Lurking would answer all of these questions.
1. whatever you are comfortable with and your hospital allows.
2. pack whenever you feel like it
3. You shouldn't do anything for a baby shower. If/ when someone offers to host one, then you can give option for dates and guest list. But that is all.

1. that is 100% up to you. I'm having my SO & my mom with me. 2. there's a thread at the top of the March 2016 BMB all about packing for the hospital. 3. this is up to your host. you shouldn't be planning your own. my host has decided on some time in February. 4. talk to your OB about this.

Sure, I'll be Dr. Google for you.
1. Whoever you want within the hospital's policy. 2. Whatever/whenever you want. But here's a nice list to start you off. I suggest that you consult Google or use the handy search function to search for "hospital bag" and read the countless hospital bag posts if you want additional lists and suggestions. 3. Please don't throw yourself a baby shower. That's tacky and gift-grabby. If someone OFFERS to throw you a baby shower, consult with that person. 4. It's never too early to register.
And since I just read your other eloquent post about "how i make a birth plan?", I'd like to remind you that YOU ARE CAPABLE OF DOING YOUR OWN RESEARCH. Please stop relying on random internet strangers to hold your hand.

1. This is entirely up to you. I am only having my DH. My mom can be in the room but once pushing/delivery is happening I don't want her in there since she will stress me out.
2. there are tons of lists online but I plan on having: loose clothes (big t-shirts/sweatshirts and pajama pants/sweatpants), nursing bras/tanks/shirts, soft socks, uggs and 1 pair of flip flops, toiletries (mini shower bottles, toothbrush, deodorant, etc) maxi pads (my hospital does provide them but I am bringing a few), I got cheap underwear so I don't ruin my favorite ones, nursing pillow, nipple cream, nursing pads and any electronics/dvd's. For the baby, I will bring her coming home outfit, a fuzzy blanket and maybe 2 outfits. My hospital will just keep them in t-shirts and a diaper if no outfit.
3. Has anyone started planning? If someone will be hosting a shower that is the first thing to be discussed. I wouldn't do less than a month out. I say a month or more if you can before your due date cause you never know when they will come.
4. I would ask your hospital or doctor about it. My hospital has a link where you can pre-register so maybe if your hospital has a website you can find a way to register, if not, I would call to ask.

1. the number of people you want to have in the delivery room, limited by the hospital policy 2. Stuff that you want while you are at the hospital 3. when your hosts want to have your baby shower 4. when your hospital requires you to register
