Gestational Diabetes

I just got a phone call from my doctor and it has been confirmed that I do have gestational diabetes.  Has anyone else been diagnose this too?

Re: Gestational Diabetes

  • I have. My fasting number is the one that is high so I am taking insulin before bed. I test 4 times a day. It really hasn't been that bad for me once I got used to testing.

  • I have. Mine is controlled by diet for now. I am only 31 weeks, so they keep telling me that I might need insulin as the pregnancy progresses because levels rise as time goes on. Its not that bad once you get into the routine of testing after meals, etc. Though I will admit the first couple weeks were a little stressful meeting with all the diabetes specialists, nutritionist, etc.

  • I am I have been on insulin since 8 weeks luckily the amount has stayed the same.  I'm really praying I don't have to up my dosage. 7 weeks to go!

  • I am too. I am expecting my OB to put me on insulin next week bc I'm not doing very well at controlling my numbers by diet :(

  • My advice to you would be to ask to see your glucose tolerance test results with your own eyes and have them explicitly explained to you before proceeding with the diagnosis treatment. In my own recent experience... my original OB (who began seeing me from the beginning of my pregnancy) went on maternity leave when I was around 28 weeks and had yet to have taken the glucose tolerance test. So I had a fill-in doctor who interpreted the results - was fresh out of medical school new and had no idea what she was doing. Before seeing the results, I had asked how my test results went...she unprofessionally responded, "Oh they were really really bad!" (which naturally worried me immediately) She diagnosed me with GD although my fasting level (initial blood draw) was only 2 POINTS over the recommended level. It was supposed to be 95 and I was at 97. So, she had me pricking my finger for a month and said I would be put on insulin if my levels were too high, that I needed to see a diabetic counselor, a diabetic nurse, and start going to the maternal fetal medicine unit for specialty ultrasounds to ensure the baby wasn't growing too big. So fast forward a month or so later, my original doctor came back from maternity leave and said how preposterous it was that I had been diagnosed with GD and that I don't need to be watching my sugar levels. 97 is still a decent fasting number (especially since I had explained to her that I had consumed a load of pasta the night before the fast and splurged on a huge meal since I knew I wouldn't be allowed to eat for 12 hours...). So, my OB did an ultrasound and if anything the baby was growing on the smaller end but still very healthily (in the 30th percentile of growth chart... which is just fine) and that there was not too much or too little amniotic fluid (diabetics would have excess amniotic fluid due to baby urinating more frequently). She said she would know if she were looking at a diabetic baby in an U/S and I was nowhere near having GD. She reversed the diagnosis thank goodness... all of this stress and worry for no reason. So please, if you learn anything from this post, do NOT always take what a doctor says immediately. Assess the results YOURSELF, do a lot of research, and talk to other women with possible misdiagnoses of GD. If I had listened to that other foolish fill-in doctor, I'd be on insulin, labeled as a high-risk pregnancy, and be growing a perfectly healthy baby and not know it! I am enraged about this obviously.... as everything was finally straightened out by my original doctor as early as yesterday.

O.o I'm a type 1 Diabetic. My baby is just the same as any other baby on the u/s. I understand her point was that diabetics generally have bigger babies but it isn't always the case. In fact, I have found it generally isn't the case based on the many, many babies that have been born to diabetics in a FB group I have been in for years.

I'm really just venting a little here, nothing against you or your doctor. I stupidly have been reading a lot of GD threads and I repeatedly come across people making insulting comments about diabetics like our babies are wrong somehow or diseased or "less than", or it's all our fault, etc. My little vent has nothing to do with you or your doctor. Like I said, just need to rant a little and just felt a little better letting it out here than the other the other boards.
  • I went to see the high risk doctor last week about the GD. Well little did I know that it was a 4 hour class about diabetes, what foods to eat, and checking my sugar level.  Well I am on a diet and have to check 4 times a day my sugar level.  When I go back on January 25 for my follow up if my numbers aren't where they are suppose to be then they will start me on medicine and if that doesn't work then insulin.  I just want this to be over so I can meet my baby girl.  The first couple of days on the diet was very frustrating because of the high numbers I was getting but now it's starting to be at a normal level.  I am 31 weeks tomorrow.

