Baseline u/s was this morning though and yes, each cycle is SO different! IVF#1 I had only 2 follies on each ovary at baseline and this morning...5 and 3! I left feeling good about starting with more out of the gate so FX that I respond similarly to stims and produce! Tonight I start with my 300 Gonal and in the morning my 4 amps of Menopur. Let's see how many the 'lil ladies' can grow between now and my day 8 monitoring on the 24th!
@Momifbysea The trigger shot will give them a little burst of growth, so they could be in the running! I hope your ER goes well!!!

@sm05-2- 14mm and 10-11mm aren't all that different in size, I don't think. There is definitely a room for 10-11mm follicles to grow without losing the 14mm one. Grow follies grow!
I'm sure this cycle is fine, but the first cycle really is a trial-and-error, as we respond to meds differently. I'm on my third cycle and still feels like trial-and-error... sigh.
@artemis618- I totally know what you mean by feeling the time pressure. We're the same age (I'm turning 39 in a few months), and I know that whether I like it or not, IVF is only going to get tougher as I get older (especially with all the difficulties I'm going through now). Sigh. I'm on the long agonist (long lupron?) protocol. It's not a commonly used protocol for PCO ladies like me. The antagonist protocol, with lupron trigger, is more commonly used these days but I have this problem with egg maturation. According to my RE, lupron trigger is not a good protocol for people like me, and the long-agonist is still considered the gold standard. The first two cycles I did were a bit like the antagonist protocol where I used ganirelix (antagonist) with fsh, and letrozole to keep the estrogen level low. The protocol did not work for me. So yeah.

@Hopeful_mom- I took a nap this afternoon too! I'm so exhausted. I'm off this week yet I have been making daily trips to my clinic for the past five days, and tomorrow too! I'm happy that they're monitoring me so I shouldn't complain but seriously. Though it is good that it's forcing me to get out of the house and I did run errands and get chores done, so that's good!
@Momifbysea- I think both of my REs consider anything above 15-16mm mature. I think the reasoning is that even after trigger the follicles continue to grow at 1-2mm/day pace so by ER they get to 18 mm-20mm? I think.
@kiwisun4385- I'd transfer my two best. Or 1 5AA and 1 5BB? I don't know too much about grading as I get my embryos PGS tested, but it seems that poor quality doesn't necessarily mean that it won't implant. I'd say ask your RE what is the best way.

@Momifbysea each place is different, the trigger does help the smaller ones mature. 15 and 16 are solid and will make it through!!

AFM- Another day of coasting. My E2 level went down a bit (seriously, only a bit), from 8400 down to, I think, 8200? At least it's not rising anymore. I'm coasting for another day, hoping tomorrow's blood work result shows reduced E2 level. Ugh, it's tricky. So I've been reading a bunch of papers and it seems that most clinics have shown no detrimental effects when coasted up to day 3. 4 or more days starts to show decline in # eggs retrieved and maturity and so on. This is my 3rd night of coasting, so hopefully I get to trigger tomorrow night, but at my current E2 level, I doubt it'll go down to <3000, where most clinics consider it safe E2 level for trigger to reduce the incidence of OHSS. So I'm left with these choices tomorrow:
1) E2 still too high, cancel the cycle? 2) E2 still too high, but trigger with reduced HCG (but will likely result in low % mature eggs) 3) E2 still too high, coast for another day and trigger a day later with a higher dose of HCG (this may result in fewer number of eggs retrieved, as some eggs may be post mature)
Oy. More reading to do.

cashewchicken glad u have the time off to stay up all night reading It still seemed like ur follies had some movement in growth. Were they already in their 20's? It sounds like you have such good ones cooking, I'm hoping u dont have to cancel...i have to go for more monitoring myself in a few...of course i can't fall asleep and i'm sure i'll be in need of another nap tomorrow At this point I hate the monitoring BUT I'm also close and we dont want to miss the good ones i have going HOPEFULLY!!! Its tough as I've had 2 different techs measuring me this cycle and I feel like its been a lil off. Hoping I can get the same tech from today so i can really see if there has been growth.

