  • @aera11 I'm the crazy one who tested out her trigger shot and then kept testing every day until two days before original planned beta (when I started spotting). My trigger actually only took 5 days and then I was 100% clear. 

    And I'm traveling for a mix of work and pleasure this time. One week with work, but actually not to work - it's the last-but-one week of my Exec MBA program in class, the final week will be in Munich in November. (How this will fit with my ER is going to be "interesting"!!!) Then for the second week DH will join me and I'm so excited as it will be his first time in China! So I plan to show him around my old haunts :-) 

    @dragonette505 the "why no Menopur" question is pretty high on my list. The Pergoveris pen I used was the only stim. Last time I only had the one follie grow but I can see in my notes I had an AFC of 3. So as I really need all to grow each time and something to stop one racing off ahead and selecting itself at the detriment of the others would be good. Otherwise I may as well do the stim phase as a natural IVF cycle if we only get one each time (but still prevent O and trigger).

  • @emmasemm I also tested out my trigger when I transferred only in that case I never got a negative, so it was even more confusing... but still I think testing out the trigger is a useful thing to do and will do it next time. 
    I also think it's possible a little estrogen priming or some sort of priming might help in that case unless there is some reason to believe priming would be detrimental. Estrogen priming as I understand partially can suppress you but is mostly to allow all of your follies to grow together instead of allowing one dominant follie to get in the lead.

  • @dragonette505 another question I'm going to ask is about how fast I tested out the trigger shot... to be honest I'm not really sure if it "took" properly at all as the lines were very faint on the HPTs and I mean five days is very fast... I have a feeling my body just took over and over-ruled all the meds... I mean of course I'm *never* bossy so no reason my body might decide to be bossy ;-)

  • @aera11 oh wow!!! Fx for beta tomorrow! That's the right way to start our board! Now everyone else just needs to copy lol!

    As I basically need a nuclear missile to get my ovaries doing anything in the first place, definitely no BCP for me as if we stop them they might never start again!!! 
    If i do any priming I guess it will be Estrogen, but let's see. I see from everyone else Lupron is either heaven or hell, similar to the discussion about HGH and also DHEA. 
    Not enough evidence to know which patients it will be awesome for and which the opposite, so if you want to try you have to be prepared to be a guinea pig... 

    Do you remember your AMH and FSH numbers? And what was your protocol this last time? Especially as it seems to be a good one for you ;-)

    Haha I know!  But I'm only cd26, does that matter since retrieval (cd13) & transfer (cd16) were early?  So far af has arrived by cd27 so I'm holding my breath til tomorrow but thank you both!!  I get nervous every time I go pee & wipe now

    @steph8986  Do you have to do OCP?  I know everyone’s different, but I didn’t do anything before starting this last cycle and it was kind of awesome.  OCP makes me batty.

    @laurad75 I have to try to maintain balance with the Christmas decorating because I grew up with our parents’ house participating in Christmas home tours :p. DH starts to feel like it’s more “my” house than “ours” if I overdo it...  But still so much fun!

  • @artemis618 so I am usually hungry so I have small meals or snacks throughout the day, but just try not to eat much after dinner...I felt like I needed a second meal after dinner even, it was crazy.  Happy Early Birthday!

    @steph8986 I know 17 days sounds like a lot but time will go by quickly; I've been at this for so long & I often feel like we've just begun.  Hoping your bleeding stops soon for your sake :) how many boxes of tamps have you gone through?! 12 days...jeez, I had a two week period after my *TW* d&c and it felt like forever.

    You guys, what the heck is going on?!  I got the call from my nurse & my beta is 108 & progesterone 44 (is that good? She said everything looked good so I am assuming but I don't remember from last time)!!  HOLY CRAP.  Like the nurse said, we have a lot of hurdles to go (I love that they are realistic with me b/c that's how I am).  There was not one thing in this round that gave me any hope, so I am blown away.

