Reflux/Heartburn/Indigestion

Okay, so a week or so back we were having MAJOR issues with LO's tummy.  We are now on Nutramigen by Enfamil and have seen huge improvements with being able to pass bowel movements and overall behavior of DS. It's been a week since the formula switch and while many things have gotten better, one thing that is still prevalent is his constant wheezing/hacking/gurgling noises that he makes when trying to sleep after his feedings, particularly at night.  He's eating in longer stretches now (every 4 hours usually, minimum every 3) so I know it's not because we are over feeding him.  He gets 3 oz. at every feeding and is 7w old.  There are a few things that we are noticing DS do, that are making me think he may have something else going on such as; reflux, heartburn, or indigestion.

*Spitting up more- sometimes its projectile and other times it just dribbles out of his mouth
*When we feed him he often takes the bottle into his mouth almost aggressively while arching his head back and getting very stiff on us
*If we are holding him in a cradle position or if he's sitting somewhat upright against my legs while they are bent, he tires to push off of me extending his legs out and screeching/letting out a cry.  Sometimes his back arches when he does this, but his whole body gets very stiff
*Random (although I don't really think they are, we just can not see any obvious reasons) cries out and/or screeches, He will grip onto my finger tighter when he does this as if he is barring down and trying to work out a gas bubble
*Making a lot of loud banter and clearing of his throat- often this noise making will go on for 25-30 minutes and sometimes end with him falling asleep, other times it's just a back and forth battle for 1-2 hours +, of putting the pacifier back into his mouth to try and settle him into sleep
*I can literally hear his stomach churning during feedings, like serious stomach gurgling going on.  Sometimes it will lead to gas being passed, but not always

For those of you with LO's who have been diagnosed with having reflux/hearburn/indigestion by their Pedi's, what kinds of things did they recommend or prescribe? Were the things that I'm noticing with DS, things that your LO's were doing that led to them telling you LO had something such as, reflux or heartburn.  I will probably call mine tomorrow or Wednesday if we go through another consistent day of this behavior just to see what they say.  I have already done the burping more in between feedings, sitting upright for 20-30 minutes, sleeping somewhat on an incline, etc. I'm not a huge fan of gripe water, gas drops, because I really don't know if that stuff makes it worse for him or not.

  • DD was prescribed Ranitidine yesterday to help her spitting up. She is constantly spitting up and sometimes it's so bad and cortège cheesy and mucus. I was surprised she's gaining weight but I can tell she's uncomfortable because she makes a "gross taste" face sometimes and that's the stomach acid coming up. She takes 1mL twice a day from a syringe so I mix it with expressed milk and give it to her a little at a time. She's getting stubborn and won't take bottles without a fight. 

    She also has very bad gas. We we also had tried the gas drops but I felt it wasn't doing anything to help her. If anything it upset her tummy. So I stopped giving that to her and started burping her a lot during feedings. I hear her swallow air and I immediately take her off and burp her. It helps a lot, with gas and spitting up. It kind of sucks because we side lay for feedings but it's the only way it works for us so I'm constantly up down up down. And the bicycle legs works wonders. 
  • Sounds like what I noticed in DD before taking her to the pediatrician.  She doesn't spit up a ton except for a few times during the day, but swallows a lot and cries out when she does spit up. She got put on Zantac for reflux. It doesn't stop the spit up, but it helps the pain.  I would call your pediatrician sooner than later bc your little guy us probably really uncomfortable.  And definitely check out some of the other reflux posts.
  • LO will sometimes get the cottage cheesy kind of milk in his mouth and I feel he is very phlegmy.  Its as if after he finishes his bottle no matter how good he burps or amount he spits up, it all sounds like it's pooling right in his throat and he cant quite clear it out of there. 

