Trying to Get Pregnant

IF Testing & Treatment w/o 8.23

This is a place for those of us who have already started TTC, but have started infertility testing and/or treatments. You can express your thoughts, feelings, and frustrations on infertility. TTC can be a long journey naturally, then add in the difficulties associated with infertility, and it becomes a whole new ball game. The road to infertility can be lonely, frustrating, and complicated, so let's make this a place where we can vent, ask questions, and support one another. Feel free to resurrect this thread at any point in the week if you have something to say. Treat this as an ongoing conversation.

Re: IF Testing & Treatment w/o 8.23

  • @mokay19 good luck!!! Hopefully the minimal Gonal F dose does the trick without making the other follicles grow too. Sounds like you have a very busy few months coming up so 🤞🏻 that everything works out.

@gingermama29 I can’t remember: you tried IUIs yet? Does your OB do them at their office? But definitely try and stay optimistic while also planning for the future. I find doing that helps me with my anxiety.

Diagnosis (if you've been): Officially, it’s unexplained. Unofficially it’s Hashimoto’s, AMA, and sperm-driven fertilization issues.

    Status (WTO/TWW/TTA): WTO

What are you doing this cycle? (Testing? Treatment?): follow-up with RE on Thu; and I guess trying naturally this cycle so as not to waste one? Also, detoxing from my vacation.

How are things going?: good good; been doing lots of thinking over the past few days about what we might want to do next. But ultimately, we want to talk to the doctor on Thu to see what he thinks.

Part of me wants to get the transfer portion started while we see what comes out of the previously frozen eggs I have up in Canada (11 mature eggs). But the anxious / pessimistic part of me (and remember that I’m a project manager, so that part of me is especially vocal) wants to do another retrieval because I feel like we just got lucky last time.

    TW friend’s failed transfers
A close friend of ours has had 2 failed transfers of genetically normal embryos (having had both Receptiva Dx and ERA) so I’m basically counting on the worst case scenario. I think the “unexplained infertility” diagnosis makes it especially bad because we’re essentially checking off things that could have gone wrong while trying naturally. And so far, everything - more or less - has gone right so of course now I’m thinking that it must be an implantation issue.

    Any questions?: nah

GTKY: If you could be on a game show (past or present), which would it be? Oh, definitely Price is Right. I’d love to win a showcase showdown lot!

  • @acleverusername no we haven’t tried IUI yet. This cycle is technically my 4th medicated one but *TW* two of them have ended in a MC. That’s such a hard decision you have ahead of you but I think @keikilove put it perfectly. I hope your doctor helps you choose whatever will be best for you and we’ll give you THBs!

@keikilove oh friend, I’m sending you so many hugs right now. It’s insane how unfair this process has been for you and topping your final cycle with YH’s news is so much. I’m glad you’re able to see this journey to its conclusion and I’m praying that it’s successful and you finally get your THB. I wish there was more that I could say or do. Just know that I’m thinking of you and crossing everything for you!
  • @keikilove I'm so sorry your family is going through this-- I had a close (young) family member also have a cancer diagnosis after IF, and it just felt like, supremely unfair. I'm glad you're going through this last retrieval, and I really, really hope it leads to the outcome you want, I also just see the benefit in knowing it's over one way or the other.