  • Thank you @hopefulmom (I'll get back to your msg later tonight) & @baseballhoney!  Still having pgal brain but I think that's something you never truly shake.  One day at a time though, right?  My next beta is Saturday!  My dr waits 4 days so the nurse says it should hopefully be at least 400, so crossing everything I've got.

    this also might be a *tw* for some...

    we had signed up 5 weeks ago for this adoption seminar that's tonight, so we're still planning to go.  A small part of me feels guilty sitting there knowing I'm pg but I'm trying to remind myself not all people adopting are doing so bc that's their only choice.  I just feel like I still want the information bc it might be something we choose to do regardless.  I know this isn't a board for adoption but it's part of our current sitch & my current anxiety.

  • I've just started my first round of IVF so not sure yet when I will actually be doing my transfer, but so far this thread seems...the most relateable?   : ) I have been giving myself injections of "c-hcg" and "gonal-f"; had baseline ultrasound last weekend (another today and next one Friday)I have a fridge full of meds that I haven't even started yet so I'm really not sure of what sort of timeline we are looking at in terms of retrieval!

  • @sunnyd_lite how many days have you stimmed? Most people stim for 10 days then trigger and have retrieval 2 days later. Of course it's different for everyone and how long you stim is based on how fast your follicles grow.

  • @dragonette505 thank you so much! Its been about a week...a few of my follicles today were measureable and were much higher than when I was in Sunday so hopefully when I go in on Friday they will be even better!

  • UPDATE: My doc says she can remove the polyp during my next ER and I would be able to transfer with my next cycle.... I decided to go with that plan :)

  • Question for the IVF Veterans: What was your AFC and AMH relative to the number of eggs retrieved? My AFC was 24 and AMH 2.9, My doc thinks we can reasonably expect up to 20 eggs, but never having done it, who knows?

    @steph8986 Its still subject to other things like, age and other conditions (pcos, endo and etc) I'm in my 40's and my AFC is usually around 9 and my AMH was 2.3 at last check...
    I get around 18 eggs on a good cycle (largest retrieval I got 24). But ive also had awful cycles with 5 or 6 eggs retrieved. My doctors were surprised, but this sort of thing can happen. The younger you are and if you don't have any other known conditions, you can generally expect those basic numbers to correlate.

  • Hi, 

    I'm hoping joining this board will give me the positive juju I need to push for a November IVF cycle.  

    A quick background, I am 32, have severe anovulatory PCOS, my DH is 38 with no known problems.  We started TTC 3 years ago, but after no success in 2 years, tried for an IUI both September and October 2016, however I did not respond to letrozole so both cycles were cancelled.  My RE would not try injectables because I was too high risk with PCOS and my AFC of around 50.  So we proceeded to IVF, had first retrieval in early December 2017, had to do a freeze all because my progesterone rose before retrieval and I developed mild OHSS.  16 eggs retrieved, 12 mature, 9 fertilized, 2 embryos frozen on day 5. First FET in January, BFN and same result with second FET.  Afterwards, my RE informed me that embryos were only fair-poor quality (she rated a C+) so she started me on merformin 2000mg daily.  We completed another round of IVF in April, again freeze all, 15 eggs, 12 mature, 9 fertilized and 3 embryos (B+/A-quality).  So we transferred 2 in May, and another BFN.  

    I then completed ERA test in June and had a very abnormal result (2+ days preceptive, only 5% accurate) with the crinone which I took for 4 days, so with my RE's recommendation completed ERA again in July, this time with 6 days of PIO and thankfully got a Receptive outcome.  So we were prepping for another round of IVF in August (as I only had one frozen embryo left and wanted tot transfer too), but after a few complications decided to move forward with the single embryo FET.  Prepped for the FET in September and had the transfer on October, my RE really reassured us that this would be it as my lining was perfect, embryo was already hatching and we knew it was the appropriate implantation day.  

