Easy/basic reward chart

I think I'm going to start using a reward chart with my almost 3 year old DD.  Does anyone have one they can share that is fairly basic?

I just Googled one for my DS.  Sorry, I don't have the link anymore. But if you just search for kids reward charts there are lots=)

I just draw a grid on a piece of paper for the chart. I usually use construction paper or something more fun than just a white piece of paper.

What chores do you have listed? I am thinking about doing something like this for DS -he doesn't have "chores" per say but I do want him to start being more responsible for himself. I was thinking of some sort of chart for like brushing his teeth, putting his dirty clothes in the basket, cleaning his toys up, throwing away garbage/putting dishes in the sink etc......what ideas do you have??
