Trying to Get Pregnant

IF/Testing Checkin Week of 10/10

**General TW that this thread can and will mention MC and loss.***

This is a place for those of us who have already started TTC, but have started infertility testing and/or treatments. You can express your thoughts, feelings, and frustrations on infertility. TTC can be a long journey naturally, then add in the difficulties associated with infertility and it becomes a whole new ball game. The road to infertility can be lonely, frustrating, and complicated, so let's make this a place where we can vent, ask questions, and support one another.  Feel free to resurrect this thread at any point in the week if you have something to say, and treat this like an ongoing conversation.

Diagnosis (If you've been):

Cycle/CD:

Status (WTO/TWW/TTA):

What are you doing this cycle? (Testing? Treatment?)

How are things going?

Any questions?

GTKY: If a genie gave you three wishes, but they all had to be superficial nonsense type wishes, what would you wish for?

Re: IF/Testing Checkin Week of 10/10

  • Diagnosis (If you've been):  Unexplained secondary (with a mostly-obstructed left tube and low morph on SA, neither of which explain our issues according to our RE)

    Cycle/CD:  cycle 12 / cd 17

    Status (WTO/TWW/TTA):  TWW

What are you doing this cycle? (Testing? Treatment?). First-ever treatment cycle: Femara + Ovidrel + IUI + progesterone

How are things going?  3 dpiui, so I start progesterone suppositories this morning.  Not super looking forward to that, but I'm sure I'll figure out the mechanics once I try.  Yesterday was the first day I didn't feel completely demolished by the Ovidrel trigger, so now that the fake-pregnancy side effects from that seem to be fading, I'm really hoping the progesterone doesn't add a whole bunch of new ones!  I'm feeling pretty positive and hopeful this week -- and that scares me.

    Any questions?  None right now.

GTKY: If a genie gave you three wishes, but they all had to be superficial nonsense type wishes, what would you wish for?  I'm not sure these count as superficial enough but here we go... 1) For the place I live (necessitated by DH's job) to have 4 real seasons so I could stop missing the place I grew up, 2) For my house to be just one room bigger so we wouldn't have to move if we get pregnant!, and 3) For a higher GRE score without having to waste time re-learning silly high school geometry just to execute my back-up plan for if we can't get pregnant.
  • Loading the player...
  • Cycle 22, CD23
    TWW - 7 or 8 dpo
Me - unexplained bleeding, DH -low morph
Waiting for appointment with new RE - 1 month down, 6 to go.
This is my third cycle since moving and my third cycle with minimal spotting. It's so weird, it's like something about my old job/city/life was the "unexplained" thing that was causing the bleeding.
GTKY: 1) The landlord increased the number of pets we can have 2) that we get the super cute kitten DH wanted to adopt 3) A super cute, very warm winter coat and boots
  • It's completely normal to O later than normal after an HSG @britters314
  • @BaileyM321 I would wear a panty-liner... those progesterone suppositories make a mess...

@LadyMillil I'm sorry you have such a long wait to see your new RE.

@britters314 Glad your DH, even if reluctantly agreed to explore all avenues to becoming a parent.

@mrsdaddario I'm so sorry your feeling out and hitting your IF wall. creepy internet hugs. AF after AF is devastating and numbing all at the same time. There are no words to make it better, I'm thinking about you.

  • Thanks @LadyMillil

@mrsdaddario  I'm sorry you're feeling down. Does your insurance require you to go through a certain number of IUI cycles before trying IVF? Or do they not even cover IVF?
  • @britters314 Unfortunately our insurance doesn't cover IVF at all. But the price is extremely reasonable at our clinic compared to others I've seen. However it's still a larger chunk than we just have on hand, so we would need to save.
  • @mrsdaddario My commuting lifestyle was really hard on my body so I wouldn't be surprised if removing that combined with the changes I've been making in preparation for IVF helped. I've been trying to do some research to figure out what could have been going on, because all my tests were normal and uterine bleeding was really the only physical symptom that went away after moving.
I won't go on the cancellation list until about January. From what I understand they have a waiting list of 300-400 people and schedule from the top of the list about 3 months out. Once you're scheduled for an appointment you go on the cancellation list, so it's possible to cut a month or 2 off the wait but much more than that is unlikely.

@britters314 I had my HSG this cycle too and my ovulation has been funky. I have had positive OPK's for 6 days straight (like strong positives) which has never happened in any of my previous cycles. Of course I took this cycle off from temping so I am even more in the dark. I think based on CM I have already ovulated but if I haven't this would be a delayed ovulation for me. I'm on CD 18 and in the last year it has always fallen between CD 13-15 with a 14 day LP.

Diagnosis (If you've been): Looking like unexplained but have my follow up appt with the RE today to go over everything and come up with a plan moving forward.

    Cycle/CD: 13/18

Status (WTO/TWW/TTA):  TWW... I think? (not temping this cycle)

What are you doing this cycle? (Testing? Treatment?) Just testing this cycle. Hopefully will have a plan for next cycle after my follow up appt today.

