We use reusable sandwich bags, or waxed paper bags if none happen to be clean at the time.

Flatware- we use whatever mismatched that we have, so it doesn't matter very much if somehow it gets lost/stolen. They do make bamboo flatware but we don't use that. I don't see a point of buying something new when what we have works perfectly.

What sort of reusable sandwich bags do you have? The bamboo sounds great since I can not use my moms flatware out of the house. Love the lunch box you use, it would give ds a better one for school next year and the option of putting each persons lunch separate for ease of sorting to eat.
You should definitely check out www.reuseablebags.com. I've bought a bunch of stuff from them for DH's lunches. They have TONS of options. We use the Wrap-N-Mats for sandwiches, they're super easy to clean and have been working great for 2 years.
if you go on Etsy, try https://www.etsy.com/shop/moocowmomma because her bags are slightly larger than the waste not ones. I personally really like the zipper style bags. They are fantastic, and no crumbs get stuck (I always get crumbs, tiny hairs, & ickies in the velcro and have to clean them out).
Mamamade also will custom make some for you if you want, she does good work. She sells ready made ones, but I prefer not to use PUL with food, even though many people do it and say it is fine. I'm weird though.
Re: Please name your lunch box solutions
DH and I pack our lunches.

For our lunch bags, we use the 6-pack size Acme bag from reusablebags.com, those bags are made in the USA. They work really well and are quite durable. However, no way are they a picnic basket. They work great for us though. https://www.reusablebags.com/store/reuseit%E2%84%A2-made-insulated-pack-lunch-medium-p-1927.html
This might be an option for your carrier https://www.ebagsetc.com/servlet/the--dsh--CoolTote-Travel-Cooler/Categories
Here's what we use in our lunches. We're not into the "take and toss" thing, so everything is washable and reusable.
sandwiches = https://www.amazon.com/Trudeau-Corporation-Reusable-Sandwich-Box/dp/B002TG0K80 (BPA free, and the only plastic we use in our lunches)
snacks, sides = https://www.amazon.com/Pyrex-Storage-14-Piece-Round-Clear/dp/B0000CFTB0/ref=sr_1_3?ie=UTF8&s=home-garden&qid=1274394021&sr=1-3 (glass containers with BPA free lids)
silverware = old silverware that doesn't match our current set
