  • How do I know if I have a plugged duct or mastitis?

    I woke up very achy, have a fever of 99.8, and my whole left breast is very sore, tender. Its especially the top and outside under arm. Its a little red but no streaks or pus. And no specific area with a lump. Also when do I get help and who do I go to-my OB?

    Ive tried warm compresses, massages, and pumping.

  • I haven't posted much lately but I have a question. Brooke is almost 3 weeks old and is still nursing like 15 times a day totaling over 5 hours. Shouldn't she be able to go more time between sessions now?

    She has wet diapers but her poops are more like...well, wet farts. She has a good dirty diaper about every other day but almost every wet diaper has some poop in it.

    It's probably just the fantom milk refill pain we sometimes get after a feed. However feel around for lumps or hard spots there might be a clog in there maybe?

  • Hi Ladies! I don't post much, but read threads on this board throughout my pregnancy. My LO was born three weeks ago and I've been reading this thread along the way, so I hope you don't mind me "crashing"!

    I'll try to keep this short:
    We had a tough first couple of days, including the lactation consultant and nurses at the hospital telling me her mouth was too small while frantically trying to get her to latch. I left the hospital with a painful patch, but hoped we would figure it out at home. I did see a different lactation consultant when LO was a week old and she said the latch was good, but I'm still in a lot of pain that seems to increase throughout her feeding and am getting the "lipstick" shaped nipple when she's done.

    I've watched tons of videos and read all kinds of tips and feel like I'm doing everything right. It seems like LO's jaw is really tight and even if I can get her to open wide at the beginning, she tenses up as she continues feeding. Pulling down on her chin either doesn't do anything or makes her tense up even more.

    I'm planning on making another appointment with the lactation consultant on Monday and/or going to the local LLL meeting on Wednesday, but figured I'd see if any of you have had a similar issue (tight jaw) or if it's possible she just has a small mouth like they said at the hospital.

    Sorry this is so long! If I have any luck I will certainly share in case anyone else is in the same situation!

    Advice on getting baby to spit up less?

    I feel so sad for her she spits up a lot Everytime I feed her lately. I try not to lay her on her back right afterward but it doesn't matter. :(

    Eta: After she is sick, she wants to eat again right away.

    Burp her in the middle of her feeding. As long as spitting up doesn't hurt, it's not usually indicative of a problem (well...other than a laundry problem). Keeping her upright after feedings instead of laying her down may help too.

    Spitting up usually peaks between 2-4 months. It's also very common when mom has an oversupply, so if you have a bit of an oversupply, she may spit up less as your supply regulates.

  • 3 days still not much going on. I just want to feed my baby. Supplementing is okay but she is such a hungry thing it's more like 3 4oz bottles in a 24 hours. I can nurse for 60 mins every two hours but she is starving still. So i find myself mixing a bottle :( . Just hoping it comes in soon. I'm starting to get discouraged. This thread is amazing. So are the ladies in it. So helpful.
  • So it seems my nipple issues are just not going to end and it's looking like I will be EPing again. We've had multiple LC appointments, I've seen my OB and been treated with oral antibiotics for thrush, she got her tongue and lip tie clipped on Thursday, I've been using the APNO since Thursday, and she has seen an OT. I'm starting to come to the conclusion that I have super sensitive nipples and she has a bad latch with her bottom lip that I can't fix. No matter how wide she opens she seems to pull her lip in right as I latch. When I try to pull her chin down she clamps down more. When I try laid back nursing she holds her head up to get a shallower latch. It's not painful after the first minute while feeding after a long break in nursing, but my nipple is flattened on the bottom and it hurts at the end and there is visible damage to the nipple. It hurts more at each subsequent feeding to the point that I can't nurse her more than twice in a row without a long break in between the next session. She's three weeks and two days and I've never nursed her more than four times in a day and that has not happened often. She will latch on a shield but doesn't have the patience to transfer milk that way.

    I guess this is really a rant. I'm just frustrated that I've had very little improvement. I had what I thought was a great day on Thursday but by the third session in a row my nipples were a mess. The LCs that have seen my nipples have commented on how bad they look and that they haven't seen this much damage in a while. The abrasions that look like oval shaped white blisters that cover half of one side of my nipple have not improved at all. It looks like a blister but the LC and OB think it's just nipple trauma. Unfortunately the mostly pumping and letting them heal has only worked enough for me to get in one or two pain free sessions before they need to heal again. I know it can take time to improve her latching following the tongue and lip procedure, but I'm
    not sure how she will ever learn if I can't tolerate her nursing more than a couple times a day. It just feels like an endless cycle of the same thing.

    I know my baby will be fine no matter how I feed her, it's the not being able to do something I've worked hard to do that is hard to deal with mentally. I just needed to get all that out. Thanks for listening.

