@purplg8r I think Ovidrel was about $117 OOP for me. Letrozole was not OOP, but from what I understand, it's inexpensive. Progesterone leakage can be pretty bad. I take it 3 times a day, and I usually wear a pad after about the third day of taking it. If I'm feeling risky, I'll just wear a pantiliner, but I have had moments where the progesterone leakage was so bad that I wasn't sure if I was peeing myself a little or leaking.
purplg8r Super sorry I missed your earlier question to me! I am really out of it right now I guess! Anyway, from what I understand, a progesterone biopsy measures your progesterone levels. However, unlike the test(s) done on a certain day that measure your levels at that moment, the biopsy is supposed to give you an overall view of your progesterone levels in general. However, it sounds like no one else has heard of this test, so I guess I'm running on the faith of my RE.

Sorry for being so rubbish at keeping up this week- crazy busy at work and trying to emotionally deal with the hangover symptoms from last IVF cycle. But I had the follow up with my RE and I've put all the details in the spoiler (and when I say all, I mean everything I noted down...) so if any of you are having appointments soon you can use anything if you want - and of course if you get totally different input would be interesting to know! Thanks to all who sent good wishes for the appointment, it really helps to boost my mood when I get stuck in "what ifs".

@rvamom315 sorry I don't remember your issue - but in general the older you get the more aggressive they suggest to be. Did you get AMH and day 3 FSH results back already? And do you know what your CD3 AFC was?

@avrilmai thank you for being so open. I think anxiety is not spoken about enough. @50wife I've tried yoga a few times but could never get past the first few sessions each time. I use counting and also writing - I wouldn't call it so much journaling but just downloading my thoughts on paper. I don't do it often, just a few time she a year if I feel things getting too much.
@madspunk what progesterone have you used? I had the crinone gel and found so long as I clear out the build up every few days I had no leakage
@purplg8r sounds like you had a really thorough appointment too! Hope the SA shows good results today. See above I can recommend crinone gel for less leakage
@dpjennifer did they explain more about the progesterone biopsy? Is it like an ERA?

