Currently, I get 19 days PTO which covers everything from sick time to vacation to personal days. I have the option to "buy" 5 additional days. We also get about 8 holidays, just the bare bones schedule.
Starting in January, I get an extra 5 PTO days!
Paid vacation: 2 weeks, with the option to purchase a 3rd week (which I did)
Sick time: 3.5 weeks
PTO: 5 days

3 weeks of vacay. Unlimited sick. We are pretty flexible here. I can't remember the last time I put in any paperwork for a day off and I don't ask my employees for their paperwork either.
If it gets abused, I'll address it, but it's never been an issue.
PT - 2 days per year
Vacation - 15 days per year
Sick time - 15 days per year
I've been here for 9 years.
ETA: Holidays are the bare bones Christmas, New Years, Thanksgiving, Labor Day, and Memorial Day, and 4th of July
I get 20 days all PTO to be used as sick or vacation. I also have the option of buying 5 additional days which I usually do. Next year I go up to 25.
10 personal days, 15 vacation days, 7 sick days, 1 floating holiday (and major holidays off)

New job, so people who have been around longer get a bit more...
Sick time: accumulate 8 hours/month, so 12 days/year.
Vacation time: accumulate 10 hours/month, so 15 days/year
PTO: We get two "personal holidays" per year.
We get the standard holidays (not Veteran's Day or Columbus Day) plus the Friday after Thanksgiving and the day before Christmas.
In a year's time:
- 3 weeks vacation
- 3 Family Medical Leave days
- 3 Personal break days (you can leave without an excuse)
- 3 Floating holiday days off?
- Unlimited sick days (if you get to 10 days, you do get talked to)?
It restarts in January of every year.?
We get 3 weeks of vacation plus Christmas week (so 4 weeks total). We don't have "sick" days per say - its at your manager's discretion.
Best part of my job though is our schedule - we work a 9/80. Its a two week rotation, so M-Th I work 9 hour days, the first friday we work an 8 hour day, and the next Friday we have off. I don't know what I do now without that every other Friday off!

I've been at the same job for 5 years and am up to 20 days vacation off. As far as sick time/family sick time, we get as much as we need. I also get all stats off (or a day in lieu if it falls on a weekend) plus the week between Christmas and New Years when our office is closed (doesn't go towards vacation days).
My company just got even stingier than they already were.
I get 10 days vacation, 15 after 8 years (I've been here 4).
5 sick/personal days.
I am able to work from home sometimes.
10 vacation days (starting in January I'll get 15 days since I'll have been with the company for 5 years)
6 sick days
2 floating holidays
We have 10 set holidays (days off) per year.
25 PTO days (next year I hit 10 yrs so I'll get 30)

Jeez! Where do you people work? I accrue 15 days of PTO per year which must be used for vacation AND sick. I also get 1 personal day and only 7 holidays.
My new job is 26 days of PTO that includes vacation, sick, AND holidays. They are "progressive" in that they don't close for holidays due to having people from different religious backgrounds and such. So if you are Jewish and don't mind working Christmas, you can use your PTO for something else. I will need to take to Christmas as a PTO day though. I actually think this is a very smart way to approach it - because there are times when I would rather choose my day off that be forced to stay home on say, President's Day or something!
Also, due to all my time off this year traveling and such I am currently ONE WEEK in the hole for PTO. Which means I won't even get paid for my last week here. Sucks.
Next year, I will get 30 days PTO (vacation, sick all together) and I can carryover 10 days.
I'm down to about 3 days left this year since I didn't accrue leave while I was out on maternity leave and ended up taking an extra week of leave b/c I didn't understand how the my leave had to "time out" to go onto STD.

For my first year it's 10 sick days, 10 vacation days and all of the bank holidays. Woot.
I believe starting your second year it's 10 sick, 20 vacation and all of the bank holidays. And starting out in the middle of the year this year, they were awesome; 5 sick, 5 vacation, the remainder of 2009 bank holidays.
We get 3 weeks of vacation (so 15 days) and 1 planned week of vacation between Christmas and New Years (the company is closed that week).
We also get 3 floater/personal days to be used whenever.
Our sick policy is 5 "occurrences" so if you are out 1 day or 3 days, that is 1 "occurrence". After the 3rd day, it goes to short term disability. Once you get to anything past 3 occurrences, you are given a warning or notice that you are approaching your limit. But people go over it all the time when they call out a day here or a day there and nothing ever comes of it.
Personal Time: 12 days a year, and you can carry over five unused days to the next year. This time can be used in increments of 15 minutes.
Vacation Time: 10 days + 1 day for every year of service, and the time starts on your anniversary. In January, I will get 17 days of vacation. this time can also be used in 15 minute increments.
Half Fridays: 12 a year. You can stack two of these to take the whole day off, but they can only be used on Fridays.
Holidays: New Years, Memorial Day, July 4th, Labor Day, Thanksgiving (Thursday and Friday), Christmas Eve, Christmas Day and Birthday.
Vacation: 15 days
Sick: 12 days (you accrue 1 day each month)
Holidays: We get all the normal holidays and we are closed dec. 23- Jan 4th. So, that's a big chunk of time there too paid.
Right now I get 13 sick leave days a year and 19 annual leave days. Every three years of overseas service I have to take home leave of at least 20 working days. No other PTO.
ETA: Whoops. Forgot holidays. Federal holidays, plus ten Indian holidays (about every other day is a bank holiday here, so our local staff vote on the ten Indian holidays they want us to recognize).
More than I can use. I currently have 33 accrued.
We get 1 1/2 vacation days per month, 1 sick day per month, 1 personal day per month. Vacation days you keep indefinitely. Some sick & personal days are "use or lose". You lose all but 20 sick days and all but 4 personal days at your rollover date.
I work for a university so I also get two weeks off at Christmas, plus major holidays.
