My back went out the other day while I was trying to get DS in a back carry with my DIY wrap and I collapsed with him still on my back. Not exactly what you meant, but the good news is even being jolted around he was fine. Me, not so much. I think the likelihood of you completely falling on your face or something isn't that high. I wouldn't stress out about it or anything.
When DD was little, maybe two weeks old, I'd put her in the Ergo and taken the dog out for a quick walk. I was wearing flip flops, and stepped on a patch of tiny gravel right as the dog pulled a little left. I slipped, and hit the ground on all fours. Julia never even woke up, but it scared me pretty bad and I cried for a while. Now, I always always walk the dog with appropriate footwear (sneakers with some traction) and just pay close attention to where my feet are going. Nothing like that has happened since! I am also suuuuper careful going up and down our stairs.

This is almost exactly what happened to us when M was tiny, except I was wearing loafers that unbeknownst to me, were falling apart on the bottom and I caught a rock in a flap of the shoe's sole.
Just this weekend I had M on my back at a Christmas thing downtown and slipped on a patch of ice on the sidewalk. We were both totally fine but it (combined with his being cold and maybe hungry) freaked him out pretty good:-( The next day when we went to cut down our Xmas tree and were trudging through waist deep snow, he'd start to fuss every time I seemed a little unsteady. For the next few weeks I think we're only going to wear him when we're super steady so he rebuilds his trust (which is hard to do in all this ice and snow!)

Think of it this way, it's possible to fall just while carrying LO so if they're "tied" to you, then when you fall you have both hands to catch the both of you instead of trying to catch LO and hurting either of you worse!
I'm a worst case scenario thinker to but I take the same amount of care (if not more) while wearing DD than if I'm carrying her and know that if something happens I did my best to protect her.
Yes. I tripped hiking in the woods and went down hard. It was a tummy-to-tummy carry and he was absolutely fine. My knee, hand and elbow were skinned to all hell. I got 3 stitches in my knee.
It was crazy, I was so concerned about him that I used one hand to hold him and I took all of the fall on me. It was instinctual; I lightly skinned the back of one hand because I cradled his head; I don't even remember putting it there.
I completely used my own body to shield him, and I didn't even notice that I was bleeding until I had checked every part of him! Something primal just took over. If I had fallen "normally", I wouldn't have been hurt badly, but he would have taken a hit. Instead, something made me take the damage to make sure he was safe.
I havent fallen with DS while wearing or carrying him but it is one of my greatest fears, but i agree with PPs you will instinctually protect your LO and fall approprately. I fell while PG and managed to instead of face plant land more on my side and back and DH how tried catching me, was so suprised i grabbed for my bump first with one hand and tried to protect it with my other. I think your body knows and protects your LO even if they are in or out.

I tripped over the threshhold (in my ski boots) and fell. I took most of the force on my knees, but her head did bump the ground.
This is why I don't bw more. I mainly do so around the house when DS wants to be held, and at the grocery store, target, or when running errands. I am the biggest clutz in the world. Also why I didn't run outside while pregnant...
Ask yourself this... In what situation would it be better to be carrying baby and fall vs BWing and fall. IMO pretty much none.
I fell once holding (not wearing) DD and she never touched the ground but it was hell on me since i didn't catch myself at all.
I slipped on a magazine left on the stairs. DD was in a front carry and I fell backwards and then slid down the stairs. I sat at the bottom of the stairs crying; she didn't wake up until the dogs came over to see what was up and started licking her face.
Thank God I was wearing her; I had my hands available to whip back and grab the banister. If I was holding her I don't know what I would have done or how I would have fallen. I'm much more afraid of falling while holding than while wearing.
I was in the grocery store with DS in the Beco and almost slipped. There was just a small amount of water/liquid on the floor and my feet almost flew up underneath me. If I hadn't caught myself I would have landed on my back though, but it will wouldn't have been good. Scary.
I complained/told them of the water. Told them they were very lucky I hadn't fallen....

I basically did the same thing and DD never even woke up. I did twist my right knee pretty bad when my right leg didn't come down the same number of steps (it folded up underneath me). If I hadn't been wearing her, it would have been worse since I was able to catch myself.
i had her in a hug hold a couple months ago, slipped going up concrete steps, and went flat down like a tree, right on my front. between falling and landing i managed, i think totally on instinct, to end up on my elbows and forearms cradling her. i caused myself a lot more pain and injury than i would have if i had braced myself `properly` but she didn`t get hurt at all. she looked startled but didn`t cry. her neck jerked a little from the impact, and i was worried about whiplash or something, but she was fine. i think if you do fall, you`ll be so conscious of her that you`ll both be alright