How are things going? Good - just ready to have a plan moving forward to next cycle - makes me feel like I'm somewhat accomplishing something and moving in the right direction.

Any questions? Not right now but I'm sure as I enter this treatment phase I will have plenty!

GTKY: If a genie gave you three wishes, but they all had to be superficial nonsense type wishes, what would you wish for? 1) a bigger and fenced in backyard so DH can have the puppy he's been begging me to get for as long as I can remember 2) a daily hair/makeup person and fashion stylist so I can wakeup every morning and have other people take care of it for me (I loathe getting ready and picking out clothes in the morning) 3) unlimited PTO and a private plane so I can vacation anywhere at moments notice.

  • @LadyMillil I just looked at your chart, it looks verrrry nice lol. FX SO HARD! I keep saying I want to get a new job closer to home, I'm in the car for 3+ hours four days a week. An hour and a half to work each way. But I live in the middle of no where and the jobs are few and far between for the amount of money I need. What things are you doing to prepare for IVF?
  • Diagnosis (If you've been): Unexplained IF

Cycle/CD: Cycle 12 (month 14 or something), no idea on CD because I don't chart with monitored cycles

Status (WTO/TWW/TTA):  TWW, but waiting on AF.

What are you doing this cycle? (Testing? Treatment?)  Our last IUI after AF starts

How are things going?  Welp, another negative beta.  I really am having a hard time holding out hope for anything, including IVF if it comes to that.  I'm also having a hard time coming to terms with IVF at all.  I'm not sure why, I was feeling really good about it.  I've had pretty bad anxiety recently so I'm wondering if that's part of it.

Any questions?  Not at the moment.

GTKY: If a genie gave you three wishes, but they all had to be superficial nonsense type wishes, what would you wish for?
Good question!  1) The ability to change my hair, Tonks-style (ok so this gif isn't her hair changing, but HP fans probably get what I'm saying).

    2) A bigger condo, or the ability to change the size as easily as it changes on The Sims.

    3) A driver, I got hit by a drunk driver right after I got my license and just never got over the fear of driving after that.

I've been researching acupuncture and TCM and will probably start that by the end of the year.
  • @LadyMillil
just lurked your blog hard and I love it! <3
  • @britters314 My HSG didn't push my O date back at all, but I tend to O later anyway. I did read that it can be expected, but I hope you'll O on time, too!

So sorry, @mrsdaddario! That's such a tough place to be, but I'm thinking of you!

@LadyMillil Your wait sounds brutal. I can't even imagine. It sounds like you're doing all the right things in the meantime, though. It's also great that your bleeding has gotten better!

    Diagnosis (If you've been): endometriosis

Cycle/CD: I'm going to go ahead and call it CD 1 of Cycle 13/Month 15, but that might actually end up being tomorrow.

    Status (WTO/TWW/TTA):  WTO

What are you doing this cycle? Hopefully getting an appt with an RE and making a plan!

How are things going? Okay, I guess. AF should be here today (temp drop this morning) or tomorrow at the latest. I went to my follow up this morning with my OBGYN and he referred us to an RE, which I was expecting. I'm working on the paperwork now, and then they'll call me to schedule an appointment, but I have no idea what to expect as far as their wait time.

With the rest of my day off, I went shopping! I got H a new pair of pants for work and a couple of shirts, and a couple of new cute fall things for myself. Period presents. :)

    Any questions? nothing right now.

GTKY: 1. The ability to be present (or be teleported in) whenever @britters314 conjures her cheese. ;)
    2. For my house to be magically remodeled exactly how I want it.
    3. A closet full of cute clothes that renews itself each with season and trend (the ones I like, anyway!)

How are things going? Negative Beta today after BFN 9dpo and 11dpo (glutton for punishment).   Bummed. Looks like we are headed toward IVF.  Insurance requires 2 clomid cycles before they will even consider IVF even though my dr doesn't think it will do much for me.

Any questions? We start clomid this cycle. Any tips / things to look out for?  I'm terrified about multiples even though I know this is a) unlikely and b) we will have plenty of monitoring and c) this would be a blessing.

  • @Marley629 will you remind me, are you doing IUI? were your follicles all similar sized or did you have a few more dominant ones? That is a lot of folicles, but if some are much smaller than its probably not a big deal. I know that most RE's won't trigger you if you have more than 2-3 dominant follicles due to the risk of HOM.

@MMMMM27 fx that your just testing early. TW***as a MOM I was scared when I first found out, I knew the increased risk during pregnancy and the potential challenges once they arrived. They are a lot of work but they are the biggest blessing x2.  I can't imagine them as singletons. DH only wanted 2 but here we are trying for more. You cross what ever bridge when you get there. • @clairecf thank you! RE suggested 100mg Clomid cd 3-7 and IUI so that's what we're going to try to start. It feels good to be moving in hopefully a good direction! RE also 'officially' diagnosed me as unexplained which I was anticipating. FX for you this cycle!

