I just want to say... if you are not handicapped DO NOT use the handicapped stall! Sorry I just had to get that out. We have 5 normal stalls and 1 handicapped stall at work and people still feel the need to use the handicapped stall. Ugh, drives me crazy.

2 -3 cars parked in the handicapped parallel parking for vans so that the van has to park in a regular handicapped and its wheel chair ramp sticking out into the lane.
women who are not even noticably pregnant parking in the pregnant spot, this especially annoyed me when I was out alone at 39 weeks and very sleep deprived.
When people park so horribly that no one can park on either side of them or get out if already their when the person parked...my parents witnessed this. a guy in a huge truck parked so that the cars on either side couldn't get out without hurting the truck and their own vehicle. when one of the other vehicle owners came out he just stood and stared at the truck dumbfounded...jaw hanging open. It looked like he was typing the info in his phone so he could go back in the store and the manager could page the owner to move his truck. at that moment the truck owner came storming out of the store, bag in hand, red faced and raging. started screaming at the car owner typing on his phone, got in his face. Dad was ready to break things up if needed, but the truck owner cussed the other guy out got into his truck and squealed away...luckily without hurting either car. The other owner looked at mom and dad completely flabbergasted, they chatted and had a disbelieving laugh over it then the guy left. I can't stand bad parking.
Uneducated assumptions...not that the person is uneducated in general but that they make assumptions about a certain subject without properly educating themselves about that subject.
I can't stand when I am in a narrow shopping aisle with DS in the cart and someone (without a LO) MUST squeeze their cart down the aisle. Before I had DS, I always left the cart at the end, retrieved what I needed, and walked back.
On a similar note, it bugs me when somebody blocks the whole aisle with the shopping cart, and I have to say, "EXCUSE ME!" like, three consecutively louder times until they move. How about just being polite enough to pull to the side in the first place?
I could go on and on.