My original glucose test was 150 so they have me coming back for the three hour test but since I was unable to get there for a week they asked me to test it on my own for a couple days before my appt. My fasting is between 111 and 120 and my 2 hr after eating is 165-172. What do you think of those numbers? Also I'm about 30 weeks.

This is definitely something you need to talk to with your doctor. For me, my doctor has said that my fasting should be under 90 and 2 hours after eating should be ~120 and occasional numbers in the 130-140 range shouldn't be much of a worry. But your doctor may have completely different standards.

When is your doctors appointment? If you're concerned about the numbers you're getting then you should definitely make a call to them and talk about what range you should be in and how you can change your diet until you can be seen.
  • my fasting numbers should also be under 90, and 1 hour after a meal they should be under 130.  if I have 2 consecutive days of anything over the 90 I have been advised by my endocrinologist to increase my bed time insulin dose.  I have had numerous ultrasounds with my son who is now 3, and he was born 7lbs 5oz at 40+1.  I am 33 weeks pregnant tomorrow and have had quite a few ultrasounds this time as well, and not one person has ever been able to tell I was diabetic by looking at either of my babies on an ultrasound.  That just seems like an odd thing to say to me.

  • my fasting # has to be under 90 and 2 hours after a meal my number should be under 120.
  • Hi all - I failed my 1 hour by one point, and then failed only 1 of my four blood draws at the three hour.  Everything I see online says I "passed" my three hour, but my Dr. still wanted to call it a "fail" and send me to an endocrinologist.  I told her that we needed to compromise, and asked to self monitor blood glucose in the week I would have had to wait anyway for the endo appointment.  She agreed.  I was given a home glucose meter script, and targets of less than 90 upon waking, and less than 120 1 hour after a meal.  Of course, the internet is a blessing and a curse, and when I looked these targets up I was frustrated to see that they are more aggressive targets than those given by the NIH, CDC and ADA.  Those sources say its okay if it takes 2 hours to get to 120.  So far I have not had any 1 hour after eating results above 120, but I have had some 119s.

I am curious to know what blood glucose targets in what time frame you ladies were given.  The more I research gestational diabetes, the more I find that there is not consensus in the medical community on diagnostic thresholds or optimal blood gluocose levels.
  • @mamaroar those were the targets I have been given. I'm having a bit of an issue with my fasting number as I tend hover around 86-91. I meet with my diabeates counselor again next week and have already decided that I will refuse insulin with my current numbers, however if they get worse I am completely open to medication
  • Thanks SarahJean!  I got a reprieve today - since I had good numbers, I don't have to go to endocrinology tomorrow, but I am to keep monitoring and turn all of my numbers in at my next OB appointment on the 25th.  I told my doctor that I am not trying to be a pain here, but I think with the "iffy" test results it's fair of me to hold off on doing anything drastic at least until I get measured again.  My baby has not been measuring big.

Maybe I was made more skeptical by the fact that I asked to take the 3 hour test again for a more conclusive result, even if it had to be at my own expense, and they flatly denied that request.

Any of you ask for a retake of the 3 hour or do a retake of the 3 hour to confirm your diagnosis?
  • I failed the first test I was out of range by 10 points so they sent me to have the 3 hour test, I had gastric bypass a few years ago so I don't drink soda I don't drink juice either, this drink made me immediately sick. I tried so hard to hold it down but within 10 minutes of drinking it, it was out and in the toilet. Luckily I had a follow up scheduled with my doctor and I told him I cannot tolerate that test, it's impossible for me. So he decided we'll proceed with treating me as if I have GD. He wants me to track my blood sugars until our next appointment and at that time he'll decide if I need further treatment. I'm praying I'm fine. I don't feel any different. We shall see.
  • They tend to be more aggressive with the target numbers when you are pregnant.  It does seem to vary slightly from doctor to doctor and I think endocrinologists tend to be more stringent.

  • I just found out today that I failed my 3 hour test. I was late taking my 1 hour because was putting it off. My doctor is just having me test my level 3x a day and I'll report them when I go in twice a week. My level was 144 on the 3 hour test. I'm delivering in 3 weeks so she's not going to treat it unless for some reason makes a huge leap. She wants me to avoid sweets and carbs until then.
  • I wound up passing my 3 hour. I did fail the one hour draw but only be 3 points. I'm feeling pretty relieved, but I'm thinking of you ladies.

