We vaccinated according to adjusted age and we are using an alternate vaccination plan (Dr. Sear's, Vaccine Book). It just made us feel better. I wasn't comfortable with vaccinated them when they were so little.
I was told to vaccinate at their actual ages. ?DS still hasn't had his first vaccination, though, as he still hasn't met all the health requirements for it.
I did adjusted age. Pedi wanted actual age but I explained my reasons and my pedi is laid back, so she was OK with it (we do actual age and Dr. SEars alternate schedule becuase we are going in monthly for Synagis anyway, so why not spread out the shots too). Reasons were - DDs were nothing like their actual age in development, vaccines are likely to be more effective at adjusted age because the body's immune system has likely more time to respond as it is supposed to, DDs stay home with only 2 nannies and are not exposed to much (like no daycare or older siblings)
We did actual age except for the Pediarix at the neonatalogist's recommendation. It would have been administered right after she came home and he had seen babies in the NICU have adverse reactions (having to go back on oxygen, etc) so we waited a month until she was bigger.
We have been doing actual age and he has been doing well with them.

our pedi is not doing any kind of sched. he just finished getting his 2 month shots last week. She was only giving him a shot @ a time. Plus he was getting Synagis as well.
We did actual age for everything until this point. However, a one year old would typically get the MMR and Chickenpox vaccine and we're not giving either next week when he goes for his one year checkup. I think we're going to skip chickenpox completely and we're going to delay the MMR indefinately for now. I'm trying to locate the vaccines components separately but Merck just discontinued it and they were already on backorder so I'm not sure I'm going to have any luck. DS also stays at home with a nanny and has limited exposure to other people and no siblings. If something changes and he has to go to daycare, we'll vaccinate at that point because it's a requirement.

Actual age... preemies are more at risk for serious problems/death if they are to contract anything, so it is more importatnt for them to get vaccinated on time so they have at least some protection.
