I found my ds to be a bit too tiny for the 0-3 month clothes at the beginning. ?I put him in the sleepers anyways, but probably he would have fit better in the newborn. ?He was an average size newborn at 7lb13oz and almost 21 in. ?However, on a side note, the size 1 diapers fit just fine and we bypassed the newborn diapers.
Its the pounds. Newborn starts smaller and 0-3 is like 8 pounds or so. I may be off on the lbs but you get the idea. If you read the tag it tells you the pounds.

When i had registered, i didn't register for anything newborn because i was afraid nothing would fit if i had a big baby. After Aubreigh was born, she was so tiny, (5 lbs 12 oz) that only the newborns fit properly, so I ran out and bought her some sleepers.
Now she has outgrown all the newborn things. I would say that if you register for newborn, just register for a few things, i would say sleepers, onsies, etc. I wouldn't register for outfits because they are so seasonal, that she may not get to wear them...we have several outfits like that. We also got some things newborn as gifts when she was born.
I have also noticed that gerber brands run smaller than carters...especially in the onsie department.
Newborn are a bit smaller. Your baby will wear newborn stuff for like the first and second week and then 0-3 should fit. Unlses you have a huge baby then you will never use newborn haha.

It really depends on the size of your baby, and the brand. Miranda was too small even for newborn at birth, then wore it from about 1-3 months, then 0-3 months sizes from about 3-6 months. She's still wearing one 0-3 month outfit at 11 months!
I would definitely get a couple of newborn outfits for the first few weeks. My dd was big, 8lbs 13oz/21.5 inches at birth, and the 0-3 stuff was so enormous on her in the beginning that she looked uncomfortable. She wore nb clothes for at least 3 weeks. I always think clothes that fit appropriately are more comfortable, and hopefully it's not going to break the bank to get a couple of newborn sized sleepers.

Ethan was pretty big at almost 9lbs, and we wore newborns until he was 10 weeks or so... And even in diapers, we used newborns until he was about 11 weeks old, too. We are still in ones and Ethan is nearly 15 lbs.