  • I was diagnosed at 27 weeks. 4 sticks a day. My rest is supposed to be <95 and post meal 2 hour is <120. I was hitting the 120 but the rest was always between 100 and 110. I got my followup call from the specialist today and I told her my numbers (including my Christmas spike and since I'm from new orleans and its king cake season, my spike on epiphany) she said not to worry about my rest numbers that might just be my natural rest... um....ok... I was told as long as I don't dip below 70 or above 260 I was ok. I get an hour of exercise a night due to the restless leg syndrome (I pace from my kitchen to living room) and the diet is easy, but I have a hard time eating the amount of food required. (3 carb portions, 1 cup of cooked veggies, 3 oz of meat and 2 fats?! Seriously I'm so big and my stomach is so squished I can barely eat a whole sandwich without getting winded! ) I make sure I get all of the food groups, but in smaller portions and that seems to help keep my numbers stable. Breakfast was the hardest for me. I never eat breakfast. I really have to force myself to cook breakfast every morning. My doctor has me on ultrasound appointments every two weeks now and said we are looking for 3 things. Placenta to pull away from the uterine wall due to small blood vessel issues thanks to the betus, baby girl drinking all the amneotic fluid to supplement nutrition, or baby's growth not where it should be. Since I've started the diet, I've lost 2 lbs and baby gained one. My due date was pushed back a week the same day I was diagnosed since she was a bit smaller than she should be. I think over all everyone is different. So long as she's moving around in there and my uterus is providing a stable environment, I'm good. Good luck to all you ladies. And remember nobody knows your body like you :)

  • I was diagnosed at 29 weeks. Failed the 1 hour oral glucose tolerance test. After w hearing the news that I did have GD, I was told to do Blood sugars 4 times a day and log it. I also researched and modified my diet. After the first couple of days of trial and error, tweaking what I eat... all my numbers have been good and even lost 5 lbs so far. I guess that can't be bad considering I started out overweight. Seeing my doc the end of this week to follow up.
  • @matteosmom2012 no, I was only given the 2-hour glucose test. After being 2 points over on my fasting level, I was diagnosed immediately with GD and wasn't given the 3-hour test.

  • just went in for a follow up yesterday for my sugar levels and now I have to take medicine to help keep it down. I also have to have 2 non-stress tests a week. I am just over it all. I want to have baby girl now because I am over all the symptoms I have been having.  I turn 33 weeks tomorrow. They told me that I would be delivering earlier than my due date which is March 16.  I'm hoping to find out soon when will that be so I can prepare myself and also prepare my family.
  • @chime Of course those with diabetes prior to pregnancy do not always have big babies. I wasn't insinuating that this is the case. I am aware that diabetics are able to have "normal-size'' babies if the condition is controlled. I was just detailing what my doctor was specifically looking for in the ultrasound ... which was to ensure the baby wasn't getting too large. Just wanted to straighten that out.

I turned in my numbers to my OB yesterday and they were good - I was given the option to continue tracking, or not.  Everything was fine for me - no glucose or protein in urine.  Baby's heartbeat was strong and fine.  My blood pressure is still almost ridiculously low, and my belly is measuring right at 32 centimeters.

She's not "officially" diagnosing GD but wants me to try to eat close to the GD diet.  After being more aware of my macronutrients while I was tracking - I think the diet is okay and has a bright side - I have only gained 2lbs since 28 weeks (I'm currently 32+6) and I feel like I'm eating less crap than before to make sure I get "good" scores.  Since I've gained 27 pounds to date and was concerned about weight gain, I'm not going to complain about it.  I did tell her though that I feel like I need a day off from time to time - like for my shower, that random day when me and my husband REALLY want to go to the Indian buffet for lunch, stuff like that.  She said that if I am pretty on top of this most of the time a cupcake at my shower or a piece of naan at the buffet isn't the end of the world (especially if splurging every once in a while keeps me in line the rest of the time.)

I have a growth scan in two weeks.  I told her that I was skeptical that I had GD from the start, and absent any of my measurements being problematic (I'm soooo 50th percentile in like everything ) it's hard for me to see a problem, but that if my growth scan in 2 weeks indicates that he's suddenly measuring big I'll 86 the occasional naan splurge.