  • Because she is so spitty, the clinician on duty last night told me to not let her feed more than every 90 min and no more than 15 min per side. How do I tell her no to eating though? She wants to eat and it has been 45 min...
    She also is not birth weight yet. Shouldn't I be feeding on demand? I feel like I am getting conflicting info. :-<

  • Feeding, spitting up a lot, then rooting and wanting to feed again. All day long. Normal, or should I be asking the pedi about reflux or something? It isn't after every feed but is after a lot of them. We try to keep her upright but she spits up if you lay her down to change her or put her in the carseat - even if it has been 30+ minutes.
    Trying to decide if it is just a laundry problem...

    Same here!! I may ask pedi since we have an appt on Wed. I called the pedi nurse line this weekend and they didn't mention reflux at all. They made it sound normal and like my milk hadn't regulated yet. I just feel sad for her cause she gets fussy right before she spits up.

  • Please help! I can't stop crying over how much my nipples hurt. I'm sorry if this has already been brought up in this thread but I can't sift through everything and I'm seriously desperate. I think LO has a shallow latch but I can't seem to fix it. He's 4 days old, and the lactation consultant when I was in the hospital worked with me and said his latch looked really good (a lot of breast tissue in his mouth, lips flared), so I can't figure out what's wrong. My nips are on fire 24/7, the lanolin and gel soothies are barely helping.

    I've been researching ways to improve the latch and have been using the cross cradle hold, but I can't figure out what I'm doing wrong. My right nipple has the lipstick look, but it's the one that hurts less. It's the smaller of my two nipples, so I think that's why he has an easier time latching, but it still hurts and it still looks like lipstick. When I was in the hospital this nipple was doing great, nice and elongated after baby detached from feeding, so I really have no idea how this all went to shit as soon as I'm home.

    Left nipple is different. This has always been my slightly bigger breast, and the nipple on it is bigger/puffier than the right nipple. I honestly think it's too big for his mouth. Even when he opens wide and takes a big "bite", a good amount of areola still shows. This nipple doesn't have the lipstick look, instead it is flat on top with deep horizontal creases in the circumference. The pain on this side is seriously excruciating. It feels like my LO has a mouth made of glass shards when he feeds (most of the pain is in the areola where his lips are latched). I have tears in my eyes right now just thinking about being due for another feed on this side.

    I will be contacting the LC first thing tomorrow since the office is closed today, but I need help or some insight/suggestions. LO also has a pedi appointment and they will have another look at his tongue tie (LC in the hospital did say it looked like he has a slight one but didn't think it needed to be clipped). I just don't know how I'll get through the night like this :(
  • Hoping one of the breast feeding gurus can help me out :) I saw the LC again and she said I should start giving the baby less in the bottle when I supplement after BFing. I was giving 3-4oz. When we did a weighed feeing she got about 1 oz so she said I should give 2 oz after feeding her, if she acts hungry. Question is: she told me that if within the hour after feeding her she is looking for food, give her the bottle and if over an hour just BF her because it is a new feeding session...So, do I count from the time I start feeding her or when I finish with regard to feeding her the bottle? Meant to ask her there bit forgot and she doesn't give out her email or number so can't contact her. I've been counting an hour after the time I finish breastfeeding..thanks!

    @FamousEa Is LO congested? Could he have a cold or an ear infection? Try some different positions to see if he will latch onto that breast in another position.

  • @JessAnnJ‌ (Hugs). I am in the exact same boat as you with shallow latch and I am at my wits end trying to figure out what to do. We have a good day and I start feeling optimistic and the we have a bad day and I am near tears. The APNO doesn't seem to be helping these Grand Canyon fissures in my nipples and it hurts so bad. I pump here and there to give my nips a break but am primarily nursing still. I am sick of just toughing it out.

    I plan on going back to the Breastfeeding support group tomorrow for support and some time with a LC.

    I really hope this gets better soon.

  • I've been skimming through the board and I thought I saw some discussions buts haven't seen this exact question. Sorry if it is a repeat. I've tried to begin pumping but I am hardly getting anything out when I do. I am only getting about 1-1.5 ounces from both breasts. I am breastfeeding full time and not supplementing at all so is this a normal amount? Will this increase over time? I want to start storing milk but getting so little makes it a very daunting process.

    Hmm...that's odd advice, IMO. I would recommend feeding on demand, keeping her upright and burping frequently. Nursing less often won't keep her from spitting up. It'll just make her hungry and grumpy! Strange rec...

    Is she uncomfortable from spitting up? Because there's really nothing that needs to be done about it unless she's in pain. It should go away on its own in the next several weeks.

    Does your breast soften after nursing or pumping? I'm not sure I entirely understand...the "hard as a rock" thing confuses me. :P If your breast is engorged even after feeding LO, you can offer it to her again or express enough to make yourself comfortable. It's just this way in the one breast?

    Sounds like a laundry problem. This is THE time for spitting up. As long as she's not uncomfortable or in pain when she's spitting up, she's just fine. :) Could be either the immaturity of her digestive system or you might have a bit of an oversupply/forceful letdown. More frequent burping may help too.

