Follistim and Menopur, what's your opinion?

I received the rest of my info in the mail yesterday for whenever I decide to cycle.

In going over the protocol changes I had a question... We're keeping the same protocol, but upping my doses. I'll be on 225 follistim/225 menopur adding ganirelix later (not sure dosage). My doc says the amount of meds it takes to stim me is really high. (last time was on 150/150 and had 11 follies at ER) Says it shouldn't take this much for someone of my age; Which is another reason why he thinks I could be DOR despite "normal" AMH/FSH and decent AFC (15 and 11).

What do you think? What are your experiences? Is this a lot? Do you think this shows DOR?

  • i can only tell you my experience, i'm 35 and my FSH is like 6

    IVF#1 i did gonal f 150 for 9 nights and (didn't have ER & ET due to failed mTESE) would have had around 18-20 eggs

    IVF#1.5 gonal f 150 and menuopor 75 units for 10 nights, 22 eggs, 17 mature

    IVF#2 lowered stims and started menopur later and changed protocol around a little and then raised stims (highest being 225 gonal f) 11eggs, 5 mature

    IVF#2 protocol was not good for me and my RE and I both agree i need menopur from the beginning.

    i have no idea what's right in terms of dosages.  how old are you?

    It's been a long journey. TTC since 9/06. multiple IUI's and IVF's and 4 m/c's. IVF#3 = BFP, twins, induced at 34w6d due to baby b passing away (no explanation). Delivered on 35w1d, Baby A - baby girl, and Baby B - baby boy, our little angel.

  • I tested normal on all counts for DOR but my response to meds were horrible. For my first cycle I was on a total of 450iu of a mix of meds (I can't recall which ones as they mixed them up a bit on me each cycle). My last cycle, they upped the dose to 600. I got a total of 6 eggs on the last one.

    Poor response to meds can be a sign of DOR.I was given that dx after my first IVF (unexplained before).

    Do you know the quality of embryos - as poor quality can also be a determining factor.

    um- I don't really-  I don't think they were good. I mean on day 3 I had a 4 cell grade 2 and one that matured over night that was day 3 a 4 cell grade 3. I have my chart so I can look up and try to understand the reports maybe? I dunno.

  • I started out at 150/150 too...and that ended up being WAY too much for me.  I'm 35 so they figured that was a good place to start.  My FSH is 5.4.

    I stimmed for 8 days...and i went way too fast.  I actually ovualted some of bigger eggs and lost them :(  I had 11 at ER.

    My dose was lowered to 75 follistim then to 50 follistim.  Some nites no menopur, or 1/2 or 1.

    I am 32 (until tomorrow anyway) and have DOR.  I do 300 iu total per day.  This is mostly gonal-f or follistim.  I add in menopur on two days and those two days my gonal-f/follistim is reduced.  That being said, I had 4 eggs retrieved with IVF #1 and 6 with IVF #2.  My problem is that I have had really low AFCs at the begining of both of my IVF cycles (way lower than my off cycle AFCs).  So, they aren't going to get more follies by upping my stims and I only stim 7-8 days.

    ~SAIF/PAIF/Everyone Welcome~ 

    Me= 37 and DH = 41 

    Dx: DOR, Endo, APA+ (really high beta 2 glycoprotein antibody and high everything else tested), heterozygous MTHFR mutation, positive for lupus anticoagulant, high FSH, low AMH and both tubes blocked (per HSG on 3/8/11)

    IVF #1 - long lupron (with HGH, intralipids, lovenox and BA); 4 retrieved, 3 fertilized; ET 2 blasts and 1 frozen = BFN

    IVF #2 - a version of antagonist with EPP (with HGH, intralipids, lovenox and BA); 6 retrieved, 4 mature, 3 fertilized, 2 blasts and 1 frozen blast transferred on day 5 = BFN.

    IVF #3 April was postponed to May, May was canceled. June/July was canceled. Had a cyst aspiration and then began IVF #3 in August. ER on 8/22; ET on 8/24 with AH. +HPT on 9/5. Beta #1 (11dpo) = 3; Beta #2 (15dpo) = 29; Beta #3 (17dpo) = 60; Beta #4 (19 dpo) = 118. Heartbeat at 6 weeks 6 days =132.  Lil is here!

    TTC#2:  Trigger + TI = BFN; Clomid + Trigger + IUI = BFN.

    IVF #4:  BCP + MDLF + Lovenox = 7R, 1F = Transferred 1 6-cell embryo on day 3 = BFN

    IVF #5:  MDLF + Lovenox = 4R, 1F = Transferred 1 10-cell compacting embryo on day 3 = BFN

    IVF #6:  (New RE):  Long Antagonist November 2014 (transferred two 8 cell grade 1 embryos and froze one blast) = BFN

    FET#1:  BFN

    Had it been up to me I would have preferred to up the Menopur, too.

    I haven't been diagnosed as DOR but apparently I have egg quality issues.

  • It sounds like my first IVF was exactly the same protocol, but lower doses. I did 150 Follistim, 75 Menopur for 8 days (I'm 28). I only had 9 mature eggs though and only 2 made it to transfer (1 frozen).

    For this time around, I'll be on Follistim and Menopur again but higher doses. I'll be on 375iu Follistim and 75iu Menopur. No Ganrilex this time around, stupid Lupron instead. But if it leads to better results (ie pregnancy!), I'll deal with it.

  • I never made it to ER, but I was on 450 iu of bravelle per day.  I am possible DOR as well, but I didn't make it to ER b/c I was over surpressed.

  • I was 38 for my OE IVFs with borderline high FSH and a poor response to stims. We tried three different protocols, but I was usually on 300-450 gonal f and 75-150 menopur. I am going off memory here, but I think that was about right. One cycle was converted to IUI for poor response. One had 6 eggs retrieved w/ 5 mature. One had 7 retrieved w/ 4 mature. Straight antagonist was the best protocol for me.
