My doctor triggering me to soon? Feel like they won't listen!

I started Follistim two months ago, and for this cycle I stimmed for about 13 days (last two days with added Ganirelix) And on  day 14 they called and said you have follicles sized 20.2, 15.3 14.5 13.5 13.7 12.2, with an E2 of 600 so we want you to trigger! I was shocked! because I felt I only had one good egg! I tried to beg for 2 more days of stims, but they said if I did that, they would cancel me. Now when I was on Femara, I would always get one or two nice follicles. So when we starter injectables, I was expecting them to stim me till I got 3-4. Do you think I'm wrong? I'm terrified of needles, and the shots give me horrible migraines the entire time I'm on them. I quit my job because I couldn't function. They said that the 14.5, 15.3 and the 20.2 were all mature? But I feel only the 20 was ready. What the heck do  do?!

    They might be expecting that the 14.5 and 15.3 will grow quickly over the next 36 hours or so given your E2 levels. I've had follies that grew up to 3mm overnight the day before I triggered, so definitely not impossible. I also only had one mature follie (for sure), which was less than what I'd get on Femara. But, one was all I needed (I'm now 20 weeks with a little boy). I don't know what my E2 levels were, but they called in a panic to schedule my IUI the next morning because my LH was surging (I was not on Ganirelix).

  • I think that their fear is that if you continue the follistim you will end up with way too many mature follicles by the time you ovulate.  Your follicles continue to grow after you are triggered until ovulation occurs.  I would follow their recommendation!

  • My understanding is that the bloodwork is controlling here.  But you should ask if you are not comfortable--ask your RE how s/he thinks you responded to these meds, ask whether any of the other follies may mature.  I would probably go along with the instructions this time, because timing is pretty tight, and then have a consult with your RE where you can talk about your concerns and get your RE's reaction to how you responded to these meds without the pressure of having to decide whether or not to trigger.  And I am sorry about the migraines, have you mentioned this side effect to your RE?

    I had 4 mature follies on my first 2 Clomid/IUI cycles.  On my third cycle, I only had 1 mature follie (maybe two more that could have perhaps caught up between u/s and IUI).  Based on bw, I triggered the night of my monitoring and had the IUI 36 hrs later, and whatever I ended up with was all I needed to at least get a BFP (hoping it stays that way).

  • I know it's really hard to trust them- I have struggled so much with this but they know what they are doing . It is possible for those other follicles to continue to grow . Even a day can make a huge difference . I wouldnt be discouraged . You have responded really well !! Besides , all it takes is one :) good luck !!!

  • I had around the same number of follies last IUI. I was on 75 of follistim, with 3 at 15mm, one 21mm,2 13mm. My level was at 745. They said I produced to many mature follies. So this month they lowered the follistim to 50. I would talk to your RE and tell him your concerns. Have you told them about the migraines? Best of luck.
  • I did talk to them about the migraines. They just think that I'm really sensitive to hormonal changes, based on the fact that crinone is supposed to be much easier than other progesterone supplements, and it still makes me puke! I know I need to trust them, and I also appreciate the fact that I have people on here who take the time to understand and listen. It's so hard when I feel so disheartened...This was my 6th IUI (2nd injectables cycle) Just wish this was all over, ya know?

  • I hear you @RipleyLV426.  Just a thought...have you ever tried acupuncture?  I see a provider who specializes in fertility issues, and the treatments while on stims are meant to help with side effects.  I don't know how much it helps, but for me it's in the "can't hurt, plus it's relaxing" bucket.

  • @RipleyLV426 - I'm sorry you're dealing with your 6th IUI right now.  I am keeping my fingers crossed that this is it for you!!  I've always been migraine prone myself, but I started getting regular chiropractic adjustments when starting fertility treatments and it has REALLY helped.  I rarely get headaches or migraines now, even with all the hormonal changes due to the meds.  A lot of the girls on here find that acupuncture helps as well.  I hope you feel some relief soon!

    I'll give the chiropractic treatments a look! Thank for the advice. I've done acupuncture, and I really liked it! But at $80 a pop, it's a bit more than we can afford to do on the regular.

  • I know you were asking for advice yesterday, so it's too late to really advise, but with the numbers you posted, you had 1, maybe 2 follicles in the running. If you waited an extra day, you were running the risk of having SIX follicles in the running, which is definitely grounds to cancel, to avoid the risk of high order multiples.

    I know you were hoping for 2-3 big fat follicles, but it sounds like a very good response, and reason to be as optimistic as you can during the TWW!
    Me- 39 (turning 40 in April), TTC for the first time ever (since Jan 2015), low ovarian reserve
    Married 3/14/14 to my wonderful wife, but her sperm count is rather low
    TTC with frozen donor sperm and science

    7 IUIs, 7 BFNs.
    2 IVF attempts, both cancelled and converted to IUI, both BFNs.
    Decided that my tired old ovaries are ready to retire.
    Next step- reciprocal IVF, using my wife's eggs, my uterus!  
    fresh 5 day transfer (2 embryos) 4/17/17- BFP! 
    Identical twins "due" 1/2/17 (but anticipated arrival sometime December)

  • I always try to be optimistic. I just kinda suck at it now! Is it stupid that I'd almost was the 6 follicles? Just to feel like I'd have a better chance this month.
