DS is on antibiodics for two ear infections and a sinus infection. He is having frequent loose stools and it has caused a diaper rash from hell that I can't get rid of. He's taking probiodics every day and it isn't helping much. The skin on his bottom is broken or cracked, it's just flaming red and he FREAKS every time you have to change his diaper or give him a bath. Anything that touches it causes him to scream. It's breaking my heart. Right now I am applying Triple Paste and/or Aquaphor with every diaper change. Does anyone have a fool proof treatment for this type of a diaper rash? Any suggestions are welcome.

For cleaning wash cloths and warm water only, no wipes.
For bathing consider adding baking soda to your bath. Separately have your LO wear a diaper to create a barrier between the water.
For creams lanolin is best - lanisoh nipple cream with desitin max as a layer on top.
We had an awful rash in the NICU and 8 weeks later at home. This was the only thig that worked for us.

I just went through that about 2 weeks ago. Worst diaper rash I ever saw on a child.
Things that helped me:
I finally had to stop with all the creams..it was like all that lotion was creating slime making it worse.
I did baking soda baths and they helped more than anything. (I just set her in..didn't do the extra diaper thing).
Then I let her be naked followed by a good dose of cornstarch.
Bag Balm works like a charm. Every time. You can get it online or at any pharmacy, Target, Walmart, etc.

Our doctor also recommended Maalox! We layered it with Triple Paste on top. It was gone in 2 days.. this was after antibiotics had left LO with a horrible rash.
I just spoke to our pedi for the same issue. I was told to give an infant probiotic, usually behind the counter at a pharmacy because it needs to be cold, Lotrimin antifungal cream twice a day and A & D diaper rash cream. She also suggested lots of apples and bananas and no veggies.
I have also been rinsing him in the tub at home when we have a messy diaper and while he is still raw and sore since we just started this after work the quick rinse bath prevents a lot of the screaming.
My son was having horrible diaper rashes during his horrible "teething" poops- so much so that his little bum was starting to break open. Just super acidic. So after talking with the pharmacist he recommended that I dab mylanta liquid on his bottom, let it dry and even let him run around without a diaper for a 'breather' and then A&D ointment. The mylanta neutralized the acid before it could reach his bottom and the ointment allowed his skin to heal. He quit crying during diaper changes and it was almost over night that he was not as red and it was clearing up! I have 2 children and have always used desitin and powder but for the big rashes and even the small ones now I just use A&D ointment and the mylanta! Awesome!
My LO used to get really bad rashes and we were always wrestling with how to deal with them. We finally figured out to use A&D {the Target brand equivalent works just as well for almost half the price} mixed with Mary Kay's Extra Emollient Night Cream. I think you buy it by itself or with the Satin Hands set. I know it sounds weird, and I don't work for Mary Kay so I'm not trying to endorse them necessarily, but this combo works miracles on bad rashes. My LO's diaper rashes were so bad, she was bleeding and would scream and we all dreaded diaper changes. Getting as much air to the rash as possible was good but you can only hang out with a naked baby for so long. We would combine those two things and rub a bunch on and within a day, the rash was gone or at least almost there.
My niece gets bad diaper rash when she's on antibiotics, too. For those bad rashes, my SIL has to get a prescription cream to make them go away. Good luck! Bad rashes sure are miserable.

Air and light are key to curing diaper rash. If you can, have him nap naked. If you're using ANYTHING that is a barrier. (coconut oil, lotions, Butt Paste, Aquaphor) make sure that his bottom is completely dry before putting them on. If the diaper rash looks like it has little dots in it (or around the edges of it), he has a yeast rash as well and the typical diaper rash ointments won't work on it. You would need something to address the yeast issue.
If you are breastfeeding the breast milk can help. Also, it's not going to solve the problem, but going on a bland diet yourself can cut the amount of acid in his bowel movements. Avoid anything acidic or spicy and stick to boring foods for a few days. It might sound extreme, but there is also the option of trying to catch him before he has a bowel movement, ripping his diaper off and holding him over the toilet. It would keep it from sitting on his skin any longer than the minute or so it takes for him to finish and you to get anything else off his bottom.
When my son has diaper rash I also splurge and buy the huggies naturals diapers. I don't know that it actually helps all that much, but they're the softest disposables that I've found and it helps me feel a little better.
This may sound extreme and bizarre, but it's amazing: White Vinegar.
This is if the rash is Yeast based, but if you're unsure, just give it a go. And no, it doesn't burn.

I would check to see if it is yeast based. It sounds like it. You can use an OTC cream like you would use if you had a yeast infection. A lot of kids can yeast infections that look like diaper rash. If it gets too bad you will need a prescription that can be pricey- like $50-$75 for a small tube.
As far as I know the <a href="https://www.scutece-adulti.ro/produs/2230-Sudocrem-125-g" target="_blank" title="Sudocrem"> Sudocrem </a> is quite good for diaper rush.
Usually a rash cream should contain Zinc or/and Panthenol.
I would recommend taking him/her in to the doctor to check it out. They will eb able to tell the difference right away. If it is a yeast infection (yep, they can get them on their bum) you may need a prescription ointment to cure it. If it is indeed just a diaper rash I highly recommend baking soda baths and then letting that booty air out. I put the smallest size cotton underwear on my son when he had diaper rash to keep the raw skin protected from dirt etc but we would let him go diaper-less for most of the day. If he peed it was a tiny amount to clean-up (and pee is strerile) and if he pooped the underwear would mostly contain it. That booty needs air to heal!
When a diaper is necessary petroleum jelly works fine as a barrier for their skin and it's cheap.
My great grandmother swears by burned flour... apparently when I was little, mom refused to try it and went with all kinds of fancy creams... eventually she gave in and the rash went away within a very short amount of time. I also agree with pp's if you can let the baby go without a diaper for a while, that would help as well.
If it seems like a very serious rash, then go to your dr.
My close friend swears by cornstarch- nothing more, nothing less. She told me her baby boy had such a bad case of rash that his little tooshy was bleeding red and cornstarch was the only thing that soothed and healed it. I hope you find something that works, rash is so horrible/painful
We had the exact same situation last week..double ear infection and awful cold
FIRST - Get bananas, rice and anything else that your LO will eat that is binding and feed it to them.
SECOND - I found that Desitin worked best.
THIRD - When you are home don't use wipes, use warm water with a little bit of your LOs body wash. Try to let it air dry and if possible let LO sit there without a diaper for a few minutes.
Once the loose stools stop it will clear up quickly just continue to glob on the diaper cream. I mean put on as much as possible every diaper change. It really really helps.
Here's some ideas that worked for my niece when she had one that bad...
Lay down a towel in the floor and let baby go without a diaper for a while...the air will help alot.
Blow on area when you wipe.
Use A+D ointment...not the diaper cream the original ointment (may have to buy WalMart Equate brand, but it's the same)! All other creams did nothing for my niece...but the A+D cleared it up to where she wasn't screaming within the day.