@mmmmm27 I start Clomid this cycle too. DH's main concern with doing any fertility medication/treatments was multiples. RE assured us the likelihood was low so that helped calm his fears a bit. I think I'm more anxious about what/if any side effects I may have from it. I spent all day yesterday googling it which was a bad idea! haha

I asked this over in WTO and got one response. Then I searched the boards and sort of scared myself. H's SA came back with normal numbers in count and motility but 4% in morphology. The test itself said "normal". Internet has conflicting studies. My search of the boards says 4% is a one way ticket to IUI but more likely IVF with ICSI. Anyone have any insight here? My f/u appt with the RE is not until the 25th but I would like to mentally prepare myself for possible treatment recommendations. Thanks so much.

    ETA: spelling.
  • @mrsdaddario  good luck today! Thinking about you.

@Saralee797 we did not have sperm problems but with morphology being lowish I bet they would recommend at least IUI. They may also do a repeat SA to see if this is a persistent problem or just a one off thing. GL.

  • @saralee797 4% or greater in morphology is considered normal per the World Health Organization's standards. My RE doesn't even assess morphology when analyzing the samples produced for our IUIs because they care only about motility (although it was included in the initial SA). If you're concerned, I'd look into it more in peer-reviewed research, much of which you can find for free on PubMed. I found this one, which seeks to explain the reasoning behind the 4% threshold: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3739680/ How good that particular journal is, I could not tell you, but it may give you some advice.
  • *TW previous pregnancies mentioned*

@Saralee797 My DH also has normal SA except for low morphology (3-4%).  My RE considers us "secondary unexplained" despite this.  Basically what she told us is that sperm morphology is unlikely to change over the years, and since our first two pregnancies were both first-cycle-trying (ugh I hate my last-decade self so much for taking this for granted!! *vomit*) his low morph clearly wasn't a factor then and thus she believes isn't the factor now.

I've honestly been having some doubts about this, because we've been trying for 14 months now and we're both still in our early 30s, so clearly /something/ changed and the low morph was the most significant finding on all the tests.  But FWIW, that's what we were told.  (Maybe it makes a difference that it's secondary infertility?)

Anyway, we're doing IUI for now.
  • @BaileyM321 I don't mean to just jump in. But did you husband have an SA before the 1st kids? There could have been something between then and now that has changed. It is entirely possible.

@Saralee797 We have a morph issue as well. It is a very divided topic, but in the end...I think we see a lot of people here with morph issues that aren't getting pregnant. It seems that it is an issue to me. We are going straight to IVF with ICSI, our MD won't even try IUI unless morph is at 3% and even then he doesn't give it a very high % chance of working. MFI's best shot is really IVF with ICSI. I am sorry about the news, but hopefully you can get a good treatment plan in place and move forward! It is frustrating, but there are ways to circumvent.

  • @Zoey1019 No, DH did not have an SA before the previous pregnancies.  But my RE swears that morphology should not change -- something about because the sperm are generated from cells that line the testes and should match them, and they tend to be stable?  (I am the farthest thing from a life sciences sort so I'm probably explaining this wrong, but she was really really confident that whatever his morphology was when we conceived before, that's what it is now!)

When I said before that I'm beginning to doubt her, that's what I was saying I'm beginning to doubt...
  • @BaileyM321 @zoey1019 @wabash15 @Bababatty thank you so much for all your input! I guess I'll see what the RE has to say but at least I'll be mentally prepared to potentially move straight to treatment. At least it will be progress of some kind!
  • @Saralee797 @Zoey1019
This thread got me worrying about our low morph and whether my RE was ignoring it when she shouldn't, so I spent the afternoon doing some research.  I hadn't known the 4% threshold for normal was a recent thing and that it used to be 14%!  Found lots of confirmation that many REs won't even try IUI with morph lower than 4%.

Then I decided to follow @Bababatty 's excellent lead about this and go searching for peer-reviewed science.  Here are the two main recent pubs:

2012: This one concluded that lower morph does not have a statistically significant impact on IUI outcome as long as the woman is under 35, and concluded that IUI should still be used as the first line treatment option for morph <5% unless the woman is 35+ (in which case she should move straight to IVF+ICSI).  The numbers here for low morph pregnancy rate are definitely lower than the other morph subgroups... but the difference is not statistically significant.

    https://www.ncbi.nlm.nih.gov/m/pubmed/25439801/
2014:  This one makes an even stronger statement, and unlike the studies above the different pregnancy rates are basically identical:  "There is no statistically significant difference in per cycle PRs when comparing patients with a strict sperm morphology of ≤4% versus >4% who undergo IUI-P (17.3% vs. 16.7%; odds ratio 0.954, 95% confidence interval 0.66-1.37). .... Strict sperm morphology ≤4% is not associated with lower PRs in couples undergoing IUI-P, and thus should not be the sole reason for advancing to IVF."

So based on that, I understand more where my RE is coming from on trying IUI.  That seems very in line with the modern science.