@cashewchicken I will be 40 in October *sigh*. Ah, I was wondering why no lupron trigger. Doesn't sound like longer lupron is common for AMA ladies, but I think I'm a fan of fully shutting down the pituitary gland before starting stims. My first disastrous (no blast) cycle was antagonist (as was my egg freezing cycle :-/), and the change to luteal lupron (no OCP) made the second cycle night and day different from the first. I did also notice that my E2 was higher (though not as high as yours, but more than double what it was the day before trigger last cycle at 3220 vs 1593 - I'd never head of above 3000 for OHSS risk...hmm...) than in the antagonist protocol. I stimmed one day longer with this last cycle though (lead follie was 23 the day before I triggered). It is frustrating that we mostly have to trust the professionals with this stuff - hopefully this protocol will work better for you. Good luck!

Thanks for calming me down, everyone, I think I'm in a more positive state of mind now. Certainly the side effects of the injections don't help, especially after upping Gonal-f to 450 last night, and this morning I started Cetrotide, and I'm also turning 40 on Sunday (which is hard to believe!). But I do feel better after a good night's sleep.
Also, are you all familiar with Circle and Bloom's IVF meditation program? It's a little pricey, but worth the relative peace of mind for me, and I'll probably buy their FET one when it's time for me to do that. @cashewchicken I haven't done the research, so I can't say what the risks/benefits are exactly. However, if you get to where you're on the fence as to whether you should cancel the cycle, I want to encourage you to make the choice that will best protect *your* health.
OHSS is a serious risk-- really, one of the most dangerous risks we face with these procedures.
Again, I don't want to tell you want to do, but I know what it's like to want to power through something, when you have a goal in mind. Please make sure you weigh how valuable your health is.

@Hopeful_mom- A couple of them were in the 20's, some 15-18mm, and the rest under 15 mm. Even in the absence of FSH, the follicles that are > 15mm continues to grow, apparently. The smaller ones under 15 mm will eventually die off and that's how you see the decline in E2 (most of the E2 is produced by smaller follicles, not the mature ones).
There is a tech-to-tech difference in U/S analysis, for sure. Some nurses and doctors are more thorough and some are not. I'm always amazed by how they count the follicles and not be confused which ones they've counted already.
@artemis618- Above 3000 seems like the cut off many clinics use, but there is a clinic-to-clinic variability and there doesn't seem like a consensus on how to deal with runaway E2 and when to coast, how long and when to trigger.
@sm05-2- I've never heard of Circle and Bloom, but I will check it out. Thanks! I need to calm myself down, IVF is taking over my life!
@KLake42- Thank you for your concern. It's so hard to make a decision when it's your cycle, but you're right, OHSS is a serious stuff and my health comes first. As for my risk of developing OHSS... it depends on what my E2 is and what my dose of HCG will be. I had a blood work this morning so we'll see where I am and what my RE feels comfortable with it.

So many new posts to catch up on! I've been offline for the past couple days and am really looking forward to catching up with everyone. But first, I've gotta share the past 24 hours with you guys. It's been absolutely bananas. i'm going to start from the beginning...i had my trigger shot sunday night, went fine but made me very sleepy. then came the egg retrieval. oh boy. it was yesterday at 645a. While I was under and in OR, OH was giving his sample. I woke up to find they retrieved 27 eggs. I was thrilled! Holy cow!! 27?!?
And then came the scary part. RE came to tell OH that he needs to give another sample bc his sperm weren't moving as well as they should be. This was shocking to us both bc he never had a movement issue, it was a volume issue. While I was recovering, he went to try to give another sample. Poor guy was in there for over an hour trying to do his part of this whole day of baby making, again. Unsurprisingly, he was not successful the 2nd time around due to all the stress he was experiencing. Finally, he came out feeling defeated and we were discussing it, both a little bewildered. Then it hit us - we had intercourse about 4 hours before the retrieval and used a lube...a lube that was NOT Pre-seed so I can only assume that it had spermicide in it. OMG. We are idiots.
So the nurse told us that if we can come back by 2p with a new sample, and it's a better one, then they can go ahead with fertilization. If not, they would freeze my eggs for a later time when we could get a better sample. I was terrified that we had screwed this up. On top of this, I was in excruciating pain from the retrieval.
So we go home, OH goes upstairs to take a shower and take care of business. He comes down at about 130p with his little specimen cup ready to go. I was very proud of him for this being the 3rd time he's produced in less than 12 hrs.
Long story short, I found out at 6p yesterday that the afternoon sample had worked and my eggs had been fertilized!! Whew! What a relief after such a nerve-wracking day.
As of this morning, out of the 27 retrieved, 17 were mature, and 13 normally fertilized
@wanderlust14 - those are interesting stats...can i ask where you found that out? My RE had given me more vague % of success, but was for in general, not for specifically graded embryos...I want to learn more! lol!
Wow, @i-wanna-b-the-girl-w-most-cake, what a story! I'm glad it eventually worked out, but that must have been a tense time. Sounds like a great number fertilized, and I hope you're in less pain after ER!

