@optimistgardener I did find an academic article suggesting that metformin can lower AMH in women with PCOS, so I wonder if that's the case? Because metformin did significantly regulate my cycles over the last 4-5 months, so maybe I'd have been more high-end normal without it? It's hard to say without having an AMH reading from before. I supposed I'll get a definitive answer on it come the ultrasound whether or not my ovaries are polycystic there.

@optimistgardener thanks for the tag re: PCOS question! @fantasyflyte I had indeed been misdiagnosed with PCOS in my early twenties. The difference though was that it was based on an ultrasound (TBH that ultrasound was done NOT on Day 3 of my cycle) and my complaints of irregular periods. In the past, determination of PCOS was done if the woman was presenting with 2/3 possible symptoms, the 3rd being high androgens. However, when I started all my fertility testing in 2020, all my ultrasounds/androgen markers were within normal range. However I still had quite a bit of variability when it came to period lengths. In my case, it turned out to be thyroid issues driving the cycle irregularity (but they never were drastic... ranged from 24-42 days). One thing to remember is that there are different phenotypes of PCOS. You can read more about that here (note this will download the full text article) and here. Bottom line: you might still have it.

How are things going?: Okay ladies, got our blast report back and it's.... exactly average. Stats and discussion in spoiler:
We've ended up with 5 embryos: 2 x 3AA, 2 x 3AB, 1 x 3BB. Why average? Because our doctor said that we should expect:
50-80% of resting follicles to respond to sims (we beat that; had 14 AFC, but had 16 follicles at retrieval) 80-85% of retrieved eggs to be mature (beat that too, 13 out of 14 were mature) 85% to fertilize (here, we're right on the money with 11/13 of 13 fertilizing) 40% of fertilized eggs to make it to blast (just slightly above average, but let's call it average for the sake of the argument, since it would have been 4.4 and we got 5)
So, by average, we can expect about 40-45% of our embryos to be euploid. So, we're expecting that number to be 2. And maybe 1 more mosaic.
Btw, none of the eggs from my "Canada eggs" made it to blast.
TL; DR we've decided that 2021 will be our retrieval year, so we're gearing up for another retrieval. After that, we'll close the chapter on retrievals, and move forward with transfers in 2022. I feel good about this.
I'm also truly ecstatic about how this fresh cycle turned out. And who knows, we might get lucky and end up with more euploids than the average, but I need to "protect" myself. But, I cannot lie: I am extremely super bummed about my younger, frozen eggs. Like... REALLY really bummed. Alas, it is what it is. I also know that once I start transferring, I will not want to go back to "retrievals" and put my body through that again.
There are also insurance implications here. I have enough Progyny benefits for either 2 transfers or 1 more retrieval + fertilization, but no genetic testing (the latter would cost $6900 OOP). So, MH and I are strongly considering not doing genetic testing on this upcoming batch. This will be our "backup batch" (subject to change depending on the genetic results of present batch). And retrievals (including meds) COST SO MUCH MORE then transfers so doing a retrieval would help us max out our insurance benefits. But then, of course, all transfers would be OOP for us. Considering cost of transfer is 1/4 of that of retrieval, we are OK with that.
Also, to be honest, I wasn't ready for a transfer this year. It would have been the week before we left to go to AZ for the holidays, and I would have had to have a beta at Quest / LabCrop and I wouldn't be able to enjoy a glass of wine or egg nog with my in-laws. So, yeah, this is the better plan.

But really great about your current report. 5 is a great number I hope you get pleasantly surprised for more than average but I understand reserving your expectations. My IVF was very similar. I still wish I could have done back to back ERs. I think you’re making a good decision to focus on ERs now and then just do transfers. And selfishly, timing wise we may end up transferring at the same time

And you know I am in favor of embryo banking-- especially since (a) for you, it seems to be working, and (b) since you are "unexplained", the transfer side could be an issue. Obviously I'm sad that you won't be joining the exodus this time, but I think it's sensible. Also-- it's totally normal to be bummed about your Canada eggs. It always feels nice to have an insurance policy and it's highly disconcerting when it disappears on you. But it's better that you know now so you can bank the embryos you need.

And now I feel really good about this decision, albeit a little bummed about not joining the current exodus. However, I've never seen anyone on here say "gee, I wish I didn't do so many retrievals;" however the opposite is oft the case (although back in the summer there was an off-hand comment someone made about 'trying to figure out what to do with their remaining embryos'. I think many of us would like to have that problem.
And yes @hanshotfirst77, maybe we'll be in the same exodus cohort along with @kiki047 and hopefully @keikilove and maybe even @walkawaynee. Woot! Plus, a transfer in late Jan/early Feb will yield me a Libra TBH, and I'm a Libra myself.

