Will you help me make my list?

Hi. Today I am headed to what I called my WTF 1.2. After my failed cycle my WTF appointment with my RE at the time just didn't sit well with me. I thought he would have more to say or bring to the table but it seems like all he was doing was reading our chart then and there and basically chucked it all to bad luck. Today after almost 3 months I am ready to see an RE again. I am going to the same practice, as I think they are a good clinic, but am seeing another RE. This RE is actually the one that did my ER and my ET and I felt more of a connection to. I already have a few questions for her but I was wondering if you could proof read my list and perhaps add on things I have forgotten.

1. In your opinion why do you think our cycle failed?
2. I know my MRI showed I have a severe arcuate uterus and no septum was seen, but I have read and heard from other women that a SHG has shown uterine defects that MRI has failed to identify. Would a SHG be beneficial in my case?
3. We were told if we attempt an IUI our chances are roughly 8%, could you elaborate as to how this percentage of success is reached?
4. My husband has low morphology and has dx with slight MFI, apart from this humongous list of supplements he is taking anything else you can add?
   

What else could I ask. What did you guys ask in your WTF?

I asked if they felt a protocol change would help and which one / why.