The worst of it is cold cereal for me.  I LOVE the horrible ones, like Count Chocula, Cookie Crisp - seriously the crappiest ones are my favorites (and I have them stashed in a cabinet so that my stepson and stepdaughter do not find them because I only let them have corn flakes with a half teaspoon of sugar for breakfast.)  But my husband says he'll bring me some Lucky Charms and milk in the hospital if I want him to.  :)
  • I'm 31 weeks and just got the call with the diagnosis. I knew as soon as I heard that I failed the one hr with a 190 that I wasn't going to pass, and I was right. I failed all 4 of the blood tests for the 3 hr. I'm going in tomorrow to talk to my practitioner about everything.

  • PSA - I've been on the hunt for delicious things to eat that are low carb - because seriously enough with the whole wheat english muffins with peanut butter!

I struck gold today with Panera.  Most of the things on their "pick two" menu, as long as pick a soup and one of the flatbreads (it's whole grain flatbread) will give you a nice little meal with less than 60 carb grams.  I typically find Panera a little pricey but adhering to this diet gets a little boring and it's worth the few extra bucks sometimes to really love what I'm eating.

  • I know what you mean about the wheat English muffins and peanut butter. That's exactly what I have been eating every morning and it gets boring. I love eating at Panera Bread and yes it is pricey.  I also get the pick 2.
  • Hey all - I have been lurking the GD threads & am now frustrated enough to post/complain. I'm 34+ weeks now. 5/6 weeks to go!

My morning BS is usually 100-110, I've run high my whole life as diabetes runs in the family, but now they want to put me on a pill in the evenings to combat the overnight spike. I have no problem with that, Baby J is estimating in the 84th percentile right now, around 7lbs, & they're concerned about how big she is. I don't want a 10lb baby & I want her to be healthy so I'm not going to argue a pill. I flat told them I was not going on insulin, though, as I've modified my diet & continue to make changes as time goes on to combat the BS rising. I've cut out high carb/sugar & high salt foods & stopped drinking milk except maybe 12 oz a day. I'm usually anywhere from 115-140 on the 1-hour meal test. I have spiked a few times up to around 170 & had ONE that was 210 but it was a very high-stress day, which can affect it. I see that some people have said they are on a 2-hour test instead of 1-hour, that intrigues me. If I tested at 2 hours I'd be below the limits easily...I feel like this is a trick.

I have a problem, though, with my doc going hard-nosed on me & she's telling me to come in TWICE A WEEK until I deliver so they can check heartbeat, blood pressure & my sugars. I think she's retaliating against me for cancelling my last appointment. I hadn't been in since the first week of December, I was crabby about some charges on my bill, & nothing has been going wrong or feeling wrong so my thought was, what's the point of going in when all they're going to do is lecture me on my weight & sugars & charge me thousands of dollars for it? I've also retained fluid like crazy & they told me there's nothing I can do to stop it.

Just very frustrated. Very, very frustrated.
  • @rlbjones what's your reasoning for refusing insulin?

It sounds like standard practice to me. My doctors are also seeing me twice a week now. But they split the visits- one with my OB and one with my neurologist. I think since you haven't been to an appointment since December (That's 2 months ago!), your doctor probably sees you as non-compliant and needs to monitor the baby more closely. Insulin is the safest diabetic medication during pregnancy. I understand the point of view that you may not want to stick yourself for insulin injections, but you're already sticking yourself several times a day for blood sugar checks right?

As to it being a "trick" for monitoring sugar at 1 hour instead of 2, it's not! At the 1 hour mark you should be less than 140 and at the 2 hour mark, you should be less than 120. They may have picked the 1 hour mark due to the results of your OGTT. Either hour is fine. I actually monitored mine at 1 and 2 hours.