Oh and I wanted comment on the grading of embryos. Different labs grade them in different ways! They all have their own system. My clinic's lab doesn't grade them at all.
Anyway, I asked by RE about the grading on mine, and that's what she told me (while also saying that the embryologist thought mine looked very nice).

@i-wanna-b-the-girl-w-most-cake, congrats!! what great numbers you have! Home those 13 keep on going!! were they all icsi or ivf! your OH was a trooper thrilled they were able to make it work! You were naughty and didn't follow directions?? Our RE tells us to have sex the night of the trigger and then abstain. WOW your OH deserves a gold metal for sure. I see many men come into the clinic and get embarrassed that they're up to bat. How many are you transferring?
@cashewchicken how did your appt go? you have me concerned with my level now. I dont think my smaller follies are causing it to go up that much as I dont have so many.
Momifbysea congrats how exciting! Hoping to hear a great fertilization report! AFM: Was a lil concerned with my folliles as some measured a lil smaller then the day before. My ES has gone up to 1495 hoping thats a good thing and i have maturing follies! Right now it looks like 6 are in the running so really hope that at least those will pull through and be my strong golden eggs! I will be triggering tonight with some extra gonal F with ER way too early friday morning! I'm exhausted! Still have to go back to the clinic for more bw tomorrow

AFM-I had my retrieval this morning and they retrieved 40 eggs. 40!!! I was in so much pain when i woke up but after some percocet I was feeling much better. That wore off and I'm back on extra strength tylenol. I'm praying ohss stays away and I'm drinking gatorade and eating lots of salty food. I'll find out tomorrow how many fertilized and then they go off to testing after 5 days. We should have a final answer in about 2 weeks.

@Ash_SE - I was also worried about egg vs. embroyo freezing when I was told yesterday that I would possibly need to freeze my eggs. I thought that eggs were much more fragile but the RN assured me that that wasn't the case.
....For everyone who has gone through stims and ER multiple times, I admire you. After ER yesterday, I have a whole new respect for you, seriously....
@KLake42- how are you planning to remain sane during 2WW?! Are you so excited???
i didn't actually think anything of it! my clinic really didn't give us any 'rules' about sex...or much else for that matter. Was wishing they would had, especially after yesterday. I mean, the thought crossed my mind that having sex could jeopardize his sperm count, but honestly since we were doing ICSI, I figured they only need a few. If it wasn't for that damn lube we would've been fine. I have since trashed the bottle. lol.
i'm still up in the air on how many to transfer...we have until Sunday to figure it out!

@i-wanna-b-the-girl-w-most-cake The 2WW is pretty uneventful. I still haven't heard back on how the rest of our embryos did (the ones that were still morula when we had our embryo transfer). Even though I hate being busy at work it is helping this week go by faster. Yesterday 2 patients asked me out of the blue if I was pregnant...I think it was the dress I was wearing but I'm going to pretend they saw some sort of a glow

