@lurvleybunchococonuts Sorry you find yourself here. These ladies are my people, and thumbs up to all they're saying. 4 more months of feeling like you're doing nothing with an IF diagnosis and 12 months behind you of trying is HARD. And a dx of PCOS is HARD. So give yourself a little bit of a break - pursuing treatment as aggressively or non-aggressively as you want is all up to you, and we will support you either way you choose. I agree with @kristokekerooni's timeline of what I would do. The extra testing is important, especially if you're pursing treatment. Unfortunately, it's not guaranteed that PCOS is the only thing playing a part in the IF. Hopefully it is, and if it is, it will be a relief to know that.
One note about pursing treatment through an OB - as long as they're monitoring, that's great! And it sounds like you've got a good OB. I'm not sure what they're recommending - Clomid or Letrozole, but note that with PCOS you can sometimes be resistant. I was, and I wouldn't have known that if I'd done treatment through my OB because she wasn't monitoring besides OPKs which I would have had false positives on. (But not to be a total debbie downer - it's more likely that you'll respond to Clomid and Letrozole than that you won't.)

@emmasemm I'm so, so sorry you are going through this but I recognize your fighting spirit and I salute you. You're inspiring. Have you heard of New Hope Fertility Center in NYC? I have a friend who went there after trying multiple IVF cycles at other places and she really liked their approach, with fewer medications and focusing on quality eggs, not quantity. I don't know if you would qualify for their clinic, but one round of IVF there is much more affordable than places (I think she mentioned something like $5k rather than the standard $10). She was very happy with the care she received and loved her doctor, who seemed like he was very accessible and answered all her questions (and she had a lot). Worth looking into if you haven't already...
@lurvleybunchococonuts Hey girl. Sorry you find yourself here. But man, this group is a lifesaver - I don't know what I'd do without them! I don't have much more to add that the other ladies haven't already said, but I definitely agree that get the SA done sooner than later - if there is an issue (which I hope there isn't!) it can take up to 3 months for changes to be made.
AFM - IUI #2 is a bust. AF showed up last night, and I'm going in today for my U/S. I had a meltdown this morning to DH because I wanted him to go to this appointment (which I thought would be tomorrow) and my doc isn't even there today. I wanted to talk next steps. But DH and I talked about it - unless the doctor today says something earth-shattering, we're going to do one more IUI, then we're going to start the IVF process. It was a great talk with DH, because I was afraid he's just flat out say no to the IVF, but he didn't. He said he'd do whatever it takes. He's a keeper

Id def get those tests done and start looking to schedule treatment based on the results. ART actually can take a long time (I am almost one year from my last FET.....) because in between stuff crops up or you find out something else you have to resolve before you move ahead and etc... I say the sooner you get yourself moving the better. Hoping they can get you sorted quick and on the baby train asap!!

So I had acu this morning at 7, with my scan/BW at 8:30. I fell asleep at acu, they forgot to set the timer... so I was a little late to the scan. oops. Oh well. So I must have misunderstood my AFC, because I asked how many follicles there were when she was looking at lefty. Lefty is my lazy little guy (Why do I think of my ovaries as a little GUYS? No idea. how odd), so I wasn't expecting many. She said "about 20" Wut. I didn't ask about right, because he's my champ. She said they are measuring around 1 mm and less. I have only had three nights of stims... is that the right size? I was expecting them to be like... 5-10 mm... not <1mm. Is that normal?

@rvamom315 oh I’m sorry this IUI was a bust. And so good your DH is open to trying more intervention. That’s a big step forward in itself. But you can join our weekend wine and cocktail drinking club in the meantime!!!
@madspunk yes we’re looking to get started with a new clinic right away. With DOR every month counts. And at my age the percentage of normal eggs reduces exponentially. So hardly any eggs, and those that are just crumbling more and more every day equals no time to hang around. We are looking at IVF because although it doesn’t help us get large numbers of eggs (which is the biggest benefit these days as if you’re a normal responder you might get 20 months worth of eggs in one month, so reducing your time to conceive by 1.5 years), we do still have a chance for more than one, and it takes us several steps ahead of an IUI - Firstly with an IUI you can’t guarantee the sperm and egg meet (and if they do you don’t know if they fertilize or not), and then you can’t guarantee the fertilized egg would be in the right place at the right time. (I know there is a risk with IVF of ectopic, but we don’t know if the little villi in my Fallopian tubes are working properly, especially with my inflammation issues). But IVF is high stress and more risky (as well as expensive...), so let’s see if at some point we don’t change the plan. But right now I’m putting my faith in science. (Though magic spells still welcome )

@KristoKekerooni I think most of mine were between 5-10 on day 5 of stims. When do you go back for a follow up? If they don't like your progress, they may have you change around some of your meds and increase your dose going forward.
@emmasemm , I am doing so much better. I have basically resolved that I am not poking myself. If I get to a point where I have to, I will, but that day is not today, so I'm just taking it in stride. There are bigger things to think about. Thank you for asking. Just remember when you fly for IVF, directly call the airline and explain it's for a medical procedure. It saved me literally thousands of dollars worth of discounts, points, and change fees when I had to change them last minute.
@50Wife ,That would make sense, since I have only had three days. Surely they'd be up to fiveish in a couple days. Especially when you consider that I do typically ovulate a tad later, On CD16-18 instead of 13-14. So I am on course for me, I think. And we are early enough to correct. I mostly just don't want to run out of meds. We are on a tight budget.