@wishiwaspreggo concert sounds fab! And like @adirat said it varies per person and per cycle. Without suppression or priming then most start CD3 and ER might be anywhere from 8-20 days later, then if fresh transfer 2-6 days after that, then your luteal phase should be normal length post ER (supported by progesterone) or AF often comes within a week if doing a freeze all (and thus no progesterone). There's a lot more to it than that but it should cover 80% of people.
Yesterday was my follow up. And I have to say bless my RE we were 1.5 hours with him going through everything!!! - First of all he did an US and there is no cyst, so that's good. Also I have one follie who looks like he's the "one" this month as it's CD7 and he's already 15mm. There are another two on the left around 5mm and one very small on the right. So an AFC of 4 which is really really good for me. Also my lining was already 9.5mm and tri-laminar. So he suggested HIO for the next 4 days ED until I fly to China as you never know! - The cramping I'm feeling he thinks is just the last effects from the progesterone. There was nothing to see on US but I've released my pelvic MRI to him (done by my RA) so he will check that to see if there's anything to see though we don't expect anything. - With my AFC of 1-4 and AMH of 0.65 he reconfirmed we should only expect 1-2 eggs each cycle. Whilst the insurance is paying for meds he said no harm to try again with meds but after that he's happy to take us without stims, just Ganirelix and trigger to not waste money and have less side effects if we need/want - We will start with same stims as last time (Pergoveris 225 - which is the FSH/LH combi) and potentially step down or step up. And if one doesn't work try the other in cycle 3. We don't add Menopur as it is the same as the Pergoveris. We discussed potentially using FSH only in a future cycle but he is concerned about lack of egg maturity then. - We discussed suppression and priming at length. He still thinks suppression will give no real benefits for me, risk not getting started well again after and will waste a1-3 months. With Estrogen priming with my low AFC he doesn't think it will help, but might add some Estrogen in after the ER with the progesterone this time so long as my levels are like last time as again at that point it might help lining etc and won't do any harm. For testosterone he said with my numbers he has never seen a positive outcome and again has seen negative ones so does not recommend it "pure". The DHEA he can't prescribe in country, however I could do self pay through his colleague in New York but they already discussed my case and neither of them thinks I'm a good candidate. - We talked HGH and this again is not possible domestically so he said we should think about it if we get to cycle 4 and take the additional prescription from his colleague and have it shipped in. If we use DHEA or HGH in an insurance cycle it would invalidate the coverage. But he said the research is so far only anecdotal (he even showed me on the screen the research he has reviewed again in the last weeks) doesn't suggest I would be a good candidate - As my follie seems to be selected early he's happy for me to start stims whenever I want CD2-4 - We will start monitoring on day 3 of stims (so after only 2 nights of injections) in case we want to start the Ganirelix earlier - He confirmed my embryo from last cycle was beautiful. I even signed off that he could use a photo of it in a paper he is writing. He said it was ahead of the average development until the point we transferred which he sees as a good sign. Also why he didn't want to change much in the stim part of the cycle. - But to give any future for embies the best chance we will go with assisted hatching from now on. He had already got pre approval from the insurance for it. - Having previously said I need three failed cycles before I can have further testing (or three MCs or a loss/child with proven genetic issues) he had taken my case to the genetics approval board and insurance company and has got approval to run full genetic screening for me as well as an RPL panel. So I gave 14 vials of blood today and signed all the paperwork! Results should be back in around 2 weeks so we can factor anything in before we start. - Based on my previous BW, what he saw in ER/ET, other history and conditions he is expecting to need to prescribe heparin but needs the results first as it's strictly controlled here - I asked about baby aspirin as a starting point and he said I should use it for this natural cycle until we get the results, but if results come back for any IVF cycles we hit it hard. - We talked potential intralipids and he will follow up with the national specialist Centre who is 1.5 hours from me to see if he can get me in (as it doesn't do any harm) - He has discussed with my RA and they are waiting for my BW from this Monday to come back but might double my RA meds through the next cycle to see if that helps, or add in some additional meds. This month no harm to go with the Benadryl as a temporary measure. - We talked acupuncture and he fully supports it. He said he can't explain why it has a beneficial effect always but again it does no harm and clearly shows a positive correlation to good outcomes so gave me some pointers and I'll be trying to get in at a good one tomorrow. - At the moment as we aren't freezing in any cycles due to low numbers then an ERA doesn't really help and if we do a mock cycle we are again delaying (and I'm really on a clock!) - but he said when we think it might be our last OE cycle he would suggest to do it before. But it's up to me, he would be happy to do it any time and then we would have to figure out a strategy to achieve it if it showed a problem with timing - Lastly I asked if he thought we were crazy still trying with OE and if he was just not saying but thinking we should go to DE. Here he said for him if I can yield mature eggs he is happy to keep trying - but it should not be about him, it should be about us. That there's the money - when is our limit and how many cycles can we afford. And the time/effort - we both have big jobs and especially for me each cycle means a lot of time out and limits on traveling. And of course the emotional part - he said some couples can really keep going for ten cycles without success whereas for others one or two cycles is the limit. He also reconfirmed that DE is not available in Germany, so we would have to get frozen eggs from another country (and go through some major legal hoops) or go overseas for DE treatment. But for now it's plan B.
Overall was a very intense discussion but I came away feeling really listened to, taken seriously and happy that my RE suggested half the things on my list himself without me asking. I feel like we are on the same path and I think that really helps in the tough times.

I'm so glad that you had such an in-depth (albeit involved!) discussion with your RE and really felt that they are on the same page as you. That is very important. I don't really feel that way with my RE, and that's something I have to really think about moving forward.
Update (DD ahead): On my last day of Clomid and I am an emotional mess. There was a miscommunication at work (it's not even a huge deal) and I'm just so sad and want cry. I just want to go home and curl in a ball! Ugh. And my gonal-f injectables haven't arrived yet, and the RE isn't calling back (and my nurse is on vacation this week). I'm not cut out for this

@RVAmom315 hang in there! Just breathe...you will get through this. You are strong and you are definitely cut out for it - for anything! It's okay to feel overwhelmed, but that doesn't mean you can't handle it. It just means right now you need to breathe and give yourself space to get through this.
Update: I called the RE again and got the nurse who was there with me on Monday. She gave me the number of the pharmacy to call for the injectables. They had the order, but no other information - ie my address, email, phone or insurance info!!! WTAF! And now I have to pay a $35 service charge to get it rush delivered on a Saturday.

