I have a couple of ideas, but I was looking for some ideas. I know it's kind of late, but I never sent a thank-you yet and I want to send a pic. of Natalie and card to the staff there.
I just wrote a thanks for their skilled care of my daughter and support helping us all through the challenging time and mentioned what a difference they made in our lives.
I sent a photo collage and a handwritten note updating them on Jake's progress and thanking them for helping us to get through a frightening experience. His nurses and docs were amazing. And his night nurse was on the phone with me every morning at 4am. She was a huge comfort when I couldn't be with him.

My Dad is a social worker in the NICU and I can tell you that you're definitely not late sending the card. Sending anything at all is more than most folks do. And the ladies (and men, haha) LOVE seeing pictures of older babies. It's fun for them to see the huge change from newborn to healthy, bubbly eight month old!
It was at about 9 months that I started feeling really guilty that I hadn't done any formal thank yous to the NICU. We did just the basic ones but for LO's 1st birthday we are doing a then and now collage (including pic similar to siggy) with a big thank you and baked goods.
Re: What to write in NICU thank-you card?
That is so sweet of you! I honestly have no idea. I wouldn't know where to begin. Maybe try asking on the preemie board?
I guess I would just explain how much you appreciate all they did for your LO and you will never forget it.
