Add heat either from a heating pad or warm washcloth, etc and take an ibuprofen. I'm in the middle of this also, and that combo along w/ what you've posted above has been working for me. (But mine is in a different location) Good luck!
Baby Boy Born 3.15.11. 8lb 9oz, 21.75in.
6 month stats: 20lb 11oz (92%tile), 30.12in.(100%tile - who knew it went over 99th? Which means 100% of other 6 month olds are shorter than mine!)
Nurse him while he is laying on a couch/ bed and you are hanging over him. Let gravity help. Have your significant other massage while doing so. It will hurt like heck, but this is the only thing that every helped me.
Take a disposable diaper and fill it with warm/hot water and secure it to your breast. Sounds crazy, looks crazy, feels WONDERFUL! It stays warmer longer than a hot towel. Take your ibuprofen, massage, nurse often emptying the breast. Watch out for mastitis, I just got over it.

This will work. Also try a hot shower and massaging the clog while trying to manually pump. I had a killer clog once (had it for 2 days), and this is what finally got it to go away.
Ditto to all the suggestions. I found I had better success w/ my hand pump than the electric one. I guess because I can control the speed/force of pumping?
Look for a teeny tiny white blister on your nipple, if you have one, pop it with a sterilized needle, instant relief! I've had success with this twice. Milk will shoot out, the massage the rest out. Just did this last weekend...