Just a little info, and like @KLake42 said, not all labs grade embryos the same, but this is the info my embryologist provided, might be helpful for others too..
The first number indicates the stage of development /expansion for the embryo:
6=Hatched Blast, 5=Hatching Blast, 4=Fully Expanded Blast, 3=Expanded Blast,
2=Early Blast, 1=Early Blast. The cells making up the blastocyst are represented
by letters: A=Good, B=Fair, C=Poor. The first letter following the number
describes the cells of the inner cellular mass, the portion of the blast that
will develop into the fetus. The second letter following the number describes
the cells of the trophectoderm, the portion of the blast that will develop into
the placenta. We use this system to try to determine which blasts will continue
development and ultimately lead to a successful pregnancy and healthy child.
With that said, just because we grade an embryo as "good" doesn't mean it will
lead to a successful pregnancy, and just because we grade an embryo as "poor"
doesn't mean that it won't develop into a beautiful baby. The grading is very
subjective and does not necessarily correlate to the quality of the embryo. We
urge clients to not focus all attention on embryo grades.
@cashewchicken I forgot to get back to you yesterday. The medication my RE put me on to reduce symptoms of OHSS is called cabergoline (or something like that). Maybe you can mention it since you're at risk for ohss.

So I got back the E2 result. 6700, lower than yesterday, but still kinda high. My RE is happy that it's come down, so he wants to trigger but with lower dose of HCG. He feels that the highest dose I can trigger without developing OHSS is 1/3 of the full dose (~3300 IU instead of more typical dose, 10,000 IU). I have a history of low % of mature eggs, so I'm afraid that 3300 IU isn't going to be enough to get mature eggs. Several papers suggest that 3300 is fine for trigger. I think I'm expecting about 20-30 follicles at this point.
So this afternoon I was trying to decide whether to cancel the cycle, or just go through with it. If I cancel, I'm out $7000 and will have no embryo, and will never know if this protocol works or not (my previous two cycles were completely different protocol from this one).
If I go through with it, I'm out an additional $5500, but I will know whether this is a step in the right direction, and maybe I'll get lucky and will get a few blast to test.
After deliberation, I'm going to go through with it. Maybe this is a complete waste of money, but I will have more answers. I think. I hope.

@i-wanna-b-the-girl-w-most-cake- Wow that was an ordeal! I'm a bit jealous that you and your husband were in the mood for BD before your ER. At this point, my DH is not going to touch me until I'm fully recovered after ER. Not sure if it's caused by the high estrogen level or not, but my joints are sore and I have really sore, tender breast.
Congrats on 13 fertilized embryos! That's an awesome number, and you're most likely to have a good number of blast. Are you getting them tested, or are you transferring this cycle?

@Hopeful_mom- My appointment was... well, see the post above. E2 still high, but did come down by about 1500 so I'm triggering tonight with a low dose of HCG. I'm just hoping that I get a decent number of mature eggs this time. Last time I retrieved 15, only to get 3 mature eggs... That was a sad day.
Your estrogen level looks great. I think you're supposed to get about 200 pg/mL for each follicle, so the number looks right for ~6 follicles!
@brooklyngirl18- 40 eggs! Wow that's so promising! I hope you get lots of fertilized embryos! My RE did prescribe me cabergoline and letrozole. I will be taking them to reduce the risk of OHSS. Thanks!

Now that I've made up my mind, I'm at peace with it. Sort of. Now I'm stress eating and I just ate chicken wings and veggie sticks for dinner. Hey, got to eat protein, right? And I'm making my own gatorade that I will be sure to be drinking lots of, in the next couple of weeks. I'm pre-diabetic so can't drink gatorade as it contains too much sugar. And I have to be careful with artificial sweetener as that can mess up your insulin levels too. So I decided to make my own. Not tasty, but it'll do.

@cashewchicken sounds like a great plan and I'm glad you're at peace with it, I would do the same. And no, the RE didn't prescribe me fentanyl, they used it for my I've, but man was I out, and all day yesterday felt really drugged up.

Who knew trying to have a baby requires like zero sex for long periods of time. oh and my dr also recommended coconut water to drink post retrieval if you aren't feeling your homemade gatorade. @klake42 ahh so excited, let the symptoms spotting begin! when do you plan to test? sorry if I missed it.
I woke up today in a little pain but I took tylenol and am feeling better now. I'm loading up on gatorade and lays chips. gotta love dr.'s orders I'm anxiously awaiting news of how many fertilized. I'm honestly expecting a huge drop off because I had a large number of eggs but a normal level of estrogen.

