Can someone help me make a (simple) graphic "transparent"

My sister is helping my dad with something and needs the following (or something like it) with a transparent background.

I don't know how to do it, can anyone help me out? Either with instuctions or just create the image?

the image is rough, my sister sent it to me in a word doc. I copied it into paint, and then fiddled with it in gimp based on instuctions I found on-line. I don't think it worked though

So one question... do you want the image itself to have a transparent background or do you need the image to be white, with a transparent background?