  • @deezidee04‌ Get that tongue tie evaluated for revision ASAP! If you're experiencing this level of nipple trauma and pain due to clearly shallow latch, the tie IS interfering with breastfeeding which is absolutely good reason to have it clipped.

    I'm sorry you're having such a rough time and are in so much pain. Calling an IBCLC tomorrow morning is an excellent plan. As much as you can, leave your nipples open to the air (go shirtless or wear a sweater or button down shirt open) and express breastmilk onto your nipples and allow them to dry. If necessary, nurse LO for tonight on the good side and pump the side with the trauma.

    I wonder if you have a slight oversupply in that breast and a forceful letdown. Babies often fuss at the breast and pull back/arch their backs when mom has a forceful letdown, because the milk flow is so fast and strong.

    This is a normal amount for a mom who is breastfeeding full time. The more you pump, though, the more you'll yield. You'll become a bit better at pumping and your supply may increase slightly from the extra stimulation.

    Pump after a morning feeding, when your supply is highest. :) Good luck!

    At 5 days old, it could just be engorgement from your milk maturing (coming in), but I'm surprised that it doesn't drain even after pumping and nursing. When you pumped, how much did you yield? I wouldn't want to encourage an oversupply by extra pumping, but I would also be concerned about discomfort and leaving that much milk in your breasts causing plugs. I might put baby to the breast again the next time she stirs, to see if she will drain it for you.

  • So I don't post often, but I have been following this thread mostly and have been a lurker for a long time. This may have been discussed but there are a lot of posts to go through.

    My babe is four weeks old on Tuesday and feeding ALL DAY pretty much. She only goes about an hour (or less) before she's hungry again...she's either eating, asleep, or fussy for the most part. She will also nurse for looong periods of time. She isn't always transferring, but if I unlatch her instead of waiting for her to unlatch, she gets mad. She also does the sticking out the tongue/sucking motion hunger signal immediately after a good feeding. Is she really always hungry? My boobs are constantly feeling flat and empty bc she's eating so frequently, and my nipples are sore even though her latch is great (we have seen an LC and had her tie clipped) just from the amount of time she spends nursing. I know it could be a growth spurt; but she had kinda been like this since birth. Am I not making enough? She has plenty of wet/dirty diapers and gaining fine; so I don't think that's it, but I just don't know what else it could be! I can also only pump about .5 oz after morning feeding (been pumping for a week).

    My mom suggested that she just has a strong need to suck, but I've tried five different kinds of pacifiers and can't get her to take one.

    Any tips/suggestions/ideas?

    I should also add I burp her well and she doesn't really spit up, and I keep her upright after feedings, so I don't think the fussiness is gas or reflux.

    If baby is more than 6 days old, she should be having a minimum of 6 wets per day. I think that is pretty strictly followed.
    She is in day 6 today. Hopefully she starts meeting this today. Knowing if my baby is getting enough to eat is by far the most stressful part of being a mom :-(

    @angeltennis3‌ I'm lucky to get one dirty diaper from DS every day. It's not the norm, but it doesn't typically indicate a problem unless they aren't gaining well. Hopefully LO's weight will set your mind at ease at your pedi appt today!

  • I have had extremely sore and cracked nipples since day one. I think we had some bad latching at the beginning and now I can't get it to heal bc LO is feeding every 2-3 hours. I have tried lanolin and cool gel pads, I rinse after every feeding, I let them air out when I can (between people visiting). At this point, I don't think they are ever going I heal so I dread every feeding. She is getting plenty of the nipple and areola in her mouth and has a wide latch. I may have to break down and go see the lactation consultant.
  • The consensus is to not wake baby to feed, correct? Or only if she hits four hours (which always during day, never at night...shell shocked from first night still)? I would assume that I shouldn't pull her off a breast even if it's been 20-25 minutes to feed off the other?

    I was also wondering about diet - I think my LO has her freak outs due to gas so wondering if my diet is making her so uncomfortable. I will eat asparagus wrapped in beets with a side of grilled kale to get 3 straight hours sleep....

    Definitely go see the LC if you can. She can tell you whether a scrip for APNO will be helpful in speeding your nipple healing. :) Also, instead of rinsing your nipples after nursing, just express some breastmilk onto them and allow them to dry in the open air.

    FFMC If my baby is crying and chomping her hands at an hour I'm feeding her no questions asked.

    I thought it was weird advice.

    Good for you for following your instincts instead of weird pedi advice. Nursing on demand is the best policy. And different mothers have different storage capacity. Baby will grow at a natural and proper rate if he can nurse as needed.

    These misinformed pedis make me want to pull all my hair out! I suppose someone had to graduate bottom of their class in med school...

    ETA: sorry. Hormones. I'm sure your pedi is really good and just made a not so great rec. I'm just sleep-deprived and grouchy

    Yeah...I'm sorry. I got barely any sleep last night because while MIL is here, both kids are in our bed because she is in DS1's. So I was grumpy. :P I'm sure she really is quite competent.