@mokay19 that's great news that your follicles are growing! i've got my fingers crossed for you that things will continue on this track and you'll get lucky!
@gingermama29 i know it's not in any way really "good" news, but your track record demonstrates that sperm and egg are meeting, which indicates you've got great timing, sperm know how to swim in the right direction, open tubes, and therefore will probably not improve your chances much with an IUI unless you do it while superovulating. I hope this is your lucky month and you don't need the RE!
@acleverusername i hear you on the sequential anxiety. Now that you've got those good embies, of course it makes sense to worry about implantation. I think is a good idea to really listen hard to RE's plan on how he intends to maximize your chances by cultivating a plush, healthy lining. Definitely pull on TEW's ear if you can about this, also. I get the desire to play things safe and try to line up some more embryos just in case, but remember it's a lot to put your body through, and perhaps unnecessary when you may well have all the embryos (and healthy youngish eggs) that you may need! If we presume your canadian eggs are similar or better in quality to the ones you just fertilized, there are likely more viable embryos there after you fertilize that batch.
edit to add a key word in gingermama's note.
    History
I am doing this on my own. Left ovary and fallopian tube removed due to cyst/necrosis just after birth. Right ovary and tube still purport to function.
Began TTC in Aug '18 @ age 35
5 or 6 (or 7?!) cycles using frozen donor sperm, ICI. All BFN
    Mar-Jun '19 IUIs, all BFN
Began process of referral for RE in spring of '19, blood tests confirm low AMH (.54) but all else is good.
May '19 HSG shows open tube, but ultrasound suggests fibroids
MRI in June '19 confirms two large fibroids, one growing through wall of uterus
In June '19 I move from California to Oregon, disrupting my insurance, referrals, etc. 5 months spent piecing it all back together.
Nov. '19 attempted myomectomy (vaginal approach) failed. 2nd surgery using laparoscopic method in Jan '20 is successful.
April '20 --> June '20, natural IUIs, all BFN.
July? saline ultrasound and bubble test demonstrate open tube
July '20 clomid cycle cancelled for thin lining
Aug-Oct, 3 femara cycles, all BFN
RE referral. While waiting to set up an appointment Nov-Jan,I continue with natural cycles and ICI, using a known donor, BFN...
Jan '21 more bloodwork, AMH is worse (.30), FSH high albeit not catastrophic at 13.2. hysteroscopy all good. "looks cozy!" says the RE.
Feb '21, injectables (menopur, novarel), iui, BFN (doc said the frozen donor sperm sample had the "best numbers she'd ever seen!")
Mar '21, injectables, iui cancelled due to lack of follicles, converted to at home-insem. BFN
May '21, first IVF round cancelled due to cyst.
July '21 IVF: standard antagonist. 225menupur/225gonal-f/cetrotide/5000u trigger, AFC 7; 6 days of stims. 8 eggs retrieved, 7 mature, 5 fertilize, 4 made it to blast. 3 are 4AB, 1 is 4BB. PGT-A results all abnormal.
Oct '21 somewhat improved AMH (.51) and significantly improved FSH (8.9). Maybe it's all the pills I swallow.
    Dec -21, standard antagonist. 225menupur/225gonal-f/cetrotide/5000u trigger, AFC 5; 11 days of stims. retrieval yields 5, 4M, 2 reach day-6 blast (4AB & 4BB). 1 low-level mosaic (chr. 3 deletion), 1 complex abnormal.
Feb '22 FET (low-level mosaic): CP.
May '22 egg retrieval, lupron flare protocol. HGH priming for 1 month before stims; md lupron 20/20u daily; menopur 225, follistim 225, 10k trigger after 8 days of stims; AFC 10; 13 retrieved; 10 mature; 8 fertilized. All frozen at 2PN for later thaw, grow, and biopsy.
July '22 egg retrieval same protocol: 9 days of stims; AFC 8; 10 retrieved, 8 mature, 4 fertilized and frozen.
Sept '22 ER same protocol: AFC 10; 12 retrieved, 11 mature, 11 fertilized.
From the 3 retrievals, 7 survived to day 5-6 blastocyst, from which I have 2 normal embryos!
Jan 20, Fully medicated FET, baby aspirin, dexamethasone, acupuncture, and as much woo as I can stand (which isn't very much). BFN.
July 23, Natural FET + baby aspirin. BFN.
  • @mokay19, so happy to hear that the injectibles seem to be going well!!  I'll have all my fingers and toes crossed for you!

@optimistgardener, great news that your RE doesn't seem perturbed by this last cycle and is on the 'bad luck' boat.  It is weird how knowing the sex somehow shifts things so they feel more real despite no actual change in circumstances.  I think that's one of the most challenging aspects of this whole process - thinking of things in a scientific, analytical way is tough when you suddenly start assigning human characteristics to the same piece of the process that you're trying to (hopefully temporarily) stay detached from.

@keikilove, I am so, so sorry to hear your news.  My heart aches for you and your H.  I'm glad that your TTC journey will at least reach a conclusion but am hoping that it ends on a high note and that this newest unexpected hurdle proves to be as small as possible.  Nothing I can say seems like enough, but know that you are in my thoughts.

