside manner? That can make a difference in how satisfied you are with your treatments, for sure.

@kiwi2628 That is very helpful to hear. I think this is all so new to me that I keep thinking, yeah, but there must be SOME explanation! Basically I'm having a case of "I'm a special snowflake!" I'm also nervous about the medications because I tend to be someone who gets the weird drug side effects. When they say "only 7% of people will experience xyz side effect," I'm usually in that 7%. Taking hormones scares me because, although I logically know this is a totally different drug, birth control pills were a nightmare for me. Major mood effects on three different very low dose pills. I think switching to a doctor who will acknowledge my concerns would make me feel a lot better about everything.
I am using a desk top and I am not seeing my lovetit button. Has anyone ever had this? I love titted everyone in my mind

@Redpuma119 I’ve been on letrozole for about 7 different cycles. Barely any side effects at all I was just teary and emotional at times but any hormone med will do that. My REs never put me on clomid. Only thing is I produced about 1-2 mature follies from it each cycle which could’ve been the same for me with clomid though so who knows

@Redpuma119 this is my first cycle on Femara and I haven't had any side effects. I was on Clomid for three cycles and had horrible side effects and produced 3-4 eggs each time. This cycle was just one egg.

They said only 2 made it to day 5 and they are watching 2 more. They also said they will not be able to do PGS testing on any of them, given the number of cells that they are seeing. To add insult to injury, they still said we won’t be able to do our FET until December.
I feel like I’ve somehow done something horribly wrong. How did we go from 16-18 fertilized to only 2 making it? Even if the other 2 do make it, that’s a horrible percentage. And I am really torn up about not being able to do the PGS testing. I just want to leave work and cry.
