Advice needed

Feel a little like an outsider here.
I am hoping for some advice 

I am 26 years old. Have pcos and never have ovulated on my own. Started ttc at 22 years old. Went to a specialist and started iui 
Second try was successfully (who knew how blessed we are) this was with 13 injections of menopur 50iu. 

So fast forward to now. We have tried iui again 2 times. But my resistance to hormones is worse. (I need 24-30 injection to start making a folical)
I worry this means I have less eggs or poor quality? 

My last cycle ending in a "chemical pregnancy" (hate the term because it feels like a loss to us)
This was with femzole 5mg for 5 days while taking, menopur 75iu and puregon 50iu. They also had to trigger. My poor tummy and emotions after this cycle. 
The miscarriage left me with 2 large corpus luteum cyst so if these are gone when AF arrives on the 8th of Oct we will try IVF as the doctor has said IUI is not for us. Thankfully we can afford 1 try. But that's all. 
So I came to this form looking for advice from someone who had the same/similar. 

I am not sure why I need so many hormones now... I am also so afraid I don't get any eggies or they are all poor quality... But I know I would regret it more if we didn't try.

  • Did they run bloodwork and do you know your AMH? That's usually a good predictor of egg reserve or response to meds.

    Fall 2015 - Summer 2016 - Further testing all normal. 3 IUI's -- BFN. Recommended move to IVF. Planned cycle for fall 2016.
    September 2016 - Surprise natural BFP. MMC @ 8 weeks. RE expressed confidence that we just needed the 'right' embryo.
    Fall 2016 - Spring 2017 -- Break from TTC
    June 2017 - Started IVF; egg retrieval for freeze all cycle. 9 mature eggs retrieved, 5 fertilized. 2 4BB embies on ice.
    August 2017 - FET transfer both embies. BFP.  Twin pregnancy confirmed by ultrasound. EDD 4/28/18
    September 2017 - Twin B stopped developing; Twin A doing perfectly! Graduated from RE @ 10 weeks
    March 2018 - Baby Girl born via C/S due to pre-eclampsia -- strong and healthy!

    January/Feb 2021 - Freeze-all IVF cycle 
    March 2021 - FET of 1 PGS normal female embryo. BFP! Beta #1 156,  #2 472, #3 1241, #4 5268 EDD 12/5/21 - Christmas baby!

  • They did blood work 2 years back. Not sure about an AMH. Assuming that's another blood test? 
  • Anti-mullerian hormone -- it's usually drawn with standard blood work. The level of the hormone (more is better) often correlates to egg quality, reserve and response to stim meds. It's usually interpreted in the context of all the other bloodwork, but generally the lower it is compared to the average for your age group, the harder it is to produce eggs and embryos.

    Asked about the test and they said they do not do the test when they can see I don't have a low reserve. 
  • Hi @dan-g Sorry you're feeling so down, and I'm very sorry for your loss.
    I can't speculate why your dosage went up, other to say that for many of us this is what happens - the doc will keep upping dosages and adding more protocols until they get results. I took increasingly higher dosages of Puergon+trigger+TI for >5  cycles and never got any results before moving on to ivf.
    One thing to try to keep you positive is to remember that IVF has a much higher success rate than IUIs. Yes, unfortunately there will be more shots and it is more invasive (I'm with you on hating the injections - I've never got used to them), but I really hope it will all be worth it for you. If you're going through IVF in October you should join in the Oct IVF thread - there'll be lots of ladies there going through the same thing and supporting each other.