Diagnosis (if you've been): primarily MFI

    Status (WTO/TWW/TTA):  WTO

What are you doing this cycle? (Testing? Treatment?):  Testing before treatment

How are things going?:  Good.  My initial consult with the new RE went well this AM.  As expected, I have to do some standard bloodwork.  They also want to do an HSN to make sure that things are all clear for implantation and that there are no scar tissue issues.  Assuming that nothing pops up with any of that though, the RE is good with moving into an FET.  I have to wait until CD 7-12 for the HSN and need to do 30 days of BC prep before the transfer cycle, so the timeline is still TBD depending on how long this cycle goes on.  Overall though, I feel pretty good about things.

    Any questions?:  not right now.

GTKY: If you could be on a game show (past or present), which would it be?

@keikilove I am so so sorry to hear that!  It strikes a cord since my husband had cancer awhile ago, but in our case it paused our TTC journey.  (Thankfully he is in remission now - but I remember the gut wrenching pit in the stomach when I found out).

I hope that your TTC journey ends on a high note, and with all my heart, hope that everything turns around with your husband.  :hugs: to you and wishing you all the best.
  • @kiki047 / @bumblebee0210 I was cross-posting with you guys!

@kiki047 I've been anxiously awaiting your update. My 2 cents about your dilemma is: I started out with a clinic that was "nearby" with an RE I liked. Then went to another one with similar characteristics. Finally, I went with TEW and when insurance kicked in, with someone whom she recommended. Both of them are 50 min drive away. And whenever someone at my clinic asks "wow, you live in SF but come all the way here" I answer: "yes, it was worth it for me to have the right doctor." I also think what @bumblebee0210 said is right about weighing against any potential regret. I totally get the conflict with work, but this is also the most important thing you're doing (more important than work!). I think you know what I think. :)

@bumblebee0210 Oooph, interesting question about TCM/acupuncture. I've started going down that route recently, with guidance from our lurking acupuncturist @lickastick. While I'm not sure I'd ever want to go super deep myself on it, she's given me some really good "basics" that are "easy to follow". What to take in between/after ERs but before transfer; what to do in preparation for a transfer (eat red foods: good qual red meat, red chard, pomegranate, etc.) It's very easy for me to have "all or nothing" mindset, so she's helped me understand that it's not getting to some sort of "perfect" state; rather getting to a better state than where I am now. I think the best thing to do is find a provider you trust and discuss with them what makes sense. Hope this helps.

I was actually doing acupuncture when I get pregnant with my daughter. Ya'll know my feelings about correlation =/= causality, but when I first saw her, she basically said I needed to process the trauma of my previous miscarriage, and I sort of thought it was BS and then went to her for a few weeks and after every treatment felt better than I had felt in a long time. I got pregnant within 2 months. Again, I have always gotten pregnant (fairly) quickly so don't take that as "she fixed me", but I did feel better. I have found myself feeling bad again in similar ways, and I wonder if I need another sort of "tune up", or if nothing else, it will help me reduce stress and anxiety while going through IVF.

My practitioner (whom I loved, but has stopped practicing), was also big on small changes-- her advice to me was typically to eat meat, also some red foods, also to keep my feet warm and eat warm things. Always very pleasant and do-able. But when you read the books ("Yes, You Can Get Pregnant" and "Making Babies", among others), it's like "don't eat soy, gluten, dairy, anything cold, any sugar, or anything fried, and take 15 dubious supplements, ok bye". And the A-student in me has a hard time just taking what I want and leaving the rest.

Anyway-- I have an appointment at a new acupuncture clinic next week, so hopefully it is relaxing and pleasant! I'll report back if she says anything interesting.

  • @kiki047, that is a tough call.  I feel like when you get in these situations where it feels like you could spend hours making pro/con lists and still get nowhere, that initial gut feeling is your best guide.  I once read somewhere that if you're struggling with making a decision between 2 things, you should flip a coin.  Not so that the coin can decide for you, but because most of the time as soon as the coin hits the air you'll know how you want it to land.  Not the most sophisticated tactic, but it's worked for me from time to time in the past, lol.

@acleverusername, no, of course I missed the HSN window by a whole 2 days...whomp whomp.  And because my last cycle was a nice 30 days, I'm sure this next one will be 40+ because that's just the way my cycles roll and now I'm trying to get things done.  The bloodwork doesn't need to be done on a specific CD though, so I can at least get that out of the way while I'm waiting.

I just realized I never answered the GTKY.
I think I'd have to go with Cash Cab.  Seems like a fun way to kill some commute time.