Plenty of time for more to catch up, she said. And she thinks their measuring isn't the greatest anyway, which doesn't instill hope in me, but I guess it doesn't matter too much.
I am staying on the same plan, but adding cetrotide tomorrow morning, and going back in on Monday for another check.

The doctor said that they like to try treatment at least 2-3 times before moving on. I spoke with the IVF coordinator afterwards, and she said that I could start the IVF process with my next cycle if this one doesn't pan out. They took some blood to check my AMH so they could develop the protocol for the IVF.
Hey ladies, sorry I didn't get back to comment/etc yesterday. Holy wow this thread can be intense to catch up/keep track on. Again, you all are wonderful! Much love for #teamgetshitdone and I have soooo much appreciation for all the wonderful insight and thoughts on my next steps! Went this AM for prolactin blood draw. I think they like to stab me for each individual test. I'll be requesting they group together any blood tests that need to be done in the future. Drawing a vial for one test then going in for another is annoying. Sigh... @KristoKekerooni holy goodness that's a lot of little follies. Fx they keep growing and doing their thing @RVAmom315 hopefully this next IUI will work and you won't need to continue to IVF.

Got the call that I'm dropping my Gonal F dosage tonight and tomorrow night. Tomorrow morning is still 1/2 Menopur (which I've been doing a few days now) and Cetrotide. Sunday morning is ONLY Cetrotide, and appt at 815 Sunday! Eeeeeeeeeeeeek!!!!!!
@KristoKekerooni Yes!!! Room temp is a BREEZE! Literally don't even feel it! Cold really stung, like alcohol being poured on a scrape kind of burn and it got red and blotchy right away. The red was gone after about an hour though so no biggie really. But the burning sucked. If you don't have time to take it out of the fridge early enough, just at least hold it in your hand for as long as possible to warm it up as much as you can! GL! ETA: I only got red around the injection on the day that it was too cold. Other than that, no signs of the shot except a tiny red spot where the needle went in.

So I just got off the phone with my insurance to see if they would cover an HSG and SA, and the lady I spoke with was weeeiiird. Like I didn't even get to ask about the SA, she was all "your doctors office has this information regarding if that procedure will be covered or not. They should be able to answer this question." then proceeded to put me on hold and leave a message with the doctor's office that they need to find out the info and get it to me. O.o watdafuq?

of beers last night. My first in three months! It was glorious.
@KristoKekerooni I couldn’t even handle giving myself the
trigger shot for my IUIs! MH (who is afraid of needles) had to do it for me. Glad
things are looking good with your monitoring!
@lurvleybunchococonuts I’m sorry you find yourself here, but
as everyone has said, you are so welcome. *hugs* I don’t have any new advice to
add to the awesome words of wisdom you’ve already been given, but I just wanted
to say feel free to message me any time with questions or to vent. Also, your
insurance lady is totally weird. They should know what they do and don’t cover!
I’d call back and talk to someone else.
My appointment with the RE went well today. This was the
first time I’d met her in person since switching to her, and I still really
like her. I expressed my concerns about seeing different nurses every time and
not feeling like there was enough continuity in my care, and she said she will
call me with all results and information from now on, which I really
appreciate. They drew blood today to re-test my thyroid, and we’re going to get
a repeat SA and a sperm penetration assay. She was fully supportive of MH and I
taking a few months off from medical intervention and we planned out what the
next IUI protocol will be if/when I go back. She wants to try me on clomid
instead of letrozole, because apparently there is research suggesting it can be
more effective for unexplained infertility. She doesn’t want to do injectibles
for me for an IUI because with my high AMH there is too much risk of multiples/OHSS.
She also wants to do two trigger shots because there is some chance that with
my BMI (29) one shot isn’t enough. Next time we’ll monitor on cd 12 instead of
11, and aim for follies of at least 20mm. She’s going to monitor estradiol
(because you can on clomid, but not on letrozole) and measure progesterone the
morning of my IUI and the day after, to check on the timing of my ovulation
relative to the IUI. Overall I felt like she heard my concerns and explained
things to me well. I’m still looking forward to time off, but it makes me feel
better to have a plan in place for the future.Now I'm heading out to meet MH for dinner and drinks. to all of you.
@darkstar42 I'm glad your appointment went so well and you have a good plan in place! Having a doctor that listens is so important - it's amazing how many don't! @lurvleybunchococonuts Welcome from another IF newbie - I also just intro'd this week! I definitely hear you on feeling that compared to some of these ladies our struggles seem minor so far but I do agree with the others that it's still hard to be waiting the amount of time we have been and not getting anywhere. Hopefully we both find a path that works soon!
Re: IF Testing and Treatment, Week of 1/22
One note about pursing treatment through an OB - as long as they're monitoring, that's great! And it sounds like you've got a good OB. I'm not sure what they're recommending - Clomid or Letrozole, but note that with PCOS you can sometimes be resistant. I was, and I wouldn't have known that if I'd done treatment through my OB because she wasn't monitoring besides OPKs which I would have had false positives on. (But not to be a total debbie downer - it's more likely that you'll respond to Clomid and Letrozole than that you won't.)