    At 2dp5dt the symptoms really started this time, nausea, cramping- much more than I ever experienced before.  I thought for sure it would be positive.  Yesterday on 9dp5dt decided to take a test as I could not stand the stress and anxiety of one more day of waiting especially with the symptoms I was having and the busy schedule I have today.  So I test and nervously look, and it's another BFN.  

    I go for beta today (because I have to, even through dreading it), and I usually get the call between 2-4pm, they also usually send a portal message.  Well I am really surprised when they call at noon and not only send a portal message but also orders (never done this before after beta).  So I read the orders and the covering MD informs me that my beta is a 2 and that I need to continue meds and go tomorrow for recheck.  She said it's technically considered negative as its less than 5, and that it's residual from trigger shot. Uhh, this is an FET, and we did not do a trigger shot.  So I guess this get's to drag out a little longer...but on the positive side, I guess it wasn't 0 as my last 3.  

    I have set up a follow-up consult with new RE for November 7th (although am on cancellation list and hoping to get appt next week).  If everything goes as planned, I am hoping to start stims in early-mid November and expect retrieval soon after Thanksgiving.  we will have to do a freeze all, not only based on my history, but also because I need 6 days of PIO instead of 5.  

    This journey of IVF is so stressful, but am happy to be a part of these supportive groups.  Looking forward to hopefully a positive month for all of us.

I'm currently starting my 7th cycle and have had 4 losses, so I know how tough this can all be.

  • Hey all - sorry I've been a bit MIA. Been crazy busy with work as I'm out for the next 3 weeks traveling and getting everything organized has kept me more than busy, along with dealing with all the coming down from the last cycle. So apologies for a long update  - which I've put in a spoiler.

    @aera11 wow congrats! That's awesome news and fx everything runs smoothly!
    @artemis fantastic news you can go straight to FET now. And yes the LH in the Menopur is apparently to help maturity, but my Pergoveris also has LH.
    @dragonette505 oh what a relief she can deal with the polyp during ER and not affect your transfer! Where are you on DPO?

    So the short version is I'm looking at next CD1 being Nov 1 or 2 and free light for round two with extra testing between now and then.

