Currently, I get 19 days PTO which covers everything from sick time to vacation to personal days. I have the option to "buy" 5 additional days. We also get about 8 holidays, just the bare bones schedule.
3 weeks of vacay. Unlimited sick. We are pretty flexible here. I can't remember the last time I put in any paperwork for a day off and I don't ask my employees for their paperwork either.
If it gets abused, I'll address it, but it's never been an issue.

I am a teacher. I have 3 days off is sept, 2 days in oct, 3 days in nov. we get the dec 23rd to jan.2 nd i think. we get the week off in feb. we get a week and a half for spring break. a day off in may none in june then summers off. We earn a day a month for absences.

I've been at the same job for 5 years and am up to 20 days vacation off. As far as sick time/family sick time, we get as much as we need. I also get all stats off (or a day in lieu if it falls on a weekend) plus the week between Christmas and New Years when our office is closed (doesn't go towards vacation days).
I'm a professor, so I get all school holidays off on the academic schedule, though I am expected to be available to students via email and working on my research during that time. I technically get 3 weeks PTO a year as well, but generally we don't have to take it because we can find someone to cover our classes or schedule appointments around our teaching schedules.
Jeez! Where do you people work? I accrue 15 days of PTO per year which must be used for vacation AND sick. I also get 1 personal day and only 7 holidays.
My new job is 26 days of PTO that includes vacation, sick, AND holidays. They are "progressive" in that they don't close for holidays due to having people from different religious backgrounds and such. So if you are Jewish and don't mind working Christmas, you can use your PTO for something else. I will need to take to Christmas as a PTO day though. I actually think this is a very smart way to approach it - because there are times when I would rather choose my day off that be forced to stay home on say, President's Day or something!
Also, due to all my time off this year traveling and such I am currently ONE WEEK in the hole for PTO. Which means I won't even get paid for my last week here. Sucks.
Next year, I will get 30 days PTO (vacation, sick all together) and I can carryover 10 days.

I get 10 days vacation, 15 after 8 years (I've been here 4).
5 sick/personal days.
I am able to work from home sometimes.