    I'll keep that medicine name in my head, as I am waiting to hear back from the pediatrician. 
  • @mhuber223 When you got prescribed the Zantac, did you have to ask/push for it or did the Doctor outright recommend it, based on what was going on with your LO?  I ask because I don't think my Pediatrician is going to be quick to mention putting him on something, hence so far being recommended the usual, "hold upright after feedings", "burp a lot in between feedings", etc. As well as let's treat him for a cows milk intolerance and put him on Nutramigen. Which, I do like the Nutramigen and even though it's costly, I can't complain.  It's gotten him to have regular bowel movements and has done a lot at keeping his stomach issues down. I just think there's more that's wrong with him. Sigh! 

    Within the last 3 1/2 weeks, I have brought him in twice to see the doctor and we've been on 3 different formulas. 
  • My LO spits up a lot as well. I brought him to the pedi a week and a half ago and she didn't seem too concerned since he was gaining wet and wetting diapers. I did ask for a prescription but the pedi wasn't really reluctant to write one. I have yet to pick it up bc she recommended me just trying to do what you said your pedi advised. I've also tried gripe water and little remedies stomach drops. They seem to work every now and then. 
  • This sounds EXACTLY like my daughter. She's 5 weeks and dr prescribed Rantinidine (sorry that's probably horribly mis spelled) 
    but I'm told the biggest thing is time, and reflux issues usually improve at 2/3 months of age apparently (hopefully)
  • @teachmegs817 After I told the pediatrician what I'd been doing,  she recommended the Zantac. I think it helped,  though, that DD spit up about 4 times during the visit and was visibly (and audibly) in pain. I'd fed her an hour prior and held her up, etc., so it was clear that she needed something more. 
  • At our 3 week apt our ped gave us probiotics and had me cut dairy based on a fussy baby and a diaper rash that wouldn't go away. At almost 5 weeks she gave us a prescription for ranitidine for basically the same symptoms you've described (gagging, choking, clearing his throat). We didn't have to beg for them either. He seems to be doing better. The worst part for him now is after his middle of night/early morning feed. If he's not really awake he doesn't burp well. I have to make sure he doesn't eat too much too. I keep him upright for a little bit. I also have to be careful what I eat before bed (yellow corn tortilla chips didn't work the other night
  • I suspected Henry had silent reflux and asked his ped for an rx of Zantac and she wrote one without hesitation. He was like a new baby after it started working, he still has to sleep at an upright angle but it is so much better.  He also farts a lot after we give it to him.
    Sometimes if he has bad gas we "bicycle" his legs to help get it moving. Have you tried that? Also, we were told to expect him to start drooling and/or spitting up at our 8 week appointment due to his digestive system starting to mature. 
    It sounds like he has classic signs of gas and reflux. Poor baby :/ 
    I've heard probiotics can be helpful for gas. 
  • @Gingermom15 How do you have Henry sleeping? Do you prop the crib/bassinet or is he somewhere else? 
  • Talk to your doctor about hyosciamine it's the only thing that helped after doing many things you have done too. My son's on soy formula too. We also do a probiotic and now give him an ounce or so of water in between feedings to help him poo.

    These poor kids! It is not completely better but significantly better than it was

    He naps in his Rock & Play during the day when we can supervise him. He sleeps in his glider at night because it has shoulder straps to keep him from lurching forward, which he tends to do if he wakes up with gas. We tried wedges for the crib and bassinet but they didn't prop him up far enough. The pediatrican says most kids outgrow reflux by 4-6 months, so fingers crossed. We aren't worried about transitioning to crib until he can comfortably sleep flat.

    @Gingermom15 Thanks!  It became really clear last night that the reflux has been causing DD's recent crib issues.  I've also tried a wedge and a rolled blanket,  separately, with no luck.  When the pediatrician called (love her!) to check how the Zantac is going, she said to just let DD sleep in the rnp if I do the buckle and sleep next to her. She thought that was safer than trying to prop the crib more. Nice to hear from other moms in the same boat!

    I ended getting a prescription for Axid. Similar to Zantac. We also were told we could put a teaspoon and a half of rice cereal in with the Nutramigen to thicken it up a bit and help keep it down. Crossing our fingers that it will bring him and us some relief.