@acleverusername That is a fantastic update! Make the argument for “average” all you want😄 but those results are wonderful! However, I’m very sorry about your younger eggs. That is understandably disappointing. As others have said, it would’ve been nice to have those as your back-up plan. But you seem to have a great back-up plan formed with the current use of your insurance. It sounds like a really smart choice. And it allows you to fully enjoy the holidays, which is always a treat. I hope the exodus continues from now through the holidays and into early next year!
AFM, bloodwork came in very late last night. My numbers look excellent, which really surprised me being that this is the luteal phase. My body is acting like I’m in follicular phase so it’s a go for the IUI this Saturday. I’m just glad this whole cycle is drawing to a close. I welcome some boring normalcy after this.
@acleverusername my plan is if i get blasts it's likely i'll get them tested. i'm still within the window for my "multiple cycles" set charge with cooper genomics, so it seems like it would make sense to do the testing even if i only get a couple? i hadn't even thought about that til they sent me an email this morning reminding me that i wouldn't need to do anything more than pay shipping for more tests. anyway, that pushes me out into next year for a frozen transfer if i get that lucky.

Second, 5 blasts is great!! If you're anything like me, I'm sure you were hoping to be above average, especially with your windfall # of eggs going into this round, but it's also oddly reassuring to come in around the average too. My old RE used to say average is good, average people get pregnant, we want average. Really sorry about your CA eggs though, especially because you froze them at a time when they were meant to be a backup plan, and now that you needed them it didn't work out as you hoped. I get that. FX you can more than make up for them in your next ER. I'm glad you're feeling good about that part - maybe we'll be cycle buddies! @keikilove I didn't realize you had that much sperm available to you - in that case, it sounds like this is just what you need to salvage this cycle. FX for you friend!!

I came across a rather shocking pregnancy announcement from an ex-friend who plastered it all over all her socials that she's having her "miracle" second baby after "IF" (she called it IF, I called it her being impatient because it took her more than a year so she went straight to IVF and was successful on her first try). Which makes her yet another person lapping me and she will very well have the 2nd before we even get to transfer again.

@optimistgardener this is the girl that told me I should "just do IVF" (like her) at a time when we were still exploring other options. I pointed out that just because it worked for her doesn't mean it'll work for me, and she accused me of throwing her PG in her face because I'm jealous. That was the breaking point. That was also my first IF-related breakup, but let's be honest, she was toxic to begin with so it wasn't really about IF. But speaking of friends, I'm so glad you had that drink! I find it so hard to make new friends as an adult so hopefully this is the start of some new bonds formed with your colleagues. I plan to indulge in some much-needed wine tomorrow night as well.

Sorry it's taken me so long to respond this week. I got called in to work Sunday night and then worked my normal three 12 hour shifts Monday-Wednesday so I've either been sleeping or working all week. Thank you all so much for the kind words. Update below in spoiler:

I started having heavy period-like bleeding on Wednesday so I figured my body would be getting back to "normal" soon. Well I went for a repeat beta this morning and my HCG went from 27.8 on Monday to 41.8 today. My RE called me and said that sometimes part of the pregnancy can be still there and causing the HCG to be produced even if the pregnancy is abnormal. So now I have to go back for another blood draw next Wednesday to see if the number has gone back down and if it hasn't then she'll have to give me medication to get it down. The even more frustrating part of this is that it delays everything even further and she said there's no way we'll be able to fit in another FET before the end of the year. I'm disappointed both because I hate being in limbo but also because I've hit my insurance deductible so another transfer would be completely covered this year but now I'll have to shell out the deductible again in January. It's not the worst thing ever, but after all of this it just adds to my frustration. I'm just tired and feeling very defeated right now, especially because I have absolutely no control over any of this.

There's an evangelical quality, sometimes, to people for whom a treatment has worked. it can be hard for them to acknowledge that, indeed, everyone has their own path and also their own medical realities that are not everyone else's medical realities. sorry you had to deal with that.
but also YES! about making friends as an adult. this is a huge challenge! through my late 20s and 30s i used my graduate program as my social foundation, but when i left grad school and moved back "home" (or at least back to my home state) i realized how hard it is to build a community from scratch in your 30s. this was exacerbated, of course, by the arrival of covid. there's been a whole lotta me, myself, my couch, and my pup going on around here. human friends would be lovely.

so i am on CD 10, having inserted my nuvaring on CD 1 as directed, and i'm STILL BLEEDING. Kinda off and on, but most days definite "heavy" spotting and today it's more like a "light day" for a real bleed. According to my IVF calendar, i'm supposed to remove the nuvaring this Wednesday the 17th, and my baseline is Sun. the 21st.
The clinic nurse was very on the fence about how to deal with my calendar when she decided to do this short BC prep phase. I think she would have preferred more time, like another week or two, on the Nuvaring, but didn't want to have me on for almost 2 whole cycles, as would have been the case if she'd started me in October. But my baseline, U/S check-in appointments, and stimming calendar were already set so she sort of crammed me onto this short BC prep. I'm a bit worried it's too short and it will not have done its job of calming my hormones down by the time I arrive for my baseline. Does anyone have any guesses as to whether this ongoing spotting/bleeding means anything in particular about my hormone levels?

