Infertility

Im confused... help please!

When I started with clomid and 7dp"O" blood work early last fall I was told by my OBGYN that anything over 12 shows ovulation. Well I have only had a number above 12 two times since then. Today my REs office calls me and tells me that my b/w from yesterday showed my progesterone was a 6.4... she said they like to see it over 10 but that ANYTHING OVER A 3 SHOWS OVULATION?!?!?!?! what? My RE also during consults tried to act like we have had "successful"cycles at ovulating a bunch of times. Are they full of it?! This really confuses me and actually really makes me mad. please help.

  • This is why you should never start infertility treatments with an OB.  Your RE is right (though my office says anything over 4 confirms it, it varies a little by office).

  • I believe it's actually if your progesterone is over 5 from what I have heard.  Also I think your age plays a part in that as well. For example if you're dealing with something like DOR I was told it would be lower. So no I don't think they're crazy.

  • Hey Linds! I was pretty confused when I switched from OB to RE. Their numbers were completely different from each other. I'm with L&R in that the RE knows better. Sorry you're confused and irritated :)

  • Well my OB did bloodwork to see if there was an issue to begin with because I was missing periods. If she didnt do the blood work I would have been in the dark till about now (1 year of trying). My nurse today said that they werent happy with the results and that they like to see if over 10.... but then why is that if ovulation is anything over 3?!

  • My RE said anything over 5 confirms ovulation and everything I've read about progesterone confirms that number.

  • Also, if anything over 4 or 5 shows ovulation then why do they want it to be above 10 and consider this to be a failed cycle. I get to confused about all this sometimes,  lol!

It confirms ovulation, but I don't think a 4 or 5 P4 level is high enough to sustain a pregnancy, which is why many RE's use progesterone supplements for medicated cycles.

This is how it was explained to me as well.  That a level over 4 or 5 means you ovulated but you need your P4 to be over 10 or 15 (my RE likes over 15 for medicated cycles) to sustain a pregnancy.

This is why I was always so curious as to why you were never put on progesterone supplements post IUI/ trigger - it is such an easy way to ensure your levels are high enough to sustain a pregnancy.

I think they like to see a higher number with medicated cycles...

this. Although a 5 indicates that you've ovulated it is not strong enough to sustain a pregnancy. My RE wants it 15 or higher at 7dpo blood check. I would ask for progesterone supps. for your future cycles.

  • I didnt end up doing IUI this time, i was cancelled due to no mature follies. I am moving to injectables this cycle so hopefully this will all change =) and I think I am going to ask about progesterone suppliments but im thinking my RE wont want to because he likes to be able to check my progesterone for O and especially changing meds I think he might wanna see it. Never hurts to ask though, right?! thanks girls!

  • Yes, your RE and what the other girls said is correct. Anything over 4-5 is considered an ovulatory cycle, but for medicated cycles they like it over 15 because it means you had a stronger ovulation and can sustain a pregnancy. If you are on vaginal progesterone suppositories (which I hope you are in this case) that extra progesterone doesn't register in the bloodwork so it can still sustain a pregnancy with low progesterone numbers on b/w. It doesn't necessarily mean this cycle is a bust if you're on supplements. My progesterone 7dpo was 7.0 and I now have a 2 yr old as a result of that cycle. Good luck to you. I hope this helped answer some questions.

It will not interfere with letting him see that you ovulated. You don't start P4 supps. Until after you've ovulated.
