If you've had a vaginal birth - how's your pelvic floor doing?

I am deciding between trying for a VBAC and a csec after having had an emergency csec first time around.  I had a nice easy recovery with the csec.  My sister also had a csec with her first and a VBAC, but totally regrets the VBAC.  She had some complications afterwards related to swelling and tearing that required follow up stitches, and she says she still can't run/jump/sneeze without peeing much of the time.  She did have a big baby the second time (over 10 lbs).  She does not do kegels.  

So my questions are, if you've had a vaginal birth:

1.  How's your pelvic floor generally?
2.  Did/do you have hemorrhoids?
3.  Do you pee when you laugh/sneeze/run/etc?  How often and how inconvenient is it?
4.  Did/do you have to do kegels?
5.  Sex- does it feel the same as before the birth, basically?

    1. Not good (had two babies with off the charts heads). It pretty much feels like it is going to touch the floor all the time no matter how many kegels I do. I have to have surgery eventually for my rectocele (it's painful)

    2. Yes awful ones very regularly. I think they are somewhat genetic and that is an unfortunate thing that runs in my family. With dd1 I had one the size of a half dollar that thrombosed and had to be drained. That sucker was worse than childbirth! My experience here is NOT the norm though.

    3. No thank goodness. I do sometimes pee a little if I waited way too long to go. I peed myself a few times this pregnancy while throwing up violently, but I don't really count that.

    4. I do kegels several times daily

    5. I feel like there is more space in my vagina and it definitely isn't as tight obviously, but my orgasms are much more intense than before children, which is a plus :)

    I didn't seem to have any issues that I knew of before I got pregnant again. This pregnancy I have noticed it's harder to hold pee in and maybe feels a little heavy down there.

    2. Did/do you have hemorrhoids?
    Never

    3. Do you pee when you laugh/sneeze/run/etc? How often and how inconvenient is it?
    Only now that I'm pregnant again.

    4. Did/do you have to do kegels?
    No not regularly.

    5. Sex- does it feel the same as before the birth, basically?
    Yes it did eventually feel exactly the same, obviously not for a few months.
    I had what she called a routine tear, basically the same as what an episiotomy would have been. No issues and healed fine.
  • Let me preface this by saying I have one daughter and she was delivered vaginally without an epidural. She was only 7 lbs 9 oz 19.5 inches long

    Generally it is just fine. It was a little more sore than normal going back to work (I only took 6 weeks off with dd1) but it went back to normal a few weeks after that
    2. Did/do you have hemorrhoids?
    Never did and never have
    3. Do you pee when you laugh/sneeze/run/etc? How often and how inconvenient is it?
    Again not at all
    4. Did/do you have to do kegels?
    I didn't with dd1. I have done them a few times this time around simply because everything feels more swollen/achy and it just makes my vagina feel better after being on my feet teaching when I do them verses when I don't. I've been lacking on this since its summer time but am sure I'll start up again next month when school starts again!
    5. Sex- does it feel the same as before the birth, basically?
    Sex has been a difficult thing for me always. I really think I am just undiagnosed as small small down there as dh and I struggled for a while to find a comfortable sex position (even after almost 4 years missionary is what works best for us and makes pregnant sex difficult). For the first few months it was difficult again. Everything felt very tight and uncomfortable (I had several 3rd/4th degree tears and has to have multiple stitches). But after the (what I assume was) swelling finally went down it felt like there was more room down there (which was a good thing for me).

    I have no scientific proof to back this up but along with what pp said about genetics I think age plays a part too. I was 24 when dd1 was born and will be 26 when dd2 makes her appearance. My mom had me at 32 and ahe felt with a little incontinemce and eventually her entire bladder fell and she had to have surgery to repair it (but that was 16+ years after I was born)

    Has never happened to me.

    I do them when I see them mentioned, but it's not a regular practice for me.


    5.  Sex- does it feel the same as before the birth, basically?
    No, I'm actually a little tighter than before DD1. Had a slight tear and got stitched up. Doctor told me that she'd make hubby happy, lol. Beyond that, things feel pretty much the same.
  • In a nutshell the biggest issue I've had with my pelvic floor after vaginal birth is peeing when exercising. I always have to make sure I empty my bladder before exercise otherwise I'll pee my pants (jumping jacks are the worst!) I will say though that the more I exercise the more control of my bladder I have. I also never did kegels too much. Sex is fine for me, still enjoyable not sure if DH noticed any difference. I do get hemorrhoids on occassion
  • 1. Generally, it's great! I haven't had any issues with my pelvic flooring at all.

    2. No hemorrhoids for me.

    3. I have snissed during this pregnancy, but I did that during my first pregnancy too and it seems about the same.

    4. If I get bored or someone brings them up I do a few, other than that no.

    5. Sex is both the same and better than before birth! Some days it's just really sensitive and others are just normal. I didn't have an episiotomy, but I did tear internally. Only needed four tiny stitches, so I'm not sure about others. DH has not complained about it being different, though the first couple times he was happy cause it was tighter? But I think it had just been 2 months since the last time and he was wrong.

    1.  My pelvic floor/bladder control took about 8 months to return completely to normal.  But it did.  I had some tearing at birth, but it was minimal.  At this point in my pregnancy, it's weakening again.

    2. Yes and yes.  I had haemorrhoids after giving birth the last time, and I have them again now.  And from nothing more than squatting in the bathtub to clean the tub the other day.  WTF?

    3. Sometimes.  But a 'pee' is more like a half of a thimble full of urine.  Still enough that I want to change my undies, but it's not insane.  I don't really get it from laughing, but a hard cough or sneeze could do it.  Again, my control over this returned to normal by the time I was about 7 or 8 months PP with my first, and has only returned now in my third trimester of pregnancy.

    4. Yes and yes.  Though this time I'm working more on strengthening my core.  IMO kegals did nothing for me. 

    5. It took a little bit for sex to feel like it did before, but it did return to normal.

    Better when I'm not pregnant, although if I really need to pee and I sneeze, I pee myself. My son was huge. He was 9.7 and that took a toll of my pelvic floor.

    Never expierence a a hemorrhoid in my life, and hoping I never do.

    More so when I'm pregnant, when I really need to pee, and it's not much, few dribbles.

    I did after having my son, I don't so much bother with them anymore.

    My sex life after birth got so much better, everything became twice as sensitive and orgasms became routine.
  • O man I feel like the only one. .. I must have a really weak pelvic floor.
    1. Pregnant or not it's weak. More so pregnant of course. This will be my third vaginal delivery. I had a fourth degree episiotomy with my first. Tore a little with my second.

    2. I remember having a hemorrhoid after one of my births but that was my one and only.

    3. I have to brace myself for each cough, sneeze and laugh. Like I cannot walk and sneeze or we'll need a mop on aisle 3. It's that serious. Sometimes squatting I'll leak. But I've also gained a lot of weight the past few years that may have an effect on things.

    4. Kegels are not very easy for me. I do them some but I don't feel like I have control really. Like I can very weakly tighten them but can't hold it if that makes sense.

    5. We have a very healthy sex life. I think things went back to normal in that sense. Still have no problem reaching orgasms. (Except now that I'm pregnant. Baby kicking distracts me too much)
  • No issues with any of the above after delivering natural vaginally. I do Kegals to prepare for this delivery, but have never had leaky or issues. Sex at first was uncomfortable - I was anxious that it was going to hurt and I was dry from lack of estrogen from nursing . But that didn't last long.
  • This is all really helpful, thanks everyone!  I still can't decide what to do.  I have a feeling this is going to be a game time decision.