I posted this just now on the over 35 board too but didn't want to type it twice...a little sad right now.
Ok I got the fertilization report and am disappointed. Of the 8 eggs retrieved, 4 were mature, and only 2 fertilized via ICSI. I'm a bit nervous, because I know getting to 5-day blast stage and the freezing and thawing and the testing make this a pretty small chance. I realize it only takes one though so hopefully these are the strong ones. A bit overwhelmed right now. Thinking of if we would do a second cycle squeezed in before this summer's out of state move, coupled with the financial piece. We were pretty certain this was our only cycle because it maxes out the insurance lifetime limit. Just a really hard day.

Baseline u/s was this morning though and yes, each cycle is SO different! IVF#1 I had only 2 follies on each ovary at baseline and this morning...5 and 3! I left feeling good about starting with more out of the gate so FX that I respond similarly to stims and produce! Tonight I start with my 300 Gonal and in the morning my 4 amps of Menopur. Let's see how many the 'lil ladies' can grow between now and my day 8 monitoring on the 24th!

I'm sure this cycle is fine, but the first cycle really is a trial-and-error, as we respond to meds differently. I'm on my third cycle and still feels like trial-and-error... sigh.
@artemis618- I totally know what you mean by feeling the time pressure. We're the same age (I'm turning 39 in a few months), and I know that whether I like it or not, IVF is only going to get tougher as I get older (especially with all the difficulties I'm going through now). Sigh. I'm on the long agonist (long lupron?) protocol. It's not a commonly used protocol for PCO ladies like me. The antagonist protocol, with lupron trigger, is more commonly used these days but I have this problem with egg maturation. According to my RE, lupron trigger is not a good protocol for people like me, and the long-agonist is still considered the gold standard. The first two cycles I did were a bit like the antagonist protocol where I used ganirelix (antagonist) with fsh, and letrozole to keep the estrogen level low. The protocol did not work for me. So yeah.

@Momifbysea- I think both of my REs consider anything above 15-16mm mature. I think the reasoning is that even after trigger the follicles continue to grow at 1-2mm/day pace so by ER they get to 18 mm-20mm? I think.
@kiwisun4385- I'd transfer my two best. Or 1 5AA and 1 5BB? I don't know too much about grading as I get my embryos PGS tested, but it seems that poor quality doesn't necessarily mean that it won't implant. I'd say ask your RE what is the best way.

AFM- Another day of coasting. My E2 level went down a bit (seriously, only a bit), from 8400 down to, I think, 8200? At least it's not rising anymore. I'm coasting for another day, hoping tomorrow's blood work result shows reduced E2 level. Ugh, it's tricky. So I've been reading a bunch of papers and it seems that most clinics have shown no detrimental effects when coasted up to day 3. 4 or more days starts to show decline in # eggs retrieved and maturity and so on. This is my 3rd night of coasting, so hopefully I get to trigger tomorrow night, but at my current E2 level, I doubt it'll go down to <3000, where most clinics consider it safe E2 level for trigger to reduce the incidence of OHSS. So I'm left with these choices tomorrow:
1) E2 still too high, cancel the cycle?
2) E2 still too high, but trigger with reduced HCG (but will likely result in low % mature eggs)
3) E2 still too high, coast for another day and trigger a day later with a higher dose of HCG (this may result in fewer number of eggs retrieved, as some eggs may be post mature)
Oy. More reading to do.

Thanks for calming me down, everyone, I think I'm in a more positive state of mind now. Certainly the side effects of the injections don't help, especially after upping Gonal-f to 450 last night, and this morning I started Cetrotide, and I'm also turning 40 on Sunday (which is hard to believe!). But I do feel better after a good night's sleep.
Also, are you all familiar with Circle and Bloom's IVF meditation program? It's a little pricey, but worth the relative peace of mind for me, and I'll probably buy their FET one when it's time for me to do that. OHSS is a serious risk-- really, one of the most dangerous risks we face with these procedures.
Again, I don't want to tell you want to do, but I know what it's like to want to power through something, when you have a goal in mind. Please make sure you weigh how valuable your health is.

