How often do you change out LO's pj's?

Every single night.  In fact, I wash everything he wears after one wear, unless it's a jacket or somthing like that... or if he only has something on for a little bit.

How do you handle hats?  I usually wash DD's when I wash her coat on Friday, but I often wonder if I should wash them more often.

I'm in the minority, but I switch them out every other night (unless they're dirty of course).  She gets a bath every other night and gets new jammies then.
