My biggest waste of money was the diaper bag that I had to have, another would have to be the infant car seat. We should have just bought one of the convertible ones since he only used the infant one maybe 5 or 6 times.

definitely the infant seat, although we got it off our registry. Our baby was born 11 lbs and hit the weight limit quickly. Plus, he hated the thing, we got a convertible seat early on.
Most of the baby stuff I've bought, I've been able to sell on craigslist, so it wasn't a total waste. The only thing I took a big hit on was my babyhawk carrier. I never used it. I preferred our sling when he was a baby, and then we bought an Ergo.
We didn't really get a lot of extra stuff since we had to buy for two, so there wasn't that much that I would call a waste.
We definitely had a ridiculous number of receiving blankets, but most were hand me downs or gifts so no big deal. Same with bibs and baby towels.
The biggest waste was probably the Vera Bradley diaper bag I first had. It seemed huge at the time but once I actually loaded it down with stuff for twins it just didn't cut it, plus it only had shoulder straps which are a pain in the ass when lugging two infant seats. I replaced it with a huge $20 messenger style diaper bag from Carter's and never looked back.

The bedding set. I almost didn't get it because I knew we wouldn't use it but I was talked into it. I was right, we didn't use it.
I bought a mei tai carrier off of Etsy. It sucked! The shoulder straps were so narrow it about killed me. I got a brand new Infantino mei tai with awesome padded shoulder straps at a second hand store for less money than I paid for the crappy one.
Doh! I forgot the sling I never used because it never fit right. I should put it on Craigslist.

I think I may be the only mom that actually used our baby shoes. DS wore shoes to church every week. Granted, the nursery ladies thought I was crazy, and he never left in them but he always showed up wearing shoes (they were gifts)
I LOVED our infant seats! We used them for 7.5m and they were worth every penny. I don't know; maybe it wouldn't have been as big a deal if we had one baby at a time but logistics with twins can be tough and the Snugrides provided an easy way to get both babies between the house and car, pop them into the Double Snap n Go, etc.
I did a lot of research in advance so there didn't end up being much that was a waste of money. The only thing coming to mind is the fleece slippers I just bought the boys from TCP; they don't stay on at all! ;P That's probably not what you had in mind though, eh?
The biggest waste for us was all the infant clothes she got from the baby shower, plus the tons of clothes the grandmothers bought her. I should have taken them back to Carters immediately and traded them all for 12/18 months or even 2T-3T. We have 3 rubbermaid tubs of clothes she's outgrown, a lot of which she only wore once and a few she didn't wear at all. If this baby is a girl, we are set. If it's a boy, it's going to be one heck of a yard sale!
I loved my infant carrier, loved the jumparoo, never bought a pnp because mom and mil had them and we just borrowed theirs when we went OOT.
Moby (carrier) - B despised it, though I can see how it would work for kids who enjoy being carried facing inward.
Mei Tai (carrier) - Never used it. Not once. By the time B would have been okay with a back-carry, he was too big to want to be carried.
Sophie - cute, but worthless for it's intended purpose.
Mesh feeders - B hated the feeling on his gums, so they were worthless.
Baby tub - cute, but didn't last very long. It was a gift though, so not a personal money waste.
Play mat - very rarely laid on it, and never played with the dangling toys.
The travel swing would have been a waste had we bought rather than borrowed. He never liked it, and barely tolerated it past a month or so.
Various toys that he gave a cursory glance before ignoring completely.

For us the whole travel system. DD hated the stroller when she was tiny (we ended up having to get an ergo and wear her everywhere) and it took up way too much room in our vehicle. We finally got smart and invested in a really good umbrella stroller, which she still isn't crazy about but I have a bad back and I can't carry her all the time.
The Jumper, she would never sit in it longer than 5 min

The millions of receiving blankets we were given. I used the swaddle me and didn't even touch a receiving blanket more than twice.
DD hated the bouncy seat, her swing, and the moby. Total wastes of money for us.
Mesh feeder: never used it.
Baby legs: never used those either...but insisted to my skeptical DH that I HAD to have them. Maybe this time around...
I did use my Peanut Shell sling...but not enough to justify the $$. The Ergo was (and is) our go-to carrier.
Luckily, DH talked me down from buying more than one diaper bag. I'm needing help resisting a new one this time around, too...our Skip Hop was just fine.
I loved our infant carrier! I can't wait to use it with DD. It was so much easier carrying him around in that as opposed to taking him in and out of the convertible every time. Especially in the winter, it was so nice to just put him in the seat in the house where it was warm, and bundle him up in there. And going to the grocery store...much easier just taking the seat out of the base and sticking it in the cart! And more convenient with the stroller too!
But the biggest waste was probably the jumper. He did like it when in it, but he wouldn't stay in long and I personally thought it was a PITA to srt up each time.
I bought too many cute clothes and not enough sleepers when she was tiny. The swing I bought also ended up unused because she would scream every time I put her in it and the breast feeding items (pillow, pump, etc) also ended up being a waste of money because I was not able to breast feed.
I also received baby socks and baby shoes as gifts which were totally useless.
Not much, we used all of the baby "gear" (infant seat, snap 'n go, swing, exersaucer, jumperoo, etc) until DD was over a year. I miss that stuff!
We didn't get much use out of the Bumbo, though I was still glad to have it. We also didn't use the mesh feeders, or baby towels. I switched to a regular towel when DD was just a few weeks old. Those things are worthless, IMO. I also didn't like any embellished or thick burp cloths. I liked the cheap, thin cloth diapers for burp cloths. She spit up so much at first, we went through those like crazy.
I also wish I wouldn't have bought a $300 breast pump. I only ended up BFing for about 8 weeks, so I wish I would have rented one. I ended up selling it on CraigsList for about $200. Next time, I'll rent one, and see how things go before investing.
The only thing I could think of would be the baby bathtub. He HATED that thing. After awhile we just started taking baths with him in the main tub, and we still do it that way.
He really didn't like his swing, but it was a hand-me-down, so that doesn't matter.
We absolutely needed The Boppy (still use it), The Ergo and Moby, the infant carrier, and a lot of other items that ppl have listed.
Pretty much everything we have for him was hand me down or given, so we didn't really spend much or waste much.
I had way too many 0-3 clothes, especially impractical things like jeans and non-onesie button up shirts. And baby shoes. So dumb.
He slept in the swing for like the first 5 months. We were given a hand made cradle which he slept in once and hated. I feel bad when I look at it. Maybe the next one. We also used the crib positioner maybe twice.
I spent way too much on cloth diapers when I was trying to figure out which ones worked for us. But I sold them and pretty much made my money back.
My biggest waste of money was the Pack-n-Play. He hated napping in it. He hated playing in it. Right now it's a giant toy storage bin in my office (he goes to work with me part-time). Waste of money.

Honestly, not a whole lot. I did a ton of research ahead of time and tried not to go overboard getting similar items.
The only things that were a waste were the Angel Care monitor (which was a gift) that we never used and were able to return for full price. And the My Little Lamb bouncy seat... it was not stimulating at all... we used it at MIL's house for about 2 months until we replaced it with the Rainforest boncy seat. I didn't purchase any of these things myself though.
The number of crocheted blankets we received was a bit over the top... I didn't use any of them.
Swing...he used maybe for like the first month then hated it (it was a gift though)
