  I was BFing and did it at 12 months, although she still nursed twice a day until 18 months.  At around 11 months and 2 weeks I was worried about not having enough BM while I was at work so I asked my pedi if I should give formula or WCM if needed and she said WCM was fine and that I should start off by mixing WCM and BM and slowly transition her to WCM.

  I mostly breastfed but had to supplement toward the end of his first year.  I mixed formula with whole milk and he wasn't a fan (I started this around 11.5 months) so I stuck with formula.  At 12 months I cut out formula and bottles altogether.  He never really took to milk, although he will drink it at daycare for some strange reason. 

  So to specifically answer your question; we started giving milk at 11.5 months and we basically did it cold turkey.

  I just mixed it with formula and gave it in a sippy.  We've been 100% WCM since around Christmas.

  At 11.5 months I just started giving her a cup of milk in place of a bottle, then moved those times to meal times by her birthday. I never mixed formula and WCM in the same cup. She didn't have any problems with it.

  We started about 2 weeks before she turned one, started mixing 2 oz of WCM into her bottles one week, 4 oz the next, 6 oz the next.  She never had any problems.

  Right around the time he turned one. I put whole milk in a sippy cup and got rid of bottles. He did fine. Never even flinched. Maybe your LO will do the same

  DS2 never had formula, and at 11m we started offering him whole milk (with approval from his pedi).  I didn't reduce nursing until well over 12m, so offering whole milk at that point was just to let him get used to it.  It was then a gradual process between cutting nursing times/day and my body producing less to adjust to him needing less as he drank more and more of the whole milk.  And I never put the whole milk in a bottle, we went straight to the sippy.
