I don't have a very adventurous answer; I'm somewhere between the trunk of our Honda Civic, the Wal-Mart bathroom floor, and the Great Cloth Diaper Change.
The Wal-Mart bathroom floor was definitely the nastiest, but I did put her on a changing pad and she didn't touch the floor, but still... ew. I had walked all the way back to the back of the store because I knew that bathroom was less busy than the front and the changing table was "Out of Order," however that happens... I was tired and lazy.
And the Great Cloth Diaper Change... there's nothing wrong with it, but I found I was not nearly as crunchy as everyone else, and it was just a strange experience. My husband saw so many boobs that day, haha! I wanted to go because they were giving away a lot of prizes and freebies. But then we had to stand around for a while waiting for the time to change the babies and we were all crammed into a small space and roped off. It was an interesting, strange, and kind of fun experience.

I second PPs about restaurants needing changing tables. I've been to way too many that don't.
That said, we road trip a lot and I've changed DD on more bathroom floors than I can count. Ick. Now that we have a bigger car (a Suburban), there's plenty of room to change her on a seat when we stop.

At a bar, in DD's stroller all the way reclined. They didn't have a changing table in the bathroom (surprise!). We were out with co-workers for happy hour. It was a lovely poop diaper, much to the delight of everyone around us.

I went into a bathroom with a changing table that came up to my shoulders. Ridiculous. I laughed I'm 5'9" so I can't imagine anyone shorter than me trying to use it! Impossible!

In a charging booth. There was no public bathroom. I needed to buy a dress for my sisters wedding. My lo was only a month old. I was wasn't ready to leave her with anyone. It was raining out.
Like the rest of you have said I am shock at how many places don't have changing tables and yet are a kid friendly environment. I have definitely changed her on a few bathroom stall floors. Now I just assume places wont have a changing table and change her in the car before I enter the store or restaurant, etc. When she was a baby this was fine but now it is hard to change her cuz she is 35" tall. But she is also started not to fit on changing table too. I guess potty training is in our near future.
I've so wanted to leave a dirty diaper at these places to let them know what I think about them not spending a little dough on a changing station.

Probably the hood of our car in front of a gas station. We were stuck in traffic, and when we finally reached an exit, the line for the bathroom was coming out the door! So H just changed him on the hood.