Have you heard of New Hope Fertility Center in NYC? I have a friend who went there after trying multiple IVF cycles at other places and she really liked their approach, with fewer medications and focusing on quality eggs, not quantity. I don't know if you would qualify for their clinic, but one round of IVF there is much more affordable than places (I think she mentioned something like $5k rather than the standard $10). She was very happy with the care she received and loved her doctor, who seemed like he was very accessible and answered all her questions (and she had a lot). Worth looking into if you haven't already...
AFM - IUI #2 is a bust. AF showed up last night, and I'm going in today for my U/S. I had a meltdown this morning to DH because I wanted him to go to this appointment (which I thought would be tomorrow) and my doc isn't even there today. I wanted to talk next steps. But DH and I talked about it - unless the doctor today says something earth-shattering, we're going to do one more IUI, then we're going to start the IVF process. It was a great talk with DH, because I was afraid he's just flat out say no to the IVF, but he didn't. He said he'd do whatever it takes. He's a keeper

So I had acu this morning at 7, with my scan/BW at 8:30. I fell asleep at acu, they forgot to set the timer... so I was a little late to the scan. oops. Oh well. So I must have misunderstood my AFC, because I asked how many follicles there were when she was looking at lefty. Lefty is my lazy little guy (Why do I think of my ovaries as a little GUYS? No idea. how odd), so I wasn't expecting many. She said "about 20" Wut. I didn't ask about right, because he's my champ. She said they are measuring around 1 mm and less. I have only had three nights of stims... is that the right size? I was expecting them to be like... 5-10 mm... not <1mm. Is that normal?

@madspunk yes we’re looking to get started with a new clinic right away. With DOR every month counts. And at my age the percentage of normal eggs reduces exponentially. So hardly any eggs, and those that are just crumbling more and more every day equals no time to hang around.
We are looking at IVF because although it doesn’t help us get large numbers of eggs (which is the biggest benefit these days as if you’re a normal responder you might get 20 months worth of eggs in one month, so reducing your time to conceive by 1.5 years), we do still have a chance for more than one, and it takes us several steps ahead of an IUI - Firstly with an IUI you can’t guarantee the sperm and egg meet (and if they do you don’t know if they fertilize or not), and then you can’t guarantee the fertilized egg would be in the right place at the right time. (I know there is a risk with IVF of ectopic, but we don’t know if the little villi in my Fallopian tubes are working properly, especially with my inflammation issues).
But IVF is high stress and more risky (as well as expensive...), so let’s see if at some point we don’t change the plan. But right now I’m putting my faith in science.

@50Wife ,That would make sense, since I have only had three days. Surely they'd be up to fiveish in a couple days. Especially when you consider that I do typically ovulate a tad later, On CD16-18 instead of 13-14. So I am on course for me, I think. And we are early enough to correct. I mostly just don't want to run out of meds. We are on a tight budget.

Update #2: Looks like we're doing one more IUI (I start Clomid tomorrow). The doctor said that they like to try treatment at least 2-3 times before moving on. I spoke with the IVF coordinator afterwards, and she said that I could start the IVF process with my next cycle if this one doesn't pan out. They took some blood to check my AMH so they could develop the protocol for the IVF.

@KristoKekerooni Yes!!! Room temp is a BREEZE! Literally don't even feel it! Cold really stung, like alcohol being poured on a scrape kind of burn and it got red and blotchy right away. The red was gone after about an hour though so no biggie really. But the burning sucked. If you don't have time to take it out of the fridge early enough, just at least hold it in your hand for as long as possible to warm it up as much as you can! GL!
ETA: I only got red around the injection on the day that it was too cold. Other than that, no signs of the shot except a tiny red spot where the needle went in.

I definitely hear you on feeling that compared to some of these ladies our struggles seem minor so far but I do agree with the others that it's still hard to be waiting the amount of time we have been and not getting anywhere. Hopefully we both find a path that works soon!