    Today was my follow up. And I have to say bless my RE we were 1.5 hours with him going through everything!!!
    - First of all he did an US and there is no cyst, so that's good. Also I have one follie who looks like he's the "one" this month as it's CD7 and he's already 15mm. There are another two on the left around 5mm and one very small on the right. So an AFC of 4 which is really really good for me. Also my lining was already 9.5mm and tri-laminar. So he suggested HIO for the next 4 days ED until I fly to China as you never know!
    - The cramping I'm feeling he thinks is just the last effects from the progesterone. There was nothing to see on US but I've released my pelvic MRI to him (done by my RA) so he will check that to see if there's anything to see though we don't expect anything.
    - With my AFC of 1-4 and AMH of 0.65 he reconfirmed we should only expect 1-2 eggs each cycle. Whilst the insurance is paying for meds he said no harm to try again with meds but after that he's happy to take us without stims, just Ganirelix and trigger to not waste money and have less side effects if we need/want
    - We will start with same stims as last time (Pergoveris 225 - which is the FSH/LH combi) and potentially step down or step up. And if one doesn't work try the other in cycle 3. We don't add Menopur as it is the same as the Pergoveris. We discussed potentially using FSH only in a future cycle but he is concerned about lack of egg maturity then.
    - We discussed suppression and priming at length. He still thinks suppression will give no real benefits for me, risk not getting started well again after and will waste a1-3 months. With Estrogen priming with my low AFC he doesn't think it will help, but might add some Estrogen in after the ER with the progesterone this time so long as my levels are like last time as again at that point it might help lining etc and won't do any harm. For testosterone he said with my numbers he has never seen a positive outcome and again has seen negative ones so does not recommend it "pure". The DHEA he can't prescribe in country, however I could do self pay through his colleague in New York but they already discussed my case and neither of them thinks I'm a good candidate.
    - We talked HGH and this again is not possible domestically so he said we should think about it if we get to cycle 4 and take the additional prescription from his colleague and have it shipped in. If we use DHEA or HGH in an insurance cycle it would invalidate the coverage. But he said the research is so far only anecdotal (he even showed me on the screen the research he has reviewed again in the last weeks) doesn't suggest I would be a good candidate 
    - As my follie seems to be selected early he's happy for me to start stims whenever I want CD2-4
    - We will start monitoring on day 3 of stims (so after only 2 nights of injections) in case we want to start the Ganirelix earlier
    - He confirmed my embryo from last cycle was beautiful. I even signed off that he could use a photo of it in a paper he is writing. He said it was ahead of the average development until the point we transferred which he sees as a good sign. Also why he didn't want to change much in the stim part of the cycle.
    - But to give any future for embies the best chance we will go with assisted hatching from now on. He had already got pre approval from the insurance for it.
    - Having previously said I need three failed cycles before I can have further testing (or three MCs or a loss/child with proven genetic issues) he had taken my case to the genetics approval board and  insurance company and has got approval to run full genetic screening for me as well as an RPL panel. So I gave 14 vials of blood today and signed all the paperwork! Results should be back in around 2 weeks so we can factor anything in before we start.
    - Based on my previous BW, what he saw in ER/ET, other history and conditions he is expecting to need to prescribe heparin but needs the results first as it's strictly controlled here - I asked about baby aspirin as a starting point and he said I should use it for this natural cycle until we get the results, but if results come back for any IVF cycles we hit it hard. 
    - We talked potential intralipids and he will follow up with the national specialist Centre who is 1.5 hours from me to see if he can get me in (as it doesn't do any harm)
    - He has discussed with my RA and they are waiting for my BW from this Monday to come back but might double my RA meds through the next cycle to see if that helps, or add in some additional meds. This month no harm to go with the Benadryl as a temporary measure.
    - We talked acupuncture and he fully supports it. He said he can't explain why it has a beneficial effect always but again it does no harm and clearly shows a positive correlation to good outcomes so gave me some pointers and I'll be trying to get in at a good one tomorrow.
    - At the moment as we aren't freezing in any cycles due to low numbers then an ERA doesn't really help and if we do a mock cycle we are again delaying (and I'm really on a clock!) - but he said when we think it might be our last OE cycle he would suggest to do it before. But it's up to me, he would be happy to do it any time and then we would have to figure out a strategy to achieve it if it showed a problem with timing
    - Lastly I asked if he thought we were crazy still trying with OE and if he was just not saying but thinking we should go to DE. Here he said for him if I can yield mature eggs he is happy to keep trying - but it should not be about him, it should be about us. That there's the money - when is our limit and how many cycles can we afford. And the time/effort - we both have big jobs and especially for me each cycle means a lot of time out and limits on traveling. And of course the emotional part - he said some couples can really keep going for ten cycles without success whereas for others one or two cycles is the limit. He also reconfirmed that DE is not available in Germany, so we would have to get frozen eggs from another country (and go through some major legal hoops) or go overseas for DE treatment. But for now it's plan B. 

    Overall was a very intense discussion but I came away feeling really listened to, taken seriously and happy that my RE suggested half the things on my list himself without me asking. I feel like we are on the same path and I think that really helps in the tough times.

  • @baseballhoney ; well, I cant do a fresh transfer (which I wanted to do) so that sucks, but its better than 3 months or something like that, so i'll take it!

    @emmasemm I've missed you girlie! And now I have to miss you while youre gone :) :) But you deserve a little away time for sure! I'm ovulating today, actually! lol So I'll start estrogen sometime next week, then I'm back into a cycle! When do you think you will start again?

  • @emmasemm I just actually realized you said it all in the spoiler!! lol So now I'm up to speed! yes, I'm def going to be November. Last time my doc didn't have me start stimming till CD7 so by my estimation that has me starting stims on or around 10/31
