STD claim form question

Silly question that I also asked of the girls on 3rd Tri.  Do you take the claim form with you to the hospital and have them fill it out after you give birth or give it to your OB like your FMLA paperwork prior to giving birth (with the estimates of time off)?  My STD policy is separate from my employer and they were not sure when I turned in my FMLA stuff on Friday.  Obviously, I'll ask my OB on Tuesday, but  just wondering what you ladies did?  Thanks.

Regardless of who the policy is through, your PCP which is your OB in this case, is the one who determines your inability to work so his office should take care of the paperwork.  Just make sure you let them know it needs to be sent to a different place than your FMLA documents and provide them with info (address, fax, or whatever).

I was told by my STD agent to take the paperwork to the hospital when I went into labor and have them fill it out before I leave otherwise I'd have to make a special trip to the OB's office.

I guess I should clarify.  I did bring it to the hospital with me.  My husband ran it over to the OB's office one of the days I was still a patient since the practice is essentially at the hospital.
