I got mine from USA Baby. It has staph guard and is double sided. Toddler side is plusher, newborn side firmer. It has the square corners instead of rounded so it fits closer into crib corners.
We got the LA Baby organic one from costco.com. Haven't used it for baby yet obviously, but the reviews are great and I love that it doesn't have any icky smells, etc.
I just changed mine today from the Classica Colgate recommended by Baby Bargains to a Serta Organic Mattress. BRU is selling them all Serta's 50% off when buying a crib. I'm trying to save my mom $$$$.
Unlike previous posters have said, the other L.A. baby one mentioned in this post only has organic cotton, it is not chemical free. I decided to pay a little more to get something chemical free, and I like that the fire retardant aspect is due to coconut rather than wool (not so hot and less likely to be an allergen.)
Re: Crib mattress suggestions?
