Usually if you, as a patient, have THIS many issues finding a provider & staying with them you have to look at the common denominator: perhaps you are the issue?
If you doctor hop trust they know where your going & have to send the records to the three places that request them. If you are a problem "hot potato" client there is no reason for them to take you back. They don't want to deal with a hassle client.