There is a tech-to-tech difference in U/S analysis, for sure. Some nurses and doctors are more thorough and some are not. I'm always amazed by how they count the follicles and not be confused which ones they've counted already.
@artemis618- Above 3000 seems like the cut off many clinics use, but there is a clinic-to-clinic variability and there doesn't seem like a consensus on how to deal with runaway E2 and when to coast, how long and when to trigger.
@sm05-2- I've never heard of Circle and Bloom, but I will check it out. Thanks! I need to calm myself down, IVF is taking over my life!
@KLake42- Thank you for your concern. It's so hard to make a decision when it's your cycle, but you're right, OHSS is a serious stuff and my health comes first. As for my risk of developing OHSS... it depends on what my E2 is and what my dose of HCG will be. I had a blood work this morning so we'll see where I am and what my RE feels comfortable with it.

i'm going to start from the beginning...i had my trigger shot sunday night, went fine but made me very sleepy. then came the egg retrieval. oh boy. it was yesterday at 645a. While I was under and in OR, OH was giving his sample. I woke up to find they retrieved 27 eggs. I was thrilled! Holy cow!! 27?!?
Take a nap and post nothing on internet (or at least, nothing on facebook) until the drugs have worn off completely!

AFM-I had my retrieval this morning and they retrieved 40 eggs. 40!!! I was in so much pain when i woke up but after some percocet I was feeling much better. That wore off and I'm back on extra strength tylenol. I'm praying ohss stays away and I'm drinking gatorade and eating lots of salty food. I'll find out tomorrow how many fertilized and then they go off to testing after 5 days. We should have a final answer in about 2 weeks.

@i-wanna-b-the-girl-w-most-cake The 2WW is pretty uneventful. I still haven't heard back on how the rest of our embryos did (the ones that were still morula when we had our embryo transfer). Even though I hate being busy at work it is helping this week go by faster. Yesterday 2 patients asked me out of the blue if I was pregnant...I think it was the dress I was wearing but I'm going to pretend they saw some sort of a glow

So this afternoon I was trying to decide whether to cancel the cycle, or just go through with it. If I cancel, I'm out $7000 and will have no embryo, and will never know if this protocol works or not (my previous two cycles were completely different protocol from this one).
If I go through with it, I'm out an additional $5500, but I will know whether this is a step in the right direction, and maybe I'll get lucky and will get a few blast to test.
After deliberation, I'm going to go through with it. Maybe this is a complete waste of money, but I will have more answers. I think. I hope.

@i-wanna-b-the-girl-w-most-cake- Wow that was an ordeal! I'm a bit jealous that you and your husband were in the mood for BD before your ER. At this point, my DH is not going to touch me until I'm fully recovered after ER. Not sure if it's caused by the high estrogen level or not, but my joints are sore and I have really sore, tender breast.
Congrats on 13 fertilized embryos! That's an awesome number, and you're most likely to have a good number of blast. Are you getting them tested, or are you transferring this cycle?

Your estrogen level looks great. I think you're supposed to get about 200 pg/mL for each follicle, so the number looks right for ~6 follicles!
@brooklyngirl18- 40 eggs! Wow that's so promising! I hope you get lots of fertilized embryos! My RE did prescribe me cabergoline and letrozole. I will be taking them to reduce the risk of OHSS. Thanks!

@cashewchicken sounds like a great plan and I'm glad you're at peace with it, I would do the same. And no, the RE didn't prescribe me fentanyl, they used it for my I've, but man was I out, and all day yesterday felt really drugged up.

Ok I got the fertilization report and am disappointed. Of the 8 eggs retrieved, 4 were mature, and only 2 fertilized via ICSI. I'm a bit nervous, because I know getting to 5-day blast stage and the freezing and thawing and the testing make this a pretty small chance. I realize it only takes one though so hopefully these are the strong ones. A bit overwhelmed right now. Thinking of if we would do a second cycle squeezed in before this summer's out of state move, coupled with the financial piece. We were pretty certain this was our only cycle because it maxes out the insurance lifetime limit. Just a really hard day.
