Would you use a name that you really loved if the initials would be SOB?
DH and I have a name we both like. Both the first and middle name have significance to us, the first name is the last name of my best friend since I was 7 and a man that DH has been friends with for 25 years whom he respects and admires. The middle name is my grandfathers.
We would use a nick name that begins with F, so for common day to day use with work, school, friends and family, his first name would begin with F.

I am too busy trying to figure out what name starts with S but has a nickname that begins with F to actually ansswer the question.
SOB would be cool initials to have. you could do TWO middle names if you want to avoid it, though. My son and my brother both have two middle names..I never even figured out my son's initials until right now-WVGH.

Personally I dont think that initails are that big of a deal. Our girl name initials would have been GAS and DH did not like that. I dont care. Its not like people are going to call her that. The way I think of it how many middle names of your closest friends do you even know. I have three best girl friends that I have been friends with since the early 90s. I can only think of two of their middle names and I am not positive that I am right on both.
SOB is not a term I would think someone of young school age would even know. JMO.
I might... if it was a girl- yes, because when she gets married she's more then likely going to change it anyway.

Our daughter is AJS - her middle name is June. I could have gone with an S name and might not have even realized it!
Use the names you love.
We would go by our initials a lot in our family... and my initials (premarriage) were MJB, so my stepmom called me "The Coffee" for the longest time. It was dumb, but it didn't bother me.
I think SOB is fine. I would do it to my kid.