Ok last update: I was a ball of anxious nerves and called the nurse again to ask what we'll do if I can't get the injectables in time - and they have not called back (I left a message with the patient advocate - twice - no call back). Anywhoo, I called the pharmacy and they said they'd been trying me all week to get my info (apparently the doctor's office doesn't provide them that info). So the long and short of it all is that I am all set to get my injectibles tomorrow, after paying $600 for them. So, it looks like this will be our one and only shot with this protocol. If it doesn't work, I'm going to ask my RE if there's another protocol we can do that doesn't involve injectibles (ins doesn't cover them). If not, we're going to go back to trying on our own for 3-4 cycles and try to save for 2-3 more IUIs in the spring.

I wanted to be able to tag everyone who sent me well wishes yesterday, but I’m mobile and it’s being goofy. We just got the call and I’m genuinely shocked right now. Of the 29 eggs, 24 were mature and 16 definitely fertilized with an additional 2 that might make it. I was NOT expecting numbers like this. Wowwww!!!
My husband is being so sweet and said, “So we’re like sort of parents?” I’m glad he feels connected to this moment and the embabies, since the MFI has been such a struggle for him. I can’t wait for the day 5 and PGS reports.

@purplg8r I think Ovidrel was about $117 OOP for me. Letrozole was not OOP, but from what I understand, it's inexpensive. Progesterone leakage can be pretty bad. I take it 3 times a day, and I usually wear a pad after about the third day of taking it. If I'm feeling risky, I'll just wear a pantiliner, but I have had moments where the progesterone leakage was so bad that I wasn't sure if I was peeing myself a little or leaking.