@optimistgardener thanks for the tag re: PCOS question! @fantasyflyte I had indeed been misdiagnosed with PCOS in my early twenties. The difference though was that it was based on an ultrasound (TBH that ultrasound was done NOT on Day 3 of my cycle) and my complaints of irregular periods. In the past, determination of PCOS was done if the woman was presenting with 2/3 possible symptoms, the 3rd being high androgens. However, when I started all my fertility testing in 2020, all my ultrasounds/androgen markers were within normal range. However I still had quite a bit of variability when it came to period lengths. In my case, it turned out to be thyroid issues driving the cycle irregularity (but they never were drastic... ranged from 24-42 days). One thing to remember is that there are different phenotypes of PCOS. You can read more about that here (note this will download the full text article) and here. Bottom line: you might still have it.
Okay, time for my update. Fasten your seatbelts.
Diagnosis (if you've been): Officially, unexplained. Unofficially, Hashimoto's, AMA, wandering sperm
Status (WTO/TWW/TTA): Whatever the phase after ER and before a period is. WFAF?
What are you doing this cycle? (Testing? Treatment?): Treatment
How are things going?: Okay ladies, got our blast report back and it's.... exactly average. Stats and discussion in spoiler:
50-80% of resting follicles to respond to sims (we beat that; had 14 AFC, but had 16 follicles at retrieval)
80-85% of retrieved eggs to be mature (beat that too, 13 out of 14 were mature)
85% to fertilize (here, we're right on the money with 11/13 of 13 fertilizing)
40% of fertilized eggs to make it to blast (just slightly above average, but let's call it average for the sake of the argument, since it would have been 4.4 and we got 5)
So, by average, we can expect about 40-45% of our embryos to be euploid. So, we're expecting that number to be 2. And maybe 1 more mosaic.
Btw, none of the eggs from my "Canada eggs" made it to blast.
TL; DR we've decided that 2021 will be our retrieval year, so we're gearing up for another retrieval. After that, we'll close the chapter on retrievals, and move forward with transfers in 2022. I feel good about this.
I'm also truly ecstatic about how this fresh cycle turned out. And who knows, we might get lucky and end up with more euploids than the average, but I need to "protect" myself. But, I cannot lie: I am extremely super bummed about my younger, frozen eggs. Like... REALLY really bummed. Alas, it is what it is. I also know that once I start transferring, I will not want to go back to "retrievals" and put my body through that again.
There are also insurance implications here. I have enough Progyny benefits for either 2 transfers or 1 more retrieval + fertilization, but no genetic testing (the latter would cost $6900 OOP). So, MH and I are strongly considering not doing genetic testing on this upcoming batch. This will be our "backup batch" (subject to change depending on the genetic results of present batch). And retrievals (including meds) COST SO MUCH MORE then transfers so doing a retrieval would help us max out our insurance benefits. But then, of course, all transfers would be OOP for us. Considering cost of transfer is 1/4 of that of retrieval, we are OK with that.
Also, to be honest, I wasn't ready for a transfer this year. It would have been the week before we left to go to AZ for the holidays, and I would have had to have a beta at Quest / LabCrop and I wouldn't be able to enjoy a glass of wine or egg nog with my in-laws. So, yeah, this is the better plan.

May '22 egg retrieval, lupron flare protocol. HGH priming for 1 month before stims; md lupron 20/20u daily; menopur 225, follistim 225, 10k trigger after 8 days of stims; AFC 10; 13 retrieved; 10 mature; 8 fertilized. All frozen at 2PN for later thaw, grow, and biopsy.
July '22 egg retrieval same protocol: 9 days of stims; AFC 8; 10 retrieved, 8 mature, 4 fertilized and frozen.

@acleverusername no idea what's up with my cycle. Part of me wonders if that was just a stupidly light period because my lining was virtually non-existent after my withdrawal bleed. But honestly I'm so sensitive to any little shift in hormones and my cycles go all haywire every time so it's probably just that. I'll be really pissed if that really was AF though and now I'll have to wait a whole 'nother month. On the bright side, we'll surely have our PGT results back before we start again so there's that. They said 2-4 weeks and it's been 2 weeks today so... FX!

Oct '21 somewhat improved AMH (.51) and significantly improved FSH (8.9). Maybe it's all the pills I swallow.
Dec -21, standard antagonist. 225menupur/225gonal-f/cetrotide/5000u trigger, AFC 5; 11 days of stims. retrieval yields 5, 4M, 2 reach day-6 blast (4AB & 4BB). 1 low-level mosaic (chr. 3 deletion), 1 complex abnormal.
Feb '22 FET (low-level mosaic): CP.
May '22 egg retrieval, lupron flare protocol. HGH priming for 1 month before stims; md lupron 20/20u daily; menopur 225, follistim 225, 10k trigger after 8 days of stims; AFC 10; 13 retrieved; 10 mature; 8 fertilized. All frozen at 2PN for later thaw, grow, and biopsy.
July '22 egg retrieval same protocol: 9 days of stims; AFC 8; 10 retrieved, 8 mature, 4 fertilized and frozen.
Sept '22 ER same protocol: AFC 10; 12 retrieved, 11 mature, 11 fertilized.
From the 3 retrievals, 7 survived to day 5-6 blastocyst, from which I have 2 normal embryos!
Jan 20, Fully medicated FET, baby aspirin, dexamethasone, acupuncture, and as much woo as I can stand (which isn't very much). BFN.
July 23, Natural FET + baby aspirin. BFN.