I I think you need to hunker down these last remaining weeks: go to every appointment, and follow doctors orders as to medication and compliance. It's for the baby's sake.
  • So, I can sort of sympathize with feeling like the directives are "a trick."  I felt that way at first.  After a lot of research, and a looong discussion with a friend of mine that is in med school, I realized why it felt this way.  There is actually NO consensus among either OB's or Endo's about the right guidelines for this.  Some say you "fail" the 1 hour test at a level over 130.  Some say 135.  Some say 140.  Some practices say you fail the three hour test if you fail a single blood draw.  Some practices don't fail you on that test until you fail two of the draws.  And on the three hour some use the thresholds of 95, 180, 155, 145 and some use 100, 190, 155, 145.  This means that 10 women in different OB practices could get the exact same test results, and depending on the standards at their OB practices, half could be told they are fine, and half could be diagnosed GD.

And, when diagnosed with GD, the inconsistency lingers.  My doctor wants my sugars under 120 after 1 hour.  Other doctors say you can take 2 hours to get to 120.  Those of us that are testing our BG know that 20 points in one hour is a huge difference!  (As in, if I am at 130 - 150 at the 1 hour mark, I am SURE I will make it to 120 by two hours.  How some my doctor thinks that's a problem, but many other doctors would think I was doing great?)

The problem here is that there IS medical consensus that if you are severely GD, you need treatment, even if that includes insulin (my doctor is not considering this for me, but after reading about it, it does seem quite safe in pregnancy.  I would not be happy about taking it but would likely acquiesce to it if my doctor insisted.)  Where there is NOT consensus is if you are on the borderline - some doctors believe that borderline cases are no big deal, some believe that they should aggressively treat those cases.  The truth is that no matter what camp your doctor is in for borderline cases - it's a preference as there is not definitive, conclusive evidence for either point of view.  Look up a recent NIH conference on this and you can read doctors papers debating this point.  You are unlikely to identify a clear winner.  You are either going to do what your doctor wants, or not at the end of the day.

@rlbjones, if you are 115 - 140 at the one hour mark, that sounds very borderline to me.  Like, you could cut 5 grams of carbs or less out of whatever you at at that meal, and you'd for sure be under 120 in one hour.  That sounds very similar to how my numbers come out.  I can also get slightly different numbers on different days based on the exact same meal.
  • One reason they may have you come in twice a week towards the end of pregnancy is for non-stress tests to closely monitor the health of the placenta.  In Type 1 Diabetics (myself) the placenta can start to fail a month or so early.  My assumption is the same occurs for those diagnosed with GD.  If they aren't doing NSTs to monitor the placenta yet they can use your glucose readings.  They look for fluctuations and a pattern of lows.  If you're experiencing a lot of lows definitely get in touch with your doctor (I am not suggesting you are, just throwing a tidbit of info out there).  Things can change very quickly for diabetics so we start getting hawked over a little earlier than non-diabetics.
  • Thanks for all of the input - it's nice to have people who know what I'm talking about & understand all aspects of it.

I don't want to go on insulin for the fluctuations that it causes. I dislike knowing that I could drop low, have to eat, have a spike, inject again, repeat...I don't have the patience for that & I would rather just keep a sharper eye on what I eat. If that means I run a little high as opposed to dramatic spikes & drops then that's what it means.

@mamaroar your points about different docs with different level preferences are exactly what frustrate me with how strict my provider's stance is. If my doc says one thing but a 2nd opinion at a different facility says another then where does that leave me? Frustrated & unsure as to whether I'm good or bad or borderline. I'm fully aware of the consequences of GD. I keep an eye on Baby J's movements & notice when she's still for awhile. I know that monitoring the placenta is important, especially in the homestretch, & if it starts to age prematurely I will need to make a decision about induction. I'm also aware that she might crash after delivery but that is nothing that I can control so I resent being lectured about it. Having her delivered at a Level 1/Trauma 1 is the best thing I can do for her, so that's what I'm doing.

At the present, I had a sono this morning at 8, a non-stress test that started at 8:45 & they left me sitting there even after she passed all tests with flying colors for a half hour past just because my OB isn't available to see me yet. Now I'm sitting & waiting indefinitely, they finally came & unhooked me from the monitor about 15 minutes ago. This is a GREAT use of my time, luckily I have the day off work today but if this is what I'm going to have to do twice a week it's completely unacceptable. This is the reason I resent being shoved into a box I don't fit in - once a week of this is understandable if they pick it up a bit, but twice a week of this will use up my PTO before I even go on maternity leave.

Also, the explanation of what they're doing & why they're doing it is lacking.