@50wife I've tried yoga a few times but could never get past the first few sessions each time. I use counting and also writing - I wouldn't call it so much journaling but just downloading my thoughts on paper. I don't do it often, just a few time she a year if I feel things getting too much.
@madspunk what progesterone have you used? I had the crinone gel and found so long as I clear out the build up every few days I had no leakage
@purplg8r sounds like you had a really thorough appointment too! Hope the SA shows good results today. See above I can recommend crinone gel for less leakage
@dpjennifer did they explain more about the progesterone biopsy? Is it like an ERA?
@laurad75 hope the legal stuff has gone smoothly with your donor this week
@wishiwaspreggo concert sounds fab! And like @adirat said it varies per person and per cycle. Without suppression or priming then most start CD3 and ER might be anywhere from 8-20 days later, then if fresh transfer 2-6 days after that, then your luteal phase should be normal length post ER (supported by progesterone) or AF often comes within a week if doing a freeze all (and thus no progesterone). There's a lot more to it than that but it should cover 80% of people.
- First of all he did an US and there is no cyst, so that's good. Also I have one follie who looks like he's the "one" this month as it's CD7 and he's already 15mm. There are another two on the left around 5mm and one very small on the right. So an AFC of 4 which is really really good for me. Also my lining was already 9.5mm and tri-laminar. So he suggested HIO for the next 4 days ED until I fly to China as you never know!
- The cramping I'm feeling he thinks is just the last effects from the progesterone. There was nothing to see on US but I've released my pelvic MRI to him (done by my RA) so he will check that to see if there's anything to see though we don't expect anything.
- With my AFC of 1-4 and AMH of 0.65 he reconfirmed we should only expect 1-2 eggs each cycle. Whilst the insurance is paying for meds he said no harm to try again with meds but after that he's happy to take us without stims, just Ganirelix and trigger to not waste money and have less side effects if we need/want
- We will start with same stims as last time (Pergoveris 225 - which is the FSH/LH combi) and potentially step down or step up. And if one doesn't work try the other in cycle 3. We don't add Menopur as it is the same as the Pergoveris. We discussed potentially using FSH only in a future cycle but he is concerned about lack of egg maturity then.
- We discussed suppression and priming at length. He still thinks suppression will give no real benefits for me, risk not getting started well again after and will waste a1-3 months. With Estrogen priming with my low AFC he doesn't think it will help, but might add some Estrogen in after the ER with the progesterone this time so long as my levels are like last time as again at that point it might help lining etc and won't do any harm. For testosterone he said with my numbers he has never seen a positive outcome and again has seen negative ones so does not recommend it "pure". The DHEA he can't prescribe in country, however I could do self pay through his colleague in New York but they already discussed my case and neither of them thinks I'm a good candidate.
- We talked HGH and this again is not possible domestically so he said we should think about it if we get to cycle 4 and take the additional prescription from his colleague and have it shipped in. If we use DHEA or HGH in an insurance cycle it would invalidate the coverage. But he said the research is so far only anecdotal (he even showed me on the screen the research he has reviewed again in the last weeks) doesn't suggest I would be a good candidate
- As my follie seems to be selected early he's happy for me to start stims whenever I want CD2-4
- We will start monitoring on day 3 of stims (so after only 2 nights of injections) in case we want to start the Ganirelix earlier
- He confirmed my embryo from last cycle was beautiful. I even signed off that he could use a photo of it in a paper he is writing. He said it was ahead of the average development until the point we transferred which he sees as a good sign. Also why he didn't want to change much in the stim part of the cycle.
- But to give any future for embies the best chance we will go with assisted hatching from now on. He had already got pre approval from the insurance for it.
- Having previously said I need three failed cycles before I can have further testing (or three MCs or a loss/child with proven genetic issues) he had taken my case to the genetics approval board and insurance company and has got approval to run full genetic screening for me as well as an RPL panel. So I gave 14 vials of blood today and signed all the paperwork! Results should be back in around 2 weeks so we can factor anything in before we start.
- Based on my previous BW, what he saw in ER/ET, other history and conditions he is expecting to need to prescribe heparin but needs the results first as it's strictly controlled here - I asked about baby aspirin as a starting point and he said I should use it for this natural cycle until we get the results, but if results come back for any IVF cycles we hit it hard.
- We talked potential intralipids and he will follow up with the national specialist Centre who is 1.5 hours from me to see if he can get me in (as it doesn't do any harm)
- He has discussed with my RA and they are waiting for my BW from this Monday to come back but might double my RA meds through the next cycle to see if that helps, or add in some additional meds. This month no harm to go with the Benadryl as a temporary measure.
- We talked acupuncture and he fully supports it. He said he can't explain why it has a beneficial effect always but again it does no harm and clearly shows a positive correlation to good outcomes so gave me some pointers and I'll be trying to get in at a good one tomorrow.
- At the moment as we aren't freezing in any cycles due to low numbers then an ERA doesn't really help and if we do a mock cycle we are again delaying (and I'm really on a clock!) - but he said when we think it might be our last OE cycle he would suggest to do it before. But it's up to me, he would be happy to do it any time and then we would have to figure out a strategy to achieve it if it showed a problem with timing
- Lastly I asked if he thought we were crazy still trying with OE and if he was just not saying but thinking we should go to DE. Here he said for him if I can yield mature eggs he is happy to keep trying - but it should not be about him, it should be about us. That there's the money - when is our limit and how many cycles can we afford. And the time/effort - we both have big jobs and especially for me each cycle means a lot of time out and limits on traveling. And of course the emotional part - he said some couples can really keep going for ten cycles without success whereas for others one or two cycles is the limit. He also reconfirmed that DE is not available in Germany, so we would have to get frozen eggs from another country (and go through some major legal hoops) or go overseas for DE treatment. But for now it's plan B.
Overall was a very intense discussion but I came away feeling really listened to, taken seriously and happy that my RE suggested half the things on my list himself without me asking. I feel like we are on the same path and I think that really helps in the tough times.

I'm so glad that you had such an in-depth (albeit involved!) discussion with your RE and really felt that they are on the same page as you. That is very important. I don't really feel that way with my RE, and that's something I have to really think about moving forward.

So the long and short of it all is that I am all set to get my injectibles tomorrow, after paying $600 for them. So, it looks like this will be our one and only shot with this protocol. If it doesn't work, I'm going to ask my RE if there's another protocol we can do that doesn't involve injectibles (ins doesn't cover them). If not, we're going to go back to trying on our own for 3-4 cycles and try to save for 2-3 more IUIs in the spring.
Sorry for the novel. I don't know if it's the medicine, or its Friday the 13th and today's been weird, but I am a bundle of anxiety right now. Here's a cute gif if you got all the way through
